2. Women alone : a socio-psychological investigation

In this thesis I investigate different meanings of singleness as understood by women who are alone. Using data from interviews with 30 women aged 30 to 60 years, I examine how women work with a single identity in a social context that is changing rapidly. I focus on identity and self-representation through looking at discourse, and the discursive and conversational moves made by participants. My project is interdisciplinary in nature, benefiting from developments in sociology and psychology in an applied social science approach. My analysis draws on critical discursive psychology, and makes additional use of traditions in narrative analysis and conversation analysis. Examination of how participants talked about themselves and singleness shows linked but distinctive forms of 'trouble'. The culturally available and familiar resources for understanding singleness are highly polarised, and participants demonstrated rhetorical work in weaving their ways through the extreme contrasts of a denigrated or an empowered identity. The cultural context incorporates new representations of singleness while continuing to offer continuity with older, more devalued notions. I argue that these polarised ways of thinking create a troubled identity for single women. Telling the story of her life and relationships also presents trouble for the woman alone. The familiar and dominant cultural storylines of western society are of committed heterosexual relationships and ensuing family life. Such resources for telling a life story were at the same time useful and troublesome as participants positioned themselves in comparison with, and at times resisted the familiar storyline. Furthermore, representing themselves as possessing agency and having made choices in their lives was problematic for participants. I also focus in a more detailed way on some patterns of troubled interaction in the interview itself, and consider the relationship of interactions in the interview to broader issues of stigma and social exclusion.

Despite changing meanings of singleness, it remains a 'deficit identity' (Reynolds and Taylor, 2005) and the problem for a woman alone is to account positively for her single state. Our analysis challenges theorisations which would suggest autonomy and agency in how identity and self are constructed. It employs the methodological approaches developed in critical discursive psychology (for instance Wetherell, 1998) to look at the detailed identity work of speakers as part of the identity project proposed by Giddens (1992, 2005), Bauman (1998) and other writers associated with the 'reflexive modernisation' thesis (Adkins, 2002). BACKGROUND Degludec is an ultralong-Acting, once-daily basal insulin that is approved for use in adults, adolescents, and children with diabetes. Previous open-label studies have shown lower day-To-day variability in the glucose-lowering effect and lower rates of hypoglycemia among patients who received degludec than among those who received basal insulin glargine. However, data are lacking on the cardiovascular safety of degludec. METHODS We randomly assigned 7637 patients with type 2 diabetes to receive either insulin degludec (3818 patients) or insulin glargine U100 (3819 patients) once daily between dinner and bedtime in a double-blind, treat-To-Target, event-driven cardiovascular outcomes trial. The primary composite outcome in the time-To-event analysis was the first occurrence of an adjudicated major cardiovascular event (death from cardiovascular causes, nonfatal myocardial infarction, or nonfatal stroke) with a prespecified noninferiority margin of 1.3. Adjudicated severe hypoglycemia, as defined by the American Diabetes Association, was the prespecified, multiplicity-Adjusted secondary outcome. RESULTS Of the patients who underwent randomization, 6509 (85.2%) had established cardiovascular disease, chronic kidney disease, or both. At baseline, the mean age was 65.0 years, the mean duration of diabetes was 16.4 years, and the mean (±SD) glycated hemoglobin level was 8.4±1.7%; 83.9% of the patients were receiving insulin. �Leadership and vision' is the subject of Chapter 3 and Rogers and Reynolds look at how managers can encourage leadership from other people, whether in their team, the organisation or in collaborative work with different agencies. They explore leadership style, and the extent to which managers can and should adapt their personal style to the differing needs of situations and people. Frontline managers may not always feel that they have much opportunity to influence the grander vision and strategy of their organisation. Rogers and Reynolds argue that they none the less play a role in the vision sequence even at the level of putting it into practice and looking for better ways of doing things. Practice-led managers relay their experience from the front line back to more senior management so that they can contribute to strategic planning.

A comparison of stories told in research interviews by people who have no children with those that introduce children and grandchildren. The performance of identity work is considered, giving examples of co-construction. \ud The data include extracts from others’ interview transcripts (Qualidata studies SN 5237, SN 6011, 1998-2002) and from interviews held this year with childfree women from my original study now aged over 60 years. I discuss the resources drawn upon which accommodate the unique details of a particular speaker’s life while also doing work against commonly held assumptions that childlessness is a problematic category requiring explanation.

Objectives\ud This paper reviews transcripts of interviews with childless older people. It addresses research questions concerning how childless older people make sense of their relationships and social and emotional needs, and any gendered patterns of sense-making.\ud Design\ud Secondary analysis is applied to two Qualidata datasets, SN 5237 Adding Quality to Quantity: Quality of Life in Older Age, 2000–2002 (80 interviews with people over 65 years: 9 childless women, 5 childless men) and SN 6011 Older Men: Their Social Worlds and Healthy Lifestyles, 1999–2002 (85 interviews with men over 65 years: 16 childless men). Normative understandings of the life course tend to focus on family life: older people are largely represented as grandparents. Research has tended to ignore the specific experience and perspectives of the childless. The two datasets referred to include those without children among those interviewed, and some comparison can be made of self-representation and identity work undertaken.\ud Methods\ud These datasets were the only relevant ones available at the time of planning the study. All interview transcripts were reviewed in order to identify those without children. The data are analysed using a synthetic form of discourse analysis.\ud Results\ud The data overall resound with references to children and grandchildren. Preliminary findings in these subsets are of rhetorical work from childless women interviewed: claims of closeness to neighbours’ children, or other affiliative connections. Somewhat in response to the interviewers’ line of questioning, the identity work of male participants tends to focus on current or earlier relationships with women.\ud Conclusions\ud The paper concludes concerning the gap in the literature on older people without children and argues that it is important to learn from their perspectives in ways that respect their individuality.

37. Are there life stories other than grandparenthood?

How do people who are not parents talk about their lives, relationships and their experience of ageing? There is a widespread belief that relationships between adult children and their parents are crucial to the support of older people. There is nonetheless an increasing minority of older people without children (some 18% for women born in 1922 and projected to rise to around 23% for those born in 1972, Quinn et al., 1998). It is important to learn from the experience of those already in their period of later life. \ud This paper examines datasets from Qualidata studies (SN5237, SN6011) and data from interviews held between 1998 and 2002 with people aged over 65 years. It compares the narratives co-constructed by respondent and interviewer concerning children and grandchildren with the possibilities for a joint construction of stories from childless respondents. My analysis suggests that narrative structures for a life that progresses through stages associated with the heterosexual family are so deeply embedded in the understanding of both interviewer and respondent, that while the beginnings of other kinds of story can be identified, their telling is often inhibited.\ud Reference\ud Quinn, M., Ruddock, V. and Wood, R. 38. Childless older women: combating a deficit identity?

Government proposals on care for older people often elicit responses on the need to return to the attitude that it’s the family’s responsibility to look after its older members. Indeed social policy has tended to rely on the role of families when reducing the provision of social care through tax-supported services. How do people who don’t have any children experience the ageing process, and with reference to childlessness what kind of discursive work and conversational moves do they undertake in interview?\ud The normative expectations of a life progressing through stages associated with the heterosexual family, including love, marriage and parenthood, suggest that non-parenthood conjures up notions of loss and deficit – already an identity which can be attributed to single women (Reynolds and Wetherell, 2002; Reynolds and Taylor, 2005). In this paper I revisit narratives from childless women first interviewed as living alone in their fifties and sixties some thirteen years ago. Women demonstrated rhetorical work in discussing their handling of questions on whether or not they have children. What does non-parenting mean to them now they are in the sixties and more? How does generativity (more usually associated with having children and grandchildren) play for women entering later life? 40. Older people without children

In a recent discussion about government proposals on care for older people (July 2009), the Radio 4 news presenter referred to a possible need to return to the attitude that it's the family's responsibility to look after its older members. Yet there is often already an expectation that the family – by which is usually meant children or grandchildren – can be relied upon to take an active part in the social life and any care arrangements for their older members. Indeed, social policy in much of the UK has tended to rely on the role of families when reducing the provision of social care through tax-supported services. \ud My interest is therefore in this sizeable group that is often excluded from discussions about ageing. What happens to people who do not have children to 'care' and how do they experience ageing? \ud Several research studies have challenged from different perspectives the widespread belief that relationships between adult children and their parents are crucial to the social support of older people (Davidson, 2004, Hoeffer, 1987, Koropeckyj-Cox, 2002, Koropeckyj-Cox, 2003, Thompson and Whearty, 2004, Wenger et al., 2000). - Abstract
This chapter considers the role of senior staff in managing the use of space and the physical environment within care settings. These day and residential settings come under the broad umbrella of social work and social care but the care may also be organized from within health. This is a subject that does not appear on the curriculum of many management courses in the UK, but arguably it is critical in relation to social work and social care where managers are involved with residential homes, day centres and other provision where the physical environment can enhance or be detrimental towards service users' wellbeing. In such settings many activities go on under one roof, particularly in group care or when the care is provided in a person's own home. The care environment is complex and can be bounded within space, place, time and behaviour. Activities and time available compete or have different meanings for the participants. Managers have different kinds of relationships with workers and with service users. The environment in which care takes place often frames these.

42. Women Alone: A socio-psychological investigation

- Abstract
In this thesis I investigate different meanings of singleness as understood by women who are alone. Using data from interviews with 30 women aged 30 to 60 years, I examine how women work with a single identity in a social context that is changing rapidly. I focus on identity and self-representation through looking at discourse, and the discursive and conversational moves made by participants. My project is interdisciplinary in nature, benefiting from developments in sociology and psychology in an applied social science approach.\ud \ud My analysis draws on critical discursive psychology, and makes additional use of traditions in narrative analysis and conversation analysis. Examination of how participants talked about themselves and singleness shows linked but distinctive forms of 'trouble'. The culturally available and familiar resources for understanding singleness are highly polarised, and participants demonstrated rhetorical work in weaving their ways through the extreme contrasts of a denigrated or an empowered identity. The cultural context incorporates new representations of singleness while continuing to offer continuity with older, more devalued notions. I argue that these polarised ways of thinking create a troubled identity for single women.\ud \ud Telling the story of her life and relationships also presents trouble for the woman alone. The familiar and dominant cultural storylines of western society are of committed heterosexual relationships and ensuing family life. Such resources for telling a life story were at the same time useful and troublesome as participants positioned themselves in comparison with, and at times resisted the familiar storyline. Furthermore, representing themselves as possessing agency and having made choices in their lives was problematic for participants. I also focus in a more detailed way on some patterns of troubled interaction in the interview itself, and consider the relationship of interactions in the interview to broader issues of stigma and social exclusion.

- Abstract
Although a context might be solid - like a building - it has different meanings for the people who use it. The impact of the physical space is the focus for 'Managing environments'. Peace and Reynolds note that any one environment may be multifunctional. Taking an innovative approach to management, they argue that a residential care home, for example, may be simultaneously a place for living for residents, working for staff and managers and visiting for relatives and other professionals. The atmosphere in a care home makes a critical difference to the experience of service users who live there. The authors review how the environmental factors that influence care may be managed and how the careful design and us of space can contribute to improved quality of life in the management of care. Peace and Reynolds also consider relationships between people, places and quality, recognising the impact of the manager on caring environments. A focus on practice here emphasises the role of the manager in drawing out debates over values and best practice to help care workers feel supported and confident about the care they provide.
