4. Medicinal Gardens as an Educational Strategy in the Teaching of the Natural Sciences: A Pedagogical Proposal

Abstract

The main aim of this paper was to analyze how medicinal gardens in secondary schools can improve teaching-learning processes in rural settings. The sample comprised 179 students (69.3% girls) from a rural public secondary school in the province of Huila (Colombia). The age of the participants ranged from 15 to 17 years with a mean of 15.86 years (SD = 0.86). Seeking school motivation strategies for rural students from a rural institution in the municipality of Guadalupe, teachers of the natural sciences designed didactic-pedagogical alternatives employing situated learning that would allow them to contribute to solving school problems and environmental issues affecting rural areas. The environmental phenomena included the loss of fertile soils and the harmful effects of the use of agrochemicals. Medicinal gardens together with situated learning activities proved a useful teaching tool in formal rural educational settings.

Abstract

Stones form from bile components that have precipitated in crystal forms. Frequency of stones has had a significant increase in advanced age and in females. Currently, MRI is a first-line method with high sensitivity (93%) and specificity (96%). Objective: To determine the prevalence of bile duct stones in patients aged 35 to 85 years diagnosed by magnetic resonance cholangiopancreatography at the Institute of Diagnostic Imaging, in Cuenca, Ecuador, from January 2021 to December 2022. Method: Descriptive, retrospective and crosssectional study. Data was analyzed and tabulated according to the study variables. Results: Out of 326 patients, 281 cases tested positive: The prevalence of bile duct stones in patients aged 35 to 85 years diagnosed by magnetic resonance cholangiopancreatography at the Institute of Diagnostic Imaging in Cuenca, Ecuador, from January 2021 to December 2022, was 86.2%. Female sex prevailed (64.4%) and patients between 65 and 74 years old were the most affected (40.2%). Stones in the gallbladder reached a percentage of 39.5% and absence of anatomical variations reached 92.9%. Conclusion: There is a high prevalence of bile duct stones, which mainly affect females between 65 and 74 years of age, and their morphological location is more frequent in the gallbladder, featuring also absence of anatomical variations. Similar findings are reported in other studies.

Abstract

Background: Patient satisfaction is measured according to the quality of service provided. Therefore, the patient will be satisfied as long as that service meets their needs (1). Mammogram technique is considered a noninvasive procedure, with 95% sensitivity and 79% specificity, considering that ultrasound is the gold standard for detecting diseases of the mammary gland, whit 97.61% sensitivity and 91.49 % specificity (1). Objective: To determine the satisfaction and quality of service to female patients requiring mammograms at the imaging department of a private institution, October-December 2023, in Cuenca, Ecuador. Method: Descriptive, cross-sectional, prospective study. The sample was all surveys of patients who underwent a mammographic study. Data was tabulated and analyzed according to the study variables. Results: Out of 347 surveys, the prevailing age was 40-49 years (35%) and the youngest age was 70-80 years (10%). In the reliability dimension, 39% of the patients are moderately satisfied and 1% is slightly dissatisfied. In the responsiveness dimension, 37% are moderately satisfied and 1% is slightly dissatisfied. In the security dimension, 40% are moderately satisfied and 1% is slightly dissatisfied. The empathy dimension shows 45% full satisfaction and 1% slight dissatisfaction. In regard to the quality of service in tangible elements, 38% are moderately satisfied and 2% are moderately dissatisfied.

Abstract

Background: The main cause of shoulder pain is rotator cuff injuries; of the four tendons that make it up, the one that is frequently injured is the supraspinatus tendon (1). Shoulder MRI has a high sensitivity (around 90%) in regard to tendon injuries due to tears, a sensitivity that increases with complete tears, and a specificity of 99% (3). Objective: To determine the prevalence of supraspinatus tendon injuries diagnosed by shoulder MRI in patients aged 20- 80 years who received medical care at Medimagen, Cuenca, in 2021-2022. Method: Descriptive and retrospective study which analyzed radiological reports of patients who underwent shoulder MRI in Medimagen. Microsoft Excel and IBM SPSS Statistics 25 were used for statistical analysis. Results: 422 radiological reports of patients who underwent shoulder MRI in 2021-2022 were analyzed. Results show that 265 patients had injuries to the supraspinatus tendon. Females were the most affected (52.1%) and the most affected shoulder was the right one (64.2%); the age range with the highest prevalence was 50-59 years (27.5%). The most common injury was partial rupture (38.5%). Conclusions: The study determined that shoulder injuries occur more frequently in females. The supraspinatus tendon is the most affected, and different injuries like tendinitis, partial rupture, and complete rupture, among others. The most affected shoulder was the right one.

Abstract

Background: Vascular calcification is a powerful predictor of cardiovascular and overall mortality. Cardiac calcifications usually secondary to a degenerative process are a frequent incidental finding in tomography, which offers relevant information to establish an adequate medical or surgical treatment. Purpose: To determine the frequency of coronary artery calcifications incidentally identified on chest CT in patients aged 45-90 years, in Medimagen, 2019-2022. Methodology: We will proceed to the analysis and tabulation of data by means of a retrospective descriptive method, cross-sectional, in which information will be collected from all patients who underwent a simple tomographic study of the thorax in the established period in the Centro de Diagnóstico por Imagen Medimagen, the same will be exposed with descriptive statistics. Use of results: This study will determine the prevalence of patients with coronary artery calcifications identified incidentally by simple chest tomography in the established age range and who were seen at the Medimaging Diagnostic Imaging Center. Conclusion: It was concluded that the frequency of calcifications in coronary arteries diagnosed incidentally by simple chest tomography mainly affects males between 56 and 60 years of age, with a more frequent morphological location in the anterior descending artery, showing similar results to other studies

Abstract

Combined gene and cell therapy are promising strategies for cancer treatment. Given the complexity of cancer, several approaches are actively studied to fight this disease. Using mesenchymal stem cells (MSCs) has demonstrated dual antitumor and protumor effects as they exert massive immune/regulatory effects on the tissue microenvironment. MSCs have been widely investigated to exploit their antitumor target delivery system. They can be genetically modified to overexpress genes and selectively or more efficiently eliminate tumor cells. Current approaches tend to produce more effective and safer therapies using MSCs or derivatives; however, the effect achieved by engineered MSCs in solid tumors is still limited and depends on several factors such as the cell source, transgene, and tumor target. This review describes the progress of gene and cell therapy focused on MSCs as a cornerstone against solid tumors, addressing the different MSC-engineering methods that have been approached over decades of research. Furthermore, we summarize the main objectives of engineered MSCs against the most common cancers and discuss the challenges, limitations, risks, and advantages of targeted treatments combined with conventional ones. Abstract

Tumour-infiltrating lymphocytes are associated with a survival benefit in several tumour types and with the response to immunotherapy.sup.1-8. However, the reason some tumours have high CD8 T cell infiltration while others do not remains unclear. Here we investigate the requirements for maintaining a CD8 T cell response against human cancer. We find that CD8 T cells within tumours consist of distinct populations of terminally differentiated and stem-like cells. On proliferation, stem-like CD8 T cells give rise to more terminally differentiated, effector-molecule-expressing daughter cells. For many T cells to infiltrate the tumour, it is critical that this effector differentiation process occur. In addition, we show that these stem-like T cells reside in dense antigen-presenting-cell niches within the tumour, and that tumours that fail to form these structures are not extensively infiltrated by T cells. Patients with progressive disease lack these immune niches, suggesting that niche breakdown may be a key mechanism of immune escape. Abstract

Objective: This article examines how parent–child geographic proximity changes around the onset of parental health shocks in the United States. Differences in the likelihood of moving closer across social groups are also investigated. Background: Adult children often care for older parents with health problems, but this requires relatively close proximity. As families are becoming smaller and many adult children live away from their parents, it is unclear how responsive families will be to older adults' health problems. Method: We estimate a series of fixed effects and event study models on data from the Health and Retirement Study (2004–2018) to assess changes in parent–child proximity after parents' first onset of cognitive impairment and functional limitations. Results: We find robust evidence that parents and children tend to stay close or move closer to each other in response to parent's health declines. Moves occur immediately and in subsequent waves after the onset of health shocks. Reductions in parent–child distance are consistently larger among mother‐daughter dyads, dyads without spouses or multiple children, and non‐Hispanic white families. Conclusion: The geographic availability of adult children to provide care is responsive to parents' needs. After the onset of a serious health condition, most older adults have a spouse or child living close enough to provide care. Abstract

Repeated claims that a dwindling supply of potential caregivers is creating a crisis in care for the U.S. aging population have not been well-grounded in empirical research. Concerns about the supply of family care do not adequately recognize factors that may modify the availability and willingness of family and friends to provide care to older persons in need of assistance or the increasing heterogeneity of the older population. In this paper, we set forth a framework that places family caregiving in the context of older adults' care needs, the alternatives available to them, and the outcomes of that care. We focus on care networks, rather than individuals, and discuss the demographic and social changes that may alter the formation of care networks in the future.
