- Abstract
[Background]: Microscopic residual disease (MRD) after surgery can be a challenging situation in cutaneous squamous cell carcinoma (CSCC) and there is a lack of evidence concerning its management. [Objective]: To evaluate the prognosis of CSCC with MRD and the usefulness of postoperative radiotherapy (PORT) in CSCC with MRD. [Methods]: Retrospective cohort study of CSCC with MRD through a 10-year period (2010–2019) (n = 244). Disease-free survival and event-free survival were assessed using R (v.3.4.1), considering competing risks. Evaluated outcomes were local recurrence (LR), nodal metastases (NMs), and disease-specific death (DSD). [Results]: Median age was 88y (IQR: 10.5). A total of 145 (59.43%) were men and 69 (28.28%) were immunosuppressed. Median tumour diameter and thickness were 19 and 6.4 mm (IQR 11 and 5.5 mm). Patients treated by re-excision had a relapse rate of 4.3% compared with 11.30% and 29.71% in those who received PORT and observation (P = 0.045). The use of PORT was associated with a lower risk of LR compared with observation (HR = 0.206 [0.049–0.859], P = 0.030), but not with a lower risk of NMs or DSDs. In the multivariable models, PORT was again associated with a lower risk of LR than observation (HR = 0.167 [0.039–0.708], P = 0.014), but not with lower risk of metastasis and death. [Conclusions]: We always should try to obtain clear margins after surgery. PORT improves local control in CSCC with MRD, but when administered to the tumour bed, it does not reduce the risk of NM and DSD.

- Abstract
[Background]: Acantholytic cutaneous squamous cell carcinomas (aCSCCs) have been classically considered as a high-risk variant of CSCC. However, more recent studies show that aCSCC does not confer more aggressiveness. This study aims to establish whether the prognosis of the aCSCC is worse than that of the non-acantholytic (naCSCC) or not. [Methods]: Retrospective case-control study with 50 aCSCCs and 50 naCSCCs. For each aCSCC, an naCSCC with similar high-risk features to the aCSCC but with no acantholysis was selected. Prognosis between both groups was compared. [Results]: The mean age was 86 years (SD 9.61). Sixty-one patients were men. Thirty-nine CSCCs were located in high-risk head and neck areas. Twenty CSCCs exhibited a poor degree of differentiation, and 36 showed an infiltrative growth pattern. The tumor diameter was 18.71 mm (interquartile range, IQR 35), and the tumor thickness was 6.72 mm (IQR 15.50). Twelve CSCCs exhibited perineural infiltration, and eight CSCCs exhibited invasion beyond the subcutaneous fat. Positive margins after excision of the tumor in 22 aCSCCs vs eight naCSCCs (P < 0.02). Nineteen poor-prognosis events were observed (local recurrence, lymph node metastasis, and death from CSCC). However, no differences were observed between both groups when comparing poor-prognosis events. [Conclusion]: The proportion of unfavorable events is similar in aCSCC and naCSCC. The acantholytic histopathological subtype is not associated with a poorer prognosis than the non-acantholytic CSCC in our cohort.

- Abstract
Advances in virology and skin cancer over recent decades have produced achievements that have been recognized not only in the field of dermatology, but also in other areas of medicine. They have modified the therapeutic and preventive solutions that can be offered to some patients and represent a significant step forward in our knowledge of the biology of skin cancer. In this paper, we review the viral agents responsible for different types of skin cancer, especially for solid skin tumors. We focus on human papillomavirus and squamous cell cancers, Merkel cell polyomavirus and Merkel cell carcinoma, and human herpesvirus 8 and Kaposi’s sarcoma.

- Abstract
Introduction: Cardiovascular autonomic modulation can be considered a useful tool in determining the physiological state of the interaction between the autonomic nervous system and the cardiovascular system. Objectives: To determine the differences in linear parameters of the basal auto- nomic balance between medical students and young baseball players. Methods: A cross-sectional analytical study was carried out in the Biomedical Basic Sciences Laboratory, Faculty No.1, of the University of Medical Sciences Universidad de Ciencias Médicas in Santiago de Cuba. The population and sample consisted of 36 individuals (Group 1: 18 high-performance young baseball athletes, Group 2: 18 medical students). Data was collected using an 8-channel PowerLab polygraph and it was processed using the Kubios® Software version 3.0.4 Premium. Results: There were significant differences in the values between students and athletes: pNN50 (p=0.009), stress index (p=0.044) and in parasympathetic (p=0.005) and sympathetic (p=0.001) indexes. The discriminatory ability of the parasympathetic index to be associated with the best physical fitness of the athletes was good (area under the curve 0.784). The optimum cut-off point above which the parasympathetic index is associated with the group of athletes was set at 0.57. Conclusions: The parasympathetic index was associated with the group of athletes, showing the vagal predominance in the modulation of cardiac activity in the individuals belonging to this group., Introducción: La modulación autonómica cardiovascular puede ser considerada como una herramienta útil en la determinación del estado fisiológico de la interacción entre los sistemas nervioso autónomo y cardiovascular. Objetivo: Determinar las diferencias en los parámetros lineales del balance auto- nómico basal entre estudiantes de medicina y atletas juveniles de béisbol. Método: Se realizó un estudio analítico de tipo transversal en el Laboratorio de Ciencias Básicas Biomédicas de la Facultad Nº. 1 de la Universidad de Ciencias Médicas de Santiago de Cuba. Universo y muestra de 36 individuos (Grupo 1: 18 atletas juveniles de béisbol de alto rendimiento, Grupo 2: 18 estudiantes de medicina). Los datos fueron recolectados, mediante de polígrafo PowerLab de 8 canales, y fueron procesados usando el software Kubios® versión 3.0.4 Premium.Resultados: Existieron diferencias significativas en los valores de pNN50 (p=0,009), índice de estrés (stress index [p=0,044]), y en los índices parasimpático (p=0,005) y simpático (p=0,001) entre estudiantes y atletas. La capacidad discriminatoria del índice parasimpático para asociarse con la mejor forma física de los atletas fue buena (área bajo la curva 0,784). El punto de corte óptimo por encima del cual el índice parasimpático se asocia al grupo de los atletas quedó establecido en 0,57. Conclusiones: El índice parasimpático se asoció con el grupo de atletas, lo que evidencia el predominio vagal en la modulación de la actividad cardíaca en los sujetos de este grupo.

- Abstract
Trabajo fin de grado. Defendido en 2017. 4 carpetas: Curriculum Vitae ; Resumen ; Trabajo Fin de Grado ; Láminas El proyecto se plantea como una propuesta regenerativa para el barrio. El abandono y la desconexión entre dos zonas de Madrid se hace patente por una compleja topografía. La diferencia entre la forma de vida entre la parte alta y baja es un hecho. La parte de arriba de la cornisa se ha convertido a lo largo de los años en un espacio residual y únicamente de tránsito. Dada su localización privilegiada por la cantidad de espacios públicos de interés, zonas turísticas, edificios emblemáticos y sus vistas, se plantea una intervención urbana además del centro de salud que lo integre dentro de la misma. El proyecto se plantea como un remate a la cornisa de Madrid, ya que se encuentra en un punto donde esta se empieza a desdibujar a través de la trama urbana. El proyecto formaría parte de uno de los alzados más característicos de Madrid, contemplándose a su izquierda los Jardines de Sabatini, El Campo del Moro, el Palacio Real, la Catedral de la Almudena y finalmente el museo de las Colecciones Reales de Tuñon y Mansilla. Se ha querido establecer un diálogo con este último a través del acabado de la fachada, integrándolo con ese alzado. En cuanto al espacio público se resuelve una plaza dura que cree un nuevo espacio turístico por sus características singulares, como el fragmento de muralla árabe que casi nadie visita. Además se han incorporado diversos usos como un gimnasio al aire libre o un escenario multiusos, que le darán vida a la plaza a cualquier hora del día. Dada la importancia que cobra la plaza dentro del proyecto como elemento integrador del centro de salud en el espacio urbano, la sistemática que he seguido a la hora de contar el proyecto lo divide en dos partes: el espacio urbano y el centro de salud. Existen múltiples conexiones entre ambos que los relacionan de manera simbiótica. En cuanto al centro de salud, que es el programa principal que había que resolver, opté por un elemento lo menos invasivo posible, ya que me interesaba que estuviese integrado dentro de la plaza. Además, su modulación y diseño permiten que si en algún momento este deja de ser necesario, pueda albergar fácilmente otro uso. El diseño se basa en un largo pasillo longitudinal a la pieza que se contrapone a un ritmo marcado por muros de hormigón perpendiculares que van definiendo los usos. Esta decisión se tomó porque favorece el funcionamiento diario del mismo, ya que uno de los principales problemas de los centros de salud y hospitales es que la gente se pierde frecuentemente. En la parte posterior del edificio, la que da a la plaza, aparecen una serie de patios paralelos al pasillo que permiten tener luz natural en prácticamente todas las estancias del centro. Además permiten salir a tomar el aire sin tener que salir del edificio. Y ya que los patios eran necesarios, se han aprovechado para implantar un sistema de ventilación pasivo que actúa mediante la diferencia de temperatura existente entre la plaza de piedra y el pinar, reduciendo así el gasto energético. El centro posee dos accesos diferenciados, uno en cada extremo, cubriendo así la normativa de seguridad anti incendios. El acceso principal se encuentra en la plaza, mientras que el otro está más apartado y es exclusivamente para el personal. Como continuación de la estructura de muros se ha resuelto un pequeño espacio público cubierto que consta de una cafetería y unos baños públicos en una pieza separada. Este programa era necesario para cubrir el uso que va a tener la plaza, además es un espacio bastante atractivo arquitectónicamente ya que genera un belvedere encuadrando el oeste de Madrid. Introduction: The responses of the autonomic nervous system to situations of stress and struggle usually translate into an increase in HR, blood pressure and cardiac output. The complexity of these responses can be evaluated through the nonlinear parameters of the HR variability. Objective: To determine the modifications that the nonlinear parameters of the HR variability undergo when faced with mental stress. Method: A quasi-experimental study was conducted, with no control group, before-after, a sample consisting of 10 men in the Laboratory of Basic Medical Sciences at the Universidad de Ciencias Médicas de Santiago de Cuba, for the analysis of the RR intervals' variability of the electrocardiogram in the Powerlab 8. For the detection of RR peaks, its processing was used and the variability parameters' calculation of the Premium HRV Kubios program (version 3.1.0). Results: In the nonlinear parameters of HR variability, when comparing in individuals the mental stress induced by the arithmetic calculation test with respect to the rest state, there was a reduction in the sample entropy (p=0.047) -parameter related to the degree of complexity and adaptability of the systems- and an increase of SD2/SD1 (p=0.005) that represents the cardiac sympathetic index. Conclusions: Under conditions of induced mental stress, the nonlinear parameters of HR variability reflected an increase in sympathetic modulation over the heart and a decrease in the complexity and adaptability in the autonomic cardiac regulation. Introduction: Stress is considered by psychologists to be responsible for changes. When it continues over time can affect many psychological and physiological functions such as the immune system, metabolism, reproduction and circulation. The regulation of the autonomic nervous system on the cardiovascular system that is commonly affected by stress can be evaluated through the HR variability. Objective: To determine the effect of the Stroop word-color test in nonlinear parameters of the HR variability according to the sex. Method: A quasi-experimental study was conducted, before-after, with no control group, in 10 medical students of both sexes randomly selected in Santiago de Cuba, for the analysis of the RR intervals' variability of the second standard deviation of the electrocardiogram in the Powerlab 8 device. For the detection of RR peaks, its processing was used and the variability parameters' calculation of the Premium HRV Kubios program (version 3.1.0). Results: It was observed that men showed a significant decrease (p=0.008) of the sample entropy (parameter related to the degree of complexity and adaptability systems) before the application of the Stroop Color test (pre-test rest) with respect to women. Conclusions: In the period preceding the Stroop word-color, the men presented a decrease in complexity and adaptability in the autonomic cardiac regulation, with respect to the women, which could lead to a predisposition towards psychic stress and inadequate physiological responses. - Abstract
Introduction: Stress is the mental health problem of the future. The psychophysiological response of each individual to this noxa is diverse and the HR variability represents an efficient predictor in order to evaluate it. Objective: To determine the differences between males and females in presence of stress by analyzing the linear dynamics of HR variability. Method: A quasi-experimental study, before-after, with no control group, was carried out in 10 healthy individuals (5 females and 5 males). We analyzed the linear parameters of the HR variability and the differences between sexes at rest and before a certain stimulus (Stroop Color Test). In the statistical analysis, nonparametric techniques were used, as the Mann-Whitney U test for qualitative variables and the Wilcoxon signed-rank test for quantitative variables. Results: The low frequency band (LF [p=0.038]) had a statistically significant increase from basal state to mental stress. The HR in men was higher at both, basal state (p=0.016) and mental stress (p=0.009). Women showed a pNN50 (parasympathetic indicator [percentage of consecutive RR intervals that differ by more than 50 ms from each other]) greater (p=0.009) in presence of stress than men. In a multiparametric analysis, from the qualitative point of view, the sympathetic index was elevated in males with respect to females. Conclusions: In men, a greater sympathetic cardiovascular psychophysiological response to stress with respect to women was evidenced. [ABSTRACT FROM AUTHOR]

- Abstract
Introduction: The HR variability is the variation of this vital sign during a time interval, previously defined, never greater than 24 hours, in a study of consecutive circadian periods. Its analysis represents an effective method in the evaluation of the autonomic regulation of cardiac activity. Objective: To determine the effect of the arithmetic calculation test on the linear parameters of the HR variability. Method: A quasi-experimental study, before-after, with no control group, was conducted, where the effects of arithmetic calculation test were analyzed in the linear parameters of HR variability. The variability indexes were analyzed in a sample of 10 healthy men with an average age of 19.5 years. The RR intervals were obtained under basal (control) and stress conditions. Results: We found a decrease in the RR intervals (p=0.005), of the RMSSD (square root of the mean of the differences of the RR intervals; p=0.007), pNN50 (percentage of consecutive RR intervals, which differ by more than 50 ms from each other, p=0.005), and the parasympathetic index (p=0.005), as well as increases in mean and maximum HR (p=0.005), the sympathetic index (p=0.005) and the vagal-sympathetic ratio quotient (p=0.005). Conclusions: It was demonstrated that this invasive, quantitative and reproducible technique provides interesting information about the behavior of HR in presence of a controlled mental exercise, where mental stress is shown to be a modifier of cardiovascular variability during the arithmetic calculation test.
