4. Analysis of the image and corporate identity of the Colegio Mexicano de Reumatologia: Is it time to redefine it?

The Colegio Mexicano de Reumatología (CMR) is a corporation whose brand has two elements—image and identity—that differentiate it from other corporations. We evaluated aspects of CMR's corporate image and identity. To assess corporate image, we designed a survey using proof-of-concept and discrete-choice-experiments approaches. It assessed which definition (orthopedist, rheumatologist, or rehabilitator) was most meaningful in four pain scenarios in healthy adults from the country's Western region. We used discourse analysis and five readability indices of the CMR website to assess corporate identity. In total, 700 respondents were included. For every rheumatologist chosen in the hand scenario, respondents chose 1.13 orthopedists and 0.70 rehabilitators. For every rheumatologist chosen in the knee scenario, respondents chose 2.36 orthopedists and 0.64 rehabilitators, whereas 0.85 orthopedists and 0.58 rehabilitators were chosen in the arthritis scenario. Only 38% of the respondents preferred the CMR's definition of a rheumatologist to describe a rheumatologist. The younger age group preferred orthopedists to rheumatologists (50% vs. 31%, p < 0.001). In the arthritis scenario, the choice of rheumatologist increased from 27% in the elementary school group to 49% in the university group (p < 0.001). Mother was the most influential in healthcare seeking. The discursive analysis revealed that the CMR is positioned as a "we" restricted to "colleagues;" the patient did not have agentive representation. The semiotic structure of the CMR's mission/vision was deemed imprecise and lacking in statements of value and purpose; the readability scores indicated that the text was challenging and dry. The CMR's corporate image does not differentiate it from other health providers. CMR's identity seems ambiguous with restricted directionality. It seems pertinent to redefine the CMR. Existe un interés creciente en el potencial de la telemedicina (TM) como un medio alternativo a la consulta física. Aunque numerosos estudios prueban los beneficios de la TM en reumatología, no existen recomendaciones sobre su implementación en España. El objetivo de este estudio fue analizar la aplicación de la TM en consultas de reumatología en España. Estudio cualitativo, transversal, multicéntrico con metodología Delphi en 2 rondas de consultas. Se diseñó un cuestionario ad hoc estructurado que incluía enunciados sobre teleconsulta, teleconsulta de enfermería, teleasistencia, telerrehabilitación, telerradiología, teleeducación sanitaria, principales barreras, ventajas e inconvenientes de la teleeducación sanitaria y TM en artritis reumatoide. Los participantes fueron especialistas en reumatología que ejercían su labor asistencial en España. Los reumatólogos participantes (N = 80) tenían una edad media de 42,4 (± 9,0) años, con 12,6 (± 8,4) años de experiencia. Algunos de los aspectos de la TM que obtuvieron un mayor consenso fueron: La TM es útil para el seguimiento de algunos pacientes, ayudar a determinar si es necesaria una consulta presencial, o asistir a pacientes con artritis reumatoide si presentan baja actividad o en remisión; ciertos pacientes, como aquellos en su primera consulta o aquellos que presentan barreras digitales o deterioro cognitivo, deben ser atendidos presencialmente; la TM presenta algunas barreras técnicas y de acceso de los pacientes; la TM puede ser útil en enfermería y en la formación médica continuada. La TM puede ser beneficiosa para el tratamiento y seguimiento de los pacientes con enfermedades reumáticas, así como para aliviar la carga asistencial presencial en reumatología. There is growing interest in the potential of telemedicine (TM) as an alternative to physical consultation. Although numerous studies prove the benefits of TM in rheumatology, there are no recommendations on its implementation in Spain. The aim of this study was to analyze the application of TM in rheumatology consultations in Spain. Qualitative, cross-sectional, multicenter study with Delphi methodology in 2 rounds of queries. A structured ad hoc questionnaire was designed that included statements on teleconsultation, nursing teleconsultation, telecare, telerehabilitation, teleradiology, telehealth tele-education, main barriers, advantages and disadvantages of telehealth tele-education and TM in rheumatoid arthritis. The participants were rheumatology specialists practicing in Spain. The participating rheumatologists (N = 80) had a mean age of 42.4 (± 9.0) years, with 12.6 (± 8.4) years of experience. Some of the aspects of TM that obtained the greatest consensus were: TM is useful for follow-up of some patients, to help determine if a face-to-face consultation is necessary, or to assist patients with rheumatoid arthritis if they present low activity or in remission; certain patients, such as those in their first consultation or those who present digital barriers or cognitive deterioration, should be seen face-to-face; TM presents some technical and patient access barriers; TM can be useful in nursing and in continued medical education. The characteristics of synovial fluid (SF) in geriatric patients differ from those in younger patients. In Mexico, epidemiologic data on the incidence of different rheumatic diseases in geriatric patients are scarce. To describe the physical characteristics of geriatric SF and the prevalence of crystals in knee and other joint aspirates from patients with previously diagnosed joint disease. A retrospective study was performed with a baseline of 517 SF samples between 2011 and 2023. White blood cell count was performed by Neubauer chamber and crystals were identified by polarized light microscopy. Descriptive statistical analysis was performed and prevalence was reported as a percentage. The mean age of the adults was 73.5 ± 5.0 years, 54.4% were women and 45.6% were men. The mean SF volume was 6.3 ± 9.5 mL in older adults and 15.3 ± 24.9 mL in those younger than 65 years. The mean viscosity in older adults was 9.5 ± 4.5 mm and the mean leukocyte count was 7352 ± 16,402 leukocytes/mm3. Seventy percent of the older adults' SFs were referred to the laboratory for osteoarthritis (OA), with lower proportions for rheumatoid arthritis (RA) (14.6%) and gout (5.1%). Of the crystals observed in the geriatric population, 14.6% corresponded to monosodium urate crystals (CUM) and 18.9% to calcium pyrophosphate crystals (CPP). The characteristics of LS in older adults were smaller volume, increased viscosity, and non-inflammatory. The main diagnoses were OA, RA, and gout. El Lupus Eritematoso Sistémico (LES) es uno de los prototipos de las enfermedades autoinmunes, compromete principalmente al sexo femenino, con una proporción 9:1 mujer-hombre. Se caracteriza por la formación de autoanticuerpos con la participación de la inmunidad adaptativa e innata, en interacción con los factores genéticos y medioambientales. Estos autoanticuerpos son parte central de la fisiopatología de la enfermedad, causando desde compromiso orgánico leve hasta compromiso multisistémico grave. El diagnóstico es clínico, orientado por una anamnesis detallada, un examen físico exhaustivo y los marcadores de autoinmunidad; eventualmente podría apoyarse en criterios clasificatorios de algunas sociedades científicas. La actual revisión pretende destacar las generalidades sobre la enfermedad, haciendo énfasis en su fisiopatología, presentación clínica, y su abordaje diagnóstico y terapéutico, sirviendo de apoyo para la formación médica integral en beneficio de los pacientes.

Introducción: la Reumatología es una especialidad clínica con elevada demanda y gran desigualdad en su ejercicio en Latinoamérica y el país. Objetivos: describir el estado del ejercicio de la Reumatología en nuestro país. Materiales y métodos: estudio observacional de corte transversal que recabó información a través de una encuesta online difundida por e-mail y de un código QR en el 54º Congreso Argentino de Reumatología. Resultados: se recibieron 127 respuestas; ochenta y seis (67,7%) fueron del sexo femenino, con una mediana de edad de 45 años. Ciento trece (113; 89%) encuestados eran especialistas en Reumatología, con 20 años (RIC 14-28) de ejercicio de la medicina y 13 años (RIC 6,5-20) de la especialidad. El 55,5% ejercía tanto en el ámbito público como privado, el 38,6% solo en el privado y el 7,6% únicamente en el público. Del total, el 84,3% trabajaba en dos o más lugares y el 21,2% en cuatro o más. Sesenta de los encuestados (60; 59,8%) recibía una remuneración mensual de entre 100.000 a 249.000 pesos (U$S: 501,68-1249,18) y 27 (23,1%) de menos de 100.000 pesos (U$S 501,68). Conclusiones: nuestro estudio muestra un alto porcentaje de profesionales que se desempeña en múltiples lugares de trabajo. Una quinta parte de ellos tenía remuneración baja.

14. Valores de referencia para el 1-minute sit-to-stand test en pacientes pediátricos con artritis idiopática juvenil

Introducción: los niños con artritis idiopática juvenil (AIJ) experimentan períodos de dolor e inmovilidad que afectan sus capacidades condicionales. Objetivos: describir los valores de referencia para el 1-minute Sit-to-Stand Test (1-STS; test de 1 minuto de sentarse y pararse) en niños con AIJ como evaluación de la capacidad aeróbico-funcional y de la fuerza muscular de los miembros inferiores (MMII). Materiales y métodos: se realizó un estudio observacional que incluyó a 15 niños con AIJ de entre 5 y 16 años. Se evaluó su rendimiento en el 1-STS. Resultados: se encontró una correlación positiva significativa entre el 1-STS y el test de la marcha de 6 minutos (r=0,56; p=0,03), como con el índice de capacidad funcional (CAPFUN) (r=0,54; p=0,03). No se observaron correlaciones significativas entre el Childhood Health Assessment Questionnaire (CHAQ) y el 1-STS (r=-0,21; p=0,44), tampoco con el Juvenile Arthritis Disease Activity Score (JADAS-10) (p=0,83). Conclusiones: el 1-STS parece prometedor para medir la capacidad aeróbico-funcional y la fuerza muscular de los miembros inferiores en niños con AIJ oligoarticular.

15. Emerging trends in nurse-led programs of care for management of patients with established rheumatoid arthritis: Systematic literature review.

In patients with rheumatoid arthritis (RA), nurses are considered as essential, not only to ensure pharmacological safety, but also in the promotion in self-care and decision-making, favouring the empowerment of patients. This systematic review aimed to summarize the available literature on the health education by the nurse in patients with RA. Following Cochrane Collaboration procedures, the PRISMA statement and PRISMA checklist, relevant quantitative studies published were retrieved from the CINAHL, Scopus, PubMed and Medic databases and then systematically reviewed. The search ended in August 2021. Nineteen studies were retained for inclusion and evaluated with the Scottish Intercollegiate Guidelines Network for Systematic Reviews. We found statistically significant improvement in self-care (five studies), disease activity (three studies), quality of life (two studies), satisfaction (five studies) and adherence (one study) with the nursing-led management of patients with established rheumatoid arthritis. Although there is solid evidence of improvement in satisfaction and self-care, there seems to be a trend also to improve other outcomes, such as DAS28, from the EULAR recommendations, the expansion of the therapeutic arsenal for rheumatoid arthritis and shared decision-making. In addition, recently and due to the implementation of new technologies, the role of the nurse has been evaluated through virtual consultations. The results of recent studies have shown that this an effective and well-accepted novel approach for the management of patients with stable rheumatoid arthritis. Our study suggests that nurse-led health education, in addition of improvement in satisfaction and self-care, improve activity disease scores in RA patients. [ABSTRACT FROM AUTHOR]

Identificar percepciones en cuanto a sesgos de género en la reumatología española y cuantificar la implicación de ambos sexos en todos los ámbitos de la especialidad. Se envió una encuesta a todos los miembros de la SER, sobre participación y percepción de sesgos y de la propia capacitación y se revisaron los datos reales de participación en órganos directivos, congresos, comités y de los servicios de reumatología españoles en los últimos cinco años. La encuesta fue respondida por 95 personas, 4,8% de los miembros de la SER (14 hombres y 81 mujeres). No se detectaron diferencias por sexo en edad, nivel académico, puesto, centro de trabajo, reparto de tareas laborales y del ámbito familiar; tampoco en las invitaciones a puestos de poder en los cinco últimos años, ni en la percepción de capacidad para ocupar los distintos puestos de poder. Los hombres encuestados consideran con mayor frecuencia que actividades como la participación en un comité científico o dar una ponencia no dan poder. Un tercio de los participantes de ambos sexos considera que la SER debería revisar sus procesos con perspectiva de género y menos de un tercio cree que debe hacerse por cuotas. Tras revisar los datos de 2015 a 2020, los ratios hombre a mujer en las principales actividades de liderazgo en reumatología en España son: 1) juntas directivas de la SER 3:2 sin presidenta ni tesorera; 2) comités científicos de los congresos 2:1; 3) ponentes y moderadores: simposios satélites, 4:1; 4) editores jefes de revistas científicas 9:1; 5) puestos académicos global 1,7:1, catedráticos 9:1, profesores titulares 100% hombres y 6) jefes de servicio: 75% son hombres a pesar de representar 40% de reumatólogos en España. A pesar de un creciente número de mujeres en la reumatología española, estas no están suficientemente representadas en puestos de liderazgo, existiendo un sesgo de género no percibido. To identify perceptions of gender bias in Spanish rheumatology and to quantify the involvement of both sexes in all areas of the specialty. A survey was sent to all members of the SER on participation and perception of biases and of their own competencies, and actual data on participation in governing bodies, congresses, committees, and Spanish rheumatology departments in the last 5 years were reviewed. The survey was answered by 95 rheumatologist, 4.8% of SER members (14 men and 81 women), both groups being similar in terms of age, academic level, and position and work centre. No differences were detected in the distribution of work and non-work tasks between sexes, nor in invitations to positions of power in the last five years, nor in the perception of capacity to occupy the different positions of power, which was high for both sexes. Male respondents more frequently consider that activities such as participating in a scientific committee or giving a conference are not empowering. A third of both sexes consider that the SER should review its processes with a gender perspective but less than a third believe that this should be done by quotas. The reality of the last 5 years is that 1) there is a male to female ratio of 3:2 on SER boards of directors and in this period there has been no female president or treasurer; 2) in the scientific committees of the congresses men predominate (2:1) although slightly less in the local organizing committee; 3) there are more male speakers and moderators than women (very striking in satellite symposia, 4: 1); 4) 9 out of 10 editors-in-chief are men; 5) in academic positions there are 3 men for every 2 women, 9 to 1 in professorships or emeritus positions; although more women supervise residents; and 6) there are more women (60%) than men (40%) in Spanish rheumatology departments, although 75% of department chiefs are men. Background: Takayasu arteritis is a large-vessel vasculitis which affects large-caliber elastic arteries, primarily the aorta and its main branches. It mainly affects women between 20-30 years, so it is rare in children. Case report: We describe the case of a 15-year-old female who was followed up since she was 9 years old due to celiac disease. At the age of 13, anaemia of chronic disorders associated to elevated C-reactive protein and erythrocyte sedimentation were detected. The patient remained asymptomatic. After excluding other diseases, we requested a positron emission computed tomography (PET-CT); lesions compatible with large-vessel vasculitis were detected. Cardiology evaluation showed an aneurysm in the right coronary artery. Angio-CT suggested Takayasu type III arteritis. Conclusions: The delay in the diagnosis of Takayasu arteritis in pediatric patients is quite common. In this case, we have found phase II lesions, with no previous phase I symptoms. However, PET-CT allowed the diagnosis of vasculitis, key to the diagnosis of the patient. [ABSTRACT FROM AUTHOR]

La poliarteritis nodosa es una vasculitis necrotizante de rara presentación que afecta vasos de mediano y pequeño calibre, que puede afectar desde un órgano hasta sistemas del cuerpo humano, por lo tanto, su presentación clínica es variable y su etiología es predominantemente idiopática. Respecto a su diagnóstico, se deben descartar otras patologías y correlacionar la clínica, las imágenes diagnósticas y paraclínicos disponibles, con el fin de brindar una terapia adecuada y oportuna, teniendo en cuenta que la terapia farmacológica ayuda a disminuir el daño y la progresión más no es una cura. Se presenta el caso de un paciente de 46 años con poliarteritis nodosa cutánea rápidamente progresiva que llevó a necrosis de los dedos de las 4 extremidades, se logró estabilizar y detener su progresión mediante el reconocimiento y manejo farmacológico con glucocorticoides y citotóxicos, sin embargo, requirió amputación.

To analyse the outcome of scientific abstracts submitted to the Argentine Congress of Rheumatology (ACOR) in 2000, 2005, 2010, and 2015. Every abstract submitted to the ACOR was analysed. The number of these manuscripts published was determined through Google Scholar and PubMed searches. The impact of the scientific journals was established through the SCImago Journal (SJR) indicator. Considering the 727 abstracts evaluated, 10.2% of the articles were found in journals indexed by Google Scholar, and 6.6% in PubMed: 4.7% were published in 2000, 9.4% in 2005, 14.6% in 2010, and 11.9% in 2015 (Log Rank test 0.008), with a statistically significant increase between 2010 and 2015 compared to 2000 (HR 3.3; 95% CI 1.5–7; p 0.002 and HR 2.9; CI 1.4–6.3; p 0.005 respectively). The median SJR of the journals was 0.46 and 67.6% had SJR available. The publication rate was low, and only a few articles were published in the most prestigious journals within the speciality. Rheumatic diseases account for almost 30% of consultations attended in Spanish primary care centres. The main objective was to analyse the demand for rheumatology consultations from Primary Care and their resolution using the electronic consultation system. Retrospective descriptive study of electronic consultations from primary care centres in the health area to the Rheumatology service of a tertiary hospital, between July 2020 and May 2021. The last 500 consecutive consultations were collected. Mean age of patients was 59.5 years; 74.2% were women. Main reasons for consultation were osteoporosis and treatment of patients with rheumatoid arthritis and spondyloarthritis under follow-up by the department. Mean response time was 2 days. Fifty-seven per cent of patients required outpatient appointments. Over 40% of queries were resolved thanks to the electronic consultation system in an average of 2 days, otherwise patients would have been referred to specialized care. [ABSTRACT FROM AUTHOR]

Con el advenimiento de las drogas biológicas en Reumatología, hace más de 20 años, se inauguró un nuevo capítulo en el tratamiento de las enfermedades reumáticas, con notables mejoras en la calidad de vida de los pacientes reflejada a través de remisiones sostenidas, disminución en la progresión estructural, reducción significativa en la evolución a discapacidad, siendo que muchos pacientes pueden llevar hoy en día una vida normal aún en condiciones de enfermedades basalmente agresivas. Quince años después, con la aparición de los inhibidores JAK, la terapéutica amplió las oportunidades para los tratamientos orales con altas tasas de eficacia. Sin embargo, como todo medicamento con acción inmunomodulatoria, todos estos tratamientos no están exentos de efectos adversos, algunos de ellos frecuentes y esperables, otros más raros y con distintos niveles de intensidad.

Takayasu arteritis is a rare systemic vasculitis, characterized by the involvement of large vessels, mainly the aorta and its branches. The diagnosis is based on the suggestive clinical condition, complementary tests and the criteria of the American College of Rheumatology. Treatment is based on high doses of glucocorticoids and disease-modifying agents such as methotrexate, azathioprine, and cyclophosphamide. The aim of this study is to report a case of Takayasu's arteritis in a female patient, with extensive vascular involvement and classic manifestations of the disease. The patient was treated with high doses of corticosteroids and cyclophosphamide, followed by maintenance treatment with methotrexate. Knowing the disease is crucial to avoid late diagnosis and reduce morbidity and mortality. Treatment must always be individualized, based on side effects and availability of each medication, as well as the experience of the prescribing professional. [ABSTRACT FROM AUTHOR]

To determine the number of rheumatologists per 100,000 inhabitants working in public or private centres in Spain as a whole, and by Autonomous Community and their distribution by age and sex. Cross-sectional study based on the information contained in the database of the Spanish Society of Rheumatology. Quality control was performed by contact (e-mail and telephone call) with the heads of the clinical services of each of the hospitals (public and private). The information analysed was the age, sex and place of work of active rheumatologists in February 2020. The rates of rheumatologists per 100,000 inhabitants were calculated from population data from the National Institute of Statistics. The rate of rheumatology specialists per 100,000 inhabitants in Spain was estimated at 2.17. The percentage of women was 59.7%, with a higher female/male ratio at younger ages. The lowest proportion of specialists per 100,000 inhabitants was in the community of Valencia (1.6), and the highest in Cantabria (3.2). Variations were found in the rate of rheumatologists per 100,000 inhabitants among the Autonomous Communities. The distribution by age and sex showed a tendency towards female rheumatologists, especially in the younger age strata. [ABSTRACT FROM AUTHOR]

Identification of patients with rheumatoid arthritis (RA) has improved during the last decade, although there is still a need for biomarkers allowing an early diagnosis. In this regard, it has been recently proposed that Gal1 serum levels are increased in patients with RA compared to the general population. However, this topic is controversial in the literature. In this work, we provide additional information about the potential usefulness of Gal1 serum levels as a biomarker for RA diagnosis. We studied Gal1 serum and synovial fluid levels and clinical parameters in samples from 62 patients with early arthritis belonging to the PEARL study. In addition, 24 healthy donors were studied. We found that both patients fulfilling RA criteria and patients with undifferentiated arthritis showed higher Gal1 levels than healthy donors. Similar findings were observed in synovial fluid, which showed even higher levels than serum. However, we did not find correlation between Gal1 levels and disease activity or disability. Despite the high number of PTPs expressed in CD4 T cells and their involvement in autoimmunity, information about the expression profile of PTPs in these cells has not been obtained in patients diagnosed with autoimmune diseases. Here, we compare the expression profile of PTPs in CD4 T cells of healthy volunteers and patients submitted to an early arthritis clinic, due to suspicion of rheumatoid arthritis, an autoimmune disease mediated by CD4 T cells. We found lower transcript levels of the mitogen-activated protein kinase (MAPK) phosphatase dual-specific phosphatase-7 (DUSP7) and the cell division cycle-25B (CDC25B) in T cells of patients. While the low expression level of DUSP7 was restricted to patients with positive rheumatoid factor and anti-citrullinated protein antibodies, the altered expression of CDC25B correlated with the activity of the disease. Low levels of CDC25B might contribute to the progression of the autoimmune arthritis and/or might be consequence of the inflammatory environment in the active disease. - Abstract
Objectives: oxidative stress plays a major role in the onset and progression of involutional osteoporosis. However, classical antioxidants fail to restore osteoblast function. Interestingly, the bone anabolism of parathyroid hormone (pTH) has been shown to be associated with its ability to counteract oxidative stress in osteoblasts. The pTH counterpart in bone, which is the pTHrelated protein (pTHrp), displays osteogenic actions through both its n-terminal pTH-like region and the c-terminal domain. Methods: We examined and compared the antioxidant capacity of pTHrp (1-37) with the c-terminal pTHrp domain comprising the 107-111 epitope (osteostatin) in both murine osteoblastic Mc3T3-e1 cells and primary human osteoblastic cells. Results: We showed that both n- and c-terminal pTHrp peptides at 100 nM decreased reactive oxygen species production and forkhead box protein o activation following hydrogen peroxide (H2o2)-induced oxidation, which was related to decreased lipid oxidative damage and caspase-3 activation in these cells. This was associated with their ability to restore the deleterious effects of H2o2 on cell growth and alkaline phosphatase activity, as well as on the expression of various osteoblast differentiation genes. The addition of Rp-cyclic 3′,5′-hydrogen phosphorothioate adenosine triethylammonium salt (a cyclic 3',5'-adenosine monophosphate antagonist) and calphostin c (a protein kinase c inhibitor), or a pTH type 1 receptor antagonist, abrogated the effects of n-terminal pTHrp, whereas protein phosphatase 1 (an src kinase activity inhibitor), sU1498 (a vascular endothelial growth factor receptor 2 inhibitor), or an anti osteostatin antiserum, inhibited the effects of c-terminal pTHrp. Los cuidados paliativos (CP) se conocen como los cuidados de confort y alivio de síntomas, es la atención que se brinda para mejorar la calidad de vida de los pacientes portadores de una enfermedad crónica avanzada e incurable. Las enfermedades crónicas impactan en la morbimortalidad y en la calidad de vida relacionada con la salud (CVRS). La CVRS es un concepto multidimensional, está relacionado con el impacto individual de la enfermedad en la capacidad funcional y la percepción del paciente en los roles sociales, físicos y mentales. La medicina paliativa tiene una amplia población de atención que incluye; pacientes oncológicos con enfermedad avanzadas, enfermedades degenerativas como las enfermedades autoinmunes sistémicas (EAS), siendo este grupo las que constituye el papel central de la atención en reumatología. Los CP se centran en la atención integral activa, continua del paciente, su familia y su entorno, este seguimiento se realiza por un equipo multidisciplinario cuando la expectativa no es la curación. Proporciona mejoría en la calidad de vida y su atención cubre aspectos físicos, psicosociales y espirituales extendiéndose incluso hasta el periodo del duelo. La siguiente revisión describe como las enfermedades reumáticas impactan en la calidad de vida del paciente, las diferentes escalas aplicadas para su medición y la importancia de la integración temprana de los cuidados paliativos en Reumatología.

Cataracts are the leading cause of reversible blindness in developing countries, their causes are varied, some are secondary to medications (such as the steroids), or eye diseases like uveitis, which might be associated in some cases with rheumatic conditions. The precise mechanism by which steroids promote cataracts development is not yet known, although it is relevant to be aware of this adverse effect in patients receiving these treatments. The relationship between cataracts and uveitis is important, as it is a common complication in this setting. Surgical management in these cases is more complex compared to those in which concomitant uveitis is not present. The aim of this review is to highlight the relationship and importance of a multidisciplinary approach between ophthalmologists and rheumatologists for providing an adequate management, and achievement of a good visual prognosis. Introducción: en 2020, la rápida evolución de la pandemia por SARS-CoV-2 desencadenó una emergencia sanitaria que generó una importante reorganización del sistema de salud, lo que llevó a la discontinuación y posterior adecuación de los sistemas de formación. El objetivo de este estudio fue describir el impacto de la pandemia en un grupo de reumatológos en formación en la Ciudad Autónoma de Buenos Aires. Materiales y métodos: se envió una encuesta online por correo electrónico a los alumnos de la Carrera de Especialista en Reumatología de la Sociedad Argentina de Reumatología y de la Universidad de Buenos Aires. Incluyó 24 preguntas relacionadas con el impacto de la pandemia en diferentes aspectos de la formación y las nuevas estrategias educativas implementadas.

Introdução: A fibromialgia é uma síndroma reumatológica crónica, de etiologia desconhecida, caracterizada pela distribuição generalizada de queixas álgicas e uma multiplicidade de sintomas. O seu tratamento continua a ser um importante desafio. Alguns fármacos antipsicóticos, como a quetiapina, têm sido estudados como terapêutica adjuvante. No entanto, existe ainda alguma controvérsia relativamente à sua eficácia e segurança. O objetivo deste trabalho foi avaliar o papel da quetiapina no controlo de sintomas em indivíduos com fibromialgia. Métodos: Foi realizada uma revisão bibliográfica com pesquisa de artigos em diferentes plataformas científicas utilizando os termos MeSH fibromyalgia e quetiapine. Foram incluídos estudos que cumpriam os critérios definidos pelo modelo PICOS: (P) adultos com diagnóstico de fibromialgia; (I) plano terapêutico com recurso a quetiapina; (C) placebo ou outro tipo de comparador; (O) alívio de sintomas e perfis de tolerabilidade e segurança; (S) revisões sistemáticas e ensaios clínicos aleatorizados controlados. Foi utilizada a escala Strength of Recommendation Taxonomy, da American Academy of Family Physicians, para avaliação dos níveis de evidência e atribuição de forças de recomendação. Resultados: Dos 66 artigos encontrados foram selecionados três ensaios clínicos aleatorizados controlados. Os resultados encontrados são heterogéneos e apesar de, na sua generalidade, parecerem mostrar vantagem no controlo dos principais sintomas dos doentes com a utilização da quetiapina não foi encontrada evidência robusta que comprove um benefício inequívoco. Os autores relataram boa tolerância e ausência de efeitos adversos severos com o seu uso. Conclusões: A evidência atual é de fraca qualidade, não livre de viés e de difícil generalização, apesar de aparentemente favorável no alívio parcial de alguns sintomas de doentes com fibromialgia (força de recomendação B). Este perfil positivo não é sustentado por significância estatística. É necessário realizar novos estudos, mais robustos, com estrutura e metodologia mais específicas, claras e consensuais, de forma a chegar a conclusões de qualidade.

Introducción: La esclerodermia cutánea localizada es una enfermedad crónica del tejido conectivo, etiología desconocida, caracterizada por áreas de piel induradas. La Morfea es una enfermedad rara con incidencia de 0,3-3 casos por 100.000 habitantes / año. Más común en mujeres, proporción 4:1 mujer/hombre. Caso Clínico: Mujer, 21 años acude al Servicio de Dermatología del Hospital de Clínicas por cuadro de 3 años de evolución de mancha roja en cadera de lado derecho que luego se vuelve marrón, aparece lesión similar en muslo y rodilla derechos con misma evolución, sin desencadenante aparente ni síntomas acompañantes. Al examen físico se observa varias placas hipocrómicas con bordes eritematosos, entre 2-3 cm, límites netos y bordes irregulares en muslo derecho. Placas hipercrómicas induradas entre 1-6 cm de diámetro, límites netos, bordes regulares en cadera y muslo derechos, y brazo izquierdo. Biopsia compatible con Morfea. Tratamiento con hidroxicloroquina, metotrexate, corticoides tópicos, vitamina A, C, E con respuesta y evolución favorable. Discusión: Esclerodermia localizada aparece en adultos entre 40- 50 años, en comparación con paciente que afectó segunda década de vida. Tronco como localización más frecuente, característica de la paciente, además en miembros superiores e inferiores. Artralgias se presentan en 44% de casos, coincidentes con la paciente. Aumento del factor reumatoideo, eosinofilia, VSG guían hacia diagnóstico de Morfea en fase activa, no apreciables en el caso. Tratamiento con metotrexate como inmunosupresor es la terapéutica con evolución favorable, complementados con hidroxicloroquina y vitaminas A y E.

La ecografía se ha incorporado en los últimos años a la formación de los residentes en reumatología, fruto de su creciente expansión en práctica clínica diaria. Nuestro objetivo es evaluar la calidad percibida de la formación en ecografía de los residentes en los servicios de reumatología de España. Encuesta online dirigida a aquellos reumatólogos que iniciaron su periodo de formación en reumatología entre los años 2009 y 2019. Ciento treinta y nueve reumatólogos participaron en la encuesta, y el 97,1% contaban con al menos un ecógrafo en su centro de formación. Hasta un 51,1% realizaron una rotación en ecografía y el 56% contaban con una consulta de ecografía. El acceso a los cursos SER fue alto (87,8%) mientras que fue limitado a los cursos EULAR (17,3%); el 69,1% de los residentes no completó la acreditación de competencias. La valoración más alta en el grado de formación lo recibieron la valoración de actividad articular inflamatoria, entesis y enfermedades microcristalinas. La valoración de la formación en ecografía durante la residencia fue buena en el 36% de los casos, regular en un 28,1%, mala en un 18% y excelente en un 12,9%. Un 88% de los encuestados considera que ha mejorado su práctica clínica como reumatólogo. La mayoría de residentes han realizado rotaciones en ecografía. La participación en los cursos de ecografía de la SER es alta, moderada en los cursos EULAR, mientras que solo una minoría completaron la acreditación de competencias. El grado global de satisfacción de formación en ecografía durante la residencia es bueno y, en opinión de los residentes, contribuye a la mejora de las competencias como reumatólogos. Ultrasound has been included in the training of residents in rheumatology in recent years, as a result of its increased use in daily clinical practice. Our objective is to evaluate the perceived quality of ultrasound training of residents in rheumatology services in Spain. Online survey aimed at rheumatologists who began their training in rheumatology between 2009 and 2019. One hundred thirty-nine rheumatologists participated in the survey, of which 97.1% had at least one ultrasound machine in their training centre. Up to 51.1% performed a rotation in ultrasound and 56% had an ultrasound consultation. Access to SER courses was high (87.8%) while access to EULAR courses was limited (17.3%) and up to 69.1% of residents did not complete the competency accreditation. Training in evaluation of inflammatory joint activity, entheses and microcrystalline diseases received the highest scores. Evaluation of ultrasound training during the residency was good in 36% of cases, fair in 28.1%, poor in 18% and excellent in 12.9%. Of those surveyed, 88% consider their clinical practice as a rheumatologist to have improved. Most residents have performed ultrasound rotations. Participation in SER ultrasound courses is high and moderate in EULAR courses, while only a minority completed the competency accreditation. The overall degree of satisfaction with training in ultrasound during residency is good and, in the opinion of residents, contributes to the improvement of their skills as rheumatologists. 39. Drugs Recommended in Adult Rheumatic Diseases, But Considered for Off-Label Use in Argentina.

Off-label (OL) drug use is the prescription of a drug for indications other than those authorised in its technical datasheet. The objective of this study was to identify drugs recommended in rheumatology but considered for off-label use in Argentina. A list of medications for certain selected rheumatic conditions was compiled. A drug was considered recommended if it was endorsed by a) at least one Argentine or Pan-American treatment guideline or consensus, or b) two international treatment guidelines, or c) one international treatment guideline and one selected textbook. Approval of these drugs for any condition in Argentina until December 31st, 2018 was explored, and medicines were divided into those with on-label indications and those considered for OL use. One hundred and thirty-six medications were analysed in 13 clinical conditions. Sixty-seven OL recommendations (49%) were found, and several drugs had more than one. All the conditions included the recommendation of at least 1 OL drug except osteoporosis and rheumatoid arthritis. The frequency of OL recommendations for the following conditions was 100%: calcium pyrophosphate dihydrate crystal deposition disease, polymyalgia rheumatica, Sjögren syndrome, and systemic sclerosis. The drugs with the highest number of OL recommendations were methotrexate (in 7 conditions), and glucocorticoids and mycophenolate (in 4). There were 2 OL recommendations for rituximab and 1 for abatacept. Almost all the rheumatic disorders analysed involved the recommendation of at least 1 OL medication, and in 4 conditions all the recommendations were OL. Introducción: en el contexto de la pandemia por SARS-CoV-2, la Sociedad Argentina de Reumatología (SAR) organizó su congreso “híbrido” (presencial y virtual), en diciembre de 2021, en concordancia con la condición epidemiológica. El objetivo principal de este trabajo fue describir la frecuencia de nuevos casos de infección luego de asistir al Congreso y la opinión de los médicos sobre los aspectos de bioseguridad del evento, y como objetivo secundario, analizar las características de los asistentes durante la pandemia por SARS-CoV-2. Materiales y métodos: estudio transversal a través de una encuesta online, autoadministrada y anónima. Se encuestaron a los concurrentes (médicos y no médicos) al Congreso (presencial o virtual). La primera encuesta fue al momento de la inscripción y la segunda luego de 14 días de culminado, solo para los médicos. Se realizó un análisis descriptivo de los resultados. Resultados: 1.322 individuos se inscribieron al Congreso; 1.039 (98,9%) eran médicos. 1.051 (79,5%) completaron la primera encuesta y 501 (48,2%) contestaron la segunda. Mientras 428 (85,4%) asistieron presencialmente, la virtualidad la eligieron aquellos con más años de ejercicio (p=0,023), con comorbilidades (p=0,03) y quienes tuvieron una internación previa por SARS-CoV-2 (p=0,05). Del total, 1.028 (97,8%) estaban vacunados. El 84,6% tuvo una opinión favorable sobre la modalidad “híbrida”. Cinco (1,2%) presentaron síntomas de infección por SARS-CoV-2 y tres (0,7%) tuvieron confirmación diagnóstica. Conclusiones: cinco personas registraron la infección sintomática después del evento. Las medidas de bioseguridad tomadas fueron las aconsejadas por el Ministerio de Salud de la Nación y la opinión de los médicos sobre las mismas fueron favorables.

En este nuevo número de la Revista Argentina de Reumatología se publican cuatro trabajos originales, tres casos clínicos y una revisión. El estudio del Dr. Gustavo Citera, junto con la Dra. Virgina Carrizo Abarza como primera autora, presenta los resultados de prevalencia de depresión y ansiedad en 100 pacientes con artritis psoriásica. En este trabajo, de excelente calidad cientí ca, la presencia de estas manifestaciones se asoció a mayor actividad de la enfermedad; al respecto, los autores concluyen que la detección y el tratamiento de estos trastornos deben incluirse dentro de las estrategias de evaluación y seguimiento de estos pacientes.

Realmente fue y es un honor para mi grupo de trabajo que el Dr. Gustavo Citera escribiera un editorial basado en nuestro artículo, sobre todo siendo un referente nacional en lo que respecta a la evaluación y el tratamiento de los pacientes que padecen artritis reumatoidea (AR). También fue muy grati cante constatar que coincidimos en varios conceptos. Sin embargo, motiva esta carta el hecho que disiento con él en algunas de sus afirmaciones y me veo en la obligación de realizar algunas aclaraciones.

El tratamiento busca disminuir la sintomatología y conservar la capacidad funcional de estos pacientes.

La reumatología vive un momento histórico, hasta hace 30 años eran pocos los tratamiento disponibles para las dolencias que afectaban a nuestros paciente. El metotrexate se convirtió en la piedra angular en el tratamiento de la artritis reumatoide y otros entidades inflamatorias, sin embargo muchos pacientes continuaban con dolor e inflamación articular. El siglo 21 dio paso al inicio de nuevas moléculas más selectivas para el tratamiento de la enfermedad, fue así como aparecieron los primeros Anti TNF (Etanercep e Infliximab), con el tiempo llegaron más moléculas con diferentes mecanismos de acción.
