2. Access time to pasture under wet soil conditions: Effects on productivity and profitability of pasture-based dairying

ABSTRACT: A long grazing season improves the profitability of pasture-based dairy production. It can entail grazing under wet soil conditions and the risk of damaging swards. Housing cows either temporarily or completely while soil moisture is high can avoid damaging swards. An experiment with 4 grazing systems was conducted over 3 yr (Sep. 1, 2013, to Aug. 31, 2016). The purpose was to evaluate whether soil moisture measurements are an effective decision support to assess the risk of treading damage and effects on pasture productivity and dairy cow performance during wet soil conditions. Access time to pasture between February and December of each grazing season was dependent on volumetric soil moisture content (VSMC, m3/m3) measured each morning: Control = cows were housed at VSMC >0.5 and otherwise allowed 22 h/d access to pasture; S0.7 and otherwise allowed 22 h/d access to pasture; S7–6 = cows were housed at VSMC >0.7 and allowed 8 h/d access to pasture at VSMC between 0.7 and 0.6 and 22 h/d access at VSMC ≤0.6; S7–5 = cows were housed at VSMC >0.7 and allowed 8 h/d access to pasture at VSMC between 0.7 and 0.5 and 22 h/d access at VSMC ≤0.5. Cows with 8-h access per day received no other feeding when housed. All herds were compact spring-calving, with a mean calving date of Feb. 19. Mean stocking rate was 2.57 cows/ha. Measurements of VSMC provided an objective indicator for the risk of treading damage. Less time spent at pasture under wet soil conditions lowered treading damage but had no effect on annual pasture production (mean 14.8 t of organic matter/ha). Annual milk solids production per cow was lowest for the control herd (485 kg) and not different between the other systems (503 kg). Reducing treading damage to swards did not improve productivity or profitability of the grazing systems. Nevertheless, measuring soil moisture was a useful decision support for assessing the risk of treading damage when turning cows out to pasture.

4. What is the impact of restricted access policy on workplace violence in general hospital? A before-after study in a CHINESE tertiary hospital

Abstract Background To evaluate the impact of a restricted access policy on workplace violence in a healthcare setting. Methods We surveyed healthcare workers before and after the implementation of a restricted-access policy at a tertiary hospital in north-eastern China. Data were collected in April 2017 and January 2019. Fisher’s exact test were used to compare the difference in workplace violence prevalence between responses to two surveys. Survey 1 (S1) collected data from 345 healthcare professionals who had worked in the inpatient ward for at least 12 months. Survey 2 (S2) included 338 healthcare workers from the same ward who had been employed for more than two years. The effective response rates for the two studies was 79.31 and 83.25%, respectively. All 18 female security guards were included in the investigation in S2. Results The prevalence of psychological violence was 62.03% in S1 and 34.62% in S2, the difference in prevalence showing statistical significance (P = 0.000), while the prevalence of physical violence was 3.77 and 4.73% respectively, showing no statistical significance (P = 0.573). The change in the rate of injury caused by physical violence was also statistically significant at 76.92 and 31.25% (P = 0.025), respectively. Security guards were at high risk of workplace violence under the policy. Most healthcare professionals thought this policy ameliorated treatment order, the sense of security, anxiety about workplace violence, and so forth, but one-third of the respondents thought that it caused patient dissatisfaction. Conclusion While the restricted access policy may be effective for healthcare professionals in avoiding or dealing with violence, such policy could contribute to new problems regarding the safety of security guards and the potential dissatisfaction of patients. The policy should be further developed to alleviate this phenomenon.

5. Outpatient health care utilization and health expenditures of asylum seekers in Halle (Saale), Germany - an analysis of claims data

Abstract Background Asylum seekers are a vulnerable group with special needs in health care due to their migration history and pre-, peri- and postmigratory social determinants of health. However, in Germany access to health care is restricted for asylum seekers by law and administrative regulations. Methods Using claims data generated in the billing process of health care services provided to asylum seekers, we explore their utilization of health care services in the outpatient sector. We describe the utilization of outpatient specialties, prevalences of diagnoses, prescribed drugs and other health care services, as well as total costs of health care provision. Results The estimated prevalence for visiting an ambulatory physician at least once per year was 67.5% [95%-Confidence-Interval (CI): 65.1–69.9%], with a notably higher prevalence for women than men. The diagnoses with the highest one-year prevalence were “Acute upper respiratory infections” (16.1% [14.5–18.0%]), “Abdominal and pelvic pain” (15.6% [13.9–17.4%]) and “Dorsalgia” (13.8% [12.2–15.5%]). A total of 21% of all prescriptions were for common pain killers. Women received more diagnoses across most diagnosis groups and prescribed drugs from all types than men. Less than half (45.3%) of all health care costs were generated in the outpatient sector. Conclusion The analysis of claims data held in a municipal social services office is a novel approach to gain better insight into asylum seekers’ utilization of health services on an individual level. Compared to regularly insured patients, four characteristics in health care utilization by asylum seekers were identified: low utilization of ambulatory physicians; a gender gap in almost all services, with higher utilization by women; frequent prescription of pain killers; and a low proportion of overall health care costs generated in the outpatient sector. Further research is needed to describe structural and individual factors producing these anomalies.

Introduction: A recent law (DCTO-2020-883-APN-PTE-Law No. 27,350. Regulation) passed in Argentina put an end to the ban imposed for the last 60 years on cannabis cultivation within the country. The law permits restricted access to cannabis derivatives for medicinal, therapeutic, and palliative use by individuals and communities, allowing self- and community-based cannabis production. This is cause for concern in view of the lack of quality controls for cannabis derivatives. The several varieties of cannabis grown in Argentina have different chemical profiles and are processed in a variety of ways-mostly by alcohol extraction or maceration at different temperatures and for different amounts of times-making the cannabinoid content of these preparations highly variable. Determining the characteristics of home- and community-grown cannabis products will facilitate the implementation of public policies conducive to their safety and improvement. Objective: The aim of this study was to determine the cannabinoid chemotypes used for therapeutic purposes in Argentina and evaluate whether the cannabinoids present in homemade derivatives are comparable to those in commercially available products. Materials and Methods: High performance liquid chromatography with ultraviolet and diode array detector (HPLC/UV-DAD) analysis of 436 samples (oils, resins, and inflorescences) was carried out to determine the identity and concentration of five cannabinoids: tetrahydrocannabinolic acid (THCA), tetrahydrocannabinol (THC), cannabidiolic acid (CBDA), cannabidiol (CBD), and cannabinol (CBN). From three different sources, the samples represent the type of medical cannabis preparations to which patients have access. Results: The results indicate that the medium-to-low cannabinoid concentration in a significant number of homemade oil samples is similar to that found in commercial products. Most of the samples have a THC/CBD ratio >1 or only contain THC. Acidic cannabinoids were detected in homemade preparations, but were not reported in package inserts of commercial products. Conclusions: Our results indicate that despite their considerable variability, homemade preparations as a whole show cannabinoid levels and profiles equivalent to the commercially available products commonly used for medicinal, therapeutic, and palliative purposes in Argentina.

The problems associated with the opportunities for young farmers to have sufficient access to agricultural land and investment in land resources are particularly relevant. Farming investment in recent years has been increasingly recognized as an objective need by farmers. This in turn raises a number of problems related to access to it. On the one hand, they are generated by the nature of this fixed capital fund as an irreplaceable and limited natural resource. On the other hand, the appetite for agricultural land is further enhanced by the current system of European subsidies, which provides large companies and investors with significant revenues because of the huge agricultural holdings they have.Young farmers, in comparison with other producers, have greater difficulties in accessing agricultural land, mainly due to their lower available financial resources and the lack of preferential conditions for the use of in-vestment loans intended for young farmers. During the years of the second programming period 2014–2020, certain changes in the investment priorities of farmers are observed. Although the priority of investment is in machinery, plant and equipment remains, it is increasingly weakening at the expense of increasing investment interest in land and permanent crops. According to the potential attitudes of farmers, investment in agricultural land will play a leading role in other types of agricultural investment.The purpose of the article is to reveal the main problems of young farmers related to access to agricultural land and the level of investment activity in land resources. 10. Unraveling residential burglary trends in urban Mexico: The role of target accessibility, time and regional contexts.

This study aims to examine how contextual factors influence the risk of burglary victimization, specifically investigating whether time and regional effects shape these relationships. By analyzing data from the Mexican system of cities between January 2018 and June 2022 while controlling for population characteristics, we detected significant decreases in burglary risk among urban dwellers living in apartments and using restricted access mechanisms, although these effects are limited to certain regions. On the other hand, occupants of detached houses and those without restricted access mechanisms did not experience a reduction in burglary risk. These findings validate existing criminological theories emphasizing the importance of contextual factors. Moreover, our study underscores the necessity of further exploring why the downward decrease in burglary rates across cities came to a halt after the COVID-19 pandemic. • Residential burglary declined in Mexican cities between 2018 and 2022. • The decline differed significantly across regions • Apartments and restricted access measures decreased burglary victimization in some regions only. • Unobserved contextual factors contribute to regional variations in burglary trends. • Research is needed on how formal and informal social controls impact burglary risk. Objetivo: o investimento em ciência é essencial para a promoção do desenvolvimento econômico e do progresso científico e social de um país. O artigo analisa o financiamento de pesquisa sobre a ciência aberta sob uma perspectiva cientométrica, partindo de dados coletados em base especializada. Método: utilizou-se como fonte para coleta de dados a base Dimensions.ai, mantida pela empresa Digital Science & Research Solutions. Realizou-se consulta em janeiro de 2020, por meio da interface de acesso restrito da base (Dimensions Plus), utilizando-se busca parametrizada com o termo “open science”, restrita aos campos “títulos e resumos”, com aplicação da opção ou filtro de financiamento. Resultados: identificaram-se os percentuais de propostas financiadas e os valores investidos no período analisado, projetos de destaque no período – tanto em termos de duração do financiamento quanto em valores investidos –, valores médios investidos, principais países e disciplinas afetadas pelos investimentos. Entre os países com mais investimento, destacam-se os Estados Unidos da América, Reino Unido e a Alemanha e, quanto às áreas, temos as ciências da informação e computação e sistemas de informação, respectivamente. Conclusões: os dados explorados puderam ser relacionados com aspectos destacados na literatura, além de viabilizarem uma análise do progresso do tema ciência aberta, apontando para a sua consolidação como área de estudo passível de financiamento.

This paper addresses what it means to live with acutely restricted access to the city in the process of seeking urban asylum in post-apartheid South Africa. Our concept of apportionment specifies the gendered and racialised diminishment of space and time in the context of exclusionary and everyday violence. We focus on how the delineation and reduction of space and time is feminised, through the working lives of refugee and asylum-seeking women from the Democratic Republic of the Congo who live in Cape Town. Their embodied experiences incorporate the resonance of war in the Democratic Republic of the Congo, further sharpening their careful movements across Cape Town’s segregated geographies. Drawing on our conversations with non-governmental organisations and self-employed women over a nine-month period in 2020, we highlight how the deferral of refuge compounds precarity, significantly affecting women and those who are sexually minoritised. In connecting how state apportionment maps onto urban apportionment we reveal how an ecology of violence – of spatialised segregation, xenophobia and sexual violence – establishes a corporeal power that constrains access to the city. Crucially, these women deploy counter practices of apportionment and their precisely attuned navigations add to our understanding of the agile repertoires of working the city.

Context.— Acquiring objective, timely, and comprehensive feedback on resident diagnostic performance is notoriously difficult. Objective.— To implement a custom software application (Resident Case Tracker) to improve evaluative diagnostic analysis for residency programs. Design.— Residents and faculty use a graphical user interface with restricted access to their own cases and evaluations. For each sign-out, residents enter their diagnoses and comments for each case. Faculty are provided a sign-out queue to review the resident diagnosis and select their level of agreement alongside optional comments. After sign-out, residents can review the agreement level and comments for each case, overall sign-out statistics, and organ-specific performance, and they have the option of opening and reviewing groups of cases by agreement status. A sign-out evaluation is automatically generated and stored alongside additional reports. Administrative access allows privileged users to readily review data analytics at both an individual and residency-wide global level. Results.— A marked increase in completed evaluations and feedback was noted in the initial 36 months of implementation. During a 3-year academic period, faculty completed individual feedback on 33 685 cases and 1073 overall sign-out evaluations. Conclusions.— Resident Case Tracker is an invaluable tool for our residency program and has provided unparalleled feedback and data analytics. Throughout residency, trainees have access to each completed sign-out, with the ability to learn from discrepant cases while also monitoring improvements in diagnostic acumen over time. Faculty are able to assess resident milestones much more effectively while more readily identifying residents who would benefit from targeted study.

Unique restricted access media-magnetic molecularly imprinted polymers (RAM–MMIPs) were prepared as magnetic solid-phase extraction (MSPE) material for florfenicol (FF) and are combined with HPLC–UV to extract FF from fetal bovine serum samples. Under the optimization conditions, the resulting materials had a uniform imprinted layer, acceptable adsorption amounts (22.92 mg/g), rapid binding kinetics (80 min) and satisfactory selectivity. In addition, RAM–MMIPs presented satisfactory recoveries (95.6–102.1%), relative standard deviation (RSD) (2.7–3.9%) and lower limits of detection (LOD) (10.5 μg/L). The method was applied to the real samples. Compared with other reported methods, this method exhibits the advantages of simple, fast and, efficient operation, using a relatively small amount of the RAM–MMIPs adsorbent. The developed method can enrich FF from fetal bovine serum samples.

AIMS To demonstrate the magnitude of the potential problem caused by the trend towards using positron emission tomography prostate-specific membrane antigen scans on men in rural Australia. CONTEXT Prostate-specific membrane antigen positron emission tomography scans have higher sensitivity to detect metastatic prostate cancer than other imaging modalities, especially at lower prostate specific antigen (PSAs). This has led to proposals that prostate-specific membrane antigen be the gold standard to investigate men with a suspicion of prostate cancer. There is a trial underway in Australia examining the use of positron emission tomography prostate-specific membrane antigen in pre-biopsy men. Disparities in access to care and outcomes for prostate cancer for men in rural Australia are well documented. Incorporating positron emission tomography prostate-specific membrane antigen into the primary diagnostic pathway for prostate cancer creates a risk of further entrenching or worsening this inequity due to the lack of positron emission tomography scanners in rural locations. APPROACH As a surrogate marker for restricted access, we determined the proportion of men over 50 more than 1.5 hours from a positron emission tomography scanner, with local government areas as the unit of assessment. CONCLUSION Of the 505 local government areas, 309 were greater than 1.5 hours from the nearest positron emission tomography scanner. Of 3 793 865 men, over 50 585 689 lived in the 309 local government areas with restricted access to a positron emission tomography scanner. Our study highlights the risk of exacerbating inequalities in prostate cancer care experienced by men in rural areas. If positron emission tomography prostate-specific membrane antigen becomes the new gold standard, one in 6 men will face geographic barriers to access the standard of care. Future considerations of positron emission tomography prostate-specific membrane antigen must take this into account.

Homeless people are a socially disadvantaged group acutely affected by social exclusion and beset by various existential and health problems. Due to their life situation and the various kinds of stress associated with it, people in this group have higher somatic and psychological morbidity levels than the general population. At the same time, structural and individual barriers make it impossible for many to avail themselves of health services and treatment offers. Although it can be assumed that previous illnesses put the majority of those affected in the COVID risk group, the state of support for homeless people has deteriorated considerably in the course of the pandemic. In addition, restricted access to preventive measures such as disinfectants, protective masks and hygiene products plus the restrictions imposed in order to reduce social contacts have cut off many homeless people from low-threshold psychosocial support or medical services. The article provides an overview of the current state of support for homeless people in the context of the pandemic and outlines recommendations for improving it. Im Rahmen derCOVID-19-Pandemie hat sich die Versorgungssituation wohnungsloser Menschen durch die Masnahmen zur Pandemiebekampfung erheblich verschlechtert, wobei davon ausgegangen werden kann, dass ein Grosteil der Betroffenen wegen Vorerkrankungen zur Risikogruppe zahlt. Neben dem eingeschrankten Zugang zu praventiven Masnahmen wie Desinfektionsmitteln, Schutzmasken oder Hygienemitteln wurden viele Betroffene durch die Kontakteinschrankungen von niedrigschwelligen psychosozialen Hilfeangeboten oder medizinischen Angeboten abgeschnitten. Der Beitrag gibt einen Uberblick zur aktuellen Versorgungssituation von wohnungslosen Menschen wahrend der Pandemie und skizziert Empfehlungen fur eine Verbesserung der Versorgung. • Journal competition with endogenously choosing types and qualities is studied. • Journal competition helps to sustain high research quality of academic communities. • Coexistence of competing open and restricted access journals sustains higher quality. • Article processing charge impacts journals' decisions and research quality. Despite the recent moving towards open access trend in the academic community, both restricted and open access journals continue to coexist. New journals need to choose their journal types (open versus restricted access), while the incumbent journals may change their journal types. To better understand how the academic community is shaped by journals' choices of journal types, we constructed a game-theoretical model of journal competition with endogenous journal qualities and journal types. We found that journals' equilibrium quality and types vary by article processing charge (APC) and journals' preference for quality. Compared to the case that both journals are open access, a competition among journals of different types leads to higher journal quality standards chosen in equilibrium when APC is modest. Therefore, in the academic community where the research quality is measured by the highest quality of the journals therein, journals of different types guarantee a good degree of knowledge diffusion with a high quality. Resultados: As dificuldades reportadas foram: acesso limitado aos Equipamentos de Proteção Individual nos ambientes de trabalho; acesso restrito a testagem diagnóstica para COVID-19 e pouco conhecimento sobre a diferenciação e finalidade dos testes. Conclusão: A atuação da enfermagem aos profissionais da saúde na pandemia é de extrema importância. É relevante abordagens estratégicas de órgãos governamentais e de instituições de saúde acerca de capacitação para os profissionais da saúde e acesso aos equipamentos necessários para a saúde do trabalhador.

The restricted access window (RAW) based channel access scheme proposed for IEEE 802.11ah wireless LAN (WLAN) divides the stations (STAs) in the network into groups and allocates RAW slots for each group of STAs. In this way, the STAs belonging to each group can attempt to access the medium during their designated RAW slot by invoking the enhanced distributed channel access (EDCA) protocol. This paper describes an analytical model to evaluate the saturation throughput of IEEE 802.11ah WLAN under the RAW based scheme, when STAs carry traffic of distinct access categories (ACs) and use the EDCA protocol for medium sharing. However, when groups are formed by randomly selecting the STAs, i.e., if random grouping (RG) scheme is employed, each group will contain STAs carrying traffic of distinct ACs. This will lead to starvation and degraded throughput performance for the low priority class STAs, since they have to contend with high priority class STAs within the same group during the RAW slot assigned. To resolve this issue, we propose an algorithm to realize priority class based grouping (PCG), in which the STAs belonging to the same ACs are grouped together. We also describe an analytical model to evaluate the saturation throughput of the network under the proposed PCG scheme. We establish that saturation throughput of the network and the relative service differentiation ratio of the lower priority class STAs can be significantly improved under the proposed PCG scheme compared to the conventional RG scheme.

Context It is widely recognised that red foxes (Vulpes vulpes) are abundant within urban areas; however, it is difficult to apply lethal control measures using poison baits in cities because of concerns about the safety of domestic pets, particularly dogs (Canis familiaris). Aims We tested canid pest ejectors (CPEs) as a potential method of fox control by measuring visitation and activation behaviour of foxes and other wildlife while assessing non-target risk to domestic dogs. Methods We compared eight urban and eight peri-urban sites in Sydney, with half of the sites having restricted access for domestic dogs. We allocated five camera traps and ejectors per site. Through generalised linear mixed models, we compared the probability of ejector activation between foxes and dogs. We also assessed the relationship between dog visitation and distance to habitation and dog restrictions as measures of dog safety. This paper examines the regulatory gaps in the protection of athletes’ gender rights in sport. The regulation of human rights in sport is increasingly being scrutinised and at the same time, the societal parameters of sex and gender identity are shifting. Together, these current developments present a regulatory riddle when determining how to balance the interests of sport with the protection of non-conforming athletes’ rights, who may not strictly fit into the binary categories of sport. Despite a strong human rights framework protecting gender rights, the complex relationship between law and sport leaves athletes in a vulnerable position, with limited accountability of sports bodies and restricted access to a legal remedy. Drawing upon existing recommendations, this paper offers original insight for resolving the riddle, such as the establishment of an anti-discrimination in sport unit to enforce human rights in sport.

Since 2010, the author has been part of the Central State Hospital (CSH) Digital Library and Archives Project to digitize records from the first state psychiatric hospital for African Americans, founded in 1870 in Virginia at the pleadings of the Freedman's Bureau and run by the state since then.1 Many of the records of this hospital not yet accessioned by the Library of Virginia have now been digitized, and this project is working on a set of tools for lawful access, including one that can be used for automated redaction to protect sensitive data while responding to the needs of different stakeholder groups. Project participants were especially concerned about understanding the communities that have grown up around state-run psychiatric hospitals, as the project was done at the request of the hospital. The proposed plan is to work with the Central State Hospital and the Library of Virginia to provide the project materials to both. The records that were chosen to be digitized included the minutes of the people who first ran the hospital as well as the registers kept on the patients, which differ over time.2 In the past ten to fifteen years, professional discussion about community archives has responded to communities' desires to build their own archives so that they can be treated fairly, especially with reference to records created about them and kept by others, including records found in state archives.

27. COVID-19 Lockdown in Portugal: Challenges, Strategies and Effects on Mental Health

Identified in China at the end of 2019, the COVID-19 infection has increased exponentially worldwide. Together with the threat of physical illness, this pandemic brought negative impacts on mental health. Additionally, the pandemic also interfered with other life domains: closings of schools and companies, restricted access to public places, changes in work and home routines and increased uncertainty due to the economic repercussions. This study examines people’s experiences and mental health in the 6th consecutive lockdown week in Portugal and compares with the pre-confinement population’s mental health. Specifically, we explore the difficulties experienced by the general population while in lockdown, their strategies to cope with it, and its impact on mental health. One thousand five hundred and two participants undergoing the 6th lockdown week answered a web-based cross-sectional survey. Demographic information, living conditions, confinement-related difficulties or challenges, strategies to cope with lockdown and mental health dimensions were assessed. Some difficult confined related experiences and sociodemographic variables were associated with poor mental health. When compared to our findings, the general population’ mental health was better before the pandemic. Lockdown challenges experienced by participants and their strategies to cope with confinement predicted 10 to 42% of the variance in mental health dimensions. This study identified several mental health-protective and risk factors related to long-term compulsory confinement. Individual strategies to prevent mental health problems are discussed. Additionally, our findings may inform national health agencies and governments on measures to enhance mental health among the quarantined general population.

Objetivou-se avaliar a literatura existente sobre a abordagem terapêutica da insônia no âmbito da atenção primária à saúde. Foram pesquisados estudos originais, através dos descritores insônia, atenção primária à saúde e tratamento, em português e inglês, nas bases de dados PUBMED e Biblioteca Virtual em Saúde (BVS). Foram incluídos os artigos publicados entre 2017 e 2020, relacionados com o tratamento da insônia e suas repercussões na saúde pública, e foram excluídas pesquisas documentais ou de acesso restrito. O tratamento da insônia, abrange medidas não farmacológicas e farmacológicas. A terapia cognitivo-comportamental é considerada como tratamento padrão-ouro, com benefício a longo prazo, atuando em diversas etapas do sono. A farmacoterapia deve ser considerada em situações bem indicadas e em subgrupos especiais. Considerando-se a prevalência e repercussões clínicas da insônia, evidencia-se a importância do médico que atua na atenção primária à saúde, em dominar o manejo terapêutico da insônia.

Open Access versus Restricted Access in a General Equilibrium with Mobile Capital We consider an economy with two sectors, resource and manufacturing, in a general-equilibrium setting. Two property regimes in the resource sector are compared, open access versus restricted access, with both labor and capital mobility. We first contribute by deriving the multi-factor demand conditions under open access. We provide necessary and sufficient conditions for a factor to gain from a property regime change. Redistributive effects depend crucially on relative factor intensities. Contrary to common wisdom, labor may gain from being “expelled” from the resource sector following privatization.

BACKGROUND Fragile X syndrome (FXS) is the most common cause of inherited intellectual disability and autism spectrum disorder (ASD). In Colombia, there are no screening or testing protocols established for the diagnosis of FXS. In this study, we aimed to describe the diagnostic trends of FXS in Colombia. METHODS Data were included on 1322 individuals obtained based on data from the only 2 databases available. Sociodemographic information and data related to the diagnostic process were obtained and included in this study. RESULTS The average age at the time of diagnosis for individuals with the full mutation (FM) was of 26.9 ± 2.57 years and was strongly dependent on sex and socioeconomic status. Most individuals with a molecular diagnosis were from the main cities. CONCLUSION The overall age of diagnosis of FXS is later in life than reports from other countries. Restricted access to molecular testing through the national health system might explain this discrepancy in Colombia.

31. Advanced airway interventions in the trapped (racing) driver: a randomised cross-over trial.

Casualty entrapment occurs in 12-38.6% of road traffic incidents attended by advanced trauma teams. Half of those occupants sustaining severe trauma require advanced airway management. Suboptimal positioning and environmental restrictions make this challenging, and low success rates are described. This study uses a race car training chassis to increase scenario fidelity, and investigates airway management devices suitable for pre-hospital use. Participants (n = 72) were recruited from pre-hospital, emergency department and anaesthesia services in a teaching hospital in Singapore and stratified by experience in advanced airway management. Two supraglottic devices (Supreme Laryngeal Mask Airway, Laryngeal Tube) and three tracheal intubation devices (Airtraq, McGrath MAC, McGrath X-Blade) were tested in a simulated sitting trapped driver scenario. The primary outcome was airway intervention success (%) which was defined by the ability to place an airway and inflate the lungs of a mid-fidelity mannequin. Secondary outcomes were time to airway management, number of oesophageal intubations and perceived difficulty. Both inexperienced (n = 36) and experienced (n = 36) operators achieved 100% success rates with the supraglottic devices. The inexperienced group demonstrated the greatest success using the McGrath X-Blade (94.4%), followed by the McGrath MAC (91.6%) and the Airtraq (80.6%). The experienced group found the McGrath MAC (94.4%) the most successful, followed by the Airtraq (91.6%) and the McGrath X-Blade (77.8%). Where tracheal intubation was attempted, 12% resulted in at least one oesophageal intubation. The majority were associated with the Airtraq device (71.0%) and the inexperienced operator (78.9%). This study suggests that a supraglottic device, rather than primary intubation, should be used when the driver is trapped in the seat. If a definitive airway is required, the McGrath videolaryngoscopes are associated with a greater success rate and reduce incidence of oesophageal intubation compared with the Airtraq. Fixed radius curve blades, such as the McGrath X-Blade, may be preferred by inexperienced operators in this situation. Variable radius curve blades, such as the McGrath MAC blade, may be the more successful option when used by operators experienced in their use. [ABSTRACT FROM AUTHOR]

Systematic reviews and maps should be based on the best available evidence, and reviewers should make all reasonable efforts to source and include potentially relevant studies. However, reviewers may not be able to consider all existing evidence, since some data and studies may not be publicly available. Including non - public studies in reviews provides a valuable opportunity to increase systematic review/map comprehensiveness, potentially mitigating negative impacts of publication bias. Studies may be non-public for many reasons: some may still be in the process of being published (publication can take a long time); some may not be published due to author/publisher restrictions; publication bias may make it difficult to publish non-significant or negative results. Here, we consider what forms these non-public studies may take and the implications of including them in systematic reviews and maps. Reviewers should carefully consider the advantages and disadvantages of including non-public studies, weighing risks of bias against benefits of increased comprehensiveness. As with all systematic reviews and maps, reviewers must be transparent about methods used to obtain data and avoid risks of bias in their synthesis. 34. Medicinal plants used for diarrhoea treatment in sheep and goats among smallholders in farm settlements of Ogun state, Nigeria

The use of traditional remedies in the treatment of diarrhoea in ruminants is a common form of therapy among poor resource smallholder farmers in Nigeria with restricted access to veterinary healthcare services. The medicinal plants of antidiarrheal importance in smallholder sheep and goat production systems were studied in farm settlements namely Ado-Odo, Ikenne and Ibiade, located in Ogun state, Nigeria. Techniques including interviews, observations and guided field walks were used to collect data from 150 farmers across the farm settlements. Results showed that all respondents owned either sheep or goats with goats more preferred, while diarrhea was also observed to be a major disease problem in goats compared to sheep. About 92% of the farmers admitted using anti-diarrhoea herbs and about 80% agreed to their effectiveness in the treatment of diarrhoea in sheep and goats. The study identified 11 plant species used to treat diarrhoea with varied recipe and the route of administration was orally. Less than 12% of these plant parts were administered solely, preparing as a single-component while the majority of the identified plants were used in combination with other plant parts. Phytochemical components were analyzed for tannin, saponin, flavonoid and alkaloid which were observed in different plant parts showing their potential as antidiarrheal agents.The usage of these plants could therefore be an indispensable component of traditional medicine practice which can serve as safe alternative and complementary medicines to various allopathic drugs that could help safeguard health and obtain optimum production from animals.

The article focuses on archaeolinguistics as a separate field of knowledge and outlines the features that distinguish it from other disciplines in comparative studies. It analyses the existing text collections and shows how they may find application in a corpus-based research in ancient languages. It also discusses approaches to creating new corpora of texts. The study focuses on Old Church Slavonic and Ancient Greek, in particular, it analyses the existing corpora in these languages, e. g., Corpus Cyrillo-Methodianum Helsingiense. Most of the corpora under study are not tagged. Some of them change the original writing system (from Glagolitic to Latin, using, for instance, ASCII), while the others have a restricted access. Some of the corpora are no longer available at all or available as part of local databases only. Thus, corpus-based resources in ancient languages in question are obviously insufficient. To facilitate more effective research, the easiest possible solution is to develop new corpora by using platforms specializing in linguistic analysis (e. g., CDLI or Lingvodoc) or systems that support DIY corpora. However, such platforms are often paywalled, may have limited functionality, or lack comprehensive user guides. With all the above in mind, there seems to be no ready solution for archaeolinguists who want to use a corpus-based approach in their study. They either have to make a considerable effort to modify an existing system for their purposes, or to build one of their own. In conclusion, the article proposes one of the possible ways to address these issues.

Esse artigo tem como objetivo apresentar uma discussão acerca dos desafios para a construção e implementação de políticas públicas de lazer para a população de Lésbicas, Gays, Bissexuais, Travestis, Transexuais e Interssexuais (LGBTTI), no Brasil. A partir da análise de diferentes produções científicas a respeito do tema é possível constatar que, apesar das conquistas do movimento LGBTTI, ainda há muito a avançar quando se trata de políticas públicas de lazer. Esse público ainda encontra-se marginalizado perante o Estado e a sociedade, refletindo na incipiência dos espaços de lazer os quais, quando existentes, são privados e de acesso restrito a grande parte da população.

Devido ao distanciamento social adotado em todo o mundo, a fim de amenizar os efeitos da pandemia da COVID-19, houve a necessidade de adequação do modo de vida de todas as pessoas, inclusive nas instituições de ensino, sejam da educação básica ou do ensino superior. Nesse sentido, esta pesquisa tem por objetivo investigar as concepções de professores universitários sobre as atividades experimentais e as dificuldades enfrentadas por eles para o seu desenvolvimento durante o Ensino Remoto Emergencial (ERE). Para tanto, durante os meses de maio a julho de 2020, foram elaborados dois questionários on-line, que abordaram questões relacionadas às atividades experimentais e ao ERE, e encaminhados aos professores responsáveis por ministrar disciplinas experimentais do curso de Química de uma universidade mineira. Através dos resultados, evidenciou-se que a maioria dos professores apontaram concepções muito simplistas sobre as atividades experimentais, ainda centradas na manipulação de vidrarias e comprovação de teorias. Ainda, eles mencionaram que as maiores dificuldades enfrentadas no ERE são o acesso restrito à internet, a alta demanda de atividades nesse período e a dificuldade que os estudantes apresentaram em desenvolver a autonomia no processo de ensino e aprendizagem.

Gender equality is a situation for women and men to take the same status, in order to fully realize their human right. It has the same potential both men and women regarding to the all aspects in their life, including for fisherwomen. This study aims to (1 ) describe the social culture of the coastal communities of Ponjalae where the fisherwomen lives and (2) identify gender equality and the role of women to get education opportunity. This is a case study research where the informants are the fisherwomen in the coastal communities of Ponjalae, Palopo. Interactive data analysis are performed in this research. Findings indicate that fisherwomen and their children have restricted access to get education related to economic difficulties. In line with the previous finding , there is a misleading social stigma to the proper position of women at home. Based on the results of this study, it could recommend for government through their policies which could improve the opportunity for getting an education, especially for fisherwomen to promote gender equality.

Purpose of review The application of robotic surgery to anterior skull base disease has yet to be defined despite the potential for improved tumour resection with less morbidity in this region. Complex anatomy and restricted access have limited the development of robotic anterior skull base surgery. Recent findings A limited number of transoral robotic surgical anterior skull base procedures have been undertaken; however, there are significant limitations to the utilization of this technology in the anterior skull base. In this article, the advantages, disadvantages and limitations of robotic anterior skull base surgery are discussed. Currently, the major limitation is the size of the robotic endoscope and of the available instrumentation. Technological advancements that provide promise for the future development of robotic anterior skull base surgery are in development, such as single-port robots, flexible instrument systems and miniaturization and growth of minimally invasive platforms. Summary Although transnasal access to the skull base is not possible with the currently available robotic systems, promising technology does exist and is in development. Robotic anterior skull base surgery promises to provide greater access to skull base disease, improve oncologic results, reduce morbidity and to reduce the ergonomic burden on the surgeon.

This article presents estimates of damage and lifetime of welded 09G2S structures upon operation under severe conditions of the Arctic and subarctic regions. Accumulation of damage in steel subjected to low temperature ductile–brittle transition has been estimated by measurement of impact toughness in samples with V notch in the corresponding temperature range. On the basis of experimental data, it has been established that the decrease in temperature leads to significant loss of plasticity in the heat-affected zone, where stresses are localized and accelerated growth of cracks and microdefects takes place. As a consequence, the lifetime of steel structure decreases. On the basis of the Kachanov–Rabotnov theory of damage accumulation, a procedure has been proposed for estimating the integral damage of steel of a welded structure subjected to ductile–brittle transition depending on lifetime under certain climatic conditions. Comparison of estimates of accumulated damage in material of structures operating under extreme conditions of Yakutia and under moderate conditions of Krasnoyarsk krai (Russia) makes it possible to conclude that the mean time between failures is determined by climatic conditions. The developed procedure for estimating damage accumulation in the temperature range of ductile–brittle transition would make it possible to decrease the scope of required tests without loss of data reliability. In addition, on its basis, it is possible to diagnose all dangerous industrial objects with restricted access.

The coronavirus disease 2019 (COVID-19) pandemic has affected people from every part of the world, with the elderly and minority groups being at a higher risk owing to the high prevalence of underlying health conditions such as atherosclerosis among these groups. This study discusses numerous implications of the COVID-19 pandemic on people consuming more unhealthy foods due to restricted access to stores and stores closing early. A lot of people currently live a sedentary lifestyle because of the strenuous quarantine measures. COVID-19 impacts the nutritional status of people leaving them at risk of atherosclerosis and worsens the health condition of individuals suffering from it. Also, the use of COVID-19 medications has been observed to have side effects in patients with atherosclerosis. Thus, we review the nutritional impact of COVID-19 and its implications on atherosclerosis at length and make recommendations for reducing future implications. This study would serve as a foundation for further research on COVID-19 and its implications on atherosclerosis as only a limited number of studies have addressed it.

This comparative analysis of Latina immigrant mothers’ experiences in “Squire Town,” a farmworker community in Northern California, and Williamsburg, a recent Latina/o destination in Eastern Virginia, employs a gender lens to analyze how these women make claims to place, belonging, and inclusion through their sustained individual and collective struggles to ensure the well-being of their families. In Williamsburg they created attachment to place through resilience in facing restricted access to services and housing and overcoming fear to navigate unfamiliar physical and institutional settings. Mothers in Squire Town made politicized claims to inclusion by advocating for services in their community. Women in both sites developed support networks to assist them in obtaining or collectively demanding resources and services for their families. These mothers’ responses to exclusionary conditions in these two contexts reveal gendered, place-specific dimensions of belonging and women’s contribution to its production.

A new and simple micelles-rich restricted access supramolecular solvent-based liquid phase microextraction method (RASUPRASs-LPME) based on the ion-pair complex formation of phosphate (PO4 3-) ions with ammonium heptamolybdate and malachite green in acidic medium was developed. The phosphate ion concentration after microextraction of the ion-pair complex to the hexagonal aggregates of decanoic acid (DA) was measured with micro-volume UV-Vis spectrophotometer at 625 nm. All analytical parameters which are effective on the method such as acid type and concentration, supramolecular solvent volume, amount of the components forming the complex, sample volume, were optimized. The preconcentration factor (PF), limit of detection (LOD) and limit of quantification (LOQ) for the developed method was found to be 15, 9.6 and 32.1, respectively. The RA-SUPRAs-LPME method was finally applied for the analysis of the phosphate content of different types of water samples.

Purpose. To analyze domestic and foreign scientific publications over the past 5 years, reflecting the issues of accommodation and accommodation disorders. To assess the presence or absence of changes in the classification of accommodation disorders. To evaluate the role of various accommodation disorders in the development and progression of myopia. Relevance . Given the significant increase in the number of people with myopia in recent decades, interest in the study of accommodation and accommodation disorders has increased again. This is due not only to the fact that accommodative insufficiency (AI, weak accommodation), is considered one of the leading factors in the development of myopia, but also because the use of one or another method of myopia control affects the process of accommodation, increasing or reducing accommodation reserves, affecting the functioning of the binocular system. New developments in the field of visualization of intraocular structures (anterior segment imaging by OCT, Sheimpflug imaging, UBM) allows carry out previously impossible studies of changes in the crystalline lens, ciliary body, at different levels of accommodation response, in different age groups in vivo, which itself affects the development and testing of new concepts related to the mechanism of the accommodation. Thus, the dynamics of the changes that are taking place motivates ophthalmologists to take a fresh look at the already established concepts of the accommodation mechanism, to assess the consistency of previously adopted approaches to the classification of accommodation disorders, which in turn can affect changes in the tactics of myopia control, which is especially crucial as myopization of the population, especially in children, is accelerating every day. Our task was to evaluate domestic and foreign scientific publications over the past 5 years, reflecting the results of studies of the accommodation mechanism, assessing the influence of accommodation disorders on the progression of myopia. The task was also to assess the prospect of creating a unified classification of accommodative disturbances, the ability to create a unified algorithm for controlling progression, based on accommodation parameters. Search strategy. A review of English and Russian sources related to the study of the mechanism of accommodation was conducted; the effect of accommodation disorders on the progression of myopia and their classification was studied using the following databases: PubMed, Cyberleninka, Google scholar, Elibrary. The search depth was 5 years (20142019), excluding the historical sources. The sources included: – original research; – historical issue, scanned publications; – studies / descriptions of accommodation disorders and their relationship with myopia; – abstracts and full-text publications; – literary reviews; – monographies. The exclusion criteria were: – themed issues with closed or restricted access. During the process of searching and processing information, the data obtained were not systematized according to the level of reliability due to the fact that the purpose of the review is to obtain a general idea of the views on this issue, assess the presence or absence of unity in the formation of diagnostic criteria, without the task of suggesting practical recommendations.
