Respiratory diseases are leading causes of death and disability globally, with about 65 million people suffering from COPD, and 334 million from asthma, the most common chronic disease. Each year, tens of millions of people develop and can die from from respiratory infections such as pneumonia and TB. Systemic inflammation may induce and exacerbate local inflammatory diseases in the lungs, and local inflammation can in turn cause systemic inflammation. There is increasing evidence of the coexistence of systemic and local inflammation in patients suffering from asthma, COPD, and other lung diseases, and the co-morbidity of two or more local inflammatory diseases often occurs. For example, rheumatoid arthritis frequently occurs together with, and promotes the development of, pulmonary hypertension. This co-morbidity significantly impacts quality of life, and can result in death for those affected.Current treatment options for lung disease are neither effective, nor condition-specific; there is a desperate need for novel therapeutics in the field. This monograph sheds new light on pulmonary sensory receptors. Diving into the pulmonary microenvironment, the book focuses on the role of pulmonary neuroepithelial bodies (NEBs) as potential receptors and effectors, able to store and release neurotransmitters. It explores NEBs as potential stem cell niche and highlights the multidisciplinary approach taken to identify and study NEBs, including functional morphological investigation, live cell imaging, genetic models, and laser microdissection combined with gene expression analysis.Complexly organized NEBs are an integral part of the intrapulmonary airway epithelium of all air-breathing vertebrates. For decades a quest has been going on to unravel the functional significance of these intriguing structures that appear to be modified in the course of many pulmonary diseases. The NEB microenvironment (ME) is composed of organoid clusters of pulmonary neuroendocrine cells (PNECs) that are able to store and release neurotransmitters and are closely contacted by extensive nerve terminals, emphasizing a potential receptor/effector role and probable signalling to the central nervous system. PNECs are largely shielded from the airway lumen by a special type of Clara cells, the Clara-like cells, with potential stem cell characteristics. So far, functional studies of the pulmonary NEB ME revealed that PNECs can be activated by various mechanical and chemical stimuli, resulting in a calcium-mediated release of neurotransmitters. In the past decades, a number of publications have exposed NEBs as potential hypoxia sensors. Recent experimental evidence unveiled that the NEB ME is a quiescent stem cell niche in healthy postnatal lungs, and silencing may involve bone morphogenetic protein signalling mediated by vagal afferents. Only an integrated approach that takes all current information into account will be able to explain the full role of the pulmonary NEB ME in health and disease. Interstitial lung disease is a group of diffuse parenchymal lung disorders affecting the interstitium, a part of the lung's anatomic structure, causing inflammation, scarring, or excess fluid. Die Situation der Tuberkulosetherapie ließ sich bis vor kurzem für die zentral­ europäischen Länder mit wenigen Worten umschreiben: die Chemotherapie hat in den ersten Jahren einen großen Fortschritt gebracht, einen entscheidenden Rückgang der Mortalität, einen wesentlichen Rückgang der Morbidität, eine bemerkenswerte Ver­ schiebung der Erstinfektion und der Durchseuchung. - In den sechziger Jahren kam es dann zu einem Stillstand, zu einem Einspielen auf einem Niveau, auf das man mit Stolz hinwies, das aber noch genug unerfreulich war. Dieser Stillstand äußerte sich etwa in den Schlagzeilen der großen Übersichtsreferate:'Stagnation der Tuberkulose­ bekämpfung'(Göttsching),'Halbzeit in der Tuberkulosebekämpfung'(Haefliger). Hier hat sich nun etwas geändert: Die Entdeckung von neuen, höchst wirksamen, wenig toxischen, oral anwendbaren Heilmitteln, so Ethambutol und insbesondere Rifampicin, hat offensichtlich eine neue Situation geschaffen. Mit diesen Mitteln scheint es zu gelingen, die Stagnation zu überwinden und die zweite Halbzeit des Kampfes gegen die Tuberkulose mit Aussicht auf neue und bessere Erfolge zu begin­ nen. Mit besseren Aussichten, weil jetzt wirksamere Kombinationsmöglichkeiten die Ersttuberkulose wesentlich sicherer zur Ausheilung bringen (siehe Freerksen im vor­ liegenden Tagungsbericht:'Die Einführung des Rifampicin hat die Tuberkulose­ therapie auf eine neue Ebene gebracht. Sie wird, richtig angewandt, dazu führen, daß wir in Zukunft praktisch keine Therapieversager mehr haben.'). - Mit besseren Aussichten, weil die neue Therapie auch neue Möglichkeiten bietet, chronische Fälle zu sanieren.
