Item does not contain fulltext, About one-third of patients undergoing transcatheter aortic valve implantation (TAVI) use oral anticoagulants (OAC), mainly due to atrial fibrillation. General guidelines advise interrupting OAC in patients with a high risk of bleeding undergoing interventions. However, preliminary observational data suggest that the continuation of OAC during TAVI is safe and may reduce the risk of periprocedural thromboembolic events. The Periprocedural Continuation Versus Interruption of Oral Anticoagulant Drugs During Transcatheter Aortic Valve Implantation (POPular PAUSE TAVI) is a multicentre, randomised clinical trial with open-label treatment and blinded endpoint assessment. Patients are randomised 1:1 to periprocedural continuation versus interruption of OAC and are stratified for vitamin K antagonist or direct oral anticoagulant use. The primary endpoint is a composite of cardiovascular mortality, all stroke, myocardial infarction, major vascular complications and type 2-4 bleeding within 30 days after TAVI, according to the Valve Academic Research Consortium-3 criteria. Secondary endpoints include separate individual and composite outcomes, quality of life and cost-effectiveness. Since continuation of OAC is associated with the ancillary benefit that it simplifies periprocedural management, the primary outcome is first analysed for non-inferiority; if non-inferiority is proven, superiority will be tested. Recruitment started in November 2020, and the trial will continue until a total of 858 patients have been included and followed for 90 days. In summary, POPular PAUSE TAVI is the first randomised clinical trial to assess the safety and efficacy of periprocedural continuation versus interruption of OAC in patients undergoing TAVI.

Item does not contain fulltext, BACKGROUND: Percutaneous renal denervation (RDN) has recently been introduced as a treatment for therapyresistant hypertension. Also, it has been suggested that RDN may be beneficial for other conditions characterised by increased sympathetic nerve activity. There are still many uncertainties with regard to efficacy, safety, predictors for success and long-term effects. To answer these important questions, we initiated a Dutch RDN registry aiming to collect data from all RDN procedures performed in the Netherlands. METHODS: The Dutch RDN registry is an ongoing investigator-initiated, prospective, multicentre cohort study. Twenty-six Dutch hospitals agreed to participate in this registry. All patients who undergo RDN, regardless of the clinical indication or device that is used, will be included. Data are currently being collected on eligibility and screening, treatment and follow-up. RESULTS: Procedures have been performed since August 2010. At present, data from 306 patients have been entered into the database. The main indication for RDN was hypertension (n = 302, 99%). Patients had a mean office blood pressure of 177/100 (±29/16) mmHg with a median use of three (range 0-8) blood pressure lowering drugs. Mean 24-hour blood pressure before RDN was 157/93 (±18/13) mmHg. RDN was performed with different devices, with the Simplicity™ catheter currently used most frequently. CONCLUSION: Here we report on the rationale and design of the Dutch RDN registry. Enrolment in this investigator-initiated study is ongoing. We present baseline characteristics of the first 306 participants.

6. Reduced CD26 expression is associated with improved cardiac function after acute myocardial infarction: insights from the REPERATOR study

Abstract

Peripheral blood mononuclear cells (MNC) enhance cardiac recovery and repair after myocardial infarction (MI). The SDF-1α/CXCR4 axis plays a major role in cell homing to infarcted myocardium and is negatively regulated by CD26. Therefore, we studied the expression of CD26 during MI and its effects on cardiac function. Blood samples from forty-two patients who underwent a primary percutaneous coronary intervention (PCI) for a first ST-elevated MI were collected during primary PCI, 1 week and 3 months after MI. Soluble CD26 (sCD26) and membrane bound CD26 expression on MNCs (mncCD26) were determined. Left ventricular function and infarct size were measured within 1 day, 1 week and 3 months follow up by magnetic resonance imaging. One week post MI, sCD26 was down regulated compared to baseline, while mncCD26 was higher at baseline and 1 week compared to 3 months. Increased mncCD26 expression at 1 week after MI was associated with decreased overall recovery of left ventricular function as measured by left ventricular end systolic volume index. Furthermore, the in vitro migration capacity of MNCs to SDF-1α was decreased 1 week post MI and the migration capacity to SDF-1α was negatively correlated with mncCD26 expression. CD26 inhibition with sitagliptin - a drug currently used in diabetic patients - resulted in improved in vitro migration capacities of MNCs. In conclusion, our preliminary results suggest that high cellular CD26 expression decreases the migration of MNCs towards SDF-1α and high cellular CD26 expression negatively influences cardiac function post MI. Treating patients shortly post MI with sitagliptin to inhibit CD26 may therefore increase MNC homing to the infarct area and could improve cardiac recovery and repair.

BACKGROUND: Given the devastating consequences of ST, efforts should be directed toward risk stratification to identify patients at highest risk for ST. METHODS: Consecutive patients with angiographic ST were enrolled. Patients who did not suffer from a ST were randomly selected in a 2:1 ratio and were matched for: 1) percutaneous coronary intervention (PCI) indication; 2) same date of index PCI; and 3) same interventional center. METHODS : Sixty STEMI patients were randomised to abciximab, to high-dose tirofiban or to no additional GP IIb/IIIa inhibitor treatment. Platelet activation (P-selectin expression) was measured using flow cytometry and the level of inhibition of platelet aggregation was assessed using the Plateletworks assay. Additionally, the PFA-100 with the collagen/adenosine-diphosphate cartridge (CADP) was used to compare the levels of platelet inhibition. All measurements were performed at baseline (T(0)), immediately after (T(1)), 30 minutes (T(2)), 60 minutes (T(3)) and 120 minutes (T(4)) after primary PCI. RESULTS : The level of platelet activation in both GP IIb/IIIa receptor inhibitor treated groups was significantly lower compared with the control group at all time points after primary PCI (p=0.04). Abstract

The REPARATOR study is a multicentre clinical trial in which the effect of 80 mg atorvastatin on microvascular (re)perfusion and late ventricular remodelling, and infarct size in patients presenting with an acute ST-elevation myocardial infarction is studied. Primary endpoint is end-systolic volume index at three months measured by quantitative cine magnetic resonance imaging (MRI). Secondary endpoints are cardiac MI (CMR) measurements of global and regional left ventricular function, MRI measurements of infarct size on admission, one week and three months as well as changes between MRI investigations, biochemical markers of infarct size, blush grade, and TIMI frame count. A total of 50 patients will be enrolled. Including three months follow-up, the study will last for six months.

Contains fulltext : 79950.pdf (publisher's version ) (Closed access), BACKGROUND: There are limited data on the long-term clinical outcome after an angiographically confirmed (definite) stent thrombosis (ST). METHODS AND RESULTS: Four hundred thirty-one consecutive patients with a definite ST were enrolled in this multicenter registry. The primary end point was the composite of cardiac death and definite recurrent ST. Secondary end points were all-cause death, cardiac death, definite recurrent ST, definite and probable recurrent ST, any myocardial infarction, and any target-vessel revascularization. The primary end point occurred in 111 patients after a median follow-up of 27.1 months. The estimated cumulative event rates at 30 days and 1, 2, and 3 years were 18.0%, 23.6%, 25.2%, and 27.9%, respectively. The cumulative incidence rates of definite recurrent ST, definite or probable recurrent ST, any myocardial infarction, and any target-vessel revascularization were 18.8%, 20.1%, 21.3%, and 32.0%, respectively, at the longest available follow-up. Independent predictors for the primary end point were diabetes mellitus, total stent length, severe calcification, American College of Cardiology/American Heart Association B2-C lesions, TIMI (Thrombolysis In Myocardial Infarction) flow grade <3 after percutaneous coronary intervention, and left ventricular ejection fraction <45%. The implantation of an additional coronary stent during the first ST was also associated with unfavorable outcome. Clinical outcome was not affected by the type of previously implanted stent (drug-eluting or bare-metal stent) or the category of ST (early versus late). CONCLUSIONS: The long-term clinical outcome after a first definite ST is unfavorable, with a high mortality and recurrence rate. Abstract

In pulmonary vein isolation as a treatment for atrial fibrillation the proximal part of the pulmonary veins is catheterized. A protocol for preinterventional assessment of pulmonary vein anatomy was developed, based on contrast-enhanced magnetic resonance angiography (MRA) in combination with three-dimensional visualization to tailor periprocedural angiography. Abstract

Aims Coronary stenting is limited by a 10%–60% restenosis rate due to neointimal hyperplasia. Sirolimus is a macrocyclic lactone agent that interacts with cell-cycle regulating proteins and inhibits cell division between phases G1 and S1. The hypothesis tested in this study is that local delivery of sirolimus with an eluting stent can prevent restenosis.Methods and Results Fifteen patients were treated with 18mm sirolimus eluting BX VELOCITY™ stents. Quantitative angiography and three-dimensional quantitative intravascular ultrasound were performed at implantation and at the 6 months follow-up. All stent implantations were successful. One patient died on day 2, of cerebral haemorrhage and one patient suffered a subacute stent occlusion due to edge dissection (re-PTCA, CKMB 42). At 9 months no further adverse events had occurred and all patients were angina free. Quantitative coronary angiography revealed no change in minimal lumen diameter and percent diameter stenosis and hence no in-lesion or in-stent restenosis. Quantitative intravascular ultrasound showed that intimal hyperplasia volume and percent obstruction volume at follow-up were negligible at 5·3mm3and 1·8%, respectively. No edge effect was observed in the segments proximal and distal to the stents.Conclusion Implantation of a sirolimus-eluting stent seems to effectively prevent intimal hyperplasia. 22. Diagnostic merits of exercise testing in females.

Abstract

The diagnostic accuracy of conventional exercise electrocardiography was compared with exercise algorithms specifically aimed at application in females, with R wave and heart rate adjusted ST-segment changes, and with discriminant analysis. A symptom-limited bicycle exercise test was performed by 189 females without previous infarction with a normal electrocardiogram (ECG) at rest. Frank lead ECG was computer processed. ST-segment amplitudes in lead X were superior to measurements in lead Y, but their sensitivity amounted only to 50% at a specificity of 90%. Correction for R wave amplitude marginally increased sensitivity. It was confirmed that leads with inferiorly directed vectors such as standard leads II, III, a VF or Frank lead Y should not be used for diagnostic purposes in females. Exercise-induced increases in R-wave amplitude of at least 01 mV occurred in only 8% of females with coronary disease. Previously described discriminant function using attained heart rate, ST-segment depression and a sex-dependent ST segment correction factor yielded a sensitivity of 60% at a specificity of 90%. Sensitivities of heart rate adjusted ST-segment changes and of a previously proposed discriminant function were greater than 70% at a specificity of 90%. It is concluded that the diagnostic yield of exercise testing in females can be improved by use of more sophisticated electrocardio-graphic and exercise variables. [ABSTRACT FROM PUBLISHER]

Abstract

Recently a new noninvasive imaging technique, magnetic resonance imaging (MRI) has been developed that has the potential to assess the coronary arteries. MRI of the coronary arteries is a challenging task because of the motion of the vessels during cardiac contraction and the motion of the heart with respiration. Several two-dimensional and three-dimensional acquisition techniques have been developed to overcome these problems. In this article we will describe different conventional MR techniques such as spin-echo and gradient-echo imaging. Also, we will describe new developments in MRI as ultrafast breathhold techniques using echo planar imaging or targeted volume scanning. Other new developments are respiratory gating techniques with or without respiratory motion correction. Finally, we will review the results of these techniques in the detection of coronary artery bypass graft patency, coronary artery stenosis, and the evaluation of coronary artery anomalies. Abstract

Magnetic resonance imaging (MRI) is a noninvasive imaging technique that is becoming more and more important in clinical cardiology. Physicians must understand the basic principles of MRI before reliable use in practice is possible. Therefore, we will give an introduction to basic MRI principles necessary to understand the difficulties of cardiac MRI. First the generation of a signal by the combination of a strong magnetic field, radiofrequency pulses, and temporary changes in the magnetic field is explained. Then, the processes of localization of different points in an image, resolution, and signal-to-noise ratio are highlighted. Abstract

Intravenous coronary angiography with electron beam computed tomography (EBCT) allows for the noninvasive visualisation of coronary arteries. With dedicated computer hardware and software, three-dimensional renderings of the coronary arteries, veins, and other cardiac structures can be constructed from the individual transaxial tomograms. Interest in this technique is growing, and recently a number of clinical studies have been published comparing EBCT coronary angiography with conventional cine-coronary angiography. In this article, image acquisition, postprocessing techniques, and the results of recently published clinical studies are discussed. EBCT coronary angiography is a promising imaging technique of coronary arteries. Currently, it is a reasonably robust technique for the visualization and assessment of the left main and left anterior descending coronary artery. However, at the moment a relatively high proportion of the right and circumflex coronary angiograms are noninterpretable. Improvements in image acquisition and postprocessing techniques are expected to improve visualization and diagnostic accuracy of the technique. Copyright (C) 2000 by W.B. 27. Peripheral blood derived cells and angiogenesis in cardiovascular disease

Abstract

Patients suffering from myocardial infarction (MI), atherosclerosis and Hereditary Hemorrhagic Telangiectasia type 1 (HHT-1) all have diseased and dysfunctional blood vessels. Cardiovascular repair in these diseases occurs not only locally, but also peripheral blood (progenitor) cells and cytokines/growth factors positively contribute to repair of malfunctioning tissue. In this thesis several aspects of cardiovascular repair have been explored. First, we show that in MI patients relatively large infarctions or unfavorable hemodynamic conditions result in increased progenitor cell mobilization and pro-angiogenic cytokine/growth factor levels in the circulation. Secondly we show that, in MI and HHT-1 patients, homing of (progenitor) cells from the peripheral blood to an infarct area is stimulated by low levels or inhibition of the endopeptidase CD26. Furthermore, in MI patients, low CD26 levels are associated with an improved cardiac function and inhibition of CD26 on (progenitor) cells improved their homing. CD26 inhibition may therefore improve cardiac outcome. In a clinical pilot trial, we furthermore investigated whether pre-medication with atorvastatin in ST-elevated-MI reduces reperfusion damage. We did not find any effects of pre-treatment with atorvastatin on cardiac function, microvascular perfusion or MI size. Finally, we show that in atherosclerotic plaques with a relatively large number of microvessels, high levels of Angiopoietin-2 are found. These high Angiopoietin-2 levels may result in leaky microvessels and subsequent plaque destabilization. Taken together, the results presented in this thesis show that MI results in mobilization of (progenitor) cells and production of high levels of pro-angiogenic cytokines/growth factors, which may improve cardiovascular repair. Furthermore, our results suggest that CD26 inhibition is a promising tool to increase homing of systemically available cells which may have beneficial effects on cardiovascular repair and function, but a clinical trial is needed to demonstrate this.

28. Non-invasive cardiac imaging and the risk of coronary atherosclerosis

Abstract

In this thesis we describe how non-invasive cardiac imaging such as MDCT and dobutamine stress MRI (DSMR) have opened the way to screening cardiac asymptomatic high risk patients. We present the rationale and design of the GROUND study, a prospective, multi-center, randomized clinical trial. Patients with peripheral artery disease (PAD), but without symptomatic cardiac disease were randomized to undergo either CT calcium scoring only or to undergo a non-invasive cardiac imaging algorithm consisting of CT calcium scoring, coronary CT angiography and DSMR testing. Patients with a left main (or equivalent) coronary artery stenosis of >50% on CT did not undergo DSMR testing, but were referred to a cardiologist. Also patients with a positive DSMR were referred to a cardiologist. These referred patients were candidates for conventional coronary artery angiography and cardiac interventions. All participants entered a 5 year follow up period for the occurrence of cardiovascular events. The aim of the study was to evaluate whether a modern imaging algorithm consisting of CT and DSMR can help to reduce cardiac morbidity and mortality in a cardiac asymptomatic high risk population. We present baseline results of 231 participants of the GROUND study. We demonstrate that one fifth of cardiac asymptomatic patients with PAD have a significant stenosis in the left main coronary artery or its equivalent on cardiac CT angiography. A stenosis in this area is considered a class I indication for revascularization. Only 2 out of 76 participants who underwent DSMR had a positive test result. This finding may have major implications for the management of patients with cardiac asymptomatic PAD. More than 90% of the participants show coronary atherosclerosis and half of the patients has a significant stenosis in their coronary arteries. Also half of the patients showed soft plaque in their coronary arteries. With the use of CT calcium scoring, we show a considerable amount of calcium in the coronary arteries of the GROUND study population. We found a 6 times increase in the risk of having a class I revascularization indication for diabetics and a more than 12 times increase of this risk in patients with an Agatston calcium score of more than 400. Diabetic status and the use of calcium scoring may help in selecting PAD patients at highest risk of suffering a cardiac event. In the second half of the thesis we focus on the role of epicardial adipose tissue (EAT) as a potential risk factor for the development of coronary atherosclerosis. EAT is a source of bioactive molecules in the direct surrounding of the coronary artery vessel wall. With the use of cardiac CT the amount of EAT around the heart can be quantified in a reliable way. We demonstrated that EAT was related to multiple known cardiovascular risk factors. Peri-coronary EAT was also related to calcium scoring as a sign of coronary atherosclerosis. Furthermore, we found that EAT is related to the metabolic syndrome and that there was a graded relation between the number of metabolic syndrome factors and peri-coronary EAT.
