Introduction Hearing loss etiology depends on the population studied as well as on the ethnicity and the socio-economic condition of the analyzed region. Etiological diagnosis contributes to the improvement of preventive measures and to the early identification of this deficiency. Objective To identify the etiological factors of hearing loss and its prevalence in a tertiary hospital in southern Brazil, to verify the frequency of mutations in GJB2 and GJB6 genes, and to correlate the degree of hearing loss with the etiological factors of deafness. Methods This prevalence study involved 140 children with bilateral sensorineural or mixed hearing loss. Medical history, physical examination, audiometry, and evoked auditory brainstem response were conducted. Imaging and genetic examinations were also performed. Results Etiologies and their prevalence were as follows: (a) indeterminate causes, 31.4%; (b) conditions related to neonatal period, 22.1%; (c) genetic, 22.1%; (d) auditory neuropathy, 10%; (e) other factors (cortical malformation, intracranial hemorrhage, and internal ear malformations), 7.9% and (f) congenital infections, 6.4%. Within the genetic cases, ten homozygous and seven heterozygotes of the 35delG mutation were identified, besides two cases of rare variants of GJB2: p.Try172* and p.Arg184Pro. One case with homozygosis of del(GJB6-D13S1830) was found. Regarding severity of hearing loss, in 78.6% of the cases the degree of hearing loss was profound and there were no significant differences when comparing between etiologies. Conclusion The number of indeterminate etiologies is still high and congenital CMV infection may be a possible cause of undiagnosed etiology for hearing loss. The predominance of etiologies related to neonatal conditions and infectious causes are characteristic of developing countries. The most prevalent mutation was 35delG, the main GJB2 gene, probably because of the European influence in the genotype of our population. Resumo Introdução A etiologia da perda auditiva depende da população estudada, da etnia e da condição socioeconômica da região analisada. O diagnóstico etiológico contribui para o aprimoramento das medidas preventivas e para a identificação precoce dessa deficiência. Objetivos Identificar os fatores etiológicos da perda auditiva e sua prevalência em um hospital terciário do sul do Brasil, verificar a frequência de mutações nos genes GJB2 e GJB6 e correlacionar o grau da perda auditiva com os fatores etiológicos da deficiência auditiva. Método Este estudo de prevalência avaliou 140 crianças com perda auditiva neurossensorial bilateral ou mista. Foram submetidos a anamnese com histórico médico, exame físico, audiometria e potencial evocado auditivo de tronco encefálico. Exames de imagem e genéticos também foram feitos. Resultados As etiologias e sua prevalência foram as seguintes: (a) causas indeterminadas, 31,4%; (b) condições relacionadas ao período neonatal, 22,1%; (c) genética, 22,1%; (d) neuropatia auditiva, 10%; (e) outros fatores (malformação cortical, hemorragia intracraniana e malformações da orelha interna), 7,9% e (f) infecções congênitas, 6,4%. Entre os casos genéticos, foram identificados dez casos homozigotos e sete heterozigotos da mutação 35delG, além de dois casos de variantes raras do GJB2: p.Try172* e p.Arg184Pro. Foi encontrado um caso homozigoto da mutação del (GJB6‐D13S1830). Em relação à gravidade da perda auditiva, em 78,6% dos casos o grau da perda auditiva foi profundo e não houve diferenças significantes na comparação entre as etiologias. Conclusão O número de etiologias indeterminadas ainda é elevado e a infecção congênita por CMV pode ser uma possível causa de etiologia não diagnosticada para perda auditiva. A predominância das etiologias relacionadas às condições neonatais e às causas infecciosas são características de países em desenvolvimento. A mutação mais prevalente foi a 35delG e o principal gene foi o GJB2, provavelmente devido à influência europeia no genótipo de nossa população.

The demand for ethanol in Brazil is growing. However, although the country is one of the largest producers of this fuel, it is still necessary to diversify the production matrix. In that regard, studies with different raw materials are needed, mainly the use of low cost and high available wastes such as lignocellulosic residues from agriculture. Therefore, this study aimed to analyze the bioethanol production from corn stover. An alkaline pretreatment (CaO) was carried out, followed by enzymatic hydrolysis (Cellic Ctec2 and Cellic Htec2) to obtain fermentable sugars. The best experimental condition for the pretreatment and hydrolysis steps resulted in a solution with 0.31 gsugar∙gbiomass-1. Then, the fermentation was performed by the industrial strain of Saccharomyces cerevisiae (PE-2) and by the wild yeast strain Wickerhamomyces sp. (UFFS-CE-3.1.2). The yield obtained was 0.38 gethanol∙gdry biomass-1 was, demonstrating the potential of this process for bioethanol production. La demanda de etanol en Brasil está creciendo. Sin embargo, si bien el país es uno de los mayores productores de este combustible, aún es necesario diversificar la matriz productiva. En este sentido, se requieren estudios con diferentes materias primas, principalmente el uso de residuos de bajo costo y alta disponibilidad, como los residuos lignocelulósicos de la agricultura. Por tanto, este estudio tuvo como objetivo analizar la producción de bioetanol a partir del rastrojo de maíz. Se realizó un pretratamiento alcalino (CaO), seguido de hidrólisis enzimática (Cellic Ctec2 y Cellic Htec2) para obtener azúcares fermentables. La mejor condición experimental para los pasos de pretratamiento e hidrólisis resultó en una solución con 0,31 gazúcar∙gbiomasa-1. Luego, la fermentación fue realizada por la cepa industrial de Saccharomyces cerevisiae (PE-2) y por la cepa de levadura salvaje Wickerhamomyces sp. (UFFS-CE-3.1.2). El rendimiento obtenido fue de 0,38 gettanol∙gbiomasa seca-1, lo que demuestra el potencial de este proceso para la producción de bioetanol. A demanda por etanol no Brasil está crescendo. Porém, embora o país seja um dos maiores produtores deste combustível, ainda é necessário diversificar a matriz produtiva. Nesse sentido, estudos com diferentes matérias-primas são necessários, principalmente a utilização de resíduos de baixo custo e alta disponibilidade, como os resíduos lignocelulósicos da agricultura. Portanto, este estudo teve como objetivo analisar a produção de bioetanol a partir da palha de milho. Foi realizado um pré-tratamento alcalino (CaO), seguido de hidrólise enzimática (Cellic Ctec2 e Cellic Htec2) para obtenção de açúcares fermentáveis. A melhor condição experimental para as etapas de pré-tratamento e hidrólise resultou em uma solução com 0,31 gaçúcar∙gbiomassa-1. Em seguida, a fermentação foi realizada pela cepa industrial de Saccharomyces cerevisiae (PE-2) e pela cepa de levedura selvagem Wickerhamomyces sp. (UFFS-CE-3.1.2). O rendimento obtido foi de 0,38 getanol∙gbiomassa seca-1, demonstrando o potencial deste processo para a produção de bioetanol.

Objective: To evaluate the impact of the Universal Neonatal Hearing Screening (UNHS) on the age at diagnosis, beginning of treatment, and first cochlear implant surgery. Methods: A retrospective cohort study with children up to 12 years old with bilateral hearing loss were divided into two groups: patients who underwent UNHS and the ones who didn't. The groups were compared according to their age at the beginning of the evaluation at a specialized center, at the beginning of the intervention, and, for the ones who had indication, at the cochlear implant surgery. The group who underwent UNHS was divided between the ones who passed the screening test and the ones who didn’t. They were compared according to their ages at the same moments as the first two groups. Results: 135 patients were included. The median age at the first appointment in a specialized center was 1.42 (0.50 and 2.50) years, at the beginning of treatment 2.00 (1.00 and 3.52) years, and the cochlear implant surgery 2.83 (1.83 and 4.66) years. Children who underwent UNHS were younger than those who didn't, at the three evaluated moments (p < 0.001). In a subanalysis, children who passed the UNHS but were later diagnosed with hearing loss reached the first appointment with a specialist and started treatment older than those who failed the tests. Conclusion: Performing UNHS interfered with the timing of deafness diagnosis and treatment. However, children who passed the screening but were later diagnosed with hearing loss were the category with the most important delay.

Antecedentes : Os registros de acompanhamento de pacientes com espondiloartrite (SpA) forneceram evidencias de que os inibidores do fator de necrose tumoral (TNFi) aumentam a incidencia de infeccao tuberculosa ativa (TB). No entanto, a maioria desses registros e de areas de baixo peso da TB. Poucos estudos avaliaram a seguranca de agentes biologicos em areas endemicas da tuberculose. Este trabalho compara a taxa de incidencia de TB (TB IR) em individuos com experiencia em anti-TNF e anti-TNF com SpA em um cenario de alta incidencia de TB. Metodos : neste estudo de coorte retrospectivo, foram revisados os prontuarios medicos de pacientes atendidos na Clinica SpA durante 13 anos (2004 a 2016) em um hospital universitario. O IR da TB foi calculado e expresso como numero de eventos por 10 5 pacientes / ano; foi calculada a razao da taxa de incidencia (TIR) associada ao uso de TNFi. Resultados : Foram avaliados 277 pacientes, 173 individuos com anti-TNF e 104 com experiencia em anti-TNF; 35,7% (N = 35) dos pacientes que receberam um medicamento anti-TNF foram diagnosticados com infeccao tuberculosa latente (ILTB). O tempo total de acompanhamento (pessoa / ano) foi 1667,8 e 394,9, respectivamente. O IR da TB (IC 95%) foi de 299,8 (37,4- 562,2) para os anti-TNF e 1012,9 (25,3- 2000,5) para os anti-TNF. A TIR associada ao uso de TNFi foi de 10,4 (2,3- 47,9). Conclusoes : nesse cenario de alta incidencia de TB, os pacientes com SpA expostos a terapia anti-TNF apresentaram uma incidencia maior de TB em comparacao aos individuos que nao receberam anti-TNF, embora a incidencia de TB no grupo controle tenha sido significativa.

Ja existe um consenso da necessidade de uma mudanca do cenario energetico global para aumentar a parcela de fontes renovaveis, tornando a economia baseada em uma ampla gama de plataformas alternativas de energia, na qual o etanol pode ser apontado como um importante biocombustivel. O Brasil e o segundo maior produtor de etanol, contudo ainda e necessario a diversificacao da matriz de producao a fim de torna-lo mais atrativo para substituicao como combustivel alternativo. Dessa forma, observa-se a necessidade de estudos com diferentes materias-primas, principalmente a utilizacao de residuos de baixo custo que apresentem grande disponibilidade, como, por exemplo, os residuos lignocelulosicos da agricultura. Neste contexto, o objetivo deste trabalho foi estudar a producao de bioetanol como biocombustivel a partir do residuo lignocelulosico do milho. Para tanto, executou-se um pre-tratamento seguido de hidrolise enzimatica para obtencao de acucares fermentesciveis. A melhor condicao ocorreu com o emprego de uma suspensao de 100 g/L de biomassa, concentracao de 0,2 gCaO/gbiomassa em incubacao a temperatura de 70oC e agitacao de 200 rpm, por um periodo de 24 horas. A seguir foi realizado hidrolise enzimatica, utilizando-se enzimas comercias nas concentracoes de 2% Ctec2 (m/m) e 0,5% Htec2 (m/m), em incubacao a temperatura de 50oC e 200 rpm de agitacao, durante 24 h. Por meio desse processo, obteve-se uma solucao com concentracao de 0,31 gacucares/gbiomassa, sendo os acucares fermentesciveis quantificados via HPLC. Em seguida, produziu-se um novo hidrolisado, o qual foi fermentado utilizando uma cepa industrial de Saccharomyces cerevisiae (PE-2) e outra selvagem, isolada a partir de biomassa de milho em decomposicao, identificada taxonomicamente como uma nova especie do genero Wickerhamomyces. A partir desses processos determinou-se um rendimento de 0,376 getanol/gglicose, demonstrando o potencial deste processo para a producao de bioetanol.

The current level of air pollution, fossil fuel depletion, rising demand for fuels and rising cost, has been pressure the modern society to search clean fuels. An alternative is biodiesel, because vehicles with diesel engines can use it with little or no modification. However, the high cost of raw materials remains a challenge in to biodiesel production. Thus, the use of alternatives low-cost raw materials from industrial waste is a paramount, to reduce the process costs. In this context, the main objective of this work was the production of fatty acid methyl esters (FAME) using low cost raw material (soybean oil industry residue), with high free fatty acid content (FFA), through enzymatic route. The catalyst used was NS-40116 (Novozymes - Denmark). These analyses indicated that high FAME yields (96%) are achieved using 37 °C; 2 equivalents of methanol added in 6 shots and 3 wt%. In addition, using low-cost materials has several advantages, not competing with food products, requiring no land for cultivation and being eco-friendly. Summary, the enzymatic process is a promising alternative for biodiesel production, reducing its cost and benefiting sustainable development through the use of industrial waste.

Background Anti-tumour necrosis factor (anti-TNF) agents can induce progression from latent tuberculosis infection (LTBI) to active tuberculosis (TB) in patient with rheumatic diseases1,2. In a high tuberculosis incidence setting, TB cases developed despite the screening and treatment for LTBI. Objectives To identify, in a high TB incidence setting, the TB incidence rate in patients with spondyloarthritis (SpA) during twelve years of follow-up. Methods Electronic medical records from patients attending the SpA Clinic between 2004 and 2016 in a public university hospital were reviewed. Patients were grouped in those exposed to anti-TNF therapy and those non-exposed. The tuberculosis incidence rate (IR) was calculated for both groups and expressed as number of events per 1 00 000 patients/year; the incidence rate ratio (IRR) associated to the anti-TNF therapy was calculated. Results A total of 274 patients were evaluated, 102 exposed to anti-TNF drugs and 172 non-exposed. All the 102 patients underwent screening for LTBI before anti-TNF therapy: 38.2% (n=39) were diagnosed with LTBI and underwent 6 months of isoniazid preventive therapy (IPT). The total follow up time (in patients/year) was 729 in the group exposed to anti-TNF and 1243 in the group non-exposed. Ten patients were diagnosed with TB: 4 exposed to anti-TNF therapy and 6 non-exposed. Among the 4 patients exposed to anti-TNF therapy who developed TB, three had negative screening for LTBI. The TB IR (per 1 00 000 patients/year) among those exposed to anti-TNF was 548, compared to 321 in non-exposed; the IRR associated with the use of anti-TNF drugs was 1.7. Conclusions In a region with high TB prevalence, patients with SpA exposed to anti-TNF drugs had a higher incidence of TB compared to those who have never been exposed to these drugs. Our data reinforces the American College of Rheumatology’s recommendation that patients who live in endemic TB settings should be tested annually for LTBI.
