This unique case is the first published renal rupture with hematoma as a result of conduitoscopy. Whilst cases of renal hematoma following ureteropyeloscopy are a recognized entity, published complications following conduitoscopy are absent from the literature. This case serves as a warning that even simple conduitoscopy can result in life threatening bleeding and demonstrates the need for caution and risk management with diagnostic procedures. It is hoped that by individual patient assessment for specific risk factors, and by harm reduction methods, such complications may be avoided in the future.

Abstract Background Spontaneous formation of urinoma is a rare condition, especially for pregnant women. We report a patient in the third trimester of pregnancy with a spontaneous renal rupture who then develops a urinoma from urine leaking into the perinephric space. Case presentation A 23-year-old primagravida was diagnosed with a spontaneous renal rupture and acute left loin pain accompanied by hematuria when she was 35 weeks pregnant. A sub-capsular perinephric cyst then developed to a size of 319 × 175 × 253 mm, and because of discomfort to the patient, we performed Cesarean section. After a healthy male newborn was delivered, fluid was suctioned from a large perirenal cyst that had an estimated size of 300 × 200 × 300 mm. A percutaneous nephrostomy tube was left in the cyst until CT showed no remaining fluid. In the six-month follow-up, the patient showed no perirenal extravasation according to an ultrasound scan, and the urine analysis and renal function tests were normal. Conclusion Close follow-up should be recommended for the patient who has renal rupture after conservative therapy, especially for pregnant woman. CT or MRI should be considered in addition to utilizing ultrasound in the management of pregnant women who present with urinomas. Percutaneous nephrostomy is suggested as an appropriate treatment for large urinomas.

A 41-year-old woman presented to the emergency department with a chief complaint of hematuria three days status post extracorporeal shockwave lithotripsy. The patient described a three-day history of worsening left-sided abdominal pain immediately following the procedure. She denied any fever, chills, changes in bowel habits, hematochezia, increased urinary frequency, urinary urgency, or dysuria.Physical exam revealed tenderness to palpation in the left upper quadrant, left flank and periumbilical region with mild guarding. Laboratory studies revealed an anemic patient with downward trending hematocrit (red blood cell count of 3.41 106/µL, hemoglobin of 10.6 g/dL, and a hematocrit of 31.3% down from 43% a week and a half prior). Urinalysis revealed red and cloudy urine with 3+ leukocytes.A chest radiograph was unremarkable. A computed tomography of the chest, abdomen, and pelvis showed a laceration to the lateral aspect of the mid left kidney with a hematoma measuring 3.2 cm in thickness (Figure). Summary: A 22‐year‐old Thoroughbred mare presented to North Carolina State University Large Animal Hospital for evaluation of acute colic. After initial work‐up, the mare was diagnosed and treated for presumed gas colic. She was subsequently further diagnosed and treated for gastric ulcers and mild sand accumulation in her large colon, based on gastroscopy and abdominal radiographs, respectively. The mare initially responded well to medical management of colic, but on Day 3 of hospitalisation, developed a low grade fever and signs of mild discomfort returned. Abdominocentesis at that time revealed an aseptic peritonitis and she was placed on antibiotics and NSAIDs. When no significant improvement was achieved, the mare was taken to surgery 2 days later. In surgery, she was diagnosed with nephrolithiasis of the left kidney that had erupted through the renal capsule and into the retroperitoneal space, as well as into the abdomen. Two nephroliths were found in the omentum. The mare was subjected to euthanasia due to a poor prognosis and submitted for necropsy, where 54 nephroliths were identified, associated with the left kidney. This report describes significant unilateral nephrolithiasis (to the point of renal rupture) in a horse with no azotaemia or other clinical signs of urinary tract disease. [ABSTRACT FROM AUTHOR]

Renal injuries are commonly reported in blunt abdominal trauma and are usually associated to other abdominal injuries. Renal rupture may be misdiagnosed and its management, especially in severe lesions, is still controversial. We describe the case of renal rupture diagnosed in the Emergency Department and conservatively managed. The outcome was good thanks to the early diagnosis and the management of the renal trauma. Conservative management in renal rupture is possible in stable patients provided that the imaging is early done.

Abstract: We present a rare, but critical case of renal rupture after ureterorenoscopic holmium:YAG laser lithotripsy. This is the first case report of renal rupture after laser lithotripsy in managing an upper urinary tract stone. A 75-year-old male underwent retrograde ureterorenoscopic holmium:YAG laser lithotripsy for a left renal pelvic stone. A drop in blood pressure was noted postoperatively. Abdominal computed tomography confirmed a left subcapsular, perirenal hematoma and a stone which had dropped out to the left subcapsular space. [Copyright &y& Elsevier]

Introduction. Bullying is a form of aggression in which one or more children repeatedly and intentionally intimidate, harass or physically harm a victim who is perceived as unable to defend herself or himself. Case report. 9-year old male patient who sustained an abdominal (mesogastrium) blow from schoolmate which resulted in a left renal rupture. A laparotomy revealed a left renal rupture and large peri-renal hematoma. The patient developed renovascular hypertension. Discussion. After a blunt impact on the abdomen, physical examination, ultrasound and computerized tomography are mandatory since associated renal malformations are common. Conclusions. A cecoureterocele is a rare form of ectopic ureterocele that the orifice of the affected ureter is within the bladder, but the cavity of the ureterocele extends beyond the bladder neck into the urethra. We present a case of a newborn boy with a large cecoureterocele with contralateral renal rupture. Renal allograft rupture is one of the most dreaded complications in patients of kidney transplantation. It has a dramatic and stormy course and may be life threatening for the patient. Although rare, it has to be recognised early and surgical correction and or graft nephrectomy may often be required to salvage the patient. It is usually observed in the first two weeks after transplant. We herein report a case of spontaneous renal allograft rupture in a 50 years old female patient who had undergone a live donor kidney transplantation from her spouse. The patient had a renal rupture on ninth day of surgery. She was re-explored and the rupture was repaired using 2-0 vicryl mattress sutures over surgicell. Her graft recovered completely by day 3 after the repair. However she had pulmonary embolism a day prior to discharge and she succumbed with a functioning graft. [ABSTRACT FROM AUTHOR]

Background: Spontaneous renal rupture is a rare disease in the clinic. The causes of spontaneous renal rupture include extrarenal factors, intrarenal factors, and idiopathic factors. Reports on infection secondary to spontaneous renal rupture and the complications of spontaneous renal rupture are scarce. Furthermore, there are few patients with spontaneous renal rupture who present only with fever., Case Summary: We present the case of a 52-year-old female patient who was admitted to our hospital. She presented only with fever, and the cause of the disease was unclear. She underwent a contrast-enhanced computed tomography (CT) scan, which showed that the left renal capsule had a crescent-shaped, low-density shadow; the perirenal fat was blurred, and exudation was visible with no sign of calculi, malignancies, instrumentation, or trauma. Under ultrasound guidance, a pigtail catheter was inserted into the hematoma, and fluid was drained and used for the bacterial test, which proved the presence of Klebsiella pneumoniae. Two months later, abdominal CT showed that the hematoma was absorbed, so the drainage tube was removed. The authors present a case report of multiple renal ruptures after flexible ureteroscopic lithotripsy (FURL) with holmium laser. Multiple renal ruptures following flexible ureterorenoscopy have not been reported so far. The etiology remains unclear. Background: Undifferentiated abdominal pain is a common condition encountered in the emergency department. While a patient's age, risk factors, and comorbidities often aid in risk stratification and guide emergency department evaluation, atypical presentations of serious disease may still occur suddenly in young otherwise healthy patients.Case Report: In this vignette we discuss a case of spontaneous, atraumatic renal rupture with large retroperitoneal hemorrhage (Wunderlich's Syndrome) in a 22year old male.Discussion: Emergency physicians regularly encounter uncommon causes of abdominal pain. Wunderlich's syndrome is rare but proper understanding of the disease process will aid in decision making and improve disposition and outcomes for patients.
