1. Implantation of trabecular micro-bypass stent using a novel 'landing strip' technique

Abstract

AIM: To describe a novel technique of creating a landing strip within the trabecular meshwork to guide trabecular micro-bypass stent (iStent) implantation in patients who underwent phacoemulsification. METHODS: Thirty-four eyes from 30 patients who underwent iStent implantation after phacoemulsification from May 2014 to February 2015 were included in our retrospective study. All iStents were implanted via the “landing strip” technique. A 25-gauge microvitreoretinal blade was used to bisect the trabecular meshwork to less than 1 clock-hour, effectively creating a landing strip. The iStent applicator was pressed along the landing strip and then the stent was released into the trabecular meshwork. RESULTS: Of the 34 eyes with iStent implantation, 27 (79.4%) eyes had primary open-angle glaucoma, 6 (17.6%) eyes had pseudoexfoliation glaucoma, and 1 (2.9%) eye had ocular hypertension. At 6-month follow-up (n=17), the mean number of hypotensive medications decreased from 2.2±1.2 at baseline to 0.8±1.3 (P=0.05) and mean intraocular pressure decreased from 19.7±4.1 mm Hg at baseline to 16.7±2.1 mm Hg (P=0.58). Two eyes (5.9%) required subsequent trabeculectomy. CONCLUSION: The “landing strip” technique appears to be an effective way to assist with iStent implantation.

Abstract

Vigna unguiculata is the major grain legume in Africa and is also important in South-East Asia as a vegetable. The wild relatives, present only in Africa, are currently made of ten subspecies. However, while the different groups are well identified, the relationships between them are not firmly established. The present study of chloroplast diversity through restriction fragment length polymorphism confirms the different groups, as well as the split between savannah subspecies and forest subspecies. It justifies treating subsp. stenophylla as a subspecies independent from subsp. protracta and invalidates the subsp. stenophylla sensu lato concept. It suggests that the species V. unguiculata originated in southern Africa and that the annual subsp. unguiculata var. spontanea originated in eastern Africa. More importantly, it proves that three subspecies are of hybrid origin, between the annual var. spontanea and various perennial subspecies from southern Africa with left-twisted keel. Owing to its annual and inbred properties, var. spontanea spread across all African low- to medium-altitude savannahs, displacing the left-twisted keel savannah perennials from their warmer and drier areas.

Abstract

Background: Hereditary angioedema (HAE) is a potentially fatal disease characterized by unpredictable, recurrent, often disabling swelling attacks. In a randomized phase 2 study, donidalorsen reduced HAE attack frequency and improved patient quality‐of‐life (ISIS721744‐CS2, NCT04030598). We report the 2‐year interim analysis of the phase 2 open‐label extension (OLE) study (ISIS 721744‐CS3, NCT04307381). Methods: In the OLE, the on‐treatment study period consisted of fixed (weeks 1–13, donidalorsen 80 mg subcutaneously every 4 weeks [Q4W]) and flexible (weeks 17–105, donidalorsen 80 mg Q4W, 80 mg every 8 weeks [Q8W], or 100 mg Q4W) dosing periods. The primary outcome was incidence and severity of treatment‐emergent adverse events (TEAEs). The secondary outcomes included efficacy, pharmacodynamic, and quality‐of‐life assessments. Results: Seventeen patients continued in the OLE study. No serious TEAEs or TEAEs leading to treatment discontinuation were reported. Mean monthly HAE attack rate was 96% lower than the study run‐in baseline rate (mean, 0.06/month; 95% confidence interval [CI], 0.02–0.10; median, 0.04 on‐treatment vs. mean, 2.70/month; 95% CI, 1.94–3.46; median, 2.29 at baseline). Mean monthly attack rate for Q8W dosing (n = 8) was 0.29 (range, 0.0–1.7; 95% CI, −0.21 to 0.79; median, 0.00). Mean plasma prekallikrein and D‐dimer concentrations decreased, and Angioedema Quality of Life Questionnaire total score improved from baseline to week 105 with donidalorsen. Conclusion: The 2‐year interim results of this phase 2 OLE study of donidalorsen in patients with HAE demonstrated no new safety signals; donidalorsen was well tolerated. AbstractBackgroundCowpea is a highly inbred crop. It is part of a crop-weed complex, whose origin and dynamics is unknown, which is distributed across the African continent. This study examined outcrossing rates and genetic structures in 35 wild cowpea (Vigna unguiculata ssp. unguiculata var. spontanea) populations from West Africa, using 21 isozyme loci, 9 of them showing polymorphism.ResultsOutcrossing rates ranged from 1% to 9.5% (mean 3.4%), which classifies the wild cowpea breeding system as primarily selfing, though rare outcrossing events were detected in each population studied. Furthermore, the analyses of both the genetic structure of populations and the relationships between the wild and domesticated groups suggest possibilities of gene flow that are corroborated by field observations.ConclusionsAs expected in a predominantly inbred breeding system, wild cowpea shows high levels of genetic differentiation and low levels of genetic diversity within populations. Gene flow from domesticated to wild cowpea does occur, although the lack of strong genetic swamping and modified seed morphology in the wild populations suggest that these introgressions should be rare.

20. The Influence of Plasma Prekallikrein Oligonucleotide Antisense Therapy on Coagulation and Fibrinolysis Assays: A Post-hoc Analysis

Abstract

Background Congenital prekallikrein deficiency has been associated with increased risk of thrombosis in a few uncontrolled studies. Prekallikrein levels were reduced by 36-94% (median 75%) in a phase 2 study with the prekallikrein specific antisense oligonucleotide, donidalorsen, in patients with hereditary angioedema (HAE). Objectives To estimate the effects of plasma prekallikrein reduction on coagulation and fibrinolysis. Methods Plasma samples were obtained from 16 HAE patients treated with donidalorsen and six placebo-treated patients. Calibrated automated thrombogram (CAT), clot lysis time, high-molecular-weight kininogen activity, factor XI activity, prothrombin fragment 1+2 (F1+2), thrombin-antithrombin complexes, D-dimer, plasminogen activity, plasmin-α2-antiplasmin complexes, and α2-antiplasmin activity were measured before and after four months of treatment. Results No significant changes following donidalorsen treatment were observed between baseline and after four months of treatment: CAT lag time 3.0 min and 3.0 min, CAT peak thrombin concentration 109% and 129%, CAT endogenous thrombin potential 98% and 108%, factor XI activity 110% and 115%, F1+2 levels 251 pMol/L and 161 pMol/L, thrombin-antithrombin complexes 2.6 μg/L and 1.9 μg/L, high-molecular-weight kininogen activity 87% and 93%, clot lysis time 92% and 99%, D-dimer levels 0.65 μg/L and 0.36 μg/L, plasminogen activity 116% and 125%, plasmin-α2-antiplasmin complexes 533 ng/mL and 328 ng/mL, and α2-antiplasmin activity 122% and 127%, respectively. There were also no differences between donidalorsen and placebo treatment. Conclusions Reduction of plasma prekallikrein by donidalorsen in HAE patients neither affected thrombin formation nor fibrinolytic activity. Our data suggest that partial plasma prekallikrein reduction does not influence thrombotic risk.

22. Molecular characterization of cowpea [Vigna unguiculata (L.) Walp.] subspecies with SSR markers

Abstract

Background Cowpea, Vigna unguiculata, is an important food legume in the tropics and subtropics. However, cowpea is a complex species with more than 10 subspecies which can hybridize and produce intermediate progenies. Partly because of the complex organization of the cowpea gene pool and the lack of adequate markers for these infraspecific units, cowpea breeders are not using the wild part of the cowpea gene pool. Methods Here, we report the molecular characterization of 34 representative accessions with 61 SSR markers from coding regions. Results If SSR failed to separate the closest groups, i.e. subsp. alba, subsp. tenuis and the perennial groups from subsp. unguiculata, a combination of few SSR markers can properly identify the main cowpea subspecies. Regarding the infraspecific phylogeny of cowpea, SSR markers confirm the special status of the annual subsp. unguiculata versus the different perennial subspecies. It confirms that subsp. protracta looks like the oldest subspecies, making the origin of the species in southern Africa likely. However, it unites in a single group, clearly separated from subsp. unguiculata, all the taxa of hybrid origin, i.e. subsp. alba, subsp. tenuis, subsp. pubescens and the BWA group of subsp. unguiculata. Conclusion Although a limited number of markers was tested considering that several hundred of cowpea SSR are available, the present work shows that SSR markers can be used for molecular characterization of cowpea subspecies and can be very helpful for understanding its complex evolutionnary history.

Abstract

• PURPOSE: To describe the 15-month baseline results and costs of the Manhattan Vision Screening and Follow-up Study, which aims to investigate whether innovative community-based eye health screening can improve early detection and management of glaucoma and other eye diseases among high-risk populations. • DESIGN: Five-year prospective, cluster-randomized controlled trial. • METHODS: Individuals aged 40+ years were recruited from public housing buildings in New York City for an eye health screening (visual acuity (VA) with correction, intraocular pressure measurements (IOP), and fundus photography). Participants with VA 20/40 or worse, IOP 23–29 mm Hg, or an unreadable fundus image failed the screening and were scheduled for an optometric examination at the same location; those with an abnormal image were referred to ophthalmology. A cost analysis was conducted alongside the study. • RESULTS: A total of 708 participants were screened; mean age 68.6 ± 11.9 years, female (65.1%), African American (51.8%) and Hispanic (42%). 78.4% (n = 555) failed the eye health screening; 35% (n = 250) had an abnormal image and were also referred to ophthalmology. 308 participants attended the optometric exam; 218 were referred to ophthalmology. Overall, 66.1% were referred to ophthalmology. The cost per participant to deliver the eye health screening and optometric examination was $180.88. The cost per case of eye disease detected was $273.64. • CONCLUSIONS: This innovative study in public housing developments targeted high-risk populations, provided access to eye-care, and improved early detection of ocular diseases in New York City. The study has identified strategies to overcoming barriers to eye care to reduce eye health disparities.

27. Operational forecasts of stratospheric chemistry and aerosols by the ECMWF Integrated Forecasting System for the Copernicus Atmosphere Monitoring Service (CAMS)

Abstract

The ECMWF’s Integrated Forecast System (IFS) is the global atmospheric model used by the Copernicus Atmosphere Monitoring Service (CAMS) to provide analyses and forecasts on atmospheric composition. While the IFS configuration used operationally until June 2023 comprised a chemistry scheme and an aerosol module designed only for tropospheric composition, it has now been upgraded (cycle 48R1) to solve also for stratospheric chemistry and sulfate aerosols. We describe these two modules and their latest developments, evaluating hindcasts of stratospheric composition across eight winter-spring seasons above the polar regions and different aerosol case studies including quiescent and volcanic periods.The new stratospheric chemistry module, IFS(BASCOE), is evaluated against the BASCOE Reanalysis of Aura-MLS (BRAM) demonstrating its capacity to forecast the chemical composition of the polar lower stratosphere above both the Arctic and the Antarctic for five years with very different evolutions of the polar vortices. The current performance allows us to study the interannual variability of ozone hole episodes, including the exceptional Antarctic 2020 event.The stratospheric extension of the aerosol module has been coupled to IFS(BASCOE) through sulphuric acid, the radiation scheme, and the 4Dvar assimilation scheme. Intercomparisons with aerosol-related datasets (GloSSAC, GOMOS-AerGOM, AERONET, TROPOMI-SO2) and simulations by WACCM-CARMA show reasonable agreement for the post-Pinatubo event (1991-1995), a quiescent period (1997-1999), the early Envisat period (2002-2004) and the Raikoke eruption (June 2019). Abstract

Purpose: Despite the publication of several randomized controlled trials (RCTs), it is not clear which technique for the treatment of focal chondral and osteochondral defects of the knee grants the best clinical outcome. The aim of this network meta-analysis (NMA) was to compare the efficacy and safety of microfractures (MF), autologous chondrocyte implantation (ACI), autologous matrix-induced chondrogenesis (AMIC), osteochondral autograft transplantation (OCT) at short (< 1 year), intermediate (1–5 years) and long-term (> 5 years). Methods: We carried out an NMA with Bayesian random-effect model, according to PRISMA guidelines. The search was performed in MEDLINE, EMBASE, Web of Science, CENTRAL, CINAHL, SPORTDiscus, clinicaltrials.gov, WHO ICTRP, from inception to November 2022. The eligibilities were randomized controlled trials on patients with knee chondral and osteochondral defects, undergoing microfractures, OCT, AMIC, ACI, without restrictions for prior or concomitant surgery on ligaments, menisci or limb alignment, prior surgery for fixation or ablation of osteochondritis dissecans fragments, and prior cartilage procedures as microfractures, drilling, abrasion, or debridement. Results: Nineteen RCTs were included. No difference among treatments was shown in the pooled comparison of patient reported outcome measures (PROMs) at any timepoint. Safety data were not available for all trials due to the heterogeneity of reporting, but chondrospheres seemed to have lower failure and reoperation rates. Conclusion: This NMA showed no difference for PROMs with any technique. The lower failure and reoperation rates with chondrospheres must be interpreted with caution since adverse event data was heterogenous among trials. The standardization of the efficacy and safety outcome measures for future trials on knee cartilage repair and regeneration is necessary.

Abstract

Hereditary angioedema is a rare, genetic disorder characterized by painful, debilitating and potentially life-threatening angioedema attacks in subcutaneous and submucosal tissue. While usually unpredictable, attacks can be provoked by a variety of triggers including physical injury and certain medication and are often preceded by prodromal symptoms. Hereditary angioedema has a profound influence on the patients' lives. The fundamental cause of hereditary angioedema in almost all patients is a mutation in the SERPING1 gene leading to a deficiency in C1-inhibitor. Subsequently, the contact activation cascade and kallikrein-kinin pathway are insufficiently inhibited, resulting in excessive bradykinin production triggering vascular leakage. While C1-inhibitor is an important regulator of the intrinsic coagulation pathway, fibrinolytic system and complement cascade, patients do not have an increased risk of coagulopathy, autoimmune conditions or immunodeficiency disorders. Hereditary angioedema is diagnosed based on C1-inhibitor level and function. Genetic analysis is only required in rare cases where hereditary angioedema with normal C1-inhibitor is found. In recent years, new, highly specific therapies have greatly improved disease control and angioedema-related quality of life. This article reviews the clinical picture of hereditary angioedema, the underlying pathophysiology, diagnostic process and currently available as well as investigational therapeutic options.

46. Using Haptic and Auditory Interaction Tools to Engage Students with Visual Impairments in Robot Programming Activities

Abstract

The robotics field represents the integration of multiple facets of computer science and engineering. Robotics-based activities have been shown to encourage K-12 students to consider careers in computing and have even been adopted as part of core computer-science curriculum at a number of universities. Unfortunately, for students with visual impairments, there are still inadequate opportunities made available for teaching basic computing concepts using robotics-based curriculum. This outcome is generally due to the scarcity of accessible interfaces to educational robots and the unfamiliarity of teachers with alternative (e.g., nonvisual) teaching methods. As such, in this paper, we discuss the use of alternative interface modalities to engage students with visual impairments in robotics-based programming activities. We provide an overview of the interaction system and results on a pilot study that engaged nine middle school students with visual impairments during a two-week summer camp.
