Summary: Background: Integrase inhibitor (INSTI) with boosted darunavir (DRV/r), a regimen with a high-resistance barrier, avoiding NRTI toxicities, might be a switching option in children living with HIV (CLWHIV). Methods: SMILE is a randomised non-inferiority trial evaluating safety and antiviral efficacy of once-daily INSTI + DRV/r vs. continuing on current standard-of-care (SOC) triple ART (2NRTI + boosted PI/NNRTI) in virologically-suppressed CLWHIV aged 6–18 years. The primary outcome is the proportion with confirmed HIV-RNA ≥50 copies/mL by week 48, estimated by Kaplan–Meier method. Non-inferiority margin was 10%. Registration number for SMILE are: ISRCTN11193709, NCT #: NCT02383108. Findings: Between 10th June 2016 and 30th August 2019, 318 participants were enrolled from Africa 53%, Europe 24%, Thailand 15% and Latin America 8%, 158 INSTI + DRV/r [153 Dolutegravir (DTG); 5 Elvitegravir (EVG)], 160 SOC. Median (range) age was 14.7 years (7.6–18.0); CD4 count 782 cells/mm3 (227–1647); 61% female. Median follow-up was 64.3 weeks with no loss to follow-up. By 48 weeks, 8 INSTI + DRV/r vs. 12 SOC had confirmed HIV-RNA ≥50 copies/mL; difference (INSTI + DRV/r-SOC) −2.5% (95% CI: −7.6, 2.5%), showing non-inferiority. No major PI or INSTI resistance mutations were observed. There were no differences in safety between arms. By week 48, difference (INSTI + DRV/r-SOC) in mean CD4 count change from baseline was −48.3 cells/mm3 (95% CI: −93.4, −3.2; p = 0.036). Difference (INSTI + DRV/r-SOC) in mean HDL change from baseline was −4.1 mg/dL (95% CI: −6.7, −1.4; p = 0.003). Background: Chronic granulomatous disease (CGD) is a primary immunodeficiency due to a malfunction of NADPH oxidase. It is characterized by recurrent and severe infections caused by catalase-positive microorganisms and autoinflammatory manifestations. Recently, there has been described an NCF4 gene variant that causes a deficiency of p40phox, a subunit of NADPH oxidase. Patients with this deficiency appear to have a less severe clinical form as compared to classic CGD. Case: A 15-year-old girl with vulvar lichen planus since she was 2 years old and suspected Crohn’s disease (CD) was first seen at our hospital. At the age of 12 years, she had been submitted to sacrococcygeal cyst exeresis, without cicatrization of the surgical wound and extension of the lesion to the perianal area. The diagnosis of CD was questioned, and the patient underwent an endoscopic and radiologic assessment, which was normal. A skin biopsy from the perianal area revealed a granuloma; thus, CD with isolated perianal disease was assumed. After several different treatments including antibiotics, infliximab, and adalimumab, the perianal lesion persisted, with no associated gastrointestinal symptoms. Therefore, the hypothesis of an immunodeficiency was considered. An immunologic and genetic study revealed reduced oxidative burst in the phorbol myristate acetate test, with diminished reactive oxygen species production and a homozygous mutation in the NCF4 gene. The adolescent started prophylactic trimethoprim-sulfamethoxazole and became asymptomatic. Conclusions: The present case highlights that alternative diagnoses to CD must be considered in the presence of isolated perianal disease with granulomatous inflammation, especially when the disease is refractory to conventional CD therapy.

ABSTRACT Objective: To highlight the pathogenicity of Streptococcus anginosus, which is rare in pediatric patients, but can cause severe infections that are known to have a better outcome when treated early with interventional procedures and prolonged antibiotic therapy. Case description: The patient is a 6-year-old boy with global developmental delay, examined in the emergency room due to fever and respiratory distress. The physical examination and diagnostic workout revealed complicated pneumonia with empyema of the left hemithorax; he started antibiotic therapy and underwent thoracic drainage. Pleural fluid cultures grew Streptococcus anginosus. On day 11, the child had a clinical deterioration with recurrence of fever, hypoxia, and respiratory distress. At this point, considering the causative agent, he was submitted to video-assisted thoracoscopic decortication, with good progress thereafter. Comments: Streptococcus anginosus is a commensal bacterium of the human oral cavity capable of causing severe systemic infections. Although reports of complicated thoracic infections with this agent are rare in the pediatric population, they have been increasing in adults. Streptococcus anginosus has a high capacity to form abscess and empyema, requiring different therapeutic approaches when compared to complicated pneumonia caused by other agents.

Variações de oferta e dificuldades de acesso a determinados produtos estratégicos podem provocar mudanças na geopolítica internacional. Os choques do petróleo ocorridos na década de 1970 são, talvez, os exemplos mais conhecidos de como a reconfiguração do poder entre os países pode ser redesenhada em função de quem detém determinado produto estratégico. Um conjunto de 17 metais, conhecidos como terras raras, que possui propriedades químicas únicas, está no centro de uma tensão comercial internacional desde que a China, que exporta cerca de 95% do mercado mundial de terras raras, passou a restringir suas exportações a partir de meados de 2000. Este artigo investiga as motivações políticas e econômicas que estiveram por trás do embate na Organização Mundial do Comércio (OMC) entre a China, de um lado, e os Estados Unidos, a União Europeia e o Japão, de outro, que denunciaram a China em 2012 por impor restrições à exportação de terras raras, e analisa como o Brasil poderia aproveitar o seu potencial como produtor de terras raras para fortalecer sua posição no cenário internacional. Conclui-se que o diferendo na OMC foi apenas o primeiro capítulo de uma história de tensões internacionais que estão por vir no cenário internacional.

Theobroma grandiflorum is an important fruit tree from Sterculiaceae family, native to the Brazilian Amazon, known in the region as cupuaçu. The seeds have a high fat content (24%) with characteristics that resemble those of cocoa (Theobroma cacao) butter with potential applications in the cosmetic, pharmaceutical and food industries. The main objective of this work was to explore the seed fats from T. grandiflorum that were analyzed for fatty acid composition by Gas Chromatography with Flame Ionization Detector (GC-FID) and to analyze their activity for acetylcholinesterase inhibition. Chromatographic analysis provided detection of nine fatty acids. The major fatty acids found in the species were oleic (40.0%), stearic (32.7%), arachidic (10.4%) and palmitic (8.0%). INTRODUCTION: Nymphoplasty or labiaplasty consists of a surgical approach to hypertrophy of the labia minora, and is aimed at correcting asymmetry and removing redundant tissue of the vulva. Hypertrophy of the labia minora is characterized by tissue that exceeds the limits of the labia majora under normal conditions. Surgery is an option in patients who complain of unpleasant odor, urinary infection, interference with sexual activity, and/or personal dissatisfaction; in the latter case, the surgical indication has an aesthetic purpose. The aim of this article was to report our experience with wedge resection of the labia minora in patients with hypertrophy. METHODS: We retrospectively analyzed 53 cases of labiaplasty performed with the wedge resection technique from 2010 to 2015. The results were evaluated for postoperative anatomical appearance, the degree of patient satisfaction, and the effect on sexual activity. RESULTS: Patients were satisfied with the postoperative aesthetic appearance. There was improvement in sexual satisfaction, ability to perform physical activities, and hygiene. No complications were observed, such as suture dehiscence, infection, or tissue necrosis. One patient required reoperation in the postoperative period due to asymmetry of the labia minora. Another patient complained of excessive exposure of the clitoris, but she had preexisting clitoral hypertrophy. CONCLUSION: Labiaplasty using the wedge resection technique relieved personal discomfort and provided functional and aesthetic improvement. Due to the lack of validation and standardization of labiaplasty techniques, further clinical research should be performed.

O artigo trata do acolhimento dos refugiados na União Europeia, e possui como principal objetivo analisar esse acolhimento, considerando a securitização da migração como um empecilho para a proteção dos refugiados. Para conduzir a discussão sobre a temática, busca-se examinar a discussão política sobre as migrações forçadas na União Europeia, tendo como foco os refugiados e utilizando-se do exemplo da Itália. Além disso, outros objetivos subjacentes ao principal intento desta investigação buscam apresentar a importância da integração institucional e humanitária dos refugiados no local de acolhimento e, por fim, identificar soluções a fim de melhorar o acolhimento dos refugiados nos países da União Europeia. Observa-se que os Estados se eximem da sua responsabilidade enquanto importantes atores para a proteção desses migrantes forçados. Portanto, é necessário refletir sobre as medidas que contribuam para uma maior cooperação dos Estados, em vez de reforçar a securitização da migração local.

Apesar de sua importância do ponto de vista social e econômico, e de serem mais protegidas que as águas superficiais, as águas subterrâneas podem ser contaminadas por diversas fontes, comprometendo sua qualidade para abastecimento e demandando alto investimento financeiro e de recursos humanos para sua remediação. Nesse contexto, o objetivo do presente estudo foi analisar a qualidade da água subterrânea em 8 (oito) poços distribuídos nos bairros da cidade de Uberaba-MG e avaliar o risco à saúde com base na concentração de metais potencialmente tóxicos e de íons nitrato. Os parâmetros analisados na estação seca e chuvosa foram de caráter microbiológico (Coliformes totais e termotolerantes), físico (condutividade, temperatura, turbidez e cor) e químico (pH, metais potencialmente tóxicos, alcalinidade, demanda química de oxigênio, fósforo total, nitrato, cloreto e dureza). Todos os poços analisados apresentaram coliformes totais e termotolerantes em pelo menos uma estação. Ao comparar com a legislação vigente, os parâmetros cor, DQO, fósforo total, ferro, cádmio, chumbo, níquel, manganês e cromo apresentaram valores acima dos limites permitidos, principalmente no período seco. A análise de risco à saúde mostrou-se significativa para quatro poços estudados, sendo cádmio o parâmetro com os maiores coeficientes de risco, seguido por cromo. Essas alterações precisam ser constantemente monitoradas a fim de conhecer a real qualidade da água subterrânea para consumo. Assim, os resultados encontrados contribuem para o delineamento de estudos futuros, servindo como subsídio para gestores ambientais e de saúde pública.

The inajá (Maximiliana maripa) is a palm from Arecaceae family. The inajá distribution occurs from Amazon to the west-central of South America. The inajá has edible parts: the palm heart and oils obtained from pulp and seed. The aims of this study were to verify the fatty acid profile from pulp oil by GC-FID and analyzing acetylcholinesterase inhibition by inajá pulp oil. Chromatographic analysis provided eleven fatty acids. The major unsaturated fatty acids are oleic (22.32%), linoleic (4.72%) and linolenic acids (3.95%). The major saturated fatty acids are palmitic (20.76%), myristic (20.48%) and lauric acids (17.42%). The acetylcholinesterase inhibition by inajá oil pulp was over 63.76%. 24. Effects of Variants of Concern Mutations on the Force-Stability of the SARS-CoV-2:ACE2 Interface and Virus Transmissibility

Viruses mutate under a variety of selection pressures, allowing them to continuously adapt to their hosts. Mutations in SARS-CoV-2 have shown effective evasion of population immunity and increased affinity to host factors, in particular to the cellular receptor ACE2. However, in the dynamic environment of the respiratory tract forces act on the binding partners, which raises the question whether not only affinity, but also force-stability of the SARS-CoV-2:ACE2 bond, might be a selection factor for mutations. Here, we use magnetic tweezers (MT) to study the effect of amino acid substitutions in variants of concern (VOCs) on RBD:ACE2 bond kinetics with and without external load. We find higher affinity for all VOCs compared to wt, in good agreement with previous affinity measurements in bulk. In contrast, Alpha is the only VOC that shows significantly higher force stability compared to wt. Investigating the RBD:ACE2 interactions with molecular dynamics simulations, we are able to rationalize the mechanistic molecular origins of this increase in force-stability. Our study emphasizes the diversity of contributions to the assertiveness of variants and establishes force-stability as one of several factors for fitness. Understanding fitness-advantages opens the possibility for prediction of likely mutations allowing rapid adjustment of therapeutics, vaccination, and intervention measures.

A água é componente essencial para a vida de todas as espécies. Para os seres humanos além da necessidade de ingestão diária, ela também é utilizada em diversas atividades, como higiene pessoal, limpeza e processos industriais. Porém, somente a parcela da água não salina e isenta de contaminantes pode ser utilizada nas atividades humanas. Dentre os principais agentes poluidores dos recursos hídricos, o presente trabalho destaca os metais tóxicos. Estes compostos apresentam alta toxicidade mesmo em baixas concentrações, principalmente por sua característica de bioacumulação. Destaca-se ainda não serem facilmente degradados em componentes menos tóxicos e possuírem elevado tempo de residência no corpo hídrico, sendo um risco à saúde humana e aos ecossistemas. O bário é um metal alcalino terroso cuja toxicidade é reconhecida há bastante tempo. Ele é encontrado em pequenas concentrações nas águas naturais, principalmente sob a forma de barita (BaSO4). Não sendo encontrado livre na natureza, devido a sua elevada reatividade. As principais ações antrópicas que emitem bário no meio ambiente são: perfuração de poços; produção de pigmentos, fogos de artifício, vidros e defensivos agrícolas; e purificação do minério barita com o subsequente descarte dos efluentes. Nas águas naturais o bário apresenta concentração média de 0,01 mg·L-1 devido à baixa solubilidade de seus sais. Os sais de bário mais solúveis são os cloretos, os nitratos e os carbonatos. A intoxicação pelo bário pode causar náuseas, vômitos, diarreias, perda de reflexos e paralisia muscular e pode levar a óbito se não houver tratamento. A biossorção de metais potencialmente tóxicos tem-se mostrado uma alternativa no tratamento de águas contaminadas, por ser uma técnica de baixo custo e rápida. Dentre os biossorventes, destaca-se os resíduos da Moringa oleifera que possuem grande quantidade de poros e área superficial, indicando boa capacidade de adsorção para o tratamento de águas superficiais e efluentes, sendo uma alternativa aos tratamentos convencionais. O potencial da Moringa oleifera tem sido estudado no tratamento de água e efluentes contaminados por meio de processos de coagulação e adsorção. A biomassa da moringa demonstrou capacidade de melhorar os parâmetros de qualidade da água, incluindo pH, turbidez, sólidos totais dissolvidos, fluoreto, dureza e remoção de contaminantes. Sendo assim, este trabalho propôs a avaliação da eficiência da biomassa de Moringa oleifera em suas frações folha e vagem e semente, na remoção de bário em água. Foram realizados ensaios de adsorção conforme o planejamento experimental, para avaliação das seguintes variáveis: tipo (folha e vagem e semente), quantidade biomassa (0,32g a 1,68g), diâmetro de partículas da biomassa (partículas menores que 425 μm, partículas entre 425 μm e 600 μm, entre 600 μm e 1180 μm, entre 1180 μm e 1750 μm e maiores que 1750 μm) e diferentes concentrações iniciais de bário (6,40 a 33,45 mg·L-1). As folhas da moringa apresentaram 98,6% de remoção de Ba2+, as vagens da moringa 98,2%, e as sementes da moringa apresentaram 99,8% de remoção de Ba2+. Como a semente foi a amostra que demonstrou melhor percentual de remoção no planejamento experimental, ela foi analisada de acordo com os modelos de isotermas de adsorção de Freundlich e de Langmuir em três temperaturas (25°C, 35°C e 45°C). Com base nos coeficientes de correlação (R2), o modelo que mostrou melhor ajuste para os dados experimentais de biossorção do bário pelas sementes foi o de Freundlich, indicando, portanto, um processo de biossorção que pode ocorrer em multicamada. O estudo da cinética de adsorção também foi realizado utilizando a semente de moringa e os dados dos ensaios não se ajustaram ao modelo cinético de pseudo-primeira ordem. O melhor ajuste foi observado aplicando o modelo de pseudo-segunda ordem. Conclui-se que os resultados demonstram que as folhas, vagens e sementes de Moringa oleifera são biomassas eficientes na adsorção bário.

The unbinding pathway of a protein complex can vary significantly depending on biochemical and mechanical factors. Under mechanical stress, a complex may dissociate through a mechanism different from that used in simple thermal dissociation, leading to different dissociation rates under shear force and thermal dissociation. This is a well-known phenomenon studied in biomechanics whose molecular and atomic details are still elusive. A particularly interesting case is the complex formed by bacterial adhesins with their human peptide target. Staphylococci bacteria use an arsenal of virulence factors, mainly composed of proteins such as adhesins, to target and adhere to their host. Adhesins play critical roles during infection, mainly during the early steps of adhesion when cells are exposed to high mechanical stress. S. epidermidis SdrG:Fgβ force resilience has been investigated using AFM-based single molecule force spectroscopy experiments paired with steered molecular dynamics (SMD) simulations. However, there is still a gap between both kinds of experiments at high force-loading rates. Here, we leveraged the high-speed of coarse-grained (CG) SMD simulations to bridge the gap between the data obtained in vitro and in silico with all-atom SMD. We used the DHS theory to connect the two types of SMD simulations and the predictions are consistent with theory and experimentation. We believe that, when associated with all-atom SMD, course-grained SMD can be a powerful ally to help explain and complement the results of single-molecule force spectroscopy experiments.

Human genomic diversity has been shaped by both ancient and ongoing challenges from viruses. The current coronavirus disease 2019 (COVID-19) pandemic caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has had a devastating impact on population health. However, genetic diversity and evolutionary forces impacting host genes related to SARS-CoV-2 infection are not well understood. We investigated global patterns of genetic variation and signatures of natural selection at host genes relevant to SARS-CoV-2 infection (angiotensin converting enzyme 2 [ACE2], transmembrane protease serine 2 [TMPRSS2], dipeptidyl peptidase 4 [DPP4], and lymphocyte antigen 6 complex locus E [LY6E]). We analyzed data from 2,012 ethnically diverse Africans and 15,977 individuals of European and African ancestry with electronic health records and integrated with global data from the 1000 Genomes Project. At ACE2, we identified 41 nonsynonymous variants that were rare in most populations, several of which impact protein function. However, three nonsynonymous variants (rs138390800, rs147311723, and rs145437639) were common among central African hunter-gatherers from Cameroon (minor allele frequency 0.083 to 0.164) and are on haplotypes that exhibit signatures of positive selection. We identify signatures of selection impacting variation at regulatory regions influencing ACE2 expression in multiple African populations. At TMPRSS2, we identified 13 amino acid changes that are adaptive and specific to the human lineage compared with the chimpanzee genome. Genetic variants that are targets of natural selection are associated with clinical phenotypes common in patients with COVID-19. Our study provides insights into global variation at host genes related to SARS-CoV-2 infection, which have been shaped by natural selection in some populations, possibly due to prior viral infections.

Breast cancer can arise within a benign tumor or the tumors can coexist independently. A 48-year-old woman presented with a structured palpable nodule in the breast for 2 years. In the course of the follow-up, a growth was noted where it was decided to perform a core biopsy resulting in a low-grade invasive cribriform carcinoma in fibroadenoma. Some treatments such as quadrantectomy with negative sentinel lymph node biopsy and free surgical margins, chemotherapy, and radiotherapy were carried out at the time of follow-up using tamoxifen without complications. This case contemplates a rare association of two breast diseases and the value of the histological examination for the diagnosis of malignancy. Fibroadenomas are not normally considered a risk factor for carcinoma. In a review of the literature, an attack of a carcinoma evolving into a fibroadenoma ranged from 0.002% to 0.0125%. A carcinoma that occurs in a fibroadenoma can be considered a casual occurrence, once the epithelial component of a fibroadenoma is subjected to the same stimuli and triggers as the rest of the breast. Carcinoma may arise in adjacent breast tissue surrounding or infiltrating a fibroadenoma, or it may be entirely restricted to or, at least, predominantly, a fibroadenoma.

Trata-se de uma pesquisa qualitativa, cujo objeto de estudo consiste na análise da prática dos universitários acerca da contracepção e controle das DSTS/HIV/AIDS. E teve como objetivo identificar o método contraceptivo de preferência no universo dos universitários, bem como descobrir os aspectos que pesam na hora dessa escolha e se há relação na escolha desse método contraceptivo com a prevenção das doenças. Os resultados alertam para a necessidade de mudanças na atitude dos futuros profissionais de saúde, seja na maneira atual de ensino, seja no modo de oferecer suporte para que estes realmente entendam a importância e necessidade das práticas sexuais seguras, associadas ao uso de métodos contraceptivos.Investigación descriptiva cualitativo sobre el conocimento de los estudiantes de infermage e medicina de la universidad y de cuidado referentes la opción anticonceptiva y al control de DST/SIDA. Identificando la opción anticonceptiva del método entre éstos que gradúan, y la evidencia si ellos métodos del associate a la prevención del DST's/SIDA o no. Los datos fueron recogidos por entrevistas semi structuralized en cassette de cinta. Los datos producidos foran reorganizados y analizados basado en referencial teórico. Los resultados alertan para cambio de las actitudes de los estudiantes, de la educación de modo que realmente entiendan la importancia y la necesidad de práctico sexual seguro, y a los métodos anticonceptivos asociados a enfermedades sexuales, control de HIV/SIDA y prevención del embarazo.Descriptive research of qualitative approach, with the study object about college medicine and nursing students concerning the contraceptive choice and DST/SIDA control. Identifying the contraceptive method choice between these graduating, and to evidence if they associate the methods to the prevention of the DST's/SIDA or not. The data were collected by semi-structured interviews in ribbon cassette. The produced data were reorganized and analyzed based on theoretical referencial. Results alert for changes in students attitudes needs either in current education way, to offer support so that they really understand the importance and necessity of safe sexual practical, and associates contraceptive methods to sexual diseases, HIV/AIDS control beyond planned pregnancy prevention.

OBJECTIVES Varicella is a common, usually benign, and autolimited disease in children but can lead to severe complications and hospitalization. With this study, we aim to analyze all varicella hospitalizations to provide epidemiological information to help outline preventive policies. METHODS We assessed all varicella hospitalizations in children aged 0 to 17 years, from 2000 to 2015, in mainland, public Portuguese hospitals using a Portuguese administrative database. Seasonality, geographic distribution, severity, complications, risk factors, use of diagnostic and treatment procedures and hospitalization costs were analyzed. RESULTS A total of 5120 hospitalizations were registered, with an annual rate of 17.3 hospitalizations per 100 000 inhabitants. A higher number of hospitalizations occurred during the summer period and in Southern regions. The median length of stay was of 4 days (interquartile range: 3.0–7.0). We found a high rate of severe complications, mostly dermatologic (19.6%), neurologic (6.0%), and respiratory (5.1%). Of the total number of patients, 0.8% were immunocompromised and 0.1% were pregnant. Total direct hospitalization costs during the 16-year period were estimated to be 7 110 719€ (8 603 970 USD), with a mean annual cost of 444 419.92€ (537 748.10 USD). CONCULSIONS This is the first national study in which useful epidemiological data to evaluate the burden and impact of varicella in Portugal is provided.

O Fórum Universitário do Mercosul apresenta mais um registro de seu dinamismo. Fruto da determinação de seus organizadores, e do apoio da Universidade Estadual da Paraíba, esta coletânea é também significativa da vivacidade e da capilaridade da rede FoMerco, construída ao longo de mais de duas décadas de existência. Ao abrir este livro, que o leitor o aprecie pela qualidade de suas análises, mas também, que atente para o fato de que ele representa os avanços da integração regional, sobretudo do adensamento das relações culturais, posto que esta é uma obra coletiva de estudantes, professores, autores de diferentes nacionalidades latino-americanas, e não somente mercosulenhas, portadores de diferentes filiações intelectuais, pesquisadores de diferentes dimensões desse nosso amplo tema. A segunda observação, que cabe aqui destacar, diz respeito ao sentido da exitosa longa trajetória do FoMerco: promover e cultivar uma rede de pesquisadores cônscios da importância da produção do conhecimento a partir das perspectivas, experiências e saberes do Sul. Atentos para a necessidade de superação dos “males de origem” da América Latina, para lembrar aqui a original abordagem de Manoel Bomfim. Convictos de que a integração regional é muito mais do que uma oportunidade de vantajosos sucessos comerciais. Em 2017, ano do XVI Congresso Internacional do FoMerco, o tema Integração Regional em Tempos de Crise: Desafios Políticos e Dilemas Teóricos assinalava um contexto político regional hostil ao modelo vigente do Mercosul. O Congresso foi um sucesso, mas sua realização foi um tour de force pelos parcos financiamentos disponíveis e pelas dificuldades daí decorrentes. Mas, a inquebrantável determinação do Professor Renato Vieira Martins, à época Presidente do FoMerco, e o decidido apoio do Professor Antonio Albino Canelas Rubim, da Universidade Federal da Bahia, sede do XVI Congresso, reuniu, aproximadamente, 500 congressistas na ensolarada cidade de São Salvador. Desde então, assistimos aos crescentes ataques das forças políticas conservadoras desconformes com “a passagem do Mercosul econômico e comercial para o Mercosul social e participativo”, como assinalou Renato Vieira Martins. Nem os quase 1 milhão de óbitos da Covid-19, em toda a América Latina e Caribe, estabeleceu um limite à “razão neoliberal” que ameaça a estrutura do Bloco. A volta do conhecido modelo da “submissão estratégica” das burguesias latino-americanas, de “caráter” “entreguista” e “comprador”, como analisa Theotônio dos Santos, é acompanhado da exclusão social que caminha pari passu com o aumento da concentração econômica. Não obstante, o Mercosul completa 30 anos de existência. E as diferentes trajetórias, desafios e crises que pontuaram o processo de integração contribuíram para fortalecer a democracia na região, afastar a hipótese militar entre os países vizinhos, adensar as relações entre os povos. Quero acreditar, portanto, que são essas conquistas que orientarão o desenvolvimento das dinâmicas da integração. De sua parte, nesses mais de vinte anos acompanhando o Mercosul, o FoMerco manteve-se firme no propósito de assegurar à rede o debate democrático e a ampliação da cooperação acadêmica. Em promover as renovações exigidas pelas mudanças da passagem do tempo - o XVIII Congresso, em setembro próximo, ocorrerá em formato totalmente digital. Em registrar os debates sobre os assuntos significativos da vida do Mercosul, como as mais de 500 páginas deste livro permitem entrever. Isso tudo não é pouca coisa. Por isso, minhas calorosas saudações aos muitos amigos que comparecem neste livro e até setembro, em nosso próximo encontro. Context: Multisystem Inflammatory Syndrome in Children (MISC) is a newly and rising condition, particularly in SARS-CoV-2 high transmission communities. Objective: Analyze current literature and reported cases of MISC, concerning its clinical spectrum, complications associated, therapeutic strategies and distinguishing features of other clinical syndromes. Data Sources: Extensive literature research was performed in MEDLINE (trough PubMed), Scopus and Web of Science from December 2019 to December 2020.Study Selection: First analysis included all article titles and abstracts screening to identify relevant studies and second analysis included a full text screening of previous selected studies. Eligibility was assessed independently by two authors and disagreements were resolved by discussion and consensus. Data Extraction: Data were extracted on MISC definition, demographic data, clinical features, diagnostic tests, laboratory analysis andimaging, therapeutical approach and outcomes. Results: Common symptoms included: gastrointestinal (70%), rash (57%) and cardiovascular (52% with shock). Notable differences with Kawasaki Disease were identified including age, clinical presentation and cardiac involvement. 30% presented positive SARS-CoV-2 2 reverse transcription polymerase chain reaction and 51% positive serologies. 62% received intravenous immunoglobulin and 42% glucocorticoids. 62% required intensive care, 21 children died (
