Abstract

Despite concerns about including children and adolescents in research about sensitive topics like violence or abuse, growing evidence suggests that minors experience only small amounts of distress when being asked sensitive questions during research participation (Chu & DePrince, 2013; Chu et al., 2007; Kassam-Adams & Newman, 2005; Kuyper et al., 2012; Langhinrichsen-Rohling et al., 2006; McClinton Appollis et al., 2020; Priebe et al., 2010; Ybarra et al., 2009; Zajac et al., 2011). However, even though the long-term effects of participation in research on sensitive topics have been repeatedly investigated with adults (e.g., Carter-Visscher et al., 2007; Cook et al., 2015; Larsen & Berenbaum, 2014; McGee et al., 2005; Nielsen et al., 2016), there is a lack of research when it comes to adolescents. Consequently, several authors recommend further investigation of this issue (e.g., Jaffe et al., 2015; Jorm et al., 2007; McClinton Appollis et al., 2020; McClinton Appollis et al., 2015; Ybarra et al., 2009). Two counterexamples are the studies by Chu and DePrince (2013) and by Zajac et al. (2011), where participants were recontacted after one year. The latter study reported low levels of distress among participants and no relation to attrition, suggesting that asking sensitive questions does not have long-lasting negative effects on adolescents. Chu and DePrince (2013) found similar results concerning attrition and, additionally, reported that their participants reported positive cost-benefit ratios over the course of the year, i.e., they reported more positive aspects of their participation than negative. To further expand our knowledge about long-term effects on adolescents who have participated in research on sensitive topics, this study wants to question adolescents on a study in which they have participated one year earlier. During that study, the participants were asked to put themselves in the fictional situation of a person that was being interviewed by a police officer about one of their teachers who was accused of child maltreatment. Approximately half of the participants were further supposed to imagine that this person had indeed experienced maltreatment by the teacher. All participants were asked whether they had ever been maltreated themselves and how distressing the survey had been.

Abstract

Every year, millions of children all around the world are being interviewed on cases of sexual and physical abuse (Saywitz, Wells, Larson, & Hobbs, 2019). Nevertheless, there are no general interviewing guidelines German police officers must follow in cases of alleged child abuse, although children are being interviewed comparatively often in Germany (Brackmann & Habermeyer, 2019). The Revised National Institute of Child Health and Human Development Protocol (R-NICHD) by Hershkowitz, Lamb, Katz, and Malloy (2013) is an internationally used and renowned instrument designed for questioning children in cases of alleged abuse. It provides practitioners with “enhanced rapport-building and supportive yet nonsuggestive techniques” (Disussion section, para. 1) to strengthen the interviewed children’s cognitive processes and increase their willingness to testify. It has been tested in various field studies with allegedly abused children and shown to aid the disclosure of more detailed information (Hershkowitz, Lamb & Katz, 2014; Karni-Visel, Hershkowitz, Lamb, & Blasbalg, 2019) while not posing the threat of suggestibility because of “carefully structured prompts designed to evoke free-recall” (Lamb, Brown, Hershkowitz, Orbach, & Esplin, 2018, p. 174). In 2018, Noeker and Franke translated the R-NICHD into German. To our knowledge, only one author (Otzipka, 2019) has researched this German version of the R-NICHD and, moreover, the effects of individual questioning techniques (as compared to the effect of the R-NICHD as a whole). In a vignette study, she discovered that some of the supposedly rapport-establishing, non-suggestive techniques were in fact experienced as decreasing well-being and willingness to talk while increasing perceived pressure in an adult sample. Since the R-NICHD was designed to interview minors, the purpose of this study is to transfer these results to an adolescent sample (14-17 years). In an online survey, participants will be asked to put themselves in the position of an adolescent who has or has not been physically abused and to rate the questioning techniques regarding their effect on their willingness to talk and the perceived pressure.

31. Perspectives on physician-assisted suicide in mental healthcare: results of a survey of physicians and medical students.

Abstract

Background: Physician-assisted suicide (PAS) is typically associated with serious physical illnesses that are prevalent in palliative care. However, individuals with mental illnesses may also experience such severity that life becomes intolerable. In February 2020, the previous German law prohibiting PAS was repealed. Patients with severe mental illnesses are increasingly likely to approach physicians with requests for PAS., Aims: To explore the ethical and moral perspectives of medical students and physicians when making individual decisions regarding PAS., Method: An anonymised digital survey was conducted among medical students and physicians in Germany. Participants were presented with a case vignette of a chronically depressed patient requesting PAS. Participants decided on PAS provision and assessed theoretical arguments. We employed generalised ordinal regression and qualitative analysis for data interpretation., Results: A total of N = 1478 participants completed the survey. Of these, n = 470 (32%) stated that they would refuse the request, whereas n = 582 (39%) would probably refuse, n = 375 (25%) would probably agree and n = 57 (4%) would definitely agree. Patient-centred arguments such as the right to self-determination increased the likelihood of consent. Concerns that PAS for chronically depressed patients might erode trust in the medical profession resulted in a decreased willingness to provide PAS., Conclusions: Participants displayed relatively low willingness to consider PAS in the case of a chronically depressed patient. This study highlights the substantial influence of theoretical medical-ethical arguments and the broader public discourse, underscoring the necessity of an ethical discussion on PAS for mental illnesses.

Abstract

Pancreatic intraepithelial neoplasia (PanIN) is a precursor to pancreatic cancer and represents a critical opportunity for cancer interception. However, the number, size, shape, and connectivity of PanINs in human pancreatic tissue samples are largely unknown. In this study, we quantitatively assessed human PanINs using CODA, a novel machine-learning pipeline for 3D image analysis that generates quantifiable models of large pieces of human pancreas with single-cell resolution. Using a cohort of 38 large slabs of grossly normal human pancreas from surgical resection specimens, we identified striking multifocality of PanINs, with a mean burden of 13 spatially separate PanINs per cm 3 of sampled tissue. Extrapolating this burden to the entire pancreas suggested a median of approximately 1000 PanINs in an entire pancreas. In order to better understand the clonal relationships within and between PanINs, we developed a pipeline for CODA-guided multi-region genomic analysis of PanINs, including targeted and whole exome sequencing. Multi-region assessment of 37 PanINs from eight additional human pancreatic tissue slabs revealed that almost all PanINs contained hotspot mutations in the oncogene KRAS , but no gene other than KRAS was altered in more than 20% of the analyzed PanINs. PanINs contained a mean of 13 somatic mutations per region when analyzed by whole exome sequencing. The majority of analyzed PanINs originated from independent clonal events, with distinct somatic mutation profiles between PanINs in the same tissue slab. A subset of the analyzed PanINs contained multiple KRAS mutations, suggesting a polyclonal origin even in PanINs that are contiguous by rigorous 3D assessment.
