VERTEBRAL fractures , *DUAL-energy X-ray absorptiometry , *BONE fractures , *OLDER people , *BONE regeneration , *POSTURAL muscles - Abstract
Osteoporotic vertebral fractures represent a significant sociomedical problem that impairs the quality of life of the elderly population. Clinical examination, supplementary diagnostic methods (X-ray, Computed Tomography - CT, Dual-Energy X-ray Absorptiometry - DXA, Lateral Vertebral Assessment - LVA) and fracture risk assessment tool (FRAX) are crucial for effective assessment of fracture severity, timely decision on treatment method, and initiation of rehabilitation. Of great practical importance is the effect of antiresorptive therapy on callus formation. In patients who suffer from osteoporosis, and despite treatment have a fracture, it is recommended not to interrupt bisphosphonate therapy, which was started several months before the diagnosed fracture. Bisphosphonates should be introduced into the treatment in a period of 2-4 months from the occurrence of the fracture, depending on the location of the fracture, that is, the time required for callus formation. Recombinant parathyroid hormone is an effective anabolic therapy that accelerates bone regeneration during fractures, increases callus volume and faster bone strength. Osteoporosis therapy should not be started without checking the total and ionized Ca, P, 25(OH)D, and PTH as well as general biochemical analyses, and then introducing antiresorptive or anabolic therapy. Rehabilitation treatment is individually designed and includes balance exercises, strength exercises, range of motion and postural training leading to improvement of spinal mobility, muscle strength and overall functionality. The aim of this review was to highlight the timely diagnosis, evaluation, and treatment of osteoporotic vertebral fractures. [ABSTRACT FROM AUTHOR]

SARS-CoV-2 , *COVID-19 , *CARDIOVASCULAR diseases , *GERIATRIC rehabilitation - Abstract
Background/Aim. A very limited amount of data regarding the rehabilitation outcome of surgical patients with COVID-19 is available in the current literature. The aim of this study was to point out the characteristics of early rehabilitation of these patients and determine the predictors of rehabilitation outcomes. Methods. The study was designed as a prospective clinical trial. It included patients who had surgical treatment from April 1, 2022, to March 31, 2023, at the University Clinical Center of Vojvodina, Serbia and either had positive results for severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) preoperatively or developed coronavirus disease 2019 (COVID-19) within 72 hrs after surgery. The rehabilitation program was planned for each patient according to the type of surgical treatment, age, clinical presentation and severity of the COVID-19, length of immobilization, and comorbidities. Rehabilitation treatment (RT) started with a minimum of one 30-minute daily session, up to three 30-minute sessions daily. Patients were assessed at the beginning of RT and discharge. Outcomes were assessed with the Modified Borg Scale (MBS) for dyspnoea, Barthel index (BI) for activities of daily living, Six-Minute Walk T est (6MWT) f or e xercise tolerance, and Timed up and Go (TUG) test for balance and lower limb mobility. Results. A total of 81 patients were included in the study. RT was successful for 42 patients (24 female and 18 male) with an average age of 62.10 ± 20.07 years. These patients exhibited significant functional improvement, which was measured by all tests that assessed rehabilitation outcome at discharge: BI (p < 0.001), MBS (p < 0.001), 6MWT (p < 0.001), and TUG test (p < 0.001). The remaining 31 patients had unsuccessful RT. The binary logistic regression analysis has shown that age (p = 0.009), cardiovascular disease (p = 0.017), and malignancy (p = 0.022) were significant predictors of rehabilitation outcome. Conclusion. Results of the present study implicate that individually tailored RT during the acute phase of COVID-19 in surgical patients is very challenging. Advanced age, cardiovascular disease, and malignancy are predictors of unfavorable outcomes, and careful consideration is needed when planning the treatment for these patients. [ABSTRACT FROM AUTHOR]

periferna arterijska bolezen ,intermitentna klavdikacija ,vadba ,rehabilitacija ,dejavniki tveganja ,Medicine - Abstract

The article presents the little-known pioneering activity of doctors working in Lviv, the capital of the Galicia province in the Austrian Monarchy, for the development of therapeutic gymnastics and orthopedics from 1847 to 1918. Analysis of source materials from archives, medical magazines, and daily newspapers shows that therapeutic gymnastics was introduced for the treatment of locomotor diseases in Lviv at the turn of the 19th and 20th centuries. Initially, it was based on the German method and then replaced by the Swedish method. At the same time, orthopedics emerged from surgery, with its origins related to the habilitation of Dr Antoni Gabryszewski at the University of Lviv. The main role in this process belonged to Lviv doctors, graduates of universities in Krakow, Lviv, Prague, and Vienna. They conducted scientific research at the Medical Faculty of the University of Lviv and made numerous trips to renowned centers in other provinces of Austria, as well as in Germany, Sweden, and Switzerland. Due to their scientific work, as well as the knowledge and experience gained from foreign scientific visits, they founded and managed facilities where they applied orthopedics, healing gymnastics, mechanotherapy, physical therapy, massage, and orthopedic equipment. Most facilities were comparable in functionality to renowned foreign centers. The pioneering activities of Lviv doctors contributed to the development of orthopedics and healing gymnastics in other Galician cities and influenced the establishment of spas throughout the province. In later years, this activity furthered the establishment of orthopedics, comprehensive rehabilitation, and spa therapy in Poland and Ukraine. [ABSTRACT FROM AUTHOR]

COVID-19 , *COVID-19 treatment , *REHABILITATION centers , *COVID-19 pandemic , *SARS-CoV-2 - Abstract
Introduction. The COVID-19 pandemic caused by the SARS-CoV-2 virus has led to significant public health problems, severe complications, and functional impairment in persons who have recovered from this disease. Case report. A 60-years-old male deconditioned patient was transferred from Corona 4 Center to the Center for Physical Medicine and Rehabilitation of the University Clinical Center in Kragujevac for a postCOVID rehabilitation due to pronounced muscle weakness and inability to walk. After functional testing, an individual rehabilitation plan was created and a kinesitherapy program was adjusted daily based on the patient's respiratory status. After the kinesitherapy program, Respivol was used - a volumetric medical device for breathing exercises in patients with respiratory difficulties. At the time of admission, the patient could not move independently, could not move from the lying to the sitting position on his own, and could not maintain balance in the sitting position. Hypotrophy caused by prolonged inactivity of the muscles of the whole body dominated the clinical picture. A six-week rehabilitation treatment performed on the patient showed a significant improvement in functionality (FIM test), muscle strength (manual muscle test), daily life activity (Barthel index), as well as a reduction of anxiety (GAD-7 scale). Conclusion. The goal of the rehabilitation program is to restore the patient's state of functionality before the initiation of COVID-19 treatment, so that they can perform their regular professional activities and achieve complete independence in performing activities of daily living (ADL). DIGITAL health , *QUALITY of life , *BURN patients , *RANDOM effects model , *SCIENCE databases - Abstract
Background/Aim. The present systematic review and meta-analysis aimed at evaluating the impact of selfcare training programs based on digital health on the quality of life of burn patients. Methods. Electronic databases such as Science Direct, Medline/PubMed, Web of Science, Scopus, ProQuest, Google Scholar, and Cochrane library were searched independently by two researchers using the relevant keywords. The random effects model meta-analysis was carried out to compute the influence of common effect size, the standardized mean difference (SMD) on health-related quality of life (HRQOL) as the primary aim of the study. Funnel plots were drawn to assess the publication bias and I-squared index was utilized to assess the heterogeneity. Subgroup analyses were carried out accordingly. From a total of 105 studies, six studies were eligible to attend in the final meta-analysis. Results. The results showed a significant effect of digital health compared to normal care on the overall quality of life (QOL) in burn victims (SMD 1.2, (95% CI 0.58, 1.8), P = 0.0001, I2 = 92%). The I2 index shows a high level of heterogeneity with a value of 92%. Subgroups analysis shows a significant effect of telemedicine compared to normal care on the overall QOL in burn victims between weeks 7 - 12 (SMD 1.7 (95% CI 0.43, 3), P = 0.009, I2 = 93%), and between weeks 2 - 6 (SMD 1.4 (95% CI 0.5, 2.3), P = 0.002, I2 = 89%) and > 13 weeks (SMD 0.09 (95% CI -0.17, 0.35), P = 0.5, I2 = 0%). Conclusion. According to the presented results, it seems that new digital technologies have brought benefits including a positive impact on the quality of life score of burn victims.

MAXILLECTOMY , *QUALITY of life , *PARTIAL dentures , *APPETITE loss , *FEAR of dentists , *PILOT projects , *APPETITE disorders , *HEAD & neck cancer - Abstract
Background/Aim. Although radical surgical treatment is the method of choice in the therapy of cancer of the maxilla and maxillary sinus, it can cause oral dysfunction, social isolation, and emotional and psychological distress, which significantly affects the patient's quality of life (QoL). The aim of the study was to determine the healthrelated QoL of patients rehabilitated with obturator prosthesis (OP) after maxillectomy, according to demographic and clinical characteristics. Methods. The study included 32 patients with a mean age of 63.6 years. The measurement of QoL of patients after maxillectomy and prosthetic rehabilitation was assessed using the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire Core 30 (EORTC QLQ-C30) and EORTC QLQ - Head and Neck Cancer Module (QLQ-H&N43). Results. Functioning and symptoms were more pronounced in women, younger patients, and single patients (p < 0.05). Patients after definitive prosthetics rehabilitation had fewer problems and symptoms compared to patients with surgical and interim OP (p < 0.05). Patients with partial obturator dentures had a better perception of function than patients with total dentures (p < 0.05). Irradiated patients had more pronounced fatigue, appetite loss, pain in the mouth, dry mouth and sticky saliva, and sense and skin problems compared to nonirradiated patients (p < 0.05). Conclusion. The results of this study suggest that sex, age, marital status, characteristics of OP, and radiotherapy have a significant impact on QoL in patients after maxillectomy. [ABSTRACT FROM AUTHOR]

Aim: Utilizing the Canadian Occupational Performance Measure (COPM), the goal of this research is to identify daily life occupations that are of the highest priority for people with traumatic brain injury (TBI) and to investigate sensitivity and convergent validity of that outcome measure. Subjects and methods: The study included 37 individuals with TBI during inpatient rehabilitation. At the initial COPM assessment the most important occupations in the area of self-care, productivity and leisure were identified, followed by patients selfrating of the performance and satisfaction activities of high priority. A reassessment was administered on average 30 days following the initial assessment. Convergent validity was examined by correlations with the Functional Independence Measure (FIM). Data were analysed using descriptive statistics and by corelation analysis. Results: A total of 158 occupational priorities were identified in the areas of self-care (63.29%), productivity (24.05%) and leisure (12.66%). The difference between the initial COPM assessment and reassessment is clinically and statistically significant. A statistically significant correlation was found between the COPM performance and the total FIM result, both at the first and the second assessment. There was also a statistically significant correlation between the changes of the initial assessment and reassessment of COPM performance and changes of total and motor FIM. Conclusion: The findings of this research has shown the area of self-care, especially personal care and functional mobility to be the most important for people with TBI during inpatient rehabilitation. COPM has proven to be a sensitive and valid self-rating assessment tool of the occupational performance priorities during inpatient rehabilitation of people with TBI. [ABSTRACT FROM AUTHOR]

VIRTUAL reality , *NEURODEGENERATION , *COGNITIVE ability , *REHABILITATION - Abstract
Virtual reality is a computer-generated environment that allows the user to interact with virtual elements through special devices. This interactivity distinguishes virtual reality from conventional media such as video and television. It offers a wide range of applications, including IT, education, medicine, military, and space science. The development of virtual reality is closely linked to advances in information technology, in particular to powerful computers that enable three-dimensional interfaces. There are several types of virtual reality, ranging from non-immersive, where the user is not fully involved in the virtual world, to full virtual reality, where the presence in the virtual world is pronounced. Augmented reality integrates virtual and real elements, while mixed reality allows interaction between the two worlds. Virtual reality is proving to be an effective method of rehabilitation, especially for patients rehabilitating in a home environment. It allows control over external stimuli, flexibility of exercises and recording of progress. The use of virtual reality for upper limb and gait rehabilitation is promising as it improves motor skills and increases brain activity. For patients with neurodegenerative diseases, virtual reality is used to assess cognitive functions. In children, virtual reality can improve cognitive, motor and social skills, and quality of life. Virtual reality is therefore a versatile tool that can enhance patients' rehabilitation and improve their quality of life. [ABSTRACT FROM AUTHOR]

- Abstract
COVID-19 is a multisystem disease that, although predominantly affecting the respiratory system, can affect almost any organ system and requires structured rehabilitation of a multidisciplinary team. Acute, early, rehabilitation during the first 30 days of infection constitutes only the first phase of rehabilitation of persons with COVID-19. Symptoms and signs of physical, neurocognitive, and psychological consequences after SARS-CoV2 infection may persist for weeks or months after the end of the acute phase of the disease, and may develop in individuals who have a mild clinical picture and have not been hospitalized. The most common symptoms and signs that occur after the acute phase of COVID-19 are fatigue, shortness of breath, cough, joint and muscle pain, and insomnia and increased hair effluvium. Patients whose clinical condition in the acute phase of the disease required treatment with mechanical ventilation may have long-term consequences, including physical weakness and neurocognitive dysfunction (brain fog) characterized by loss of intellectual functions such as concentration and short-term memory. Patients with cognitive dysfunction have problems with word recollection, dyscalculia, and concentration. The PostCOVID Day Hospital for Physical Medicine and Rehabilitation rehabilitation program is implemented by a multi- disciplinary team focused on the overall functioning of patients, not just the treated organ or organ system, including the consequences of COVID-19 disease, possible treatment complications and comorbidities. The goal of rehabilitation of patients in the post-acute COVID-19 period is to achieve complete functional recovery and adequate quality of life. [ABSTRACT FROM AUTHOR]

HAND , *ROBOTICS - Abstract
Aim: This work presents a method for developing a simplified but efficient model of the complex human hand kinematics with the aim of its implementation in rehabilitation robotics. Material and methods: The approach incorporates modularity by simplifying the available model comprising 24 degrees of freedom (DOFs) to 9 DOFs, with the introduction of additional joint coupling parameters specific to different grasp types. The effect of dependent joints to the ranges-of-motion (ROMs) of the model is investigated and compared to the anatomical one. The index, middle, ring and little finger solutions to forward and inverse kinematics problems are then acquired. The implementation of the model, based on the median male bones dimensions, is made available in the open-source Robot Operating System (ROS) framework. Results: By including additional four inclination angles per finger, the devised kinematic hand model encompasses also finger curvatures, resulting in significant positioning accuracy improvements compared to the conventional model. The used 3D spatial position improvement metrics are the mean absolute (MAE) and mean relative errors (MRE). The dependent joint position MAEs range from 0.22 to 0.34 cm, while MREs range from 2.8 and 3.5 %, whereas the highest absolute and relative errors during fingertip positioning can reach 0.5 cm and 10.5 %, respectively. Conclusion: The performed investigation allowed establishing that by modelling finger curvature and assuring the adaptability of the model to a variety of human hands and rehabilitation modalities through joint dependency, represents the best approach towards a relatively simple and applicable rehabilitation model with functional human-like hand movements. - Abstract
To present a case of a patient with fibromyalgia who initially presented with chronic widespread pain and a diagnostic-therapeutic procedure. Case report: A 61-year-old patient was treated at the Department of physical and rehabilitation medicine with rheumatology for severe chronic joint and muscle pain. The patient additionally describes difficulty of sleeping, depression, and anxiety. She suffers from hypothyroidism and degenerative changes of the spine have been previously determined on magnetic resonance imaging. Extensive diagnostic treatment ruled out inflammatory rheumatic disease, neurological disease, paraneoplastic syndrome, unregulated hypothyroidism, and chronic infection. The Fibromyalgia Diagnostic Questionnaire confirmed the suspicion that the patient was suffering from fibromyalgia based on the results of the questionnaire Widespread pain index (WPI) 15 and symptom severity scale (SSS) 9. The Functional Assessment of Chronic Illness Therapy-fatigue (FACIT-F) gave an extremely poor result, 15 points. Non-pharmacologic treatment was conducted, consisted of education, individual exercise, physical therapy procedures and multimodal pharmacological therapy, with the choice of drugs affected the nociceptive and neuropathic type of pain, as well as the anxiety of the patient with a favorable therapeutic response. This approach to treatment has achieved a significant improvement in the symptoms of fibromyalgia syndrome in our patient. Conclusion: Fibromyalgia should be suspected in patients with widespread chronic pain, chronic fatigue, and mental disorders. It is necessary to exclude another serious disease by diagnostic processing. The 2016 American College of Rheumatology (ACR) diagnostic criteria are used to diagnose fibromyalgia. With this presentation of the patient, we wanted to raise awareness about this disease, which is recognized in the world as a serious health problem, although sometimes neglected and unrecognized. To analyze the impact of exoskeletal robotic device Armeo Power® (Hocoma, Switzerland) on arm and hand post-stroke rehabilitation of patients with one side movement disability. Patients and methods: Among the patients who participated in the stationary rehabilitation program at Department of Physical and Rehabilitation Medicine in Clinical Hospital Centre Rijeka we formed experimental group of 10 patients, and corresponding control group. For experimental group, we combined conventional and robotic therapy (5 times a week for 4 weeks, 45 minutes per therapy), while in the control group we applied only conventional therapy. Conventional therapy in both groups was the same (5 times a week for 4 weeks, 45 minutes per therapy). Assessment of a motoric function, functional measurement of independence, average performance, satisfaction in the therapy and estimation was done by clinical scales. Results: Statistically significant improvement was noticed in both groups with all clinical parameters (P=0,005). Comparing the two groups at the end of treatment there was no significant statistic difference in measurements of motor function on FMA (Fugl-Meyer Assessment) scale (P=0,120), or functional independence on FIM (Functional Independence Measure) scale (P=0,762). Statistically significant difference between groups at the end of treatment was registered only in results of COPM (Canadian Occupational Performance Measure), average performance P=0,014; average satisfaction P=0,022. Conclusion: Obtained results show that Armeo Power therapy in combination with conventional therapy can improve functional recovery of patients after brain stroke, although further research is needed on a larger number of patients.

CORONARY artery disease , *CARDIAC rehabilitation , *HEMODYNAMICS , *CARDIOGRAPHY , *VASCULAR resistance - Abstract
Background/Aim. Well-organized cardiovascular rehabilitation (CVR) reduces cardiovascular burden by influencing cardiovascular risk factors, improving the quality of life, and reducing mortality and hospital readmission. However, its effects on hemodynamic status are largely unknown. The aim of our study was to evaluate the influence of a three-week CVR program on hemodynamic status and to investigate if there is a correlation between physical strain tolerance and hemodynamic parameters measured by impedance cardiography (ICG) before and after the CVR program in patients with coronary artery disease (CAD). Methods. Fiftytwo patients attended a three-week CVR program. At the beginning and the end of the rehabilitation program, laboratory tests, exercise stress tests (EST), and ICG measurements were taken. Results. Patients showed better strain tolerance on the second exercise stress test (EST2) by achieving a higher strain level (Z = 2.315; p = 0.021) and a longer duration of the test (Z = 2.305; p = 0.021). There was a strong positive correlation between the level of EST2 and cardiac output (CO) (r = 0.538; p < 0.001) and stroke volume (SV) (r = 0.380; p = 0.017) on the second ICG (ICG2). Moreover, there was a strong negative correlation between EST2 level and systemic vascular resistance (SVR) (r = -0.472; p = 0.002) and SVR index (SSVRI) (r = -0,407; p = 0.010) on ICG2. There was a strong positive correlation between EST2 duration and CO (r = 0.517; p = 0.001) as well as between EST2 duration and SV (r = 0.340; p = 0.034), and a strong negative correlation between EST2 duration and SVR (r = -0.504; p = 0.001) as well as between EST2 duration and SVRI (r = -0.448; p = 0.004), according to ICG2. Conclusion. Our study showed that a well-designed CVR program can lead to better physical strain tolerance in patients with CAD. Furthermore, CVR led to a significant positive correlation between EST and CO as well as between EST and SV measured by ICG. On the other hand, there was a significant negative correlation between EST and vascular-related parameters according to ICG at the end of the CVR program. [ABSTRACT FROM AUTHOR]
