Abstract

Objective: To assess the value of an elevated (> 30 mm/h) Westergren erythrocyte sedimentation rate (ESR) for predicting early relapse and survival after therapy in patients with clinical stage I or II Hodgkin disease. Interventions: We studied 772 patients with early-stage Hodgkin disease who had participated in two separate multicenter clinical trials. Both trials used modern field radiotherapy and, in some patients, multi-agent chemotherapy. Main Results: The ESR patterns were based on pretherapy and post-therapy assessments: pattern 1, always normal (n = 261); pattern 2, elevated before therapy but normal immediately after therapy (n = 121); pattern 3, elevated before therapy but normal within 3 months after therapy (n = 89); pattern 4, always elevated (n = 48); pattern 5, normal before therapy but oscillating between normal and elevated after therapy (n = 150); pattern 6, elevated before therapy but oscillating between normal and elevated after therapy (n = 130). By multivariate analysis, independent of whether or not patients received chemotherapy in the initial therapy protocol, ESR patterns 4, 5, and 6 were shown to be the best predictors for early relapse and survival when patients were stratified according to the type of chemotherapy received and the number of involved nodal areas. Patients with ESR pattern 4 had a relative risk for death seven times that of patients with patterns 1, 2, or 3. Early relapse was the second most important factor predicting death, irrespective of ESR; patients with early relapse and ESR patterns 1, 2, or 3 had a relative risk for death of 4.5, and those with early relapse and ESR patterns 4, 5, or 6 had a relative risk for death of 15. Whether or not chemotherapy was given initially did not change the relative risk, which shows that ESR, not initial therapy, was the predictor for early relapse and death due to Hodgkin disease. 41. Utopia’s Moorish Inspiration: Thomas More’s Reading of Ibn Ṭufayl

Abstract

A promising but neglected precedent for Thomas More’s Utopia is to be found in Ibn Ṭufayl’s Ibn Ḥayy Yaqẓān. This twelfth-century Andalusian philosophical novel describing the self-education and enlightenment of a feral child on an island, while certainly a precedent for the European Bildungsroman, also arguably qualifies as a utopian text. It is possible that More had access to Pico de la Mirandola’s Latin translation of Ibn Ḥayy Yaqẓān. This study consists of a review of historical and philological evidence that More may have read Ibn Ḥayy Yaqẓān and a comparative reading of More’s and Ṭufayl’s two famous works. I argue that there are good reasons to see in Ibn Ḥayy Yaqẓān a source for More’s Utopia and that in certain respects we can read More’s Utopia as a response to Ṭufayl’s novel., L’Ibn Ḥayy Yaqẓān d’Ibn Ṭufayl consiste en un précédent incontournable mais négligé à l’Utopie de More. Ce récit philosophique andalou du douzième siècle décrivant l’auto-formation et l’éveil d’un enfant sauvage sur une île peut être considéré comme un texte utopique, bien qu’il soit certainement un précédent pour le Bildungsroman européen. Thomas More pourrait avoir lu l’Ibn Ḥayy Yaqẓān, puisqu’il a pu avoir accès à la traduction latine qu’en a fait Pic de la Miradolle. Cette étude examine les données historiques et philologiques permettant de poser que More a probablement lu cet ouvrage, et propose une lecture comparée de l’Ibn Ḥayy Yaqẓān et de l’Utopie de More. On y avance qu’il y a non seulement de bonnes raisons de considérer l’Ibn Ḥayy Yaqẓān d’Ibn Ṭufayl comme une source de l’Utopie de More, mais qu’il est aussi possible à certains égards de lire l’Utopie comme une réponse à l’Ibn Ḥayy Yaqẓān.

After a few weeks, participants expressed an interest in learning more about medical technology.

Abstract

In utero exposure to chemicals with antiandrogen activity induces undescended testis, hypospadias, and sub- or infertility. The hypospermatogenesis observed in the adult rat testis exposed in utero to the antiandrogen flutamide has been reported to be a result of a long-term apoptotic cell death process in mature germ cells. However, little if anything is known about the upstream signaling mechanisms controlling this apoptosis. In the present study, we have investigated the possibility that the TGF-beta signaling pathway may be at play in this control of the apoptotic germ cell death process. By using a model of adult rat exposed in utero to 0, 0.4, 2, or 10 mg/kg.d flutamide, we observed that pro-TGF-beta signaling members, such as the three isoforms of TGF-beta ligands (TGF-beta1-3), the two TGF-beta receptors (TGF-betaRI and -RII) and the R-Smads Smad 1, Smad 2, Smad 3, and Smad 5 were inhibited at the mRNA and protein levels, whereas the anti-TGF-beta signaling member Smad 7 was overexpressed. Furthermore, we report that the overexpression of Smad 7 mRNA could induce an activation of c-Jun N-terminal kinase, because of the observed c-Jun overexpression, activation, and nuclear translocation leading to an increase in the transcription of the proapoptotic factor Fas-L. Together, the alterations of TGF-beta signaling may represent upstream mechanisms underlying the adult germ cell apoptotic process evidenced in adult rat testis exposed in utero to antiandrogenic compounds such as flutamide.
