Abstract

Zusammenfassung: Mindestens die Hälfte aller psychisch kranken Menschen sind Frauen. Bei depressiven Erkrankungen sind Frauen sogar doppelt so häufig betroffen wie Männer. Die geschlechtsspezifischen Aspekte der Psychopharmakotherapie finden jedoch meistens zu geringe Beachtung. Dabei weiß man, dass Frauen besser auf Substanzen mit serotoninmodulierender Wirkung ansprechen als Männer, dass sich dieser Effekt aber postmenopausal umkehrt. Ferner entwickeln Frauen unter bestimmten Psychopharmaka gynäkologische Komplikationen, die den Einsatz dieser Substanzen in Frage stellen. Nebenwirkungen wie Gewichtszunahme, Hyperglykämie, Herzrhythmusstörungen und sexuelle Dysfunktionen treten bei Frauen ebenfalls häufiger auf als bei Männern. Die Schwangerschaft ist ein besonders sensibler Aspekt. Hier muss das Risiko der psychisch kranken Mutter, bei Absetzen eines Psychopharmakons erneut einen Rückfall zu erleiden, gegen das Risiko für das Kind, Fehlbildungen zu erleiden oder pränatale Komplikationen zu entwickeln, abgewogen werden.

Abstract

Zusammenfassung: In der vorliegenden Übersicht sind die in der Schwangerschaft bedeutungsvollen Gerinnungsstörungen und die Behandlung mit Antikoagulanzien bei Patientinnen mit Herzklappenprothesen, Vorhofflimmern und Zustand nach thrombotischen Ereignissen zusammenfassend dargestellt. So benötigen Patientinnen mit mechanischen Klappenprothesen eine lebenslange orale Antikoagulation mit Coumarin(derivaten). Zur Therapie und Prävention von Thromboembolien sind unterschiedliche Behandlungsstrategien etabliert. Die Vor- und Nachteile verschiedener Therapieschemata der Antikoagulation während Schwangerschaft, Geburt und Stillzeit werden diskutiert und Empfehlungen für ein Management ausgesprochen, wobei jeweils auf die Risiken für Mutter und Feten eingegangen wird.

Abstract

AIMS: Animal studies show a pivotal role of dihydrotestosterone (DHT) in pressure overload induced myocardial hypertrophy and dysfunction. The aim of our study was to evaluate the role of DHT levels and myocardial hypertrophy and myocardial protein expression in patients with severe aortic valve stenosis (AS). METHODS AND RESULTS: 43 patients (median age 68 (41-80) years) with severe AS and indication for surgical aortic valve replacement (SAVR) were prospectively enrolled. Cardiac magnetic resonance imaging including analysis of left ventricular muscle mass (LVM), fibrosis and function and laboratory tests including serum DHT levels were performed before and after SAVR. Abstract

Little is known about sex-dependent physiological and pathophysiological differences in cardiac endothelial nitric oxide synthase (eNOS) expression and activation. Therefore, we investigated cardiac morphology and eNOS protein expression, including its translocation-dependent activation and phosphorylation, in cardiac tissue of male and female wild-type mice and transgenic heart-failure mice having a cardiac-specific, 5-fold overexpression of the G[α.sub.q] protein. In addition, we measured calcineurin protein expression. Heart-to-body weight ratio was increased in G[α.sub.q] mice. Female wild-type mice showed higher eNOS protein expression and activation (translocation and phosphorylation) than did wild-type males. In cardiac tissue of G[α.sub.q] mice, these sex-dependent differences remained or were enhanced. Protein expression of the catalytic subunit calcineurin A, which has been shown to dephosphorylate eNOS, was higher in wild-type males than in wild-type females. These differences were increased in the G[α.sub.q] mice model. We conclude that sex differences exist in cardiac eNOS protein expression and phosphorylation. Increased activation of the G[α.sub.q] protein appears to alter eNOS protein expression and phosphorylation only in males. Il existe peu d'information sur les differences pathophysiologiques et physiologiques lieees au sexe dans l'expression et l'activation de la eNOS cardiaque. Nous avons donc examine; la morphologie cardiaque, l'expression de la proteine eNOS, son activation dependante de la translocation et sa phosphorylation dans le tissu cardiaque de souris de type sauvage males et femelles et dans celui de souris transgeniques en insuffisance cardiaque presentant une surexpression d'un facteur 5, specifique au coeur, de la proteine G[α.sub.q]. Nous avons aussi mesure l'expression de la proteine calcineurine. Le rapport poids cardiaque-poids corporel a augmented chez les souris G[α.sub.q]. Les souris femelles de type sauvage ont montre une expression et une activation (translocation et phosphorylation) de la proteine eNOS plus fortes que celles des males de type sauvage. Le tissu cardiaque des souris G[α.sub.q] a presentee les memes differences, voire des differences plus prononceees, liees au sexe. L'expression proteique de la sous-unite catalytique, calcineurine A, qui dephosphoryle eNOS, a etee plus forte chez les males de type sauvage que chez les femelles de type sauvage. Ces differences ont ete plus importantes chez les souris G[α.sub.q]. Il y a des differences lieees au sexe dans l'expression et la phosphorylation de la proteine eNOS cardiaque. L'augmentation de l'activation de la proteine G[α.sub.q] modifierait l'expression et la phosphorylation de la proteine eNOS uniquement chez les males. Abstract

Hypertensive disorders are the most common medical complications in the peripartum period associated with a substantial increase in morbidity and mortality. Hypertension in the peripartum period may be due to the continuation of pre-existing or gestational hypertension, de novo development of pre-eclampsia or it may be also induced by some drugs used for analgesia or suppression of postpartum haemorrhage. Women with severe hypertension and hypertensive emergencies are at high risk of life-threatening complications, therefore, despite the lack of evidence-based data, based on expert opinion, antihypertensive treatment is recommended. Labetalol intravenously and methyldopa orally are then the two most frequently used drugs. Short-acting oral nifedipine is suggested to be used only if other drugs or iv access are not available. Induction of labour is associated with improved maternal outcome and should be advised for women with gestational hypertension or mild pre-eclampsia at 37 weeks' gestation. This position paper provides the first interdisciplinary approach to the management of hypertension in the peripartum period based on the best available evidence and expert consensus. © The Author(s) 2019.
