Objetivo: identificar mediante análise macroscópica e radiográfica as alterações estruturais em tecidos dentários afetados pela hiperbilirrubinemia, normalmente constatada a partir do sinal clínico de icterícia e provoca o desenvolvimento de pigmentos intrínsecos esverdeados nas estruturas dentárias. Materiais e Métodos: a amostra foi composta por 31 dentes decíduos dividida em grupo controle (n= 7) e grupo experimental (n= 24). As análises macroscópicas foram efetuadas por meio de fotografias individuais padronizadas e as radiográficas obtidas com aquisições de imagem a 9 mA, 70 kVp, distância de 8cm, exposição 0,4 segundos e com XDR Sensor®. Introduction: Obstructive sleep apnea syndrome is a common condition in childhood and if left untreated can result in many health problems. An accurate diagnosis of the etiology is crucial for obstructive sleep apnea treatment success. Functional orthodontic appliances that stimulate mandibular growth by forward mandibular positioning are an alternative therapeutic option in growing patients. Objective: To perform a literature review about the effects of functional orthodontic appliances used to correct the mandibular deficiency in obstructive sleep apnea treatment. Methods: The literature search was conducted in June 2020 using Cochrane Library; PubMed, EBSCO (Dentistry & Oral Sciences Source), LILACS Ovid; SciELO Web of Science; EMBASE Bireme and BBO Bireme electronic databases. The search included papers published in English, until June 2020, whose methodology referred to the types and effects of functional orthopedic appliances on obstructive sleep apnea treatment in children. Results: The search strategy identified thirteen articles; only four articles were randomized clinical studies. All studies using the oral appliances or functional orthopedic appliances for obstructive sleep apnea in children resulted in improvements in the apnea-hypopnea index score. The cephalometric (2D) and tomographic (3D) evaluations revealed enlargement of the upper airway and increase in the upper airspace, improving the respiratory function in the short term. Conclusion: Functional appliances may bean alternative treatment for obstructive sleep apnea, but it cannot be concluded that they are effective in treating pediatric obstructive sleep apnea. There are significant deficiencies in the existing evidence, mainly due to absence of control groups, small sample sizes, lack of randomization and no long-term results. Resumo Introdução: A síndrome da apneia obstrutiva do sono é uma condição comum na infância e, se não tratada, pode resultar em muitos problemas de saúde. Um diagnóstico preciso da etiologia é crucial para o sucesso do tratamento dessa condição clínica. Aparelhos ortodônticos funcionais que estimulam o crescimento mandibular através do anteroposicionamento mandibular são uma opção terapêutica para pacientes em crescimento. Objetivo: Fazer uma revisão da literatura sobre os efeitos do aparelho ortodôntico funcional usado para corrigir a deficiência mandibular no tratamento da apneia obstrutiva do sono. Método: A pesquisa bibliográfica foi feita em junho de 2020 nos os bancos de dados eletrônicos da Cochrane Library; PubMed, EBSCO (Dentistry & Oral Sciences Source), Lilacs Ovid; SciELO Web of Science; Embase Bireme e BBO Bireme. A busca incluiu artigos publicados em inglês, até junho de 2020, cuja metodologia referia-se aos tipos e efeitos dos aparelhos ortopédicos funcionais no tratamento da apneia obstrutiva do sono em crianças. Resultados: A estratégia de busca identificou 19 artigos; apenas quatro eram estudos clínicos randomizados. Todos os estudos que usaram aparelhos orais ou aparelhos ortopédicos funcionais para apneia obstrutiva do sono em crianças resultaram em melhorias no índice de apneia-hipopneia. As avaliações cefalométrica (2D) e tomográfica (3D) mostraram alargamento das vias aéreas superiores e aumento do espaço das vias aéreas superiores, que melhoraram a função respiratória em curto prazo. Conclusão: Os aparelhos funcionais podem ser um tratamento opcional para apneia obstrutiva do sono, mas não é possível concluir que sejam eficazes na população pediátrica. Existem deficiências significativas nas evidências existentes, principalmente devido à ausência de grupos de controle, tamanho pequeno das amostras, falta de randomização e ausência de resultados em longo prazo.

SUMMARY OBJECTIVE To assess the patient’s understanding of the informed consent form before and after plastic surgery. METHODS This was a prospective analytical descriptive study that utilized a questionnaire on informed consent before and after plastic surgery procedures. RESULTS Comprehension of informed consent was higher before surgery than after surgery (p=0.016; question 15). The higher the scholarity, the higher the comprehension (s=0.151; p=0.045) before surgery (question 4). For the other questions, it was not possible to find a difference in the pattern of understanding and in the association with the educational attainment level after surgery (s=0.180; p=0.046; question 1). CONCLUSIONS: The patients’ level of comprehension of the details, outcomes, possible complications, and postoperative evolutions of surgical procedures, as stated by the informed consent form, is high.

To investigate the short-term effects of rapid maxillary expansion (RME) on the quality of life of children who had persistent snoring post-adenotonsillectomy (AT).The study included children with maxillary constriction aged 5 to 12 years, two or more years after AT whose parents/guardians reported that they still snored ≥ 5 nights per week. We enrolled children with sleep-disordered breathing, including children with primary snoring and children with obstructive sleep apnea (OSA). All patients underwent laryngeal nasofibroscopy and complete polysomnography. Quality of Life (QOL) Questionnaire (OSA-18), the Pediatric Sleep Questionnaire (PSQ), Conners Abbreviated Scale (CAS), and the Epworth Sleepiness Scale (ESS) were administered before and after RME.Of 24 children enrolled, 13 had primary snoring and 11 had OSA. Overall OSA-18 scores were reduced in both groups (intragroup difference, p 0.001). The PSQ total score, CAS, and ESS were significantly reduced in both groups (p 0.001) In the evaluation of snoring, there was a reduction due to the treatment effect in both groups (p 0.001). Daytime sleepiness and attention deficit hyperactivity disorders were also positively affected in both groups.Our study demonstrated the potential benefit of RME in treating children with persistent snoring and transverse maxillary deficiency (TMD). RME can improve snoring and the QOL of children with refractory SDB after AT.ClinicalTrials.gov Identifier: RBR-463byn.

Introduction It has been hypothesized that increasing the interstitial hydrostatic pressure within the sinonasal mucosa of patients with nasal polyposis (NP) might decrease the size of nasal polyps. Objective To evaluate the effects of positive airway pressure, delivered by a continuous positive airway pressure (CPAP) device, in patients with NP and in control subjects. Methods Twelve patients with NP and 27 healthy subjects were exposed to CPAP (20 cm H2O) for 2 hours. Visual analog scale (VAS), Nasal Obstruction Symptom Evaluation (NOSE) scale, acoustic rhinometry (AR), peak nasal inspiratory flow (PNIF) and nasal endoscopy (NE—Meltzer polyp grading system) were performed before and after the intervention, for all patients. Results The control group showed a significant worsening in nasal obstruction symptoms, as measured by VAS and NOSE (p Conclusions Positive pressure worsened the nasal obstruction symptoms and decreased objective parameters of nasal patency in control subjects. In patients with NP, exposure to CPAP reduced the nasal polyps' size, and the nasal patency, as measured by PNIF. However, it had no significant effects in AR and in nasal obstruction symptoms.

The cone beam computed tomography (CBCT) image provides clear differentiation of soft tissues from empty spaces. This paper presents a literature review to evaluate the effects of orthopedic and surgical treatment on the pharyngeal dimension by CBCT. It was concluded that treatments involving dentofacial orthopedics and orthognathic surgery have been related with an increase in the upper airway volume. Standardized capturing of tomographic images and more controlled and randomized studies are necessary.

Introduction: The treatment of obstructive sleep apnea with positive airway pressure in children is restricted due to concerns that it could affect maxilla growth over time. Objective: To undertake a systematic review of the literature about the long-term impact of using a positive airway pressure mask on the midface in growing individuals. Methods: The literature search was conducted in September 2019 using the keywords (“long-term” OR “long term” OR “side effects” OR longitudinal) AND (children OR child OR preschool OR adolescents OR adolescent OR infant OR infants) AND (craniofacial OR “mid-face” OR midface OR midfacial OR facial OR maxillary) AND (“airway pressure” OR ventilation) in the databases PubMed, Web of Science and Lilacs. The search included papers published in English, until September 2019, on the effects of positive airway pressure on midfacial growth. Results: The search strategy identified five studies: two case reports, two cross-sectional studies and one retrospective cohort study. All studies evaluated the long-term effects of a using a nasal mask on the midface in children and adolescents; four showed midface hypoplasia and one no showed difference post- treatment compared to a control. Conclusion: Most of the studies demonstrated that long-term use of nasal positive airway pressure in childhood/adolescence is associated with midface hypoplasia. Objetivo: Realizar uma revisão sistemática da literatura sobre o impacto em longo prazo do uso de uma máscara de pressão positiva nas vias aéreas na face média em indivíduos em crescimento. Resultados: A estratégia de busca identificou cinco estudos: dois relatos de casos, dois estudos transversais e um estudo de coorte retrospectivo. Todos os estudos avaliaram os efeitos em longo prazo do uso de máscara nasal na face média em crianças e adolescentes; quatro apresentaram hipoplasia da face média e um paciente não mostrou diferença após o tratamento em comparação com um controle. Conclusão: A maioria dos estudos demonstrou que o uso prolongado da pressão positiva nas vias aéreas nasal na infância/adolescência está associado à hipoplasia da face média.

Introduction: Obstrutive sleep apnea syndrome is characterized by repeated episodes of upper airway obstruction, associated with intermittent hypoxia and hypercapnia, and the main risk factor in childhood is adenotonsillar hypertrophy. The lymphocytes in these structures are responsible for local and systemic immune responses. Objective: Verify the levels of the inflammatory markers, IL-1β, IL-4, IL-6, IL-8, IL-10, IL-15, TNF-α, CRP and α1-GP, in the tonsils of children with and without obstructive sleep apnea syndrome. Methods: This cross-sectional prospective study included 34 children with complains of snoring, difficulty breathing during sleep or recurrent tonsillitis. Patients underwent to a complete otorhinolaryngological examination, nasal endoscopy and polysomnography and were divided into two groups with 17 children each: obstructive sleep apnea syndrome group and control group. All underwent an adenotonsillectomy. Cytokines were measured in the collected tonsils (ELISA and Multiplex methods). Results: Statistically significant increasing were observed between IL-8 and IL-10 cytokines of patients with obstructive sleep apnea when compared to the control group; also between c-reactive protein and α1-GP of the tonsils cortical region in children with obstructive sleep apnea syndrome when compared with the medullary region. There were no statistically significant differences for the remaining inflammatory mediators. Conclusion: After the analysis of the levels of pro and anti-inflammatory markers (IL-1β, IL-4, IL-6, IL-8, IL-10, Il-15, TNF-α, CRP, α1-GP) in the tonsils, we observed higher levels of markers IL-8 and IL-10 in pediatric patients with obstructive sleep apnea syndrome. Resumo: Introdução: A síndrome da apneia obstrutiva do sono é caracterizada por episódios repetidos de obstrução das vias aéreas superiores, associados a hipóxia intermitente e hipercapnia, e o principal fator de risco na infância é a hipertrofia adenotonsilar. Os linfócitos nessas estruturas são responsáveis por respostas imunes locais e sistêmicas. Objetivo: Dosar os marcadores inflamatórios, IL-1β, IL-4, IL-6, IL-8, IL-10, IL-15, TNF-α, PCR e α1-GP, nas tonsilas de crianças com e sem síndrome da apneia obstrutiva do sono. Método: Estudamos prospectivamente 34 crianças que se queixavam de ronco, dificuldade para respirar durante o sono ou tonsilites recorrentes. Os pacientes foram submetidos a exame otorrinolaringológico completo, endoscopia nasal e polissonografia e foram divididos em dois grupos com 17 crianças cada: síndrome de apneia obstrutiva do sono e controle. Todos foram submetidos à adenotonsilectomia. As citocinas foram medidas nas tonsilas coletadas (métodos ELISA e Multiplex). Resultados: Com diferenças estatisticamente significantes, observou-se aumento das citocinas IL-8 e IL-10 em pacientes com apneia obstrutiva do sono em comparação ao grupo controle, assim como aumento dos níveis de proteína C reativa e de α1-GP na região cortical das tonsilas de crianças portadoras de síndrome da apneia obstrutiva do sono em comparação com a região medular. Não houve diferenças estatisticamente significantes para o restante dos mediadores inflamatórios. Conclusão: Após a análise dos níveis de marcadores pró e anti-inflamatórios (IL-1β, IL-4, IL-6, IL-8, IL-10, Il-15, TNF-α, PCR, α1-GP) nas tonsilas, observamos níveis mais altos de marcadores IL-8 e IL-10 em pacientes pediátricos com síndrome da apneia obstrutiva do sono. Introduction: Rapid maxillary expansion is an orthodontic and orthopedic procedure that can change the form and function of the nose. The soft tissue of the nose and its changes can influence the esthetics and the stability of the results obtained by this procedure. Objective: The objective of this study was to assess the changes in nose dimensions after rapid maxillary expansion in oral breathers with maxillary atresia, using a reliable and reproducible methodology through computed tomography. Methods: A total of 30 mouth-breathing patients with maxillary atresia were analyzed and divided into a treatment group who underwent rapid maxillary expansion (20 patients, 10 of which were male and 10 female, with a MA of 8.9 years and a SD of 2.16, ranging from 6.5 to 12.5 years) and a Control Group (10 patients, 5 of which were male and 5 female, with a MA of 9.2 years, SD of 2.17, ranging from 6.11 to 13.7 years). In the treatment group, multislice computed tomography scans were obtained at the start of the treatment (T1) and 3 months after expansion (T2). The patients of the control group were submitted to the same exams at the same intervals of time. Four variables related to soft tissue structures of the nose were analyzed (alar base width, alar width, height of soft tissue of the nose and length of soft tissue of the nose), and the outcomes between T1 and T2 were compared using Osirix MD software. Results: In the TG, the soft tissues of the nose exhibited significant increases in all variables studied (p 0.05). In the treatment group, mean alar base width increased by 4.87% (p = 0.004), mean alar width increased by 4.04% (p = 0.004), mean height of the soft tissues of the nose increased by 4.84% (p = 0.003) and mean length of the soft tissues of the nose increased by 4.29% (p = 0.012). Conclusion: In short-term, rapid maxillary expansion provided a statistically significant increase in the dimensions of the soft tissues of the nose. Resumo: Introdução: A expansão rápida da maxila é um procedimento ortodôntico e ortopédico que pode alterar a forma e a função do nariz. Os tecidos moles do nariz e suas alterações podem influenciar na estética e na estabilidade dos resultados obtidos por esse procedimento. Objetivo: O objetivo desse estudo foi avaliar as alterações nas dimensões do nariz após expansão rápida da maxila em respiradores orais com atresia maxilar, usando uma metodologia confiável e reprodutível com o auxílio de tomografia computadorizada. Método: Um total de 30 pacientes respiradores orais com atresia maxilar foram avaliados e divididos em um grupo de tratamento, submetidos à expansão rápida da maxila (20 pacientes, 10 dos quais do sexo masculino e 10 do sexo feminino, com média de idade de 8,9 anos e DP de 2,16, variando de 6,5 a 12,5 anos) e um grupo controle (10 pacientes, sendo 5 do sexo masculino e 5 do sexo feminino, com média de idade de 9,2 anos, DP de 2,17, variando de 6,11 a 13,7 anos). No grupo tratado, foram realizados exames de tomografia computadorizada multislice no início do tratamento (T1) e 3 meses após a expansão (T2). Os pacientes do grupo controle foram submetidos aos mesmos exames nos mesmos intervalos de tempo. Foram analisadas quatro variáveis relacionadas às estruturas dos tecidos moles do nariz (largura da base alar, largura alar, altura do tecido mole do nariz e comprimento do tecido mole do nariz) e os resultados entre T1 e T2 foram comparados, utilizando-se o software Osirix MD. Resultados: No grupo tratado (GT), os tecidos moles do nariz apresentaram aumentos significativos em todas as variáveis estudadas (p 0,05). No GT, a largura média da base alar aumentou 4,87% (p = 0,004), a largura média alar aumentou 4,04% (p = 0,004), a altura média dos tecidos moles do nariz aumentou 4,84% (p = 0,003) e o comprimento médio dos tecidos moles do nariz aumentou 4,29% (p = 0,012). Conclusão: A curto prazo, a expansão rápida da maxila proporcionou um aumento estatisticamente significativo nas dimensões dos tecidos moles do nariz. Objective: To assess the volumetric changes that occur in the nasomaxillary complex of mouth-breathing patients with transverse maxillary deficiency subjected to rapid maxillary expansion (RME). Methods: This was a controlled, prospective intervention study involving 38 mouth-breathing patients presenting with transverse maxillary deficiency, regardless of malocclusion type or race. Twenty-three of them comprised the experimental group, which was composed of 11 (47.8%) boys, and 12 (52.2%) girls, with a mean age of 9.6 years, ranging from 6.4 to 14.2 years and standard deviation of 2.3 years; and 15 of them comprised the control group, composed of 9 (60%) boys and 6 (40%) girls with an mean age of 10.5 years, ranging from 8.0 to 13.6 years, and standard deviation of 1.9 years. All patients were scanned (CT) according to a standard protocol: Initial CT (T1), and CT three months thereafter (T2), and the patients in the experimental group were treated with RME using a Hyrax expander for the correction of maxillary deficiency during the T1-T2 interval. The CT scans were manipulated using Dolphin® Imaging version 11.7 software for total and partial volumetric assessment of the nasomaxillary complex. Results: The results revealed that in the experimental group there was a significant increase in the size of the structures of interest compared to the control group, both in general aspect and in specific regions. Conclusions: Rapid maxillary expansion (RME) provided a significant expansion in all the structures of the nasomaxillary complex (nasal cavity, oropharynx, right and left maxillary sinuses). RESUMO Objetivo: avaliar as alterações volumétricas do complexo nasomaxilar em pacientes respiradores bucais com deficiência transversal da maxila tratados com expansão rápida da maxila (ERM). Métodos: foram selecionados 38 pacientes respiradores bucais, independentemente da má oclusão ou raça, portadores de constrição maxilar, e realizou-se um estudo de intervenção, prospectivo, controlado. Para o grupo experimental, foram selecionados 23 pacientes, sendo 11 (47,8%) meninos e 12 (52,2%) meninas, com média de idade de 9,6 anos (variando de 6,4 a 14,2 anos) e desvio-padrão de 2,3 anos; e 15 pacientes para o grupo controle, sendo 9 (60%) meninos e 6 (40%) meninas, com média de idade de 10,5 anos (variando de 8,0 a 13,6 anos) e desvio-padrão de 1,9 anos. Todos os pacientes realizaram exames de tomografia computadorizada (TC), de acordo com protocolo padronizado: TC inicial (T1) e TC após três meses do período de contenção (T2). Os pacientes do grupo experimental foram tratados com expansão rápida da maxila usando um expansor Hyrax para a correção da deficiência transversal durante o intervalo T1-T2. As imagens tomográficas para avaliação volumétrica total e parcial do complexo nasomaxilar foram manipuladas no programa Dolphin ® versão 11.7. Resultados: os resultados revelaram que no grupo experimental houve um aumento significativo no tamanho das estruturas de interesse em comparação com o grupo controle, tanto no geral quanto nas regiões específicas. Conclusões: a expansão rápida da maxila (ERM) promoveu uma expansão significativa de todas as estruturas do complexo nasomaxilar (cavidade nasal, orofaringe, seios maxilares direito e esquerdo).

BACKGROUND Continuous airflow in the upper airway can cause discomfort, leading to nasopharyngeal complaints. The aim of the present study is to evaluate the acute effects of continuous positive upper-airway pressure on nasal patency in awake normal subjects. METHODS Twenty-seven adults (17 men; 10 women; age range, 18 to 43 years) were exposed to continuous airway pressure (20 cmH2 O) in the nasal cavity, delivered by a continuous positive airway pressure (CPAP) device through a nasal mask for 2 hours. Visual analogue scale (VAS) of nasal obstruction, Nasal Obstruction Symptom Evaluation (NOSE) modified scale, acoustic rhinometry (AR), and peak nasal inspiratory flow (PF) were measured before and after the use of CPAP. RESULTS There was an increase in nasal obstruction scores both on the VAS (p < 0.05) and on the NOSE scale (p < 0.05), as well as a reduction in nasal cavity volume on AR (p < 0.05) and a decline in PF (p < 0.05). Subjects were stratified into 2 groups: with and without symptoms of allergic rhinitis. Nasal parameters were significantly worse in the rhinitis group, with higher nasal obstruction scores on the VAS (p = 0.001) and NOSE scale (p < 0.001) and decreased PF (p < 0.001). CONCLUSION Acute exposure to positive pressure via CPAP is associated with subjective and objective reductions in nasal patency. In individuals with allergic nasal symptoms, deterioration is even more severe than in patients without these symptoms.

Introduction Nasal congestion and obstruction are reported in the majority of continuous positive airway pressure users and are frequently cited as reasons for noncompliance. Baseline inflammation due to allergic rhinitis could increase or exacerbate the inflammatory effect of high airflow in the nasal cavity as the result of continuous positive airway pressure and lead to greater continuous positive airway pressure intolerance. In this setting, intranasal steroids would be expected to counteract the nasal inflammation caused by allergic rhinitis and/or continuous positive airway pressure. Objective The aim of the present study is to evaluate the effects of topical corticosteroid use on nasal patency after acute exposure to positive pressure. Methods Ten individuals with allergic rhinitis were exposed to 1 h of continuous airway pressure (15 cm H2O) in the nasal cavity, delivered by a continuous positive airway pressure device. Visual analog scale, nasal obstruction symptom evaluation scale, acoustic rhinometry and peak nasal inspiratory flow were performed before and after the intervention. After 4 weeks topical nasal steroid (budesonide) application, positive pressure exposure was repeated as well as the first assessments. Results Patients reported a statistically significant improvement both on the visual analog (p = 0.013) and obstruction symptom evaluation scales (p < 0.01). Furthermore, objective measurements were improved as well, with increased nasal cavity volume on acoustic rhinometry (p = 0.02) and increased peak nasal inspiratory flow (p = 0.012), after corticosteroid treatment. Conclusion In patients with allergic rhinitis, intranasal corticosteroid therapy improved objective and subjective parameters of nasal patency after acute exposure of the nasal cavity to positive pressure.

Introduction The importance of our study lies in the fact that we have demonstrated the occurrence of mechanical dysfunction within polypoid tissues, which promotes the development of polyps in the nasal cavity. Objective To change the paradigm of nasal polyposis (NP). In this new conception, the chronic nasal inflammatory process that occurs in response to allergies, to pollution, to changes in the epithelial barrier, or to other factors is merely the trigger of the development of the disease in individuals with a genetic predisposition to an abnormal tissue remodeling process, which leads to a derangement of the mechanical properties of the nasal mucosa and, consequently, allows it to grow unchecked. Data Synthesis We propose a fundamentally new approach to intervening in the pathological process of NP, addressing biomechanical properties, fluid dynamics, and the concept of surface tension. Conclusion The incorporation of biomechanical knowledge into our understanding of NP provides a new perspective to help elucidate the physiology and the pathology of nasal polyps, and new avenues for the treatment and cure of NP.

Introduction: Maxillary constriction is a dentoskeletal deformity characterized by discrepancy in maxilla/mandible relationship in the transverse plane, which may be associated with respiratory dysfunction. Objective: The objective of this study was to evaluate the skeletal effects of RME on maxillary and nasal transverse dimensions and compare the differences between males and females. Methods: Sixty-one mouth-breathers patients with skeletal maxillary constriction (35 males and 26 females, mean age 9.6 years) were included in the study. Posteroanterior (PA) radiographs were taken before expansion (T1) and 3 months after expansion (T2). Data obtained from the evaluation of T1 and T2 cephalograms were tested for normality with the Kolmogorov-Smirnov method. The Student’s t-test was performed for each measurement to determine sex differences. Results: RME produced a significant increase in all linear measurements of maxillary and nasal transverse dimensions. Conclusions: No significant differences were associated regarding sex. The RME produced significant width increases in the maxilla and nasal cavity, which are important for treatment stability, improving respiratory function and craniofacial development. RESUMO Introdução: a constrição maxilar é uma alteração dentoesquelética relacionada à diminuição transversal da arcada superior, que pode correlacionar-se com problemas respiratórios. O tratamento nos pacientes em crescimento inclui a expansão rápida da maxila (ERM). A correção precoce resulta em maiores alterações esqueléticas e estabilidade dos resultados, podendo evitar desvios de crescimento facial. Objetivo: avaliar comparativamente, por meio de telerradiografias posteroanteriores (PA), as alterações dimensionais da cavidade nasal pré- e pós-ERM em pacientes respiradores bucais dos sexos masculino e feminino. Métodos: realizou-se o estudo de medidas lineares em telerradiografias PA pré- e pós-ERM de uma amostra composta por 61 pacientes (35 do sexo masculino e 26 do feminino), com média de idade de 9,6 anos. Todos os pacientes eram respiradores bucais com constrição maxilar esquelética, e foram tomadas radiografias PA pré- e pós-ERM (3 meses). Os dados obtidos foram avaliados nos tempos T1 e T2 a normalidade dos dados foi confirmada por meio do teste de Kolmogorov-Smirnov. O teste t de Student foi aplicado para cada mensuração, para determinar diferenças entre os sexos. Resultados: a ERM promoveu aumento significativo em todas as medidas lineares maxilares e das dimensões da cavidade nasal. Conclusão: nenhuma diferença significativa foi associada ao sexo. A ERM produziu aumentos significativos na largura maxilar e volume da cavidade nasal, que são importantes na estabilidade do tratamento Nos pacientes em crescimento e respiradores bucais com constrição maxilar esquelética da maxila, a ERM promoveu aumento do volume da cavidade nasal que melhorou o fluxo aéreo e possibilitou reconduzir o crescimento facial. As alterações transversais não foram significativas quando relacionadas ao sexo.

To evaluate the evolution of polysomnographic parameters of children with Down syndrome and obstructive sleep apnea syndrome submitted to adenotonsillectomy and the interaction of comorbidities on therapeutic outcome.Ninety patients with Down syndrome and habitual snoring were identified between 2005 and 2015 in a Pediatric Otorhinolaryngology Clinic. Parent's complaints were evaluated by the test of equality of two proportions. Wilcoxon test was used to examine pre- and post-operative polysomnographic differences. Mann-Whitney test evaluated the influence of comorbidities. A p 0.05 was considered significant.A total of 27 patients met the inclusion criteria (55.6% patients were males; mean (SD) age were 6.7 (3.6) years (range, 1.5-16 years). Significant improvement of parent's complaints (p 0.001), arousal index (p = 0.045), and minimum oxygen saturation were observed post-adenotonsillectomy (p = 0.034). Adenotonsillectomy was able to resolve obstructive sleep apnea syndrome in 29.6% of children with Down syndrome. Nineteen patients (70.4%) remained with obstructive sleep apnea syndrome and 44.4% showed a reduction of at least 50% of obstructive apnea-hypopnea index. Central apnea index post-adenotonsillectomy was worse in patients with heart disease (p = 0.022). Sleep efficiency (p = 0.031), N1 sleep stage (p = 0.001), apnea-hypopnea index (p = 0.023), and central apnea index (p = 0.008) were worse after surgery in patients with hypothyroidism. Patients with severe OSAS showed significant improvement in polysomnographic parameters after surgery.Although adenotonsillectomy improved symptoms and objective sleep data in children with Down syndrome, it was not able to resolve obstructive sleep apnea syndrome in most patients. Congenital heart diseases and hypothyroidism may affect the outcome.

Objective To evaluate otolaryngologic findings in obese prepubertal children with sleep-disordered breathing. Methods We prospectively evaluated 29 obese children referred by pediatric endocrinologist, complaining of snoring and without a history of nasal surgery or removal of the palatine tonsils and/or adenoids. Patients underwent ear, nose and throat (ENT) examination, endoscopy, measurements of weight, height, calculation of body mass index (BMI), assessment of BMI z-score and polysomnography, from which were divided into two groups: those with obstructive sleep apnea syndrome (nine children) and those with primary snoring (20 children). Then we proceeded to the statistical analysis of the data collected. Results The groups did not differ in age, gender, weight, height, BMI and BMI z-score. Among the findings of the ENT examination, the adenoid size was the only one that differed between the groups (p = 0.01). Conclusion The evaluation of the adenoid size is an important in obese children with symptoms of sleep-disordered breathing and is related to the presence of obstructive sleep apnea syndrome.

Introduction Rapid maxillary expansion can change the form and function of the nose. The skeletal and soft tissue changes can influence the esthetics and the stability of the results obtained by the procedure. Objective The aim of this retrospective study was to evaluate the short-term effects of rapid maxillary expansion on the skeletal and soft tissue structures of the nose, in mouth-breathing patients, using a reliable and reproducible, but simple methodology, with the aid of computed tomography. Methods A total of 55 mouth-breathing patients with maxillary hypoplasia were assessed and were divided into an experimental group treated with rapid maxillary expansion(39 patients, 23 of which were male and 16 female, with an average age of 9.7 years and a standard deviation of 2.28, ranging from 6.5 to 14.7 years) and a control group (16 patients, 9 of which were male and 7 female, with an average age of 8.8 years, standard deviation of 2.17, ranging from 5.11 to 13.7 years). The patients of the experimental group were submitted to multislice computed tomography examinations at two different points in time: (T1) pre-rapid maxillary expansion and (T2) three months after the procedure. The control group underwent to the same exams at the same intervals of time. Four skeletal and soft tissue variables were assessed, comparing the results of T1 and T2. Results There was in the experimental group a significant increases in all the skeletal and soft tissue variables (p < 0.05) but no significant alteration was found in the control group. When comparing the experimental group and the control group, the most important change occurred in the width of the pyriform aperture (p < 0.001). Conclusion Rapid maxillary expansion is capable of altering the shape and function of the nose, promoting alterations in skeletal and soft tissue structures. This kind of study may, in the future, permit the proper planning of esthetic procedures at the tip and base of the nose and also the performance of objective measurements in early or late surgical outcomes.

Few studies in the literature have looked into the cardiovascular and metabolic effects of Obstructive Sleep Apnea Syndrome (OSAS) in children. OBJECTIVE: This study aims to evaluate the metabolic profile of non-obese children with OSAS. METHODS: Fifty-two children were enrolled in this study, 21 girls and 31 boys. Patients were divided into two groups: OSAS (28 children) and Snore (22 children) according to polysomnographic evaluation. All children were submitted to ENT examination, measurements of weight, height and blood pressure. Blood samples were tested for hemoglobin, hematocrit, fasting glucose, fasting insulin, triglycerides, total cholesterol, HDL, LDL, VLDL, TSH and T4. The gathered data sets were compared between groups and also within the OSAS group according to the severity of the syndrome. RESULTS: The children from both groups had no alterations in blood pressure levels. The results of the blood tests were normal for both groups. Results of hemoglobin, hematocrit, and HDL were all significantly higher in the Snore group when compared to the OSAS group; by their turn, VLDL levels were higher in the OSAS group. There was no statistical difference between the groups based on OSAS severity. CONCLUSION: Non-obese children with OSAS present no significant alterations in metabolic tests or blood pressure levels.

Indicação de traqueostomia passou por transformações e sua complicação são mais frequentes em crianças. OBJETIVO: Avaliar as indicações e complicações de traqueostomias realizadas em crianças. MATERIAL E MÉTODO: Estudo retrospectivo, com revisão de prontuários de pacientes com idade entre 1 dia de vida e 16 anos submetidos à traqueostomia em um hospital universitário durante o período de agosto de 2000 a julho de 2008. Foram avaliados dados referentes à idade, gênero, indicações e complicações intra e pós-operatórias. RESULTADOS: Cinquenta e oito crianças, menores de 16 anos, foram submetidas à traqueostomia durante o período estudado. A média de idade foi de 3.7 anos. Obstrução de via aérea foi a principal indicação cirúrgica (n=40; 69%). A incidência de complicações no pós-operatório foi de 19% (11 pacientes), sendo a maioria no pós-operatório tardio. A complicação mais observada foi obstrução de cânula. Não houve complicações inerentes ao procedimento durante o intra-operatório. CONCLUSÃO: As principais indicações de traqueostomia em crianças foram obstrução de via aérea e IOT prolongada. As complicações pós-operatórias mais frequentes foram obstrução de cânula e decanulação acidental. As indicações e complicações observadas são semelhantes às relatadas por outros serviços na literatura.

Cresce o número de queixas formalizadas contra atitudes médicas. É grande o impacto social dessas denúncias. Faltam estudos, no Brasil e em Goiás, que lhes dêem significado. Este trabalho pretende quantificar e qualificar as reclamações apresentadas ao Conselho Regional de Medicina de Goiás contra médicos. MÉTODOS: Estudo descritivo, retrospectivo, sobre as denúncias formalizadas em Goiás, entre 2000 e 2006. Leitura interpretativa da evolução processual das queixas ajuizadas no Conselho e cálculo da eficácia das ações dali decorrentes. RESULTADOS: Não foi grande a flutuação na frequência de reclamações entre 2000 e 2006; 62% das queixas alegaram incompetência do profissional e inadequada relação médico/paciente. O número de queixas em cirurgia plástica e ortopedia são iguais a 50% dos especialistas. Houve 73 denúncias contra quatro profissionais da cirurgia plástica e um médico foi denunciado 49 vezes. Em 60% dos casos a denúncia foi feita por pessoa física. Discussão: Foram consideradas improcedentes 17% das denúncias e 35% das restantes se transformaram em processos éticos (10% arquivados). O julgamento levou à advertência e censura em alguns casos e suspensão (5%) e cassação do direito (3%). Mais de 90% dos casos anuais foram resolvidos. CONCLUSÃO: O problema tem sido abordado com eficácia e eficiência, apesar de graves imperfeições na gestão do banco de dados, que impedem análises qualitativas da questão. The social impact of these accusations is great, but studies in Brazil and in the state of Goiás are not sufficient to make them meaningful. This work was intended to quantify and qualify complaints submitted to the Regional Medical Council of the state of Goiás against medical acts. METHODS: Descriptive, retrospective study of the processes formally registered in the state of Goiás between 2000 and 2006; interpretative reading of the procedural development of filed complaintes at the Regional Medical Council of the state of Goiás and calculation of the efficacy of the ensuing actions. RESULTS: Variation in the complaint frequency between 2000 and 2006 was not high; 62% of complaints concerned professional incompetence and inadequate patient -physician relationship. The number of complaints regarding plastic surgery and orthopedics was significant. In 60% of cases accusations were made by individuals. DISCUSSION: The Regional Medical Council session resulted in warnings and reprimands in some cases, deferment (5%), and suspension of the licence to practice medicine (3%). Over 90% of the yearly cases were solved. CONCLUSION: This problem has been effectively and efficiently approached, in spite of severe imperfections in data base management, preventing qualitative analyses of this matter.

Os distúrbios obstrutivos do sono são relativamente freqüentes na população pediátrica, porém o impacto da perda do sono na aprendizagem e função cognitiva não está bem estabelecido. OBJETIVO: Avaliar se pacientes com distúrbios obstrutivos do sono apresentam alteração de aprendizagem, memória e atenção. CASUÍSTICA E MÉTODO: Foram avaliadas 81 crianças de 6 a 12 anos de idade, divididas em 3 grupos: grupo SAHOS (n=24), grupo Ronco Primário (n=37) e grupo Controle (n=20), através de testes de aprendizagem (Teste de Rey) e cognitivos (Dígito, Código, Cancelamento de Letras e Símbolos). Todas as crianças realizaram polissonografia. RESULTADOS: O grupo SAHOS (n=24) e o grupo Ronco Primário (n=37) apresentaram diferença estatisticamente significante nas variáveis A1 (p=0,001) do Teste de Rey quando comparados ao grupo controle. O grupo Ronco Primário apresentou ainda diferenças nas variáveis A2, A4, AT e A6 do Teste de Rey (p=0,020; p=0,05; p=0,004; p=0,05, respectivamente) em relação ao grupo controle (n=20). CONCLUSÃO: Crianças com distúrbios obstrutivos do sono apresentam piores resultados no teste de aprendizagem e memória (Teste de Rey), principalmente o grupo RP, quando este é comparado ao grupo SAHOS. Os testes de atenção apresentam resultados semelhantes entre os grupos.Sleep obstructive breathing disorders are frequent in children but the impact of sleep deprivation on the cognitive learning function is unclear. AIM: To establish whether patients with sleep obstructive breathing disorders show any functional change in learning, memory and attention. MATERIAL AND METHODS: Eighty-one children aged from 6 to 12 years were divided into 3 groups: obstructive sleep apnea syndrome (OSAS), n=24; primary snoring (PS), n=37; and control, n=20. The groups were assessed using learning (Rey) and psychological (Digit, Code, Letter Concealing, and Symbol) tests. RESULTS: OSAS and PS children showed statistically significant worse performance on the variable A1 in Rey test (learning and memory) when compared with controls (p=0.011). PS children had an even worse performance on the variables A2, A4, AT and A6 compared to OSAS participants and controls (p=0.020;p=0.050;p=0.004,p=0.05). Conclusion: Children with obstructive sleep breathing disorders, in particular PS, show worse Rey test scores. PS and OSAS children performed similarly in attention tests.

Background Acoustic rhinometry indirectly calculates cross-sectional areas of the nose at various locations from the nostrils and the total volume of the nasal cavity. These parameters are calculated from an area-distance curve displayed by the device. Methods Seventy-seven adults between the ages of 20 and 40 years who had no nasal complaints were examined by acoustic rhinometry and fiberoptic nasal endoscopy in an attempt to compare and correlate areas of nasal narrowing detected by these two methods. Results The comparison between the distances to the various narrowed areas revealed a statistically significant correlation between the first three constrictions detected by acoustic rhinometry and the first three areas of anatomic narrowing measured by fiberoptic nasal endoscopy in both nares but no correlation between the fourth constriction and the fourth measurement on either side. Conclusion The results confirm an anatomic correlation between the two examination methods, identifying the first constriction as the nasal valve, the second constriction as the head of the inferior nasal turbinate, and the third constriction as the head of the middle turbinate. The fourth constriction did not correspond to the choanal region.

OBJETIVO: Verificar se existe relação entre a presença da Síndrome da Apnéia/Hipopnéia Obstrutiva do Sono e alteração de processamento auditivo. MÉTODOS: Foram avaliados 3 grupos de crianças na faixa etária de 5 a 11 anos, incluindo um grupo controle. Vinte indivíduos que constituíram o grupo de estudo foram submetidos à avaliação otorrinolaringológica e ao exame de polissonografia (PSG) e foram divididos em dois grupos: Grupo I (RO) constituído por 10 indivíduos respiradores orais e PSG normal e Grupo II (SAS) constituído por 10 indivíduos respiradores orais e PSG alterado. O desempenho destes indivíduos foi comparado a um terceiro grupo - Grupo III (REN) composto de 10 crianças sem queixas otorrinolaringológicas. Todos os sujeitos foram submetidos à avaliação audiológica básica e à avaliação do PA. RESULTADOS: Quanto à avaliação otorrinolaringológica, houve diferença estatisticamente significante no que se refere à avaliação dos cornetos nasais e das tonsilas palatinas. No grupo II, verificou-se maior número de indivíduos com hipertrofia de cornetos nasais grau II e III (p < 0,001) e com hipertrofia de tonsila palatina grau III e IV (p 0,007). Em relação à avaliação de PA, os resultados mostraram que houve diferença estatisticamente significante (p < 0,001) no teste dicótico de dígitos, tendo o grupo II apresentado pior desempenho do que o grupo III e, no resultado do teste de memória para sons não-verbais em seqüência (p < 0,022), tendo o grupo II apresentado pior desempenho do que o grupo I. Twenty subjects who made up the study group were submitted to ear, nose and throat (ENT) exams and to polysomnography (PSG), and were divided in two groups: GROUP I (RO) comprised of 10 children who presented oral breathing and displayed normal PSG, and GROUP II (SAS) comprised of 10 children who presented oral breathing and displayed abnormal PSG. Their performance was compared to the performance of the third group - GROUP III (REN) comprised of 10 children who did not refer ENT difficulties. All the subjects completed a basic audiometric assessment as well as an auditory processing diagnosis. RESULTS: The analyses of the results revealed a statistically significant difference in ENT exams related to the turbinate and the palatine tonsils. Group II presented a higher incidence of turbinate hypertrophy levels II and III (p < 0.001) and palatine tonsils hypertrophy grades III and IV (p 0.007). Regarding the auditory processing assessment, a statistically significant difference (p < 0.001) was obtained in the dichotic digits test. Group II performed worse than group III. Also, for the non-verbal sequence memory test, Group II obtained a worse result (p < 0.022) than Group I. CONCLUSIONS: Subjects with OSAS obtained worse results in auditory processing tests.

Also, for the non-verbal sequence memory test, Group II obtained a worse result (p < 0.022) than Group I.ConclusionSubjects with OSAS obtained worse results in auditory processing tests.

Summary Polysomnography is the goldstandard exam for child OSAS. When possible, polysomnography clearly distinguishes between those with isolated primary snoring and patients with sleep apnea (obstructive, central and mixed). The most common cause of OSAS in childhood is adenotonsillar hypertrophy. Laryngomalacia is the most common cause of stridor in childhood, though its physiopathology remains unknown. Among the most prominent theories are immaturity of the cartilaginous framework of the larynx and/or neuromuscular immaturity. Objective: Our proposal was to describe polysomnographic findings in children with laryngomalacia or other isolated laryngeal alterations, that is, without other alterations in the upper airways. Methods: The sample included 29 children with exclusively laryngeal alterations. All of them underwent an otorhinolaryngological exam, nasofibrolaryngoscopy and polysomnography. Information was recorded concerning age, nasofibrolaryngoscopy and polysomnography. For analysis, the children were divided into two groups: those with laryngomalacia and those with other laryngeal diseases. Results: Among the 18 children with a diagnosis of laryngomalacia, 18 had central breathing events, knowing that the majority had showed dessaturation of oxihemoglobin and bradicardia. In this same group, 3 children had obstrutives events. On the other hand, 11 children with other laryngeal alterations showed no predominance of one type or another of apnea. Of these, 4 had central type breathing events and 2 obstructive type. Conclusion: The majority of patients with laryngomalacia showed a central type apnea. Patients with various laryngeal diseases did not present a predominant type of apnea.
