ZusammenfassungAdolf Kussmaul (1822–1902) hat in Heidelberg Medizin studiert. Der Name Kussmaul ist v.a. durch die Panarteriitis nodosa, den Pulsus paradoxus und den Venenpuls bei schwieliger Mediastinopericarditis sowie die hochfrequente und tiefe Atmung bei diabetischer Ketoazidose weltweit bekannt geworden. Kussmaul war auch Wegbereiter der endoskopischen Diagnostik und Therapie von Erkrankungen des Ösophagus und des Magens. Seine ärztliche Tätigkeit als Landarzt, die er in Kandern im Schwarzwald 1850 begonnen hat, musste er wegen gesundheitlicher Probleme nach drei Jahren beenden. Kussmaul hat dann den klinisch-wissenschaftlichen Weg beschritten. 1855 promovierte er in Würzburg bei Virchow und habilitierte in Heidelberg. Von 1859–1888 war er in Erlangen, Freiburg und Straßburg als ordentlicher Professor tätig. Nach der Emeritierung hat Kussmaul in Heidelberg weiterhin als Arzt gearbeitet und seine „Jugenderinnerungen eines alten Arztes“ verfasst.

ZusammenfassungUnter „Nasenzyklus“ versteht man die am Tage und im Schlaf wechselweise auftretenden spontanen Schwellungszustände der beiden Nasengänge bei gleichbleibendem nasalen Gesamtfluss. Der Nasenzyklus wurde erstmals 1895 von R. Kayser beschrieben. Ein Nasenzyklus besteht aus einer Ruhe- und einer Arbeitsphase. In der Ruhephase kommt es zur Schleimhautschwellung mit Regeneration und Rehydrierung. Etwa 70–90% der Menschen weisen einen nasalen Zyklus auf. Die im Schlaf ablaufenden Nasenzyklen sind hinsichtlich der Phasendauer länger als die am Tage. Unterschiedliche Faktoren wie Alter, Schlaf, Körperposition, körperliche Aktivität und mukoziliäre Clearance nehmen Einfluss auf den nasalen Zyklus. Die Langzeit-Rhinoflowmetrie ermöglicht eine Beurteilung des zirkadianen Verlaufs der wechselweise auftretenden zyklischen Veränderungen der Nasenschleimhaut.

Zusammenfassung Einleitung Die nächtliche kontinuierliche positive Überdruckatmung (CPAP) gilt als therapeutischer Goldstandard einer obstruktiven Schlafapnoe (OSA). Die CPAP-Therapie stellt zumeist eine Dauertherapie mit geringen Nebenwirkungen dar. Trotzdem ist die Therapietreue unbefriedigend. In dieser Arbeit wurden die Therapietreue betrachtet und typische Probleme katalogisiert. Methode 1078 OSA-Patienten wurden nach diagnostischer Polygrafie (PG) oder Polysomnografie (PSG) auf eine CPAP-Therapie eingestellt. Die Therapietreue wurde nach 3 Monaten nachverfolgt. Folgende Therapieadhärenz-Gruppen wurden festgelegt: 1. gute CPAP-Einstellung und Nutzung, 2. CPAP-Abbruch nach anfänglicher Nutzung, 3. mangelnde CPAP-Nutzung wegen Nebenwirkungen, 4. Abbruch aufgrund fehlender Motivation/geringem Leidensdruck, 5. Masken-Unverträglichkeit, 6. CPAP-Versagen aufgrund fehlenden Therapieeffekts, 7. Umstellung auf anderes Beatmungsverfahren, 8. keine Kontrolle erfolgt. Ergebnisse Von 1078 CPAP-Patienten erschienen 830 Patienten (77 %) zur Therapiekontrolle. Hiervon waren 450 Patienten (54,2%) in Gruppe 1, 216 Patienten (26%) in Gruppe 2, 71 Patienten (8,5%) in Gruppe 3, 35 Patienten (4,2%) in Gruppe 4, 14 Patienten (1,7%) in Gruppe 5, 3 Patienten (0,4 %) in Gruppe 6 und 41 Patienten (4,9%) in Gruppe 7. Ein geringer obstruktiver Ereignisindex, niedriger CPAP-Druck und tendenziell auch ein geringer Epworth-Sleepiness-Score waren Prädiktoren für CPAP-Versagen. Für die Therapietreue konnten keine signifikanten Prädiktoren dargestellt werden. Diskussion Eine effektive Therapienutzung von 54% nach 3 Monaten ist ein suboptimales Ergebnis. Prädiktoren für CPAP-Versagen waren Parameter, die vor Therapie auf geringen Leidensdruck der Patienten deuteten. ZusammenfassungDie Blutversorgung der Lunge besteht aus dem pulmonal- und dem bronchial-arteriellen System. Die bronchial-arteriellen Gefäße sind an der nutritiven Versorgung der kleinen Atemwege bis hin zu den terminalen Bronchiolen beteiligt. Zudem sind sie bedeutsam für die Regulation der Atemwegstemperatur, der -feuchtigkeit sowie der mukoziliären Clearance. Eine chronische Ischämie der kleinen Atemwege durch Verlust oder Verletzung der bronchial-arteriellen Versorgung erhöht das Risiko für eine Fibrosierung der kleinen Atemwege (obliterative Bronchiolitis), im Speziellen bei einer Lungentransplantation (LTx). Wenngleich sich die Überlebensrate nach LTx im Verlauf verbessert hat, ist sie noch immer deutlich schlechter als bei anderen Organtransplantationen mit einer 5-Jahres-Überlebensrate von 50 bis 60%. Es ist zu vermuten, dass eine bronchial-arterielle Revaskularisierung zum Zeitpunkt der LTx eine wichtige transplantaterhaltende Funktion hat.

Introduction Clinical symptoms of Diffuse Parenchymal Lung Disease (DPLD) are dyspnoea and dry cough. On auscultation, crackles can be detected at an early stage. The aim of this study was to analyse whether the extent of abnormal breath sounds (crackles, wheezes and coughs) provides an indication of the disease severity and aetiology. Methodology In 25 patients with DPLD, an in- hospital overnight recording of physiological and abnormal breath sounds was performed with the LEOSound® monitor. The severity of pulmonary fibrosis was assessed by body plethysmography and GAP score. The perceived breathlessness was assessed by King's Brief Interstitial Lung Disease (K-Bild) questionnaire. Results In 16 out of 25 patients, all data were completed in good quality for analysis. Five patients were women (31.3%). The patients’ mean age was 65.8 ± 7.3 years, the mean BMI 28.0 ± 2.4 kg/m2. One patient met GAP I criteria, eight patients GAP II and seven patients met GAP III criteria. The findings of eight patients with idiopathic pulmonary fibrosis (IPF) were compared with those of eight patients with secondary interstitial lung disease (ILD). Both, the number of inspiratory crackles and coughs showed significant correlations with the severity of dyspnoea and ILD-pattern. Wheezing occurred in 15 patients, with a median of 25.7 wheezes/h. Discussion Overnight long-term auscultation in patients with pulmonary fibrosis proved feasible in our study. Regardless of the aetiology and disease severity, crackles were detected in the recordings of all fibrosis patients, but not cough. Wheezing occurred in 15 out of 16 patients, the significance of which for clinical practice must still remain unclear.

ZusammenfassungDominique-Jean Larrey (1766–1842) war leitender Chirurg der französischen Armee unter Kaiser Napoleon I. Er ist als Vater der modernen Militärchirurgie sowie der Entwicklung des „Triagesystems“ bekannt geworden. Larrey war ein hervorragender und engagierter Arzt, der nicht nur die eigenen, sondern auch die Soldaten der gegnerischen Truppen medizinisch versorgt hat. Beim Fehlen einer zuverlässigen Analgesie und Anästhesie war zu damaliger Zeit die Geschwindigkeit und das Können das hervorstechendste Merkmal eines Chirurgen. Gegen den Widerstand der Administration hat Larrey eine Erstversorgung auf dem Schlachtfeld sowie eine schnelle Bergung der Verwundeten mithilfe der „fliegenden Ambulanzen“ möglich gemacht. Er galt als großer Freund der Soldaten.

There is frequent confusion between Theodor Langhans (1839-1915) and Paul Langerhans (1847-1888) in the literature. Theodor Langhans was a German pathologist who discovered and described the "giant cells" with nuclei close to the outer membrane of the tubercles. Today, these cells are called "Langhans' giant cells". The eponym "Langerhans' cells" refers to dendritic cells in the stratum spinosum of the epidermis. Paul Langerhans described these cells for a competition organised by the Berlin Medical Faculty when he was still a student. Most doctors know Paul Langerhans through the first description of the "Langerhans' islet cells" of the pancreas. Langerhans died of tuberculosis at the age of 40 after a long exile on the island of Madeira.

We investigated if overnight monitoring of wheezing and coughing is feasible during AECOPD and whether automatic long - term monitoring enables an objective assessment during and after an AECOPD.Methods: In 14 patients (age: 56– 80 years) with pre-existing COPD (stages B-D) nighttime wheezing and coughing events were monitored for a period of three weeks. The portable LEOSound® monitor recorded three nights into AECOPD (nights 1, 3 and 6) during the hospital stay, and the 20th night post- AECOPD ambulatory. Before each recording the subjective symptom severity was assessed by a COPD Assessment Test (CAT) and a Modified British Medical Research Council (MMRC) dyspnoea index questionnaire.Results: In all 14 patients, lung sounds were recorded in good quality during each of the 4 recording nights. Wheezing ranged between 5% and 90% (79 − 539.5 minutes) of the recording time on the first night. All patients showed some coughs, in four patients coughing was particularly pronounced and largely receding over the total investigation period. As group, the percentages of wheezing and the number of coughs did not show significant differences between the four recording times. The CAT scores (p< 0.001) declined over the course of investigation period, suggesting a subjective improvement of symptoms.Conclusion: The observational study showed that standardized long-term recording can be performed in high-quality during acute COPD exacerbation as it does not require the patient’s cooperation. The good-quality data of coughs and wheezing were analyzed qualitatively and quantitatively. Durch die Obstruktion im Larynx-/Pharynbereich kommt es zu hohen negativen Intrapleuraldrucken, die sich, durch unterschiedliche Faktoren bedingt, auf die Integrität der alveolo-kapillären Membran auswirken. In der Übersichtsarbeit werden klinische Symptome, Ätiologie, die Pathophysiologie sowie die therapeutischen Optionen beschrieben. Ziel ist es, den Kliniker mit den Krankheitsbildern und den Komplikationen vertraut zu machen.

Negative pressure pulmonary edema and alveolar hemorrhage are potentially life-threatening complications after relief of upper airway obstruction. The laryngeal-pharyngeal obstruction results in high negative intrapleural pressures. The increased intrapleural pressures affect the integrity of the alveolo-capillary membrane due to various factors. This review describes clinical symptoms, etiologic factors, pathophysiology and treatment strategies associated with each of these factors. ZusammenfassungMit der Entdeckung des Sauerstoffs und des pulmonalen Gasaustausches ist ein großer Schritt im Verständnis der Atmung vollzogen worden. Über Jahrhunderte hinweg wurde die Auffassung vertreten, dass die Lunge primär zur Kühlung des Herzens bzw. zur „Vervollkommnung“ des Blutes notwendig sei. Die Beobachtung einer unterschiedlichen Blutfarbe vor und nach Lungenpassage führte Richard Lower (1631–1691) zu der Annahme, dass dem Blut durch die Atmung ein spezieller Stoff zugeführt worden sein müsse. Georg Ernst Stahl (1660–1734) formulierte mit seiner Phlogistontheorie einen Feuerstoff „Phlogiston“ (phlox = Flamme), der in allen brennbaren Stoffen enthalten sei und bei der Verbrennung entweiche. John Mayow (1641–1679) erkannte, dass etwa ein Fünftel des Atemgases bedeutsam für den Atmungsprozess ist, er nannte das Gas „spiritus nitro aerius“. Erstmalig wurde Sauerstoff von dem schwedisch-deutschen Apotheker Carl Wilhelm Scheele (1742–1786) sowie dem englischen Chemiker Joseph Priestley (1733–1804) in den 1770er-Jahren – unabhängig voneinander – hergestellt. Blood supply to the lungs is carried out by the pulmonary and bronchial-arterial system. The bronchial-arterial vessels are involved in supplying the small airways all the way up to the terminal bronchioles. The bronchial-arterial system is also necessary for the regulation of airway temperature, humidity and mucociliary clearance. Chronic ischaemia of the small airways due to damage or injury to bronchial arterial supply increases the risk of fibrosis of the small airways (bronchiolitis obliteration), especially in lung transplantation (LTx). Although survival after LTx has improved over time, it is, with a 5-year survival rate of only 50 to 60%, still significantly worse than that of other organ transplants. During the reign of Emperor Napoleon I, Dominique-Jean Larrey (1766-1842) was chief surgeon in the French army. He has become known as the father of modern military surgery and for the development of the "triage system". Larrey was an outstanding and dedicated physician who provided medical care not only to his own troops but also to those of the enemy. Without reliable analgesia and anaesthesia, speed and skill were the most salient characteristics of a surgeon at that time. Against the opposition of the administration, Larrey is credited with the introduction of first-aid on the battlefield as well as a quick rescue of the wounded with the help of the so-called "flying ambulances". Nocturnal Continuous Positive Airway Pressure (CPAP) is considered the gold standard treatment for obstructive sleep apnoea (OSA). The CPAP therapy is a long-term treatment but does come with few possible side effects. The adherence to the therapy is frequently suboptimal. In this paper, adherence to therapy was assessed and typical problems were classified. 1078 OSA patients received CPAP therapy after a diagnostic polygraphy (PG) or polysomnography (PSG). Adherence to therapy was followed up three months after treatment induction. The following therapy adherence groups were defined: 1. correctly calibrated CPAP and good adherence, 2. CPAP non-acceptance, after initial use, 3. CPAP intolerance use due to side effects, 4. discontinuation due to lack of motivation/low rates of symptoms. 5. mask intolerance, 6. CPAP failure due to a lack of perceived treatment effect, 7. Change to another non-invasive ventilation method, 8. No control carried out. Out of 1078 OSA patients a therapy control was performed in 830 patients (77 %). Of these, 450 patients (54.2 %) were placed in group 1, 216 patients (26 %) in group 2, 71 patients (8.5 %) in group 3, 35 patients (4.2 %) in group 4, 14 patients (1.7 %) in group 5, 3 patients (0.4 %) in group 6 and 41 patients (4.9 %) in group 7. A mild obstructive index, low CPAP pressure and, as a trend, a low Epworth- Sleepiness score were predictors of CPAP failure. No significant predictors could be shown for adherence to therapy. An effective treatment use of 54 % after 3 months is a suboptimal result. Predictors of CPAP failure were parameters that indicated that the patient was less symptomatic prior to therapy. Despite a large patient cohort, neither anthropometric nor PSG-data provided any significant CPAP adherence predictors. Rather, experiences in the first nights of use could be decisive. CPAP devices offer comfort settings that have to be personalised to patients' needs and wants. A large selection of different mask shapes requires experience and training in patient-centred mask fitting. A three-month follow-up appointment seems too long to discuss therapy problems with the patient in a timely manner. Telemedical options or short-term telephone appointments should be considered. Hiervon waren 450 Patienten (54,2 %) in Gruppe 1, 216 Patienten (26 %) in Gruppe 2, 71 Patienten (8,5 %) in Gruppe 3, 35 Patienten (4,2 %) in Gruppe 4, 14 Patienten (1,7 %) in Gruppe 5, 3 Patienten (0,4 %) in Gruppe 6 und 41 Patienten (4,9 %) in Gruppe 7. Ein geringer obstruktiver Ereignisindex, niedriger CPAP-Druck und tendenziell auch ein geringer Epworth-Sleepiness-Score waren Prädiktoren für CPAP-Versagen. Für die Therapietreue konnten keine signifikanten Prädiktoren dargestellt werden. Eine effektive Therapienutzung von 54 % nach 3 Monaten ist ein suboptimales Ergebnis. Prädiktoren für CPAP-Versagen waren Parameter, die vor Therapie auf geringen Leidensdruck der Patienten deuteten. Trotz großer Patientenkohorte konnten keine signifikanten CPAP-Adhärenz-Prädiktoren durch anthropometrische oder PSG-Daten dargestellt werden. Vielmehr könnten Erfahrungen in den ersten Therapietagen ausschlaggebend sein. CPAP-Geräte bieten Komforteinstellungen, die kenntnisgerecht auf den Patienten individualisiert werden müssen. Eine große Auswahl an unterschiedlichen Maskenformen erfordert Erfahrung und Training bei der patientenorientierten Maskenanpassung. Eine 3-monatige Wiedervorstellung erscheint zu lang, um Therapieprobleme zeitnah mit dem Patienten zu besprechen. Telemedizinische Möglichkeiten oder kurzfristige telefonische Beratungsoptionen sollten in Betracht gezogen werden.

The discovery of oxygen and pulmonary gas exchange was a major advancement in our understanding of breathing. For centuries it was believed that the lungs were primarily necessary to cool the heart or to "refine" the blood. Richard Lower (1631-1691) observed that the blood had a different colour before and after passage through the lung. His assumption was that breathing must have been added a special substance to the blood. Georg Ernst Stahl (1660-1734) formulated a fire substance "phlogiston" (phlox = flame) with his phlogiston theory. He postulated that phlogiston is contained in all combustible substances and escapes when burned. John Mayow (1641-1679) recognised that about one fifth of the breathing gas is important for the breathing process. He called the gas "spiritus nitro aerius". Oxygen was first discovered in the early 1770 s by the Swedish-German pharmacist Carl Wilhelm Scheele (1742-1786) and the English chemist Joseph Priestley (1733-1804) - independently of each other. Excessive daytime sleepiness is a common symptom in obese patients with obstructive sleep apnea. We investigated predisposing factors of excessive daytime sleepiness by comparing obese non-sleepy with sleepy patients with obstructive sleep apnea. Excessive daytime sleepiness was determined by the Epworth Sleepiness Scale in 43 patients (34 men and 9 women) with obstructive sleep apnea (apnea-hypopnea index ≥ 15 events per hr) and obesity (body mass index ≥ 30 kg m-2 ). The Marburg vigilance test (VigiMar) is a vigilance task implemented as a four-choice reaction time task with long duration and low stimulus rate. It tests readiness for reaction under monotonous conditions characterized by sensory deprivation. This study was conducted to compare test results of subjects without sleep disorders to those of patients with untreated obstructive sleep apnea (OSA). In addition, whether patients treated for OSA by continuous positive airway pressure (CPAP) exhibited improvements in vigilance testing and whether subjects without sleep disorders have stable test results in a retest after 2 days were investigated. As test results are given for test thirds separately, these were used to determine whether there was a time-on-task effect for patients with untreated OSA and to check for internal consistency of the VigiMar test. A total of 20 patients with OSA and 20 surgical patients (knee arthroscopy) between 25 and 65 years of age were included. All patients were male. Vigilance testing was performed on the day before CPAP treatment was started or on the day before arthroscopy and 2 days later after the second CPAP night or on the first day after the surgical procedure, respectively. In the baseline vigilance test, reaction times of OSA patients were longer than those of surgical patients, especially during the last third of the test. After 2 nights CPAP, reaction times of OSA patients improved to the same level as those of surgical patients who exhibited homogenous results in baseline and postintervention testing. The VigiMar test is suitable for the assessment of impaired vigilance. Its internal consistency is high, retest reliability is satisfactory, and it is sensitive for changes in vigilance after only 2 nights CPAP treatment.

Die visuelle Auswertung der Schlafstadien nach Rechtschaffen und Kales weist eine begrenzte Interrater-Reliabilitat auf. Es wurde die Interrater-Reliabilitat einer computergestutzten visuellen Auswertung bei neun akkreditierten Schlaflaboren in Deutschland untersucht. Fur den Vergleich wurden acht kardiorespiratorische Polysomnographien (drei Probanden, drei Apnoe-Patienten, zwei PLMS-Patienten) auf CD-ROM mit einem programm zur Darstellung und Auswertung am Computermonitor zur Verfugung gestellt. Es wurden die quantitativen Werte (Schlafperiodendauer, Gesamtschlafdauer, Latenzen und Anteile der Schlafstadien) verglichen. Ein Vergleich auf der Basis der einzelnen Epochen erfolgte durch Bestimmung des Fleiss-Kappa-Koeffizienten. Neun Schlaflabore sandten jeweils alle acht Auswertungen fur einen systematischen Vergleich zuruck. Bereits der Vergleich der quantitativen Werte zeigte eine sehr hohe Variabilitat. Diese war bei den Patienten noch ausgepragter als bei den Probanden. Bezogen auf den Vergleich einzelner Epochen fand sich die beste Ubereinstimmung fur REM-Schlaf und die schlechteste fur NREM1. Die Ubereinstimmung ist generell bei den Patienten schlechter und muss bei Apnoe-Patienten mit k≤0,40 als schwach bezeichnet werden. Die Ergebnisse zeigen, dass die Schulung in der visuellen Auswertung der Schlafstadien intensiviert werden muss und dass auserdem Interpretationshilfen fur die Bewertung der Schlafstadient bei Schlafapnoe erarbeitet werden mussen.

Sleep analysis is based on polysomnography. Modern polysomnographic systems are computer based. Visual and automatic analysis of sleep and respiration is supported by most computer based systems. Four functions can be distinguished in computer based polysomnography: recording, documentation during the recording, automatic and visual analysis and report generation. This review compiles the minimal requirements for digital sleep recording, documentation, analysis and reporting. The basic principles of automatic sleep analysis are reported. The requirements and the basic principles for the analysis of non-electroencephalography (EEG) signals, such as respiration, snoring, oxygen saturation, electrocardiography (ECG) and options are reported. New developments in sleep EEG processing are discussed to enlighten how computer based sleep analysis can add quantative parameters to the rules for visual sleep staging established by Rechtschaffen and Kales 30 years ago. This helps to extend our understanding of sleep.

Electrophysiological recordings are considered a reliable method of assessing a person's alertness. The aim of this study was to show, firstly, that changes in alertness during a Reaction Time Test (RTT) can be determined with certain adaptive scoring stages but not with R&K scoring and secondly, that the different adaptive stages can explain findings in reaction time. In 17 male patients (50.8+/-9.7 years, Body-Mass Index (BMI) 31.9+/-5.1 kg/m2) diagnosed with Obstructive Sleep Apnea Syndrome (OSAS) (Respiratory Disturbance Index (RDI) 53.3+/-24.1 /h sleep) a 90 min daytime vigilance test was performed twice, after the diagnostic polysomnographic investigation and after two nights spent with nCPAP. After a computerised adaptive segmentation analysis, a visual rule-defined classification system categorised alertness into one of 12 adaptive scoring stages. 6 of the 12 stages are described by the alertness conditions comparable to WAKE and NREM1.4 stages are nearly classified as NREM2-4, Rapid Eye Movement (REM) and Movement Time (MT), and one stage reflects the increase of alertness from drowsiness. The typical stage of an alert subject increased significantly from a median of 65.9% before therapy to 80.8% in the second investigation. The percentages of clearly drowsy stages decreased significantly. In contrast, there were no significant changes in the percentages of sleep stages according to R&K criteria for both investigations. According to R&K criteria 178 of 398 failed reactions (Reaction time >10 s) occurred in stage WAKE. According to adaptive scoring, only 12 failed reactions appeared in the alert stage. During the other failed reactions the electrophysiological recordings showed decreases in alertness. Neither the visual assessment nor the descriptive statistical results of R&K scoring were helpful to interpret the patient's alertness condition. In contrast, the patients' increases in alertness with nCPAP could be described by the adaptive scoring stages. This method could be a very useful procedure, when an expert opinion is necessary. It also has an actual context to the discussion of the effectiveness of CPAP in the treatment of OSAS.

Als effektive Therapie der obstruktiven schlafbezogenen Atmungsstorungen (OSBAS) haben sich sowohl die CPAP-Therapie als auch bei Patienten mit bestimmten kraniofazialen Charakteristika die Maxillo-Mandibulare Osteotomie (MMO) bewahrt. Die erfolgreiche Beseitigung der OSBAS soll zudem zu einer Restitution des physiologischen Schlafes fuhren. Dargestellt sind die Ergebnisse von 22 OSA-Patienten, die sich nach einer CPAP-Therapie zur MMO entschlossen. Kardiorespiratorische Polysomnographien wurden jeweils zwei Nachte vor Therapie (BL), unter CPAP sowie 6–12 Wochen postoperativ durchgefuhrt. Obstructive sleep apnoea (OSA) is a common disorder with potentially serious consequences. If maxillary and mandibular deficiency, often in combination with a narrow posterior airway space is present, therapy of OSA by maxillomandibular osteotomy is possible. However, long-term follow-up of patients undergoing these procedures is lacking. We present the results of 15 OSA patients (1 female and 14 males), who underwent maxillomandibular advancement surgery with a follow-up of at least 2 yrs. Polysomnography was performed before surgery, after 6-12 weeks, and 1 and 2 yrs postoperatively. Mean apnoea/hypopnoea index (AHI) decreased from 51.4 events.h-1 before therapy to 5.0 events.h-1 6 weeks postoperatively, and was 8.5 events.h-1 after 2 yrs. Oxygen saturation significantly increased following surgery. After 2 yrs, the AHI was < 10 events.h-1 in 12 out of 15 subjects. No significant changes were found comparing the 6-12 weeks versus the 2 year follow-up data. The significant increase in stage 3/4 non-rapid eye movement (NREM) sleep and decrease in stage 1 NREM sleep, indicative of the restoration of normal physiological sleep structure, persisted in 14 of the 15 subjects 2 yrs postoperatively. Three patients, however, did not show satisfactory improvement 2 yrs postoperatively; two showed obstructive and one central respiratory events. This study demonstrates that maxillomandibular advancement is successful in a high percentage of patients carefully selected by cephalometric and polysomnographic investigation. Postoperative success has proved to be stable over a period of 2 yrs. Further preoperative evaluation seems necessary in patients with predominantly mixed or central apnoeas.

Impaired vigilance is a frequent daytime complaint of patients with obstructive sleep apnoea (OSA). To date, continuous positive airway pressure (CPAP) is a well established therapy for OSA. Nevertheless, in patients with certain craniofacial characteristics, maxillomandibular advancement osteotomy (MMO) is a promising surgical treatment. Twenty-four male patients with OSA (pretreatment respiratory disturbance index (RDI) 59.3 SD +/- 24.1 events/h) participated in this investigation. The mean age was 42.7 +/- 10.7 years and the mean body mass index was 26.7 +/- 2.9 kg/m2. According to cephalometric evaluation, all patients had a narrow posterior airway space, more or less due to severe maxillary and mandibular retrognathia. All patients except two were treated first with CPAP for at least 3 months and afterwards by MMO. Two patients only tolerated a CPAP trial for 2 nights. Polysomnographic investigation and daytime vigilance were assessed before therapy, with CPAP therapy and 3 months after surgical treatment. Patients' reports of impaired daytime performance were confirmed by a pretreatment vigilance test using a 90-min, four-choice reaction-time test. The test was repeated with effective CPAP therapy and postoperatively. Daytime vigilance was increased with CPAP and after surgical treatment in a similar manner. Respiratory and polysomnographic patterns clearly improved, both with CPAP and after surgery, and showed significant changes compared to the pretreatment investigation. The RDI decreased significantly, both with CPAP (5.3 +/- 6.0) and postoperatively (5.6 +/- 9.6 events/h). The percentages of non-rapid eye movement Stage 1 (NREM 1) sleep showed a marked decrease (with CPAP 8.2 +/- 3.6% and after MMO 8.2 +/- 4.4% vs. 13.3 +/- 7.4% before treatment), whereas percentages of slow wave sleep increased significantly from 8.0 +/- 6.1% before therapy to 18.2 +/- 12.8 with CPAP and 14.4 +/- 7.3% after MMO. The number of awakenings per hour time in bed (TIB) was significantly reduced after surgery (2.8 +/- 1.3), compared to both preoperative investigation (baseline 4.2 +/- 2.0 and CPAP 3.4 +/- 1.5). Brief arousals per hour TIB were reduced to half with CPAP (19.3 +/- 20.0) and after MMO (19.7 +/- 13.6), compared to baseline (54.3 +/- 20.0). We conclude that the treatment of OSA by MMO in carefully selected cases has positive effects on sleep, respiration and daytime vigilance, which are comparable to CPAP therapy.

Obstructive sleep apnea is the most common sleep-related breathing disorder, with a surprisingly high prevalence. The treatment of choice is nasal continuous positive airway pressure (CPAP) ventilation during sleep, which has to be applied throughout the patient's whole life. Because of various underlying pathomechanisms in patients with certain craniofacial disorders--narrow posterior airway space and maxillary-mandibular deficiency--surgical therapy by craniofacial osteotomies seems possible. A series of 38 consecutive patients were treated by 10-mm maxillomandibular advancement by retromolar sagittal split osteotomy and Le Fort I osteotomy, respectively. Obstructive sleep apnea syndrome was improved considerably in all patients; there was no significant difference compared to the results under nasal CPAP. In 37 of 38 patients, the postoperative apnea-hypopnea index was reduced clearly to under 10 per hour, oxygen saturation rose, and sleep quality improved. This was achieved by maxillomandibular advancement of 10 mm without secondary refinements in all but 2 patients. In one patient, the apnea-hypopnea index could only be reduced to 20 per hour, probably because of insufficient maxillary advancement. These results indicate that successful surgical treatment is possible in a high percentage of selected patients with certain craniofacial characteristics. In addition to cardiorespiratory polysomnography, there should be routine cephalometric evaluation of all patients. Maxillomandibular advancement should be offered as an alternative therapy to all patients with maxillary and/or mandibular deficiency or dolichofacial type in combination with narrow posterior airway space.

Obstructive sleep apnoea (OSA) is due to craniofacial changes and acromegaly. The question addressed by this study was whether growth hormone (GH) induced craniofacial changes might explain persisting OSA despite endocrine inactivity in acromegaly. Nineteen patients treated for acromegaly were examined cephalometrically for craniofacial changes and polysomnographically for OSA. Twelve patients proved to have OSA with an apnoea/hypopnoea index >15; seven patients showed no evidence of OSA at all. With respect to the endocrinological parameters, there were no differences between the two groups that would explain the presence or absence of OSA. Neither group differed with respect to sex, age, or body mass index. Craniofacial changes were predominantly found in the mandible. The group with OSA proved to have increased vertical, dolichofacial growth compared to those without OSA. Consecutively, in the OSA group the posterior airway space was narrowed, and the hyoid was displaced more caudally. Thus, it seems that craniofacial structures of patients with acromegaly and persisting obstructive sleep apnoea are different from those without obstructive sleep apnoea. Surgical corrections of pertaining acromegaly-induced craniofacial changes should be performed with an awareness of the individual craniofacial condition so as not to enhance obstructive sleep apnoea.
