Background/objectives: To compare the resting energy expenditure (REE) and the REE/Fat-free-mass (FFM) quotient in children with constitutional leanness (CL) and children with normal body weight, and to describe the within-family clustering of CL. Subjects/methods: We have studied 18 children and teenagers with CL, 10 girls and 8 boys, and 18 gender and age matched normal controls, with the same pubertal stage. All were recruited from the outpatient pediatric clinic nutrition unit. None of the children with CL showed symptoms of chronic illness, they had normal laboratory results, they had a normal caloric food intake, and they did not agree with the DSM-IV-TR criteria for anorexia nervosa. We describe the body mass index (BMI) of children and their parents. The children were classified according to Cole's recently published BMI cut-offs for thinness: under 18.5 points in CL group, stable at least in the last year, and between 18.5 and 25 cutt-offs in the control group. The body composition was calculated by anthropometric methods (skinfold thickness measurements). In addition REE was measured using fasting indirect calorimetry. Results: The CL group had a higher mean percentage of FFM, and a mean FM significantly less, relative to controls (p < 0.001). The average absolute REE was significantly lower in the CL group (1,106.55 ± 240.72 kcal) than the control group (1,353.33 ± 270.01 kcal/dia) (p < 0.01). However, the REE adjusted for FFM showed a mean significantly greater in the CL group (41.39 ± 2.26 kcal/kg FFM) (Mean confidence interval (CI) 95 %: 40.33-42.45) than the controls (37.37 ± 3.06 kcal/kg FFM) (Mean CI 95 %: 35.93-38.81) (p < 0.001). Finally, in the family study, the mean BMI of fathers of CL group was significantly lower (p < 0.01), but there were not any differences in the mean BMI of mothers. Among parents with BMI known, 8 of 35 parents of CL group had an BMI lower 18.5, and only 2 of 36 parents in the control group (p < 0.05). Conclusions: This increased energy expenditure-to-FFM ratio differentiates between CL and controls. These metabolic differences are probably genetically determined. [ABSTRACT FROM AUTHOR]

Cystic fibrosis (CF) patients require a stable and sufficient supply of micronutrients. Since copper is an essential micronutrient for human development, a cross-sectional study was carried out to investigate the serum copper levels, serum copper/zinc (Cu/Zn) ratios, and their relationship with nutritional indicators in a group of CF patients. Anthropometric, biochemical, and dietary measurements, an abdominal ultrasound, and respiratory and pancreatic tests were conducted. Seventeen CF patients were studied (10 females, 59%), 76.5% of whom were ∆F580. Their mean serum copper (113 ± 23 μg/dL) was normal, and there was only one teenager with hypocupremia (6%) and two children with hypercupremia (18%). A significant association between serum copper and zinc levels was discovered. The Cu/Zn ratio was higher than 1.00 for 94% of patients, which is an indicator of an inflammation status. There was no significant correlation between the serum copper concentrations and respiratory and pancreatic function, respiratory colonization, and the results of the abdominal ultrasound. Linear regression analysis showed that serum copper had a positive association with both the Z-score body mass index (BMI) and mean bone conduction speed (BCS). Therefore, since 94% of CF patients had a Cu/Zn ratio > 1.00, this factor must alert us to consider the risk of zinc deficiency and high inflammatory response. The measurement of serum zinc alone does not show one's zinc status. However, the Cu/Zn ratio may be an indicator of zinc deficiency and the inflammatory status of CF patients.

Microbial pathogens are the cause of many foodborne diseases after the ingestion of contaminated food. Several preservation methods have been developed to assure microbial food safety, as well as nutritional values and sensory characteristics of food. However, the demand for natural antimicrobial agents is increasing due to consumers' concern on health issues. Moreover, the use of antibiotics is leading to multidrug resistant microorganisms reinforcing the focus of researchers and the food industry on natural antimicrobials. Natural antimicrobial compounds from plants, animals, bacteria, viruses, algae and mushrooms are covered. Finally, new perspectives from researchers in the field and the interest of the food industry in innovations are reviewed. These new approaches should be useful for controlling foodborne bacterial pathogens; furthermore, the shelf-life of food would be extended.

Shiga toxin-producing Escherichia coli (VTEC) strains, including the serotype O157:H7, are considered foodborne pathogens. Transmission occurs through consumption of undercooked meat, unpasteurized dairy products, vegetables, or contaminated water. The variability of pathogenic and nonpathogenic E. coli strains growth parameters at different temperatures and in different media was studied. Bootstrap parametric (Generalized Beta of the Second Kind, GB2) or nonparametric models were used. GB2 estimations show increased growth rates and shortened lag times with increased temperature, as expected. Similar estimations were obtained using the nonparametric model. Parametric and nonparametric growth rate confidence intervals were wider with increased temperature; lag times confidence intervals were wider with decreased temperature. The nonparametric method gives similar confidence intervals to the parametric method, confirming its suitability for growth parameters estimation. Introduction: Vector bioimpedance analysis (BIVA) can be very useful for the evaluation of body composition, hydration, and nutritional status in infants and newborns. The objective of this study was to determine the impedance vector distribution for a group of healthy newborn Spanish children., Methods: This was a cross-sectional, descriptive study conducted with 154 healthy, Spanish newborns (gestational age: 37-41 weeks) aged 24 to 72 hours (79 males, 75 females). Weight, height, and cephalic-circumference were determined. Resistance and reactance were measured with a single-frequency impedance analyzer at 50 kHz (tetrapolar analysis). The newborns' specific 95% confidence intervals of the mean vectors and the 95%, 75%, and 50% tolerance intervals for the individual vector measurements were plotted using R and Xc components standardized by the subjects' lengths. The mean impedance vectors were compared with Hotelling's-T 2 test for vector analysis (significance level: P < .05)., Results: The newborns exhibited gender-related differences in the mean impedance vector (mean [SD] R/H: 833.6 [97.5] Ohm/m in males vs 918.2 [107.7] Ohm/m in females; mean [SD] Xc/H: 91.3 [34.7] Ohm/m in males vs 95.6 [23.2] Ohm/m in females). No statistically significant differences in the mean impedance vectors were observed according to days of life. Lower values of resistance and slightly higher reactance values were observed in the healthy Spanish newborns compared to Italian newborns., Conclusions: New tolerance ellipses were constructed for healthy Spanish newborns. Introduction: BMI is the most commonly used indicator to evaluate overweight and obesity, but it cannot distinguish changes in body composition. Over recent years, it has been demonstrated that bioelectrical impedance analysis (BIA) is a more accurate method for analyzing body composition. Bioelectrical impedance vector analysis (BIVA) has revealed its effectiveness as an indicator of nutritional status and hydration., Objective: To assess the usefulness of bioimpedance analysis on the study of body composition in a group of children with overweight and obesity., Materials and Methods: Cross-sectional observational study. The anthropometric parameters of 167 (79 were older than 12 years) overweight and obese children were recorded. Their body composition was analyzed using BIA and BIVA, and was classified based on different criteria. Concordance was analyzed (intraclass correlation coefficient, Bland-Altman analysis and weighted Kappa coefficient). The BIVA of the subgroups was compared using the Mahalanobis distance and Hotelling's T2. Statistical significance was considered for p<0.05., Results: The BMI revealed that the majority of the assessed subjects were obese, although 12% had a normal percentage of fat mass (%FM). The classification by Z-BMI and Z-%FM significantly discriminate between subjects with different levels of adiposity. In children over the age of 12, the classification of fat mass index also discriminates significantly between obesity and non-obesity. As anticipated, in the tolerance ellipses, most of the individual vectors were situated in the left lower quadrant., Conclusions: BIVA reflects differences in the bioelectric patterns of children who are classified as being overweight or obese (BMI) and who have different levels of %FM and FMI. Background: Zinc is an essential nutrient for all forms of life and its deficiency affects the normal growth and development of human beings., Objective: The main aim was to investigate zinc nutritional status by serum zinc concentration (SZC) and dietary zinc intake and their association in cystic fibrosis (CF) patients., Methods: A cross-sectional study was conducted in CF patients. Anthropometric measurements and respiratory and pancreatic tests were conducted. Hypozincemia was determined by SZC while using atomic absorption spectrophotometry and dietary zinc deficiency by prospective 72-h dietary surveys., Results: Mean SZC (87.2 ± 16.7 μg/dL) and dietary zinc intake (97 ± 26.9% Dietary Reference Intake) were normal. Three of 17 patients with CF (17.6%) had hypozincemia and four (23.5%) had a dietary zinc deficiency. No patient with dietary zinc deficiency had hypozincemia. A positive and significant association was observed between SZC and Z-score of BMI-for-age ( p = 0.048) and weight-for-height ( p = 0.012) and between dietary zinc intake and energy intake (EI, p = 0.036) and Z-score of weight-for-high ( p = 0.029)., Conclusion: SZC was associated with the nutritional status, expressed as BMI (Body Mass Index) and weight-for-height Z score, and dietary zinc intake with EI and weight-for-height Z-score. No patient with hypozincemia had dietary zinc deficiency. This situation should alert us to a marginal zinc deficiency and it may explain why there were no overlapping cases between the two groups. We suggest that probably 41% of the cases in this study would be at elevated risk of zinc deficiency and a zinc supplementation may be considered., Competing Interests: The authors declare no conflict of interest.

Objective: To assess the association between whole-body and calf impedance vectors and muscle mass and strength in a group of elderly individuals., Material and Methods: We carried out a cross-sectional observational study on a sample of 113 elderly people. Anthropometric parameters (weight, height and body circumferences) were determined. Body composition was evaluated using conventional bioimpedance analysis (BIA) and vector bioimpedance analysis (BIVA) (whole-body and calf BIVAs), and muscle strength was determined (manual dynamometry). The results were analyzed using the Student t-test or the Mann-Whitney U, and the correlations using the Pearson or Spearman test. To compare BIVA results among the subgroups established, the Mahalanobis distance (dM) was calculated and the Hotelling T 2 statistic was used. Statistical significance was set to p < 0.05., Results: Nearly half the sample was overweight. Based on waist circumference, 66.7% of the males and 94.9% of the females showed risk of metabolic complications; calf circumference indicated no risk of disability or skeletal muscle mass depletion. However, BIA and dynamometry detected risk of sarcopenia in more than half the subjects. Whole-body BIVA results agreed with those of the BIA, given that most impedance vectors in both sexes were to the right of major axis of the tolerance ellipses. This shows cell mass depletion. While the whole-body BIVA distinguished the subjects having loss of muscle mass and strength, the specific BIVA (calf) only did so in individuals with muscle mass loss., Conclusions: Whole-body BIVA detects loss of muscle mass and strength, while calf BIVA only distinguishes subjects having muscle mass loss. Objectives: Reference values of the bioelectrical impedance vector for the Spanish child and adolescent population are needed for assessing body composition and hydration status in this population. The aim of this study is to provide reference values of the bioelectrical impedance vector in Spanish children and adolescents aged 4-18 years from Castilla y León., Methods: This was a cross-sectional descriptive study conducted in 4401 Spanish healthy children and adolescents aged 4-18 years (2265 boys and 2136 girls). Resistance and reactance were measured with a single-frequency impedance analyzer at 50 kHz (tetrapolar analysis). The values of resistance and reactance normalized by height were used to plot the bivariate 50th, 75th, and 95th percentiles of the population by age group. Mean impedance vectors were compared with Hotelling's T 2 test for vector analysis (differences being considered significant if p < .05)., Results: Tolerance ellipses were drawn for the Spanish child and adolescent population studied. The mean impedance vector showed displacement across all age groups except for (1) girls aged 12-13 years, (2) girls aged 15-18 years, and (3) boys aged 16-18 years. There were sex-related differences in the mean impedance vector in all age ranges, even in prepubertal children. Among adolescents, the patterns of the vector displacement were consistent with the timing of normal growth and development in all groups and are attributable to the maturation process., Conclusions: New tolerance ellipses have been constructed for Spanish children and adolescents by sex and age. Objective: To compare body composition as assessed by conventional and vector bioelectrical impedance analysis according to the nutritional cataloging using body mass index (BMI) in a group of institutionalized elderly., Methods: Cross-sectional study in 38 institutionalized elderly. Body composition was estimated by bioimpedance analysis. Differences in body composition were analyzed using t-test and ANOVA, or their corresponding nonparametric tests. Statistical significance was set at p<0.05., Results: Based on BMI, the sample showed overweight (average BMI: 26.4kg/m 2 ), and women had higher BMI values than men (28.9 vs. 25.5kg/m 2 ). Based on waist circumference, abdominal obesity was detected in 60.7% of men and 80% of women. Conventional bioimpedance analysis (BIA) yielded high fat mass values and slightly depleted skeletal muscle mass, compatible with sarcopenic obesity. All individual impedance vectors were located on the right of the major axis of the tolerance ellipses, reflecting body-cell-mass depletion in all subjects, regardless of BMI cataloging., Conclusions: Bioelectrical impedance vector analysis (BIVA) detects body compartment changes in institutionalized elderly that are not identified by the most widely used clinical practice nutritional indicators, such as BMI, waist circumference, and BIA-estimated body composition., (Copyright © 2016 Elsevier Inc. Background/objectives: Bioelectrical impedance vector analysis (BIVA) is increasingly used in clinical research to assess soft tissue hydration. It is known that physical characteristics of electrodes, such as low intrinsic impedance, low electrode/skin contact impedance and type of gel, affect the reliability of noninvasive bioimpedance assessments. The aim of this study was to determine the effect of intrinsic impedance of electrode on the bioimpedance vector displacement in RXc graph., Subjects/methods: The intrinsic impedance is measured in nine pregelled disposable Ag/AgCl electrodes usually used for bioimpedance measures. The BIVA method is performed on 35 healthy volunteers using a 50 kHz phase-sensitive bioimpedance analyzer (BIA 101 Anniversary) with the lowest intrinsic impedance electrode and highest. The individual bioimpedance vector is plotted on the bivariate normal interval of reference population. The differences in the mean bioimpedance vectors obtained with each electrode are plotted, with their 95% confidence ellipses, on the dRXc graph. The paired one-sample Hotelling's T 2 -test is used to compare the differences of the mean bioimpedance vectors., Results: We found large variability in intrinsic resistance (11-665 Ω) and reactance (0.25-2.5 Ω) values of the electrodes analyzed and significant displacement (P<0.05) of bioimpedance vector positions in healthy adults according to the paired one-sample Hotelling's T 2 -test., Conclusions: A robust study of all physical characteristics of commercial Ag/AgCl electrodes is necessary to reach consensus on pregelled Ag/AgCl electrodes valid for bioimpedance measurement. This information will enable BIVA users to avoid systemic errors when performing BIVA assessments, specifically when these measurements are used for clinical interpretations.

Introducción: la alimentación de los jóvenes universitarios se aleja cada vez más de la dieta mediterránea (DM). El binomio alimentación-actividad física es fundamental para mantener un adecuado estado de salud.Objetivo: comparar la ingesta en un grupo de estudiantes universitarios deportistas frente a otro que no realiza deporte habitualmente.Sujetos y métodos:estudio observacional transversal realizado en 49 voluntarios de la Universidad de Valladolid. Se determinaron peso, talla y actividad física (cuestionario GPAQ). La ingesta se evaluó a partir de un cuestionario de frecuencia de consumo y un registro de alimentos de 3 días. La adherencia a la DM se estimó con el Mediterranean Diet Score. Las diferencias entre las variables en función del sexo y la práctica deportiva se analizaron con la t de Student o la U-Mann-Whitney. Significación estadística: p < 0,05.Resultados: todos los sujetos presentaron normopeso, sin diferencias en función de la práctica deportiva. El nivel de actividad física fue inferior en las mujeres. En todos los estudiantes la dieta fue ligeramente hiperproteica, rica en grasas y colesterol, con un inadecuado perfil lipídico y deficiente en hidratos de carbono y fibra. Se cubren los requerimientos de micronutrientes. Destaca un escaso consumo de frutas-verdurashortalizas, cereales, aceite de oliva, pescado y frutos secos; y un exceso de carne, mantequilla, bollería-industrial, dulces, snacksy refrescos. El 50% de la muestra tiene una adherencia baja o muy baja a la DM.Conclusión: las dietas de los jóvenes universitarios no cumplen los objetivos nutricionales para la población española y presentan una adherencia media-baja a la DM, independientemente de la práctica deportiva.

Anorexia nervosa (AN) is the most prevalent of eating disorders in children and adolescents, and its treatment is long and complex, involving a multidisciplinary team. Nutritional rehabilitation and restoration of a healthy body weight is one of the central goals in the initial stages of inpatient treatment. However, current recommendations on initial energy requirements for these patients are inconsistent, with a clear lack of controlled studies, available scientific evidence and global consensus on the most effective and safe refeeding practices in hospitalized adolescents with anorexia nervosa (AN). Conservative refeeding recommendations have been classically established in order to prevent the refeeding syndrome. Nevertheless, various works have recently appeared advocating a higher initial caloric intake, without observing more complications or refeeding syndrome, and allowing a shorter average stay. We present our experience in the treatment of restricting AN with a conservative progressive treatment. We have obtained good results with this approach, which was well tolerated by patients, with no observing complications. As a consequence, the medical team could establish a pact about the therapeutic goals with the patients in an easier way.

Objective: to assess the utility of body mass index (BMI) and waist circumference (WC) as surrogate indicators of adiposity with respect to the total body fat estimated with bioimpedance analysis in psychogeriatric patients., Methods: Anthropometric and hand-to-foot bioimpedance measurements were performed according to standard procedures in a sample of 128 psychogeriatric patients (87 males, 41 females). WC cutoffs proposed by the International Diabetes Federation were used to define abdominal obesity. Z-scores of fat and fat-free mass indices (Z-FMI and Z-FFMI) were calculated., Results: Males with WC values below the cutoff were normal weight, and showed normal levels of FM and low FFM (Z-FFMI below 1.5 SD). Males with WC values above the cutoff were overweight, showed high levels of FM (Z-FMI: 1.34 SD) and a slight depletion of FFM (Z-FFMI: -0.59 SD). In females with WC values below the cutoff, BMI was close to 20 kg/m(2) and both FM and FFM were depleted (Z-FMI: -0.7 SD; Z-FFMI: -1.76 SD). In females with WC above the cutoff, the average BMI was 25.6 kg/m(2) , Z-FMI was 0.48 SD, and Z-FFMI was -0.56 SD., Conclusions: Our results indicate that it is necessary to establish age and sex-specific BMI and WC cutoffs, and also highlight the importance of focusing on body composition analysis to ensure an accurate nutritional diagnosis in older-adults and in psychogeriatric patients., (© 2015 Wiley Periodicals, Inc.)

Ongoing population ageing is one of the factors influencing the increase in the prevalence of undernutrition, as elderly people are a vulnerable group due to their biological, psychological and social characteristics. Despite its high prevalence, undernutrition is underdiagnosed in the geriatric sphere. For this reason, the aim of this consensus document is to devise a protocol for geriatric nutritional assessment. A multidisciplinary team has been set up within the Spanish Society of Geriatrics and Gerontology (in Spanish Sociedad Española de Geriatría y Gerontología [SEGG]) in order to address undernutrition and risk of undernutrition so that they can be diagnosed and treated in an effective manner. The MNA-SF is a practical tool amongst the many validated methods for nutritional screening. Following suspicion of undernutrition, or after establishing the presence of undernutrition, a full assessment will include a detailed nutritional history of the patient. The compilation of clinical-nutritional and dietetic histories is intended to help in identifying the possible risk factors at the root of a patient's undernutrition. Following this, an anthropometric assessment, combined with laboratory data, will describe the patient's physical and metabolic changes associated to undernutrition. Currently, the tendency is for further nutritional assessment through the use of non-invasive techniques to study body composition in association with functional status. The latter is an indirect index for nutritional status, which is very interesting from a geriatrician's point of view. To conclude, correct nutritional screening is the fundamental basis for an early undernutrition diagnosis and to assess the need for nutritional treatment. Introduction: although there are precise and accurate techniques for estimating resting energy expenditure, like the indirect calorimetry (IC), daily practice needs faster, easier and cheaper methods as the predictive equations., Objective: the aim of the study was to develop a new predictive equation for estimating resting energy expenditure (REE) for healthy Spanish population. Méthods: the REE of 95 healthy normal weighted volunteers was determined by indirect calorimetry (IC). The new equation was obtained by multiple lineal regression by using the analytical criteria of the Cp of Mallows and the adjusted R2. Then, the behavior of the new formula was studied in a group of overweight volunteers through the intraclass correlation coefficient (ICC) and Bland-Almand plots. The level of signification was reached at p < 0,05., Results: the average age was 42 years (range: 2.0-63.2). Mean REE determined by IC was 1 589.1 kcal/d (312.0). The selected equation was: [y = 1 376.4 - 308 Sex (M = 0; W = 1) + 11.1 Weigh (kg) - 8 Age (years)] (R2: 0.68; EE: 175.95). The ICC between the new equation and the IC in normal weighted subjects was 0.901 (95%CI: 0.851 - 0.934). The new formula showed a good level of agreement in the overweight group (ICC: 0.880; 95%IC: 0.772 - 0.937)., Conclusions: we propose a new predictive equation for estimating the REE for healthy Spanish population which has an easy application and includes sex, age and weigh. Objective: the aim of this study was to analyze the agreement between the resting energy expenditure (REE) obtained by indirect calorimetry (IC) and that obtained by prediction equations in a sample of healthy adults from Spain. Méthods: a descriptive cross-sectional study was conducted in 95 healthy, normal-weight adults. REE was determined by IC and 45 population-specific prediction equations which were based on weight, height, sex and/or body composition (BC). The Intraclass Correlation Coefficient (ICC) and Bland-Alman plots were used to analyze the agreement between the REE obtained by IC and that obtained by prediction equations. The level of signification was reached at p < 0,05., Results: mean age was 42 years (range: 23.0-63.2). Mean REE determined by CI was 1589 (312) kcal/d [1822.3 (224.3) kcal/d in men and 1379.3 (216.1) kcal/d in women; p < 0.05]. The De-Lorenzo, Harris-Benedict, Schofield, and especially the Korth equations showed the greatest level of agreement with respect to IC., Conclusions: there is high variability in the estimates of REE depending on the prediction equation used. The De Lorenzo, Harris-Benedict, and the Schofield equations showed a good level of agreement in our sample; however, the Korth equation was the most appropriate. To conclude, correct nutritional screening is the fundamental basis for an early undernutrition diagnosis and to assess the need for nutritional treatment. Objective: Although dementia and nutritional status have been shown to be strongly associated, differences in body composition (BC) among older people with dementia have not yet been firmly established. The aim of this study was to assess BC through conventional and vector bioimpedance analysis (BIA and BIVA, respectively) in a sample of institutionalized older men with and without dementia, in order to detect dementia-related BC changes., Methods: Forty-one institutionalized men ages ≥ 65 y (23 without dementia [CG] and 18 with dementia [DG]) were measured with BIA and interpreted with BIVA and predictive equations., Results: Age (74.4 and 75.7 y) and body mass index (22.5 and 23.6 kg/m(2)) were similar for DG and CG, respectively. Resistance and ratio of resistance to height did not differ significantly between the two groups. Reactance and ratio of reactance to height were 21.2% and 20.4% lower in DG than in CG. Phase angle was significantly lower in DG (mean = 4.0; 95% confidence interval [CI], 3.6°-4.3°) than in CG (mean = 4.7; 95% CI, 4.3°-5.1°). Mean fat mass index (6 and 7 kg/m(2)), and mean fat-free mass index (16.4 and 16.6 kg/m(2)) were similar in both groups. BIVA showed a significant downward migration of the ellipse in DG with respect to CG (T(2) = 15.1; P < 0.01)., Conclusion: Conventional BIA showed no significant differences in BC between DG and CG, although reactance and ratio of reactance to height were about 21% lower in DG. Nevertheless, a body cell mass depletion and an increase in the ratio of extracellular to intracellular water were identified in DG using BIVA. In clinical practice, geriatric nutritional assessment usually includes nutritional screening, a simple anthropometric assessment, measurement of various biochemical parameters, such as serum albumin, and sometimes (not always) body composition analysis (BCA). However, there is a high prevalence of undiagnosed malnutrition in patients with dementia. Several factors contribute to this situation; probably, the most notable is the methodology used to assess body composition (BC). In this regard, for BCA, techniques are needed that are noninvasive, affordable, safe, simple and that require the minimum possible collaboration by the elderly patient. Consequently, body mass index (BMI) and waist circumference (WC) are widely used as indicators of overall and central adiposity, respectively; however, there is no consensus on the cutoffs for the elderly, and changes in BC (especially muscle-mass depletion) are masked by normal values of BMI and WC. Bioimpedance analysis is a simple, cost-effective and precise method for BCA, provided that cross-validated equations are used. Its main disadvantage is that it is highly sensitive to changes in body water (overhydration or dehydration), leading to substantial errors in BC estimates. However, using Bioelectrical Impedance Vector Analysis errors are minimized, as there is no need for the subject to be normally hydrated and it does not require the use of predictive models.

Background: A new analytical variation of bioelectrical impedance vector analysis (BIVA), called specific BIVA, has shown to be more accurate in detecting changes in fat mass than classic BIVA., Objective: To compare classic and specific BIVA in order to identify which is more strongly associated with psycho-functional and nutritional indicators in a group of institutionalised elderly patients with dementia., Subjects and Methods: Cross-sectional study. Fifty-four patients (34 women, 20 men) with dementia in moderately severe to very severe stages and aged 60-95years underwent geriatric nutritional assessment, including body mass index calculations, the Mini Nutritional Assessment, the Geriatric Nutritional Risk Index, and whole body composition analysis., Results: With specific BIVA (unlike with classic BIVA), significant differences were found between women with moderately severe and very severe dementia. In the BIVA conducted for body mass index, the confidence ellipses produced with the classic BIVA approach were highly overlapping; but with specific BIVA, significant differences were observed between the women in different nutritional categories (malnutrition, risk of malnutrition, normal weight and obesity). On the other hand, both approaches distinguished malnourished women from those who were at risk of malnutrition, according to the Mini Nutritional Assessment; and men with a moderate-high risk of malnutrition from men with no risk, on the basis of the Geriatric Nutritional Risk Index., Conclusion: Overall, the findings of the present study suggest that specific BIVA is more effective than classic BIVA in identifying bioelectrical changes associated with psycho-functional and nutritional indicators in institutionalised elderly with dementia., (Copyright © 2014 Elsevier Inc. Objective: To assess the nutritional status of a group of institutionalized elderly with dementia by analysing the changes in the body composition according to their evolutionary state., Method: A cross-sectional study was carried out assessing the nutritional status and the body composition (anthropometry, MNA and bioimpedance) in 63 institutionalized elderly with a diagnosis of dementia in evolutionary stages 5, 6 and 7 of the GDS (Global Deterioration Scale) scales and the FAST (Functional Assessment Stating) scale. The fat mass index (FMI) and the fat-free mass index (FFMI) were calculated. The results were analysed by one-factor ANOVA and Kruskal-Wallis factorial ANOVA and Scheffé's a posteriori contrasts. The significance level was set at p < 0.05., Results: Mean age was 80.6 years (95%CI: 78.3-83.0; Range: 51-95). According to MNA, 38.1% of the sample had hyponutrition, 60.3% was at risk for malnourishment; and 1.6% had normal nutrition. The average body mass index was 23.06 (22.01-24.10) kg/m2, without any significant difference by the degree of evolutionary state, except for those on palliative care (mean: 19.85; 95%CI: 78.3-83.0 kg/m2). The patients in the non-palliative stages GDS/FAST 5, 6 and 7 presented a Z-Score of FMI close to a 0 SD, and a Z-Score for the FFMI of approximately -1 DS. These results are compatible with a sarcopenia situation. In patients on palliative care (GDS/FAST =?7c) both indexes were significantly lower., Conclusion: The analysis of body composition should always be included in the assessment of the nutritional status. Adequate nutrient intake is an important component of maintaining bone health, and should be encouraged in all age groups. Osteoporosis is a disease in which the density and quality of bone are reduced, leading to weakness of the skeleton and increased risk of fracture, particularly of the spine and hip. Osteoporosis is a global public health problem which currently affects a great proportion of women and men, and is increasing in significance as the population of the world both grows in size and lives longer. Bone loss does not have any symptoms, and often the first sign of having osteoporosis is a fracture. Studies in children and adolescents have shown that calcium and Vitamin D supplements seems to enhance the rate of bone mineral acquisition, and can play a role in osteoporosis prevention.
