OBJECTIVES: To develop and validate a specific protocol for SARS-CoV-2 detection in breast milk matrix and to determine the impact of maternal SARS-CoV-2 infection on the presence, concentration and persistence of specific SARS-CoV-2 antibodies. DESIGN AND PATIENTS: This is a prospective, multicentre longitudinal study (April-December 2020) in 60 mothers with SARS-CoV-2 infection and/or who have recovered from COVID-19. A control group of 13 women before the pandemic were also included. SETTING: Seven health centres from different provinces in Spain. MAIN OUTCOME MEASURES: Presence of SARS-CoV-2 RNA in breast milk, targeting the N1 region of the nucleocapsid gene and the envelope (E) gene; presence and levels of SARS-CoV-2-specific immunoglobulins (Igs)-IgA, IgG and IgM-in breast milk samples from patients with COVID-19. RESULTS: All breast milk samples showed negative results for presence of SARS-CoV-2 RNA. We observed high intraindividual and interindividual variability in the antibody response to the receptor-binding domain of the SARS-CoV-2 spike protein for each of the three isotypes IgA, IgM and IgG. Main Protease (MPro) domain antibodies were also detected in milk. 82.9% (58 of 70) of milk samples were positive for at least one of the three antibody isotypes, with 52.9% of these positive for all three Igs. Positivity rate for IgA was relatively stable over time (65.2%-87.5%), whereas it raised continuously for IgG (from 47.8% for the first 10 days to 87.5% from day 41 up to day 206 post-PCR confirmation). CONCLUSIONS: Our study confirms the safety of breast feeding and highlights the relevance of virus-specific SARS-CoV-2 antibody transfer. This study provides crucial data to support official breastfeeding recommendations based on scientific evidence. Trial registration number NCT04768244.

Historic concrete buildings (end of 19th century – 1960s), because of their “experimental” character, require a specific approach to both survey and conservation. Although they were built with empirical approaches, some buildings show a fair state of conservation and resilience –even though they have already exceeded the 100-year threshold– while others of comparable age are in severe need of restoration. As part of the European project CONSECH20, aimed at contributing to the conservation of cultural-heritage concrete buildings, this paper investigates what are the most common types of damage and hypothetical causes, and what direct and non-direct parameters can lead to a faster or slower deterioration of historic concrete in the Netherlands. The research is based on an initial screening study, which will be used as a basis for a larger research among the participant countries. The current research is divided in three phases. Firstly, a selection of 15 case studies from the Netherlands are investigated; the selection was based on criteria of age, state of conservation and type of ownership. Secondly, the history and materials of the buildings are examined. Thirdly, an on-site visual survey is performed per each building, with pre-design templates, to identify types of damage, extent and severity. The data is then analysed combining different factors with a calculated index of severity. Results are discussed and contrasted to provide further clarification of the degradation of historic concrete. A fourth phase, not discussed in this paper, will use this methodology in a broader context, with a larger number of case studies in different countries.

Resumen Fundamento Conocer la situación organizativa de los hospitales españoles de cara a facilitar la atención adecuada en los servicios de urgencias (SUH) de los pacientes que acudan con sospecha de infecciones de origen tropical. Método Estudio descriptivo transversal mediante cuestionario en formato Google Forms® enviado a los miembros del grupo de INFURG-SEMES. Se estudiaron variables como el tamaño del hospital a través del número de camas, el número de urgencias de patología tropical, la existencia de protocolos de medicina tropical, de pruebas diagnósticas urgentes o tratamiento antimalárico. Resultados Se envió el formulario a 75 hospitales, obteniendo respuesta de 42 servicios de urgencias (55%), pertenecientes a 10 comunidades autónomas. Veinticuatro (57,1%) tenían más de 500 camas. Solo cinco hospitales (11,9%) podían diagnosticar malaria y dengue las 24 horas. En 19 hospitales (45,3%) no existía ningún protocolo de enfermedad tropical. En siete hospitales (16,7%) se realizaban diez o más asistencias/día. En los hospitales de mayor tamaño era más frecuente la existencia de un servicio de enfermedades infecciosas independiente del servicio de Medicina Interna, una unidad de medicina tropical, un infectólogo de guardia y un microbiólogo de guardia. No existen diferencias estadísticamente significativas entre los hospitales de mayor y menor tamaño en cuanto a la capacidad para realizar diagnósticos o tratamiento adecuados durante las 24 horas. Conclusiones. La atención de la patología importada supone un volumen no despreciable de consultas en los SUH, donde en general, se observa una ausencia de protocolos específicos, en especial, el protocolo específico de malaria, así como de escasa disponibilidad de prueba diagnóstica urgente de malaria.

- Abstract
Historic concrete buildings are at risk. Limited knowledge of concrete technology until the 1960s led to more sensitive buildings than modern concrete buildings. In addition, the lack of sensibility regarding their heritage value and insufficient protection is leading to remorseless demolition. Still, concrete has proved to be a resilient material that can last over a century with proper care. There is not yet an estimation of the status of historic concrete buildings in Europe. Until now, a few attempts have been done to secondarily, and subjectively, gauge their conservation status. This paper is the result of a joint investigation studying forty-eight historic concrete buildings distributed in four countries. They were surveyed by expert teams according to a predefined methodology. The study aims to identify recurrent damages and parameters affecting the conservation state. It also aims to serve as the first trial for an objective and measurable methodology, to apply it with a statistically significant number of cases. Damages related to the corrosion of reinforcement and moisture-related processes were the most recurrent. The use of plasters, flat roofs, and structural façade walls show a positive effect in protecting the concrete. The aim was to develop a predictive model of infection by multidrug-resistant microorganisms (MDRO). A national, retrospective cohort study was carried out including all patients attended for an infectious disease in 54 Spanish Emergency Departments (ED), in whom a microbiological isolation was available from a culture obtained during their attention in the ED. A MDRO infection prediction model was created in a derivation cohort using backward logistic regression. Those variables significant at p < 0.05 assigned an integer score proportional to the regression coefficient. The model was then internally validated by k-fold cross-validation and in the validation cohort. A total of 5460 patients were included; 1345 (24.6%) were considered to have a MDRO infection. Twelve independent risk factors were identified in the derivation cohort and were combined into an overall score, the ATM (assessment of threat for MDRO) score. The model achieved an area under the curve-receiver operating curve of 0.76 (CI 95% 0.74-0.78) in the derivation cohort and 0.72 (CI 95% 0.70-0.75) in the validation cohort (p = 0.0584). Patients were then split into 6 risk categories and had the following rates of risk: 7% (0-2 points), 16% (3-5 points), 24% (6-9 points), 33% (10-14 points), 47% (15-21 points), and 71% (> 21 points). Findings were similar in the validation cohort. Several patient-specific factors were independently associated with MDRO infection risk. When integrated into a clinical prediction rule, higher risk scores and risk classes were related to an increased risk for MDRO infection. This clinical prediction rule could be used by providers to identify patients at high risk and help to guide antibiotic strategy decisions, while accounting for clinical judgment.

La tesis doctoral titulada “El Cardenal Pedro Segura y Sáenz, 1880-1957” es una biografía política de Pedro Segura y Sáenz (1880-1957), arzobispo y cardenal español de gran relieve social, cuyos importantes pronunciamientos públicos a lo largo de la primera mitad del siglo XX tuvieron hondas repercusiones políticas durante la Monarquía, la Segunda República y el franquismo. Mons. Segura ocupó durante casi cuatro décadas importantes cargos episcopales en diferentes ciudades españolas (entre ellas, el arzobispado de Toledo, la sede más importante de todas). Abordó en su magisterio cuestiones candentes, en muchas ocasiones de forma agresiva y contra la opinión predominante de las sucesivas autoridades públicas españolas. Estas dos circunstancias determinaron frecuentes colisiones con los Gobiernos, un hecho que confiere a este personaje un indudable protagonismo en la historia nacional española. El análisis de los jalones biográficos más significativos pretende dejar al descubierto los principios intelectuales que conformaron su pensamiento, así como motrar las consecuencias sociales y políticas de su conducta, a nivel nacional e internacional. En último término, la tesis aspira a responder al interrogante de hasta qué punto Pedro Segura y Sáenz fue un arquetipo de eclesiástico tradicionalista. Definiendo tradicionalismo no la filiación o simpatía hacia el carlismo –adhesión que mons. Segura mantuvo buena parte de su vida, por lo demás– sino la percepción cultural que lleva a identificar con la fe cristiana determinadas y únicas soluciones para acometer los problemas sociales de orden temporal.

To evaluate the degree of inter-observer concordance in the blood pressure figures taken by a nurse and a doctor with a mercury sphygmomanometer and by the patient with a semi-automatic device.Observational, crossover study.A rural health centre.318 people selected by systematic sampling.Pressure was taken in different ways by 3 different "observers" (0): the patient with a semi-automatic device (01), a nurse (02) and a family doctor (03). Inter-observer concordance was assessed through the Intra-class Correlation Coefficient (ICC) and the kappa index.42.1%, 41.8% and 44.3% of patients had high blood pressure (or = 140/90 mmHg) according to 01, 02 and 03, respectively. 33% were known to be hypertense. The ICC for systolic pressure was 0.81 (95% CI, 0.75-0.87) between 01 and 02; 0.84 (CI, 0.78-0.90) between 01 and 03; and 0.87 (CI, 0.82-0.92) between 02 and 03. The ICC for diastolic pressure was 0.67 (CI, 0.59-0.75), 0.72 (CI, 0.64-0.79) and 0.79 (CI, 0.72-0.86) for 01-02, 01-03 and 02-03, respectively. The kappa index was 0.53 (CI, 0.43-0.62) for 01-02; 0.60 (CI, 0.51-0.68) for 01-03; and 0.67 (CI, 0.58-0.75) for 02-03.Inter-observer concordance was good, with no substantial differences between the measurements made by the different observers. As the figures determined by the semi-automatic device were reliable, this is a good option for the follow-up and monitoring of hypertense patients.

Introducción: El paraganglioma es un tumor neuroendocrino poco frecuente que surge de los paraganglios autonómicos extraadrenales en diversos lugares del organismo. También se les conoce como feocromocitomas extraadrenales. Alrededor de 90 % son benignos y se curan mediante la extirpación quirúrgica, el restante 10 % son malignos, y pueden provocar metástasis a distancia. El diagnóstico es importante por su riesgo de malignidad, por las implicaciones de otras neoplasias asociadas y dada la posibilidad de realizar estudios genéticos para la detección de otros casos dentro de una misma familia. Objetivo: Exponer aspectos clínico-epidemiológicos de esta enfermedad y las implicaciones perioperatorias concernientes a la conducta anestésica perioperatoria de estos pacientes. Caso clínico: Paciente femenina, de 56 años de edad con antecedentes de hipertensión arterial de más dos años de evolución y difícil control, diabetes mellitus e historia de cefalea, palpitaciones y sensación de calor. Diagnóstico imagenológico de una masa pararrenal sospechosa de paraganglioma que se confirmó por estudio histopatológico posterior a la exceresis. Conclusiones: La técnica anestésica para la resección de estos tumores es un desafío importante para el anestesiólogo que debe enfrentar dicha intervención quirúrgica, planteándose los riesgos de las etapas pre-, intra- y posoperatoria

The Spartina Schreb. genus is composed of C[sub 4] perennial grasses in the family Poaceae. They are native to the coasts of the Atlantic Ocean in western and southern Europe, north-west and southern Africa, the Americas and the southern Atlantic Ocean islands. Most species are salt tolerant and colonise coastal or inland saltmarshes. The available literature on heavy metal bioaccumulation by Spartina sp. was compiled and compared. Spartina alterniflora Loisel. and Spartina maritima (Curtis) Fernald were the most commonly researched species of the genus, whereas many species were not represented at all. In contrast, Cu and Zn are the most intensively researched heavy metals. The few studies dealing with the physiological impacts of heavy metals or the mechanisms of metal accumulation, which involve extracellular and intracellular metal chelation, precipitation, compartmentalisation and translocation in the vascular system, were documented. Bioaccumulation of metals in roots and tillers of some species of the Spartina genus (e.g. S. maritima and Spartina densiflora Brongn.) has been described as a feasible method for remediating waters and soils contaminated with heavy metals. One such example is Spartina argentinensis Parodi, which has been found to be a Cr-hyperaccumulator; it can concentrate chromium in its tissues to levels far exceeding those present in the soil. Heavy metal pollution is a major environmental problem that is rapidly gaining importance due to its impact on human health through the food chain. Phytoremediation is considered an effective, low cost, preferred cleanup option for moderately contaminated areas. Authors, teachers, students, family members, and other citizens from diverse cultures, gender options and countries have been and are developing critical pedagogy all over the world. However, there are authors of "critical pedagogy" who use this label for luxuries and egotism but who have never transformed nor supported the transformation of any school or educational project. This article presents four criteria to distinguish between the critical pedagogy that transforms reality and the "critical pedagogy" that only benefits those who use this label. The first criterion is the egalitarian dialogue that critical pedagogy authors use with very diverse citizens to achieve extensive and profound real transformations. The second one is the social impact of their work on society, especially on the oppressed, overcoming inequalities and improving their conditions of life. The third one is the equality of results of the oppressed in literacy as well as in sentiments and values, without segregating. The fourth one is the critical pedagogy style versus the Althusserian and market styles, highlighting the scientific and theoretical rigor of critical pedagogy authors opposed to the lack of theoretical basis of authors of "critical pedagogy".

Fundamento: El incremento de la enfermedad meningocócica por serogrupo C en la Comunidad Valenciana durante 1996-1997 motivó la realización de una campaña de vacunación antimeningocócica A+C en la población de 18 meses a 19 años de edad (entre septiembre y diciembre de 1997). El objetivo de este estudio es analizar el impacto de la campaña en cuanto a la epidemiología, clínica, evolución de la enfermedad meningocócica y estado de vacunación de la población infantil con el fin de valorar la efectividad vacunal. Métodos: Los datos se obtuvieron de las historias clínicas de los niños menores de 15 años, que presentaron síntomas y signos clínicos sugestivos de enfermedad invasora con aislamiento de Neisseria. meningitidis y/o que cumplían con los criterios de definición de caso establecidos, atendidos en todos los hospitales públicos de la Comunidad Valenciana entre 1996-2000. La evolución de la incidencia se valoró mediante tasas de incidencia. La clínica y su evolución (secuelas y letalidad) mediante la frecuencia y distribución por serogrupo y edad. La efectividad vacunal se calculó según la fórmula de Orestein. Resultados: Se registró un total de 302 casos de enfermedad invasora por N. meningitidis. La tasa de incidencia por serogrupo C en niños menores de 15 años disminuyó tras la campaña de vacunación desde 5,82/10(5) habitantes en 1997 a 1,68/10(5) habitantes en 1998. Tres años después de la campaña se ha vuelto a tasas similares a la época prevacunal, con un aumento por serogrupo B en los 2 últimos años. El 61% de las secuelas se da en menores de 5 años. La letalidad fue más elevada para el serogrupo C. La efectividad vacunal al tercer año tras la campaña ha sido del 83,7% para el grupo de 5-14 años y del 69,1% para el grupo de 19 meses-4 años. Conclusión: La vacuna polisacarídica se mostró efectiva para interrumpir el brote. La disminución de la incidencia del serogrupo C es atribuible a la efectividad vacunal obtenida.

Fundamento: La introducción de la vacuna conjugada anti Haemophilus influenzae tipo b (Hib) en niños ha provocado un llamativo descenso de la incidencia de la enfermedad por H. influenzae. El objetivo de este estudio es analizar las características más relevantes de la enfermedad invasora por H. influenzae en cuanto a la epidemiología, clínica, evolución y estado de vacunación de la población infantil de la Comunidad Valenciana en el periodo 1996-2000. Método: Los datos se recogen de las historias clínicas de los niños menores de 15 años que hayan presentado síntomas y signos clínicos sugestivos de enfermedad invasora con aislamiento de Haemophilus influenzae y/o que cumple con los criterios de definición de caso establecidos, atendidos en todos los hospitales públicos de la Comunidad Valenciana entre 1996 y 2000. La evolución de la incidencia se valoró mediante tasas de incidencia. La clínica y su evolución (secuelas y letalidad) mediante la frecuencia y distribución por edad. Resultados: Se registraron un total de 36 casos de enfermedad invasora por Haemophilus influenzae. La tasa de incidencia en niños menores de 15 años pasó de 3,56/10(5) en 1996 a 1,07/10(5) en 1997 (coincidiendo con la campaña de vacunación y la posterior inclusión de la vacuna conjugada anti Hib en el Calendario de Vacunaciones Sistemáticas de la Comunidad Valenciana) y 0,30/10(5) en 1998, situación que se sigue manteniendo en los años posteriores. El 53% de los casos se dan en menores de 18 meses. Tanto las secuelas como los fallecimientos se producen en la época anterior a la aplicación rutinaria de la vacuna conjugada. Ningún niño vacunado correctamente falleció. Se registraron 2 casos de H. influenzae tipo no b en niños vacunados. Conclusiones: La incidencia de la infección por Haemophilus influenzae tipo b disminuyó drásticamente desde el inicio de la vacunación sistemática de la población infantil.

49. Cross comparison of alternative diagnostic protocols including substitution to the clinical sample, RNA extraction method and nucleic acid amplification technology for COVID-19 diagnosis.

- Abstract
Background: the gold-standard diagnostic protocol (GSDP) for COVID-19 consists of a nasopharyngeal swab (NPS) sample processed through traditional RNA extraction (TRE) and amplified with retrotranscription quantitative polymerase chain reaction (RT-qPCR). Multiple alternatives were developed to decrease time/cost of GSDP, including alternative clinical samples, RNA extraction methods and nucleic acid amplification. Thus, we carried out a cross comparison of various alternatives methods against GSDP and each other., Methods: we tested alternative diagnostic methods using saliva, heat-induced RNA release (HIRR) and a colorimetric retrotranscription loop-mediated isothermal amplification (RT-LAMP) as substitutions to the GSDP., Results: RT-LAMP using NPS processed by TRE showed high sensitivity (96%) and specificity (97%), closely matching GSDP. When saliva was processed by TRE and amplified with both RT-LAMP and RT-qPCR, RT-LAMP yielded high diagnostic parameters (88%-96% sensitivity and 95%-100% specificity) compared to RT-qPCR. Nonetheless, when saliva processed by TRE and detected by RT-LAMP was compared against the GSDP, the resulting diagnostic values for sensitivity (78%) and specificity (87%) were somewhat high but still short of those of the GSDP. Finally, saliva processed with HIRR and detected via RT-LAMP was the simplest and fastest method, but its sensitivity against GSDP was too low (56%) for any clinical application. Also, in this last method, the acidity of a large percentage of saliva samples (9%-22%) affected the pH-sensitive colorimetric indicator used in the test, requiring the exclusion of these acidic samples or an extra step for pH correction., Discussion: our comparison shows that RT-LAMP technology has diagnostic performance on par with RT-qPCR; likewise, saliva offers the same diagnostic functionality as NPS when subjected to a TRE method. 50. Physiological Responses in Trail Runners during a Maximal Test with Different Weighted-Vest Loads.

- Abstract
During some trail running races, athletes have to carry hydration support, food, and technical and safety equipment, which generates an additional load that must be mobilized during the race. The aim of the present study was to determine the physiological responses to overload running and the effect they may have on metabolic zones. Seventeen well-trained male trail runners (n = 17) completed three maximal treadmill tests with weighted vests at 0%, 5%, and 10% of their body mass (L0, L5, and L10). Their gas exchange was monitored to assess their ventilatory thresholds 1 (VT1) and 2 (VT2), maximal fat oxidation zone (FatMax), and peak oxygen consumption (VO
2peak ). Their heart rate (HR), power, and velocity (V) were tracked to compare their behavior. One-way ANOVA showed significant differences in the V ( p < 0.001; ηp2 = 0.4620) as a limitation for reaching the peak velocity (Vpeak ), with a significant decrease in the Vpeak with the L10 compared to the L0 ( p = 0.002) and L5 ( p = 0.004). In addition, one-way ANOVA showed significant differences in the peak absolute power ( p < 0.001; ηp2 = 0.468) among the groups, detecting higher power production between the L10 and L0 ( p < 0.001) and between the L10 and L5 ( p = 0.015). Loads higher than L5 could generated important physiological and mechanical modifications, while a load of L5 managed to maintain the working conditions without overloading.
