Abstract

SAFRON II was a randomized phase II trial testing single vs multi-fraction (fx) lung SBRT in patients (pts) with pulmonary oligometastases (OM). Its primary endpoint was toxicity, and secondary endpoints included local control (LC) and overall survival (OS). It showed no significant difference between the study arms in terms of freedom from local failure (LF) and OS. However, a post hoc exploratory analysis suggested LF in the single-fx arm was higher for patients with colorectal (CRC) primaries. We conducted a retrospective review (RR) of our institutional experience treating CRC pulmonary OM with single and multi-fx SBRT with respect to LC and OS. We queried an IRB-approved lung SBRT registry for pts with peripheral lung CRC OM lesions between 2005-2023. Pts were assessed for LC, OS, and toxicity. Rates of LF were calculated using cumulative incidence analysis with death treated as a competing event, and rates of OS were calculated using actuarial analysis. Competing risk regression was used to identify factors associated with LF. 78 pts met eligibility criteria and 133 lesions were treated. 35 pts (44.9%) were alive at time of analysis. Median clinical follow-up (fu) time was 27.9 months (mos) (15.1 mos for single fx and 40.4 mos for multi-fx). The median age was 62.4 years (range = 42.5-88.5), 53.8% were female, and median KPS 90 (range = 60-100). 32 (41.0%) pts had multiple lesions treated and median number of synchronous lesions treated was 3 (range = 2-5). Median size was 1.1 cm (range = 0.3-4.0). KRAS status was known in 61 pts (78.2%) and 41% of all pts were KRAS mutated. 40 lesions (30.1%) were treated with single fx (30 Gy/1 fx [n = 0.8%], 34 Gy/1 fx [n = 29.3 %]) and 93 lesions (69.1%) were treated with multi-fx (50 Gy in 5 fx [24.8%], 60 Gy in 5 fx [20.3%], and 60 Gy in 3 fx [15.0%]). Cumulative incidence of LF at 12 months was 5.9% (95% CI = 2.6-11.1) for all lesions, and 6.6% (95% CI = 2.7-13.0) in the multi-fx group vs 3.4% (95% CI = 0.2-14.9) in the single fx group (P = 0.70). On univariate analysis, single fx was not predictive of LF (HR = 1.40; 95% CI, 0.312-6.29; P = 0.13), and tumor size was predictive of LF (HR = 1.83, 95% CI, 1.08-3.09, P = 0.03). When stratified by tumor size (≤1 vs >1 cm), fractionation was not significant for LF for tumors >1 cm (HR = 0.97; 95% CI, 0.12-7.9) or for tumors ≤1 cm (HR = 1.17; 95% CI, 0.12-12.0). KRAS mutation did not predict for LF (HR = 1.28, 95% CI = 0.37-3.47, P = 0.70). Median OS for the multi-fx arm was 68.9 mos and median survival for the single fx arm was not reached. There were 15 total adverse events reported. Grade 1-2 chest wall toxicity was reported in 9 (6.8%) lesions- 6 (15%) in single fx vs 3 (3.2%) in multi-fx. Grade 2 pneumonitis occurred in 5 lesions (3.8%)- 2 (5.0%) in single fx and 3 (3.2%) in multi-fx. This RR supports single fx SBRT to 34 Gy in 1 fx in the treatment of CRC pulmonary OM given its efficacy and safety. Larger lesions may be more likely to fail locally independent of fractionation. Abstract

Lung stereotactic body radiotherapy [SBRT] is the standard of care for curative management of medically inoperable early-stage lung cancer [ES-LC] due to its excellent local control and minimal treatment-related toxicity. Patients (pts) with organ transplants are chronically immunosuppressed, with increased risk of developing cancer. We wished to characterize outcomes for transplant pts with inoperable ES-LC treated with SBRT. We surveyed our institutional review board-approved prospective lung SBRT data registry from 2003 to 2023 for medically inoperable ES-LC solid organ transplant pts. Patterns of failure were assessed, as well as overall survival (OS) and disease-free survival (DFS). Univariable prognostic factors for OS and PFS were identified with Cox proportional hazards regression. For the 20-year interval, 28 of 1976 definitive pts (1.4%) met study criteria, with 17.9% alive at analysis. Median follow up was 12.4 months. Pt characteristics included: male (52.9%), median pack-years smoking 34; 7.1% smoking at SBRT; median age 70.0 years, median KPS 80. Organs transplanted were lung (57.1%), liver (21.4%), heart (21.4%). Median time from transplant to SBRT was 5.7 years. Tumor characteristics included: median size 2.4 cm, median PET SUVmax 7.8, 85.7% with biopsy proven cancer. The median SBRT schedule was 50 Gy/5 fractions (32.1%). Toxicity was reported in 9 (32.1%) pts: grade 1/2 in 8 pts (28.6%) and grade 5 (pneumonitis) in 1 pt (3.5%), with no differences in rates by organ transplanted. Failure patterns were: local 21.4%, lobar 7.1%, nodal 10.7% and distant 32.1%. First site of failure was distant [alone or in combination] in 50.1% pts. Median DFS and OS were 17.1 and 14.5 months, respectively. Metastatic cancer was the cause of death in 25% patients. Univariate analysis revealed increasing pack-years smoking was associated with increased disease failure [p = 0.0016], but not with any other patient, tumor or transplant factors. Multivariate analysis revealed OS significantly associated with KPS [p = 0.0075] and tumor size [p = 0.0181]. Compared to historical SBRT results for ES-LC, transplanted pts had higher local failure, decreased cancer control and poorer overall survival. To explain these findings, we hypothesize that transplant-associated immunosuppression promotes understaging at diagnosis, enhanced metastatic progression despite treatment and a tumor biology resistant to SBRT. Abstract

The first report on the production of all three major classes of lignin-modifying enzymes (LMEs) by Flavodon flavus, which highlights the bioremediation potential of this organism in both terrestrial and marine environments, is presented. These three major classes of extracellular LMEs are manganese-dependent peroxidase (MNP), lignin peroxidase (LIP) and laccase. Low MNP and laccase activities were observed in high-nitrogen medium, but no LIP activity was detected. Meanwhile, relatively high MNP and moderate levels of laccases were seen in media containing lignocellulosic substrates, but LIP production was either absent or minimal.

Abstract

Remnants of small and large intestines found with the skeletal remains of mastodons (Mammut americanum) in late-Pleistocene pond sediments in Ohio and Michigan were examined. Specifically, microbiological analyses of these organic masses were conducted to determine whether facultatively anaerobic gram-negative rods or obligately anaerobic organisms were associated with the assemblages of plant material, believed to be incompletely digested food remains from the mastodons and radiocarbon dated to about 11,500 years before the present, that were found in the intestines. The results are discussed.

Abstract

Laccase plate assays show that cultures of the Phanerochaete chrysosporium BKM-F1767 placed in a medium containing cellulose and ammonium tartrate produce extracellular laccase. Spectrophotometric assay and sodium dodecyl sulfate-polyacrylamide gel electrophoresis also show laccase activity in the concentrated extracellular culture medium. Induced laccase activity is observed in cultures containing cellulose but not in those containing glucose. Laccase is involved in lignin biodegradation.

Abstract

The white rot fungus Phaenerochaete chrysosporium mineralizes many chloroaromatic environmental pollutants to CO2. Congener-specific gas chromatographic analysis of this fungus shows that it degrades high concentrations of less and high chlorinated Aroclors 1242, 1254, and 1260. The ability of the fungus to degrade PCBs is not affected by nutrient rich conditions.

Abstract

The construction of DNA probes specific for genes encoding vital lignin peroxide (LIP) isozymes H2, H8 and H10 of Phanerochaete chrysosporium, was conducted. These probes were employed to examine the temporal expression of the three lip genes in determined low-nitrogen mediums. H2 gene transcripts were synthesized in high levels on days 4, 5 and 7. Low levels of H2 gene transcripts were synthesized on day 6. No relationship was discerned between lip transcript and isozyme levels.

Abstract

It was proved that mineralization of 2,4-dichlorophenoxyacetic acid (2,4-D) in nutrient-rich media occurs by wild-type Phanerochaete chrysosporium and by the peroxidase-negative mutant of this species. Nearly 83% recovery of radioactivity was revealed by mass balance analysis of 2,4-D-mineralization within malt extract cultures. Nearly 38.6% of this 82.7% was recorded to have been released as 14CO2 and 27.0, 11.2 and 5.9% were located in the aqueous methylene chloride and mycelial fractions. It was concluded that the P. chrysosporium organism successfully mineralizes 2,4-D.

The simultaneous degradation of benzene, toluene, ethylbenzene and xylenes (BTEX) is discussed. Gas chromatography, high performance liquid chromatography and mineralization experiments were made on bacterial strains of the white rot fungus Phanerochaete chrysosporium to test its degradation of BTEX. The extentive degradation of BTEX was accomplished to include its toluates.
