Purpose: Very old critical ill patients are a rapid expanding group in the ICU. Indications for admission, triage criteria and level of care are frequently discussed for such patients. However, most relevant outcome studies in this group frequently find an increased mortality and a reduced quality of life in survivors. The main objective was to study the impact of frailty compared with other variables with regards to short-term outcome in the very old ICU population. Methods: A transnational prospective cohort study from October 2016 to May 2017 with 30days follow-up was set up by the European Society of Intensive Care Medicine. In total 311 ICUs from 21 European countries participated. The ICUs included the first consecutive 20 very old (≥80years) patients admitted to the ICU within a 3-month inclusion period. Frailty, SOFA score and therapeutic procedures were registered, in addition to limitations of care. For measurement of frailty the Clinical Frailty Scale was used at ICU admission. The main outcomes were ICU and 30-day mortality and survival at 30days. Results: A total of 5021 patients with a median age of 84years (IQR 81–86years) were included in the final analysis, 2404 (47.9%) were women. Admission was classified as acute in 4215 (83.9%) of the patients. Overall ICU and 30-day mortality rates were 22.1% and 32.6%. During ICU stay 23.8% of the patients did not receive specific ICU procedures: ventilation, vasoactive drugs or renal replacement therapy. Frailty (values ≥5) was found in 43.1% and was independently related to 30-day survival (HR 1.54; 95% CI 1.38–1.73) for frail versus non-frail. Conclusions: Among very old patients (≥80years) admitted to the ICU, the consecutive classes in Clinical Frailty Scale were inversely associated with short-term survival. The scale had a very low number of missing data. These findings provide support to add frailty to the clinical assessment in this patient group. - Abstract
Although septic embolization associated with infective endocarditis is relatively frequent, mycotic cerebral aneurysms are a rare and potentially fatal complication. The authors report the case of a woman admitted with a cerebral haemorrhage due to mycotic aneurysm rupture, which led to a subacute infective endocarditis diagnosis. The patient underwent craniotomy with aneurysm excision and mitral valvuloplasty due to severe valvular insufficiency, with a favorable clinical outcome. The authors make a brief review, highlighting the challenge of the management of these patients, especially at surgical approach, which requires an individualized therapy strategy based on patient evolution.

Abstract Background The risk of coronary heart disease (CHD) is related to environmental factors and genetic variants. Apolipoprotein E (apoE) polymorphisms are heritable determinants of total and low-density lipoprotein cholesterol, with some authors suggesting an association between the ε4 allele and CHD. We investigated the relationship between apoE genotype and age at referral to a specialized lipid clinic by the primary care physician and whether the benefits of treatment with statin differed between genotypes. Methods We assessed individual apoE genotypes and lipid blood profile in a total of 463 patients followed at a specialized lipid clinic due to dyslipidemia, with a 3-year median follow-up time. The primary care physician at the time of the referral had no access to the apoE genotyping results. Carriers of apoE ε4/ε2 genotype were excluded. Results The frequencies of ε2, ε3 and ε4 alleles were 7.8, 78.9 and 13.3%, respectively. There were no significant differences between genders. Although with similar lipid profiles and antidyslipidemic drug usage at baseline, ε4-carriers were referred to the clinic at a younger age (44.2 ± 14.7 years) compared with non-ε4 carriers (50.6 ± 13.8 years) (p < 0.001), with a substantially younger age of referral for homozygous E4/4 and for all genotypes with at least one copy of the ε4 allele (p < 0.001 for trend). Although both ε4 and non-ε4 carriers achieved significant reductions in total cholesterol during follow-up (p < 0.001 vs. baseline), the mean relative decrease in total cholesterol levels was higher in non-ε4 carriers (-19.9 ± 2.3%) compared with ε4 carriers (-11.8 ± 2.3%), p = 0.003. Conclusion Our findings support the concept that there is a reduced response to anti-dyslipidemic treatment in ε4 carriers; this can be a contributing factor for the earlier referral of these patients to our specialized lipid clinic and reinforces the usefulness of apoE genotyping in predicting patients response to lipid lowering therapies.

- Abstract
Background: We aimed to evaluate differences in outcome between patients admitted to intensive care unit (ICU) after elective versus acute surgery in a multinational cohort of very old patients (80 years; VIP). Predictors of mortality, with special emphasis on frailty, were assessed. Methods: In total, 5063 VIPs were induded in this analysis, 922 were admitted after elective surgery or intervention, 4141 acutely, with 402 after acute surgery. Differences were calculated using Mann-Whitney-U test and Wilcoxon test. Univariate and multivariable logistic regression were used to assess associations with mortality. Results: Compared patients admitted after acute surgery, patients admitted after elective surgery suffered less often from frailty as defined as CFS (28% vs 46%; p < 0.001), evidenced lower SOFA scores (4 +/- 5 vs 7 +/- 7; p < 0.001). Presence of frailty (CFS >4) was associated with significantly increased mortality both in elective surgery patients (7% vs 12%; p = 0.01), in acute surgery (7% vs 12%; p = 0.02). Conclusions: VIPs admitted to ICU after elective surgery evidenced favorable outcome over patients after acute surgery even after correction for relevant confounders. Frailty might be used to guide clinicians in risk stratification in both patients admitted after elective and acute surgery.

Relatório de Estágio apresentado à Escola Superior de Educação de Lisboa para obtenção de grau de mestre em Ensino do 1.º e do 2.º Ciclo do Ensino Básico O presente relatório intitulado O património local como recurso para o desenvolvimento de competências essenciais da História no 1.º ciclo cumpre um dos requisitos para a obtenção do grau de mestre em Ensino do 1.º e 2.º Ciclo do Ensino Básico, na Escola Superior de Educação de Lisboa. Partindo da intervenção realizada no âmbito da Unidade Curricular de Prática de Ensino Supervisionado II numa turma do 3.º ano de escolaridade do 1.º Ciclo do Ensino Básico, este documento pretende refletir sobre essa prática, evidenciando como pode uma abordagem ativa ao património local contribuir para a formação de alunos historicamente competentes e, bem assim, para o desenvolvimento de competências transversais às restantes áreas disciplinares. Começou por estabelecer-se um breve quadro conceptual, no qual, fundamentalmente, se definiu o conceito de património local e se evidenciou a sua intrínseca relação com a construção de uma identidade local e nacional, se situou o estudo desta temática no 1º CEB, se esclareceram quais as competências específicas da História a atingir nesse mesmo ciclo de ensino e se justificaram as atividades e estratégias adotadas na sua abordagem. Passou-se então ao relato e à análise da prática desenvolvida, onde se procurou evidenciar que a implementação de atividades de carácter mais prático, como análises de imagens, trabalho investigativo e visitas de estudo, relacionadas com o património local tornam essa aprendizagem mais significativa, e concorre para a aquisição de competências essenciais da História, como a recolha de informação e o seu tratamento, a contextualização histórica e a comunicação em história. Tal abordagem contribui ainda para o sucesso das aprendizagens nas restantes áreas disciplinares, uma vez que os alunos desenvolvem competências transversais importantes para o incremento do seu processo de ensino-aprendizagem. ABSTRACT This report entitled Local patrimony as a resource for the development of essential skills in History in the 1st school years has the purpose of the obtainment of the master’s degree in teaching 1st and 2nd cycle of basic education, at “Escola Superior de Educação de Lisboa”. Starting from the intervention carried out under the Education Practice Course Supervised II in a 3rd year class of the 1st cycle of basic education, this document aims consider this practice, showing how an active approach to local heritage can contribute to the formation of historically skilled students, as well as for the development of soft skills related to other teaching areas. A brief conceptual framework was first established in which the concept of local heritage was determined in order to show its intrinsic relationship with the construction of a local and national identity and to situate the study of this issue in the 1st CEB. Some specific skills in history, to be obtained in this cycle of education, were clarified and the proper activities and strategies were adopted in their approach. The attention was then turned to the reporting and analysis of developed practice where one tried to show that the implementation of more practical activities such as image analysis , investigative tasks and school trips related to the local heritage make learning more meaningful and contribute to the acquisition of key competences of history, such as the collection of information and its treatment , the historical context and communication. Such an approach also contributes to the success of learning in other subject areas , as students develop important soft skills to increase their teaching-learning process.

Mestrado em Economia Internacional e Estudos Europeus A importância crescente da globalização faz com que a preservação dos valores culturais seja crítica na determinação da competitividade dos lugares turísticos. A Antiga Confeitaria de Belém em Lisboa é um símbolo regional de incontestável valor e os pastéis de Belém são, não só uma insígnia regional, mas um doce reconhecido mundialmente. A presente dissertação explora as motivações e intenções comportamentais dos turistas estrangeiros pelo doce mais conceituado da região de Lisboa, sugerindo o seu impacto na procura turística. Esta dissertação foi baseada numa análise exploratória com recurso a um inquérito elaborado para alcançar os objetivos da investigação. Este inquérito foi realizado a turistas estrangeiros dos cinco continentes que visitaram as instalações da Antiga Confeitaria de Belém. A análise exploratória na sua essência propõe-se avaliar a importância da autenticidade da doçaria regional na satisfação dos turistas estrangeiros. Os resultados mostram que os turistas estrangeiros preferem produtos típicos quando visitam um local em vez de produtos e marcas internacionais. Tais resultados foram interpretados com base na literatura do Comportamento do Consumidor, presente em artigos científicos, livros e outros trabalhos de investigação. Também se utilizou como suporte informação relativa aos pastéis de Belém, assim como dados estatísticos relacionados com o turismo. Tendo em conta a natureza exploratória deste trabalho, os resultados alavancam novas propostas que se espera poderem alimentar futuras investigações. The increasing importance of globalization makes the preservation of the cultural values critical to establish the competitiveness of a tourism destination. The Antiga Confeitaria de Belém in Lisbon is a regional symbol of incontestable value and the pastéis de Belém are, not only a regional ensign, but a sweet known worldwide. The present dissertation explores the behavioral motivations and intentions of the foreign tourists for the most renowned sweet of Lisbon region, suggesting its impact in the touristic demand. This dissertation was based on an exploratory analysis from an investigation with a survey designed to achieve the aims of the research and applied to tourists of five continents that visited the Antiga Confeitaria de Belém facilities. The exploratory analysis in its essence proposes to evaluate the authenticity importance of the regional sweets in foreign tourist's satisfaction. The results show that foreign tourists prefer typical products when they visit a place instead of international products and brands. Such results were interpreted based on Consumer Behavior literature present in scientific articles, books and other research works. This study was also supported by information about pastéis de Belém, as well statistical data related with tourism. Considering the exploratory nature of this work, the results leverage new proposals that are expected to nurture future investigations.

20. Exploring the intertwining between human rights and restorative justice in private cross-border disputes.

International human rights instruments operate on the assumption that states are the focal human rights duty bearers. However, private actors can harm human rights as well. Moreover, since mechanisms at a supranational level are lacking, these instruments rely primarily on states for their enforcement. Yet states' internal rules and courts are meant to address infringements that are confined within their borders, and are therefore often structurally unable to deal with violations having transnational impact. Restorative justice has proven to respond in depth to different kinds of wrongdoing and, although addressing the peculiarities of each case, restorative procedures can systemically prevent deviant behaviour as well. Additionally, as restorative justice relies on voluntary participation it need not operate in a specific territory. Having this broader picture in mind, the article explores whether restorative justice might be adequate for dealing with human rights infringements perpetrated by private actors that have cross-border impact. The most benign form is OI type I. The authors present a case report of a 25-year-old woman who had severe low back pain associated with incapacity to walk and breast-feed post-partum. Symptoms developed 2 weeks after delivery. The radiological examination revealed severe osteoporosis with no abnormalities in the laboratory findings. The clinical signs and a positive personal and family history of multiple fractures in childhood suggested OI type I, although other diagnosis, such as pregnancy-associated osteoporosis, was also considered. The atypical presentation of this rare disorder in adulthood calls attention to the need for early diagnosis for prompt treatment. - Abstract
Chronic periaortitis (CP) refers to a spectrum of diseases whose common denominator is a fibro-inflammatory tissue developing in the periaortic space and frequently encasing surrounding structures like the kidney and ureters. There is no unified concept regarding the primary aetiology of CP, but recent studies have demonstrated that CP may present features of auto-immune diseases. CP involves three main entities, namely idiopathic retroperitoneal fibrosis (IRF), inflammatory aneurysms of the abdominal aorta (IAAAs) and perianeurysmal retroperitoneal fibrosis (PRF). These entities are usually diagnosed using computed tomography or magnetic resonance imaging, which typically show a retroperitoneal mass surrounding the aorta and that extends laterally without displacing it. Positron emission tomography is useful for the full assessment of the extent of the disease and its metabolic activity. The inflammatory and chronic relapsing nature of these diseases compels the use of medical therapy, which is based on high--dose steroids with a tapering scheme combined with immunosuppressive agents in refractory or relapsing disease. The authors report the clinical and radiological characteristics of a nonaneurysmatic form of chronic periaortitis in a woman presented with pericarditis, pericardial effusion and a pancreatitis. PURPOSE: To document and analyse the decision to withhold or withdraw life-sustaining treatment (LST) in a population of very old patients admitted to the ICU. METHODS: This prospective study included intensive care patients aged ≥ 80 years in 309 ICUs from 21 European countries with 30-day mortality follow-up. RESULTS: LST limitation was identified in 1356/5021 (27.2%) of patients: 15% had a withholding decision and 12.2% a withdrawal decision (including those with a previous withholding decision). Patients with LST limitation were older, more frail, more severely ill and less frequently electively admitted. Patients with withdrawal of LST were more frequently male and had a longer ICU length of stay. The ICU and 30-day mortality were, respectively, 29.1 and 53.1% in the withholding group and 82.2% and 93.1% in the withdrawal group. LST was less frequently limited in eastern and southern European countries than in northern Europe. The patient-independent factors associated with LST limitation were: acute ICU admission (OR 5.77, 95% CI 4.32-7.7), Clinical Frailty Scale (CFS) score (OR 2.08, 95% CI 1.78-2.42), increased age (each 5 years of increase in age had a OR of 1.22 (95% CI 1.12-1.34) and SOFA score [OR of 1.07 (95% CI 1.05-1.09 per point)]. The frequency of LST limitation was higher in countries with high GDP and was lower in religious countries. CONCLUSIONS: The most important patient variables associated with the instigation of LST limitation were acute admission, frailty, age, admission SOFA score and country. 37. From Bronchodilation to Lactic Acidosis: A Case Report on Salbutamol's Adverse Effect.

- Abstract
Asthma is one of the most prevalent chronic respiratory diseases, characterized by bronchial hyper-responsiveness and intermittent airflow obstruction. Short-acting β
2 agonists (SABA) remain the cornerstone of acute asthma management due to its properties in smooth muscle relaxation and bronchodilatation. Rarely, these drugs might be associated with adverse effects, including the development of metabolic and hydro-electrolytic imbalances. We report a case of lactic acidosis secondary to β2 agonists in a young female patient admitted with severe acute asthma. After initial management and significant improvement of the respiratory distress with nebulized and subcutaneous salbutamol, the patient developed high anion gap metabolic acidosis due to hyperlactacidemia and hypokalemia. Alternative causes of lactic acidosis were discarded, such as severe hypoxemia, systemic hypoperfusion, sepsis, and organ dysfunction, and SABA-related lactic acidosis was suspected. This treatment was halted, and tachypnea, metabolic acidosis, and lactate levels rapidly resolved. The remainder of the hospital stay was uneventful, and the patient was discharged after a period of five days. Although rare, the development of unexplained lactic acidosis in a SABA-treated patient should alert the treating physician to this β2 agonist side-effect., Competing Interests: Human subjects: Consent was obtained or waived by all participants in this study. 39. Catastrophic Gastrointestinal Bleeding: Always Consider Meckel's Diverticulum.

- Abstract
Meckel's diverticulum, a congenital malformation of the gastrointestinal tract, is asymptomatic in the majority of patients but can be associated with some complications. Gastrointestinal bleeding is one such complication and is more common in children than in adults. Despite the variety of examinations available, diagnosis can be difficult, especially in older patients, because the sensitivity of examinations decreases with patient age. Here we present the case of a young man with gastrointestinal bleeding in whom a diagnosis of Meckel's diverticulum was made intra-operatively., Learning Points: Meckel's diverticulum is more commonly found in children than in adults and can cause gastrointestinal bleeding.The diagnosis of Meckel's diverticulum can be complicated, especially in adults because the sensitivity of examinations decreases with patient age.Despite appropriate diagnostic evaluation, Meckel's diverticulum is sometimes only diagnosed at surgery., Competing Interests: Conflicts of Interests: The Authors declare that there are no competing interest, (© EFIM 2019.)

- Abstract
Although most patients remain clinically euthyroid, some develop amiodarone-induced hyperthyroidism (HPEAI) or hypothyroidism (HPOAI). The authors present a retrospective analysis of ten patients with amiodarone-induced thyroid dysfunction. Six patients were female and mean amiodarone intake was 17.7 months. HPOIA was more common (six patients). From all the patients with HPEAI, two had type 2, one had type 1, and one had type 3 hyperthyroidism. Symptoms suggestive of thyroid dysfunction occurred in five patients, most of them with HPOAI. In HPEAI, the most frequent symptom was exacerbation of arrhythmia (three patients). Discontinuation of amiodarone and treatment with levothyroxine was chosen in 83.3% of the HPOAI cases, while thyonamide treatment with corticosteroids and without amiodarone was the option in 75% of the HPEAI cases. There were three deaths, all in patients with HPEAI. HPEAI is potentially fatal. The clinical picture may be vague, so the thyroid monitoring is mandatory.

41. [Hypovitaminosis D in patients admitted to an internal medicine ward].

- Abstract
Introduction: Hypovitaminosis D (hypoD) is a vitamin deficiency that has been rising in the developed countries, due not only to inappropriate eating habits, but also because of lower sun exposure and lack of exercise., Objective: To determine the prevalence of vitamin D deficiency and to analyze associated factors, in patients admitted to an Internal medicine Ward., Methods: Cross-sectional study which included 123 hospitalized patients admitted to an Internal Medicine Ward between April and May. Serum levels of vitamin D [25-hydroxyvitamin D, 25(OH)D] were determined and the sample divided into three groups based on 25(OH)D levels: severe hypoD (<10ng/mL), moderate deficiency of 25(OH)D (>10ng/mL and <20ng/mL) and third group with normal levels of 25(OH)D (>20ng/mL). Demographic variables were recorded as were factors potentially related to vitamin D deficiency., Results: In this sample (52.0% women) the average age was 71 ± 17 years, 67.5% of patients had severe hypoD, 25.2% moderate deficiency and 7.3% normal levels of vitamin D. The patients in the group with severe hypoD were older (p=0.027). In the same group there was a higher percentage of patients in the bedridden state (p=0.022), with higher impaired functional capacity (p=0.009) and with chronic renal insufficiency (p=0.011). In multivariate logistic regression analysis, factors associated with an increased likelihood of severe hypoD were: furosemide treatment [OR=3.673 (1.232-10.956) p=0.020] and albumin ≤3.25 [OR=5.617 (2.257-13.981) p<0.001]., Conclusion: The high prevalence of hypoD (67.5%) in this sample expresses the need for systematic evaluation of serum levels of 25(OH)D, in order to initiate early treatment in patients with inadequate levels. Furosemide treatment and hypoalbuminaemia present an increased likelihood of being associated with severe hypoD. Treatment of this hypovitaminosis is warranted not only because of the clinical consequences related to bone loss, but also because of its relationship with hypoalbuminemia which is associated with a poorer prognosis in hospitalized patients.
