Abstract

Restrictive cardiomyopathies (RCMs) are a diverse group of myocardial diseases with a wide range of aetiologies, including familial, genetic and acquired diseases and ranging from very rare to relatively frequent cardiac disorders. In all these diseases, imaging techniques play a central role. Advanced imaging techniques provide important novel data on the diagnostic and prognostic assessment of RCMs. This EACVI consensus document provides comprehensive information for the appropriateness of all non-invasive imaging techniques for the diagnosis, prognostic evaluation, and management of patients with RCM.

Abstract

International audience; Background and objectives: Hypertrophic cardiomyopathy (HCM) is expected to be caused by a heterozygous mutation in one of the 5 main sarcomere genes (MYBPC3, MYH7, TNNT2, MYL2, TNNI3). However, the routine diagnostic strategy consisting of the sequencing of these 5 genes allows identification of a mutation in only 50% of familial or sporadic cases, limiting accurate genetic counseling in families and predictive diagnosis in at-risk subjects. Purpose: We aimed at evaluating the value of Whole Exome Sequencing (WES), comparing its diagnostic yield with the routine diagnostic strategy, and clarifying the genetic spectrum of HCM. Methods: We performed WES in a large series of 200 new unrelated patients with primary HCM using the Agilent SureSelect V6 technology on Illumina Hiseq 2500. Patients were recruited between June 2015 and October 2016 in 5 French centers. In a first step of analysis, a panel of 145 genes involved in various hereditary cardiac diseases was searched for mutations. Bioinformatics analyses were performed in our lab. Only "certainly" (class 5) or "probably" pathogenic (class 4) mutations were considered, according to current guidelines Results: A total of 565 filtered variants predicted as pathogenic in 111 genes were retained after bioinformatics analysis, literature and databases review. The main results are the following:-A mutation in 1 of the 5 main sarcomere genes was observed in only 72 (36%) patients, with no mutation on the TNNI3 gene.-Conversely, 495 other pathogenic mutations were identified in 185 patients, giving a total 92.5% detection rate of at least one pathogenic gene mutation. Among them, the most prevalent mutated genes were FLNC, LDB3, MYPN, ANK2, RYR2 and NEBL. In addition, unexpected diagnosis of treatable affections such as Fabry disease (n=1) and TTR amyloidosis (n=1) were identified by WES analysis-A single mutation was identified in only 27 patients (13.5% of cases). In 158 patients (79%), multiple variants were identified with 2 to 8 mutations per patient.-Finally, in only 15 patients (7.5%), no mutation was identified in this panel of 145 genes. Conclusions: 1. Exome sequencing improves the diagnostic output in HCM with 92.5% of patients carrying at least one pathogenic mutation in a panel of 145 genes involved in various hereditary cardiac diseases 2. Genetic heterogeneity is much larger than expected with multiple variants in sarcomere genes, but also non-sarcomere, and ion channel genes unexpected in HCM. 3. Early unexpected diagnosis of Fabry disease or TTR amyloidosis can be obtained by our strategy of one-step large genetic analysis. 4. Our results suggest that the commonly accepted monogenic model of HCM involving one mutation in one gene is not the major mechanism in HCM. Rather, oligogenism with 1 major and several minor variants is our proposed new pathogenic mechanism in HCM.

Abstract

Objectives: The goal of this study was to explore sex differences in myocardial remodeling in aortic stenosis (AS) by using echocardiography, cardiac magnetic resonance (CMR), and biomarkers. Background: AS is a disease of both valve and left ventricle (LV). Sex differences in LV remodeling are reported in AS and may play a role in disease phenotyping. Methods: This study was a prospective assessment of patients awaiting surgical valve replacement for severe AS using echocardiography, the 6-min walking test, biomarkers (high-sensitivity troponin T and N-terminal pro-brain natriuretic peptide), and CMR with late gadolinium enhancement and extracellular volume fraction, which dichotomizes the myocardium into matrix and cell volumes. LV remodeling was categorized into normal geometry, concentric remodeling, concentric hypertrophy, and eccentric hypertrophy.

Background AS is a disease of both valve and left ventricle (LV). Sex differences in LV remodeling are reported in AS and may play a role in disease phenotyping. Methods This study was a prospective assessment of patients awaiting surgical valve replacement for severe AS using echocardiography, the 6-min walking test, biomarkers (high-sensitivity troponin T and N-terminal pro–brain natriuretic peptide), and CMR with late gadolinium enhancement and extracellular volume fraction, which dichotomizes the myocardium into matrix and cell volumes. LV remodeling was categorized into normal geometry, concentric remodeling, concentric hypertrophy, and eccentric hypertrophy. Results In 168 patients (age 70 ± 10 years, 55% male, indexed aortic valve area 0.40 ± 0.13 cm 2 /m 2 , mean gradient 47 ± 4 mm Hg), no sex or age differences in AS severity or functional capacity (6-min walking test) were found. CMR captured sex dimorphism in LV remodeling not apparent by using 2-dimensional echocardiography. Normal geometry (82% female) and concentric remodeling (60% female) dominated in women; concentric hypertrophy (71% male) and eccentric hypertrophy (76% male) dominated in men. Men also had more evidence of LV decompensation (pleural effusions), lower left ventricular ejection fraction (67 ± 16% vs. 74 ± 13%; p < 0.001), and higher levels of N-terminal pro–brain natriuretic peptide (p = 0.04) and high-sensitivity troponin T (p = 0.01). Myocardial fibrosis was higher in men, with higher focal fibrosis (late gadolinium enhancement 16.5 ± 11.2 g vs. 10.5 ± 8.9 g; p < 0.001) and extracellular expansion (matrix volume 28.5 ± 8.8 ml/m 2 vs. 21.4 ± 6.3 ml/m 2 ; p < 0.001). Conclusions CMR revealed sex differences in associations between AS and myocardial remodeling not evident from echocardiography. Given equal valve severity, the myocardial response to AS seems more maladaptive in men than previously reported. Abstract

Objectives We sought to evaluate the impact of the left ventricular (LV) pacing site on hemodynamic response to cardiac resynchronization therapy (CRT). Background CRT reduces morbidity and mortality in heart failure patients. However, 20% to 40% of eligible patients may not fully benefit from CRT device implantation. We hypothesized that selecting the optimal LV pacing site could be critical in this issue. Methods Thirty-five patients with nonischemic dilated cardiomyopathy referred for CRT device implantation were studied. Intraventricular dyssynchrony and latest activated LV wall were defined by tissue Doppler imaging analysis before the study. Eleven predetermined LV pacing sites were systematically assessed in random order: basal and mid-cavity (septal, anterior, lateral, inferior), apex, coronary sinus (CS), and the endocardial site facing the CS pacing site. For each patient, +dP/dT(max), -dP/dT(min), pulse pressure, and end-systolic pressure during baseline (AAI) and DDD LV pacing were compared. Two atrioventricular delays were tested. Results Major interindividual and intraindividual variations of hemodynamic response depending on the LV pacing site were observed. Compared with baseline, LV DDD pacing at the best LV position significantly improved +dP/dT(max) (+31 +/- 26%, p < 0.001) and was superior to pacing the CS (+15 +/- 23%, p < 0.001), the lateral LV wall (+18 +/- 22%, p < 0.001), or the latest activated LV wall (+11 +/- 17%, p < 0.001). Conclusions The pacing site is a primary determinant of the hemodynamic response to LV pacing in patients with nonischemic dilated cardiomyopathy. Pacing at the best LV site is associated acutely with fewer nonresponders and twice the improvement in +dP/dT(max) observed with CS pacing.
