ABSTRACT Introduction. Cardiopulmonary resuscitation (CPR) has a low survival rate after cardiac arrest. According to the Spanish Out-of-Hospital Cardiac Arrest Registry (OHSCAR), the incidence is 24.2 per 1,000,000 inhabitants, with only a 9.8% survival rate at discharge. Since 2005, international resuscitation organizations have recommended focusing on performing high-quality CPR and developing new tools to optimize it. Objective. To evaluate the usefulness of capnography and cerebral oximetry in improving the quality of CPR and predicting clinical and neurological outcomes. Methodology. Articles were reviewed in Science Direct, Medline, and the Cochrane Library, selecting primary research and systematic reviews published between 2011 and 2024. The search included terms related to end-tidal carbon dioxide and cerebral oximetry in CPR. Results. Capnography allows real-time monitoring of ventilation and cardiac performance, serving as a key predictor of the return of spontaneous circulation (ROSC). Values above 10 mmHg are associated with a higher probability of ROSC. Cerebral oximetry measures brain oxygen saturation and indicates CPR effectiveness, with values above 40% correlated with better outcomes. Discussion. Capnography and cerebral oximetry are essential tools in CPR. Although cerebral oximetry shows potential in predicting neurological damage, more studies are needed for its implementation. Integrating these measurements could improve CPR outcomes.

Introduction: Every year, five million people around the world experience an out-of-hospital cardiac arrest (OHCA) and less than 40 % receive any assistance before the arrival of the Emergency Medical Services (EMS). Ambulance operators (AO) take care of people experiencing an OHCA, stabilize and then transfer them. In Medellín, Colombia, there is a public AO and several private providers, but the information about an OHCA and the operational characteristics during the response to the event are limited. Objective: To estimate the incidence of OHCA and to explore the factors associated with survival after the event in Medellín city. Methods: Retrospective, population-based cohort study. All the medical records of patients experiencing an OHCA who were assessed and treated by ambulance operators, (AO) of the Emergency Medical Services (EMS) and private agencies in Medellin city were reviewed. Descriptive statistics were used for data analysis and the annual incidence of the event standardized for the general population was estimated. Potential survival-associated factors reported as OR with their corresponding 95% CI were explored. Results: A total of 1,447 patient records with OHCA between 2018 and 2019 were analyzed. The event incidence rate for the number of cases assessed was 28.1 (95 % CI 26.0-30.3) and 26.9 (95 % CI 24.9- 29.1) cases per 100,000 inhabitants/year for 2018 and 2019, respectively; the incidence rate of treated OHCA was 2.6 (95 % CI 2.0-3.3) and 3.2 (95 % CI 2.5-4.0) per 100,000 inhabitants/year, for 2018 and 2019. Survival on arrival at hospitals of treated cases was 14.2 % (95 % CI 5.5-22.8) and 15.5 % (95 % CI 7.4-23.5) for 2018 and 2019, respectively. Conclusions: This study portrays the operating and care characteristics of the population experiencing OHCA in Medellín city. The incidence rate of the event and the survival were lower than those reported in the literature. [ABSTRACT FROM AUTHOR]

AUTOMATED external defibrillation , *CARDIAC arrest , *CARDIOTONIC agents , *CARDIOPULMONARY resuscitation , *DEFIBRILLATORS , *DIAGNOSIS , *EMERGENCY medical services , *EMERGENCY medicine - Abstract
Sudden cardiac death is a common occurrence. Out-of-hospital cardiac arrest is a global public health problem suffered by ≈3.8 million people annually. Progress has been made in the knowledge of this disease, its prevention, and treatment; however, most events occur in people without a previous diagnosis of heart disease. Due to its multifactorial and complex nature, it represents a challenge in public health, so it led us to work in a consensus to achieve the implementation of cardioprotected areas in Mexico as a priority mechanism to treat these events. Public access cardiopulmonary resuscitation (CPR) and early defibrillation require training of non-medical personnel, who are usually the first responders in the chain of survival. They should be able to establish a basic and efficient CPR and use of the automatic external defibrillator (AED) until the emergency services arrive at the scene of the incident. Some of the current problems in Mexico and alternative solutions for them are addressed in the present work. [ABSTRACT FROM AUTHOR]

Abstract. First aid has been part of the Spanish educational system for over 30 years, gaining prominence in the last two educational laws. This study examines its presence in the current educational legislation. An analysis has been conducted on the inclusion of first aid and basic life support (BLS) content in the autonomous curricula of compulsory secondary education (ESO) in Spain. All autonomous communities incorporate first aid in a manner similar to that stipulated in the Royal Decree that establishes the minimum contents. In the first cycle of ESO, the contents focus on learning the BLS protocol, the emergency number, and the PAS protocol (Protect, Alert, and Assist). In the second cycle, these become more complex, including CPR maneuvers, the Heimlich maneuver, and the use of an automated external defibrillator (AED). There are studies that recommend initiating the learning of first aid with simple content in primary education and gradually increasing the complexity throughout the entire educational stage. Therefore, while the contents proposed in secondary education are not inappropriate, some of them might be more suitable for the previous stage, namely, Primary Education. Keywords: Basic life support, physical education, accidents, emergencies, cardiopulmonary resuscitation, curriculum.

In clinical practice, it is not rare to encounter situations in which parents and families are asked to leave the child alone with the health care team in rooms full of devices throughout the performance of procedures, which at times may give rise not only to conflicts but, more importantly, emotional sequelae in children or adolescents. We conducted a narrative review of the literature by searching the digital library of the public health care system of Andalusia for articles concerning the experiences of health care professionals and families with the accompaniment of paediatric patients during health care procedures. We restricted the search to studies published in Spanish or English and conducted in humans. The review evinced the need to humanise care in order to improve care quality. The need to accompany minors is supported by the evidence from works that have analysed the factors involved in the persistence of these behaviours and attitudes in both professionals and parents. We consider it necessary to develop institutional policies and appoint mediators to compile the statements of different national and international societies, taking into account legal aspects but, above all, the pertinent values from a health care ethics perspective, and in pursuit of the best interests of the child.

School is the optimal place to begin training in Basic Life Support and teachers can play a very relevant role for this purpose. However, the knowledge and willingness of this group to teach this subject to their students is not known. Nor has the attitude of families towards their children or guardians receiving this knowledge within the school curriculum been demonstrated. The objective of our study was to quantitatively evaluate the awareness, attitude and willingness to include this subject in the school curriculum of three key figures in the educational environment: teachers, families and students of degrees in educational sciences. Methods: A cross-sectional descriptive study was carried out using a self-administered ad hoc questionnaire. Results: 484 families, 137 teachers and 42 students of degrees in educational sciences participated in the study. The three groups agreed that learning basic life support should be part of the school curriculum, on a mandatory basis and considering primary education as the most appropriate stage for its initiation. Regarding who should teach this subject, it stands out that only 30.7% (n=42) of teachers consider that this task should be carried out by the school teachers themselves. Both active teachers and undergraduate students of educational sciences affirm that including this knowledge in the curriculum of the Degrees of Education is an efficient strategy to disseminate this knowledge among teachers. Conclusions: Based on the results, the need is concluded. to raise awareness and train teachers as a priority with the aim of increasing, in addition to their knowledge on the subject, their self-confidence, an essential fact to increase their willingness to be able to instruct their students. Key words: Basic life support; Cardiopulmonary Resuscitation; Automated External Defibrillator; teaching; teacher training

Sudden Cardiac Death (SCD) and Out-of-Hospital Cardiac Arrest (OHCA) are global public health problems suffered by ≈3.8 million people annually; they represent a challenge in public health, which leads us to work on regulations, legislation, and consensus to implement the formation of protected Cardio areas in Mexico. The increase in cardio-protected spaces and the use of Automatic External Defibrillators (AED) in the world have contributed to improving the survival of OHCA. The use of AEDs and early public access defibrillation requires training for non-medical personnel, who are generally the first to assist and start the chain of survival, with basic and efficient Cardiopulmonary Resuscitation (CPR) until emergency services arrive at the scene of the incident. In this article, we present a structured guide to the steps that must be followed to accredit a cardioprotected space in Mexico and distinguish between public access defibrillation and a cardio-protected area. Introduction: The objective is to describe the utility of the Critical Administration Threshold (CAT) as a tool in hemostatic resuscitation in patients with severe trauma and REBOA. Methods: Retrospective review between January 2015 and June 2020 of adult patients with hemorrhage secondary to trauma with or without REBOA in a level I trauma center in Cali, Colombia. Demographic variables, trauma severity, clinical status, transfusion needs, time to CAT+ and number of CAT achieved (1, 2 or 3) were recorded. Results: Ninety-three patients were included, in which REBOA was used in 36 and traditional management in 57. The REBOA group had a higher bleeding volume (3000 ml), IQR: 1950-3625 ml vs the control group (1500 ml, IQR: 700-2975 ml) (p<0.001) and a higher rate of PRBC units transfused in the first 6 hours (median 5, IQR: 4-9); p=0.015 and in the first 24 hours (median 6, IQR: 4-11); p=0.005. There were no statistically significant differences in the number of CAT+ patients between groups or time to CAT+. However, CAT+ status during the first 30 minutes of surgery was higher in the REBOA Group (24/36, 66.7%) vs. the control group (17/57, 29.8%; p=0.001), having this group a higher in-hospital mortality rate vs. CAT-patients. Conclusion: CAT is a useful tool in the hemostatic resuscitation of patients with trauma and REBOA that could predict early mortality. [ABSTRACT FROM AUTHOR]

HEART assist devices , *CARDIOPULMONARY resuscitation , *HEART failure - Abstract
Advanced heart failure is becoming more frequent and, in this population, left ventricular assist devices have emerged in the last thirty years as bridge-to-transplant or destination therapy for those with a contraindication for transplant. These devices are continuous flow, contrary to normal hemodynamic physiology, which leads to associated complications with the interaction between the device and the patient, a phenomenon known as “haemocompatibility”. One of its implications is the absence of a central pulse and the need to measure blood pressure through infrequent methods such as Doppler. Various complications are frequently associated with this and can be grouped under the mnemonic A2B2C2D2, including those specific and related to the device. Each of these must be recognized for early and prompt diagnosis and treatment, including prompt cardiopulmonary resuscitation, if indicated, to impact the survival of these patients. Introduction: Satisfactory performance in Neonatal Resuscitation requires continuous training, and several technologies can enhance its scope. This study was carried out to evaluate the use of an educational platform by medical students in a Neonatal Resuscitation virtual course. Materials and methods: Descriptive study with the data available on the Neonatal Resuscitation virtual course platform, evaluating the use of a population corresponding to the total number of undergraduate Medicine students from the Universidad Nacional Mayor de San Marcos (UNMSM) who enrolled as students to take the Paediatrics course in the last year of their studies during the years 2018 and 2019 in Lima, Peru. Results: Data from 379 sixth-year medical students in the 2018-I, 2019-I, and 2019-II semesters were included. Students entered the virtual course an average of 23 days before taking their online exam, and the average theory review time in the virtual system was 120 min. Students with outstanding performance in their scores completed 2 virtual exams, but there was no difference in the other characteristics. Conclusions: The sequence developed in the Neonatal Resuscitation virtual course, which begins with ''learn, see, and practice'', offers a greater capacity to retain the knowledge and skills acquired. The virtual offer of the theoretical component of Neonatal Resuscitation allows the use of innovative tools that make it possible to achieve a high performance of medical students when completing the virtual course of Neonatal Resuscitation.

The number of air travelers has increased over the past years, and after the limitations imposed by the COVID-19 pandemic, are expected to increase further. Given these conditions, the possibility for a health professional to find himself needing to assist an in-flight cardiac arrest is also increased. The present work describes some of the most common problems in managing an inflight cardiac arrest and emphasizes the relevance of cardiopulmonary resuscitation (CPR) performance and automated external defibrillators (AED) use on board of commercial airliners. Introduction: Basic life support training in school age is a topical issue because, with adequate training, any person can help save a life. Methods: Cluster clinical trial with data collection through an ad hoc self-administered, semi-structured questionnaire. The target population encompassed the students aged 4–6 years enrolled in 49 educational centres. The centres were randomly allocated to the intervention or control group. The intervention group was trained with the RCParvulari® methodology, consisting of theoretical and practical training on the first link of the chain of survival. The control group only received theoretical training. We evaluated participants before and immediately after the intervention and between 3 and 12 months post intervention by means of the questionnaire. We assessed the acquisition and retention over time of the knowledge and skills covered in the training compared to previous trainings in both groups. Results: A total of 1327 schoolchildren (79% of the target population) participated. The level of knowledge acquired immediately after training and after 3–12 months compared to baseline was significantly better (P

Abstract. Background: Education of schoolchildren is an important issue in promoting the involvement of the lay public in improving survival after out-of-hospital cardiac arrest. For this quality training it is necessary to know what schoolchildren know to establish what they need to learn about it. For this reason, the objective of this research was to evaluate the knowledge in SVB of schoolchildren in the 5th and 6th grades of Primary Education and students in the 1st, 2nd and 3rd grades of Secondary Education. Methods: During the first quarter of the 2021/2022 academic year, a survey was conducted of 947 Primary (n=308) and Secondary (n=639) schoolchildren between the ages of 10 and 17 from four schools supported with public funds from the Xunta de Galicia. Results: 62.81% of the students surveyed received prior training. 92.30% responded incorrectly to the correct sequence of action before a person with possible cardiac arrest and 93.14% did so to the correct order of execution of the AED for an effective shock. 82.40% of schoolchildren know the emergency telephone number. Secondary Education students and those who received prior training have better knowledge than Primary Education students (p < 0.001) and those who do not (p < 0.001), although in both cases they do not reach 5 points out of 10. Conclusions: Galician schoolchildren in Primary and Secondary Education have little knowledge of Basic Life Support, so they do not contribute to the “Children Save Lives” initiative. Background: the correct application of life support for cardiorespiratory arrest (CPR) creates a short gap between the life or death of a person, hence the importance of having the necessary preparation to perform it. Objective: to explore the training and criteria of professionals about the management of basic and advanced life support in the event of cardiorespiratory arrest, at the Canoa Health Center, Ecuador, between July 2022 and February 2023. Methods: a quantitative, descriptive, cross-sectional study was carried out in the health center and during the time period declared. The study population consisted of 15 professionals, and included medical, nursing, obstetric and dental personnel. The information was collected through an anonymous questionnaire. The results were analyzed according to descriptive statistics. Results: almost half of the professionals were doctors (46.7 %), followed by nursing graduates (33.3 %). 66.66 % have taken basic or advanced life support courses, 86.7 % have witnessed CPR, but only 40 % have participated in resuscitation. All respondents replied that they do not have the necessary equipment to act in case of a CPR. For the most part, 66.66 % reflected a high degree of knowledge compared to knowing how to act in CPR cases, declaring confidence in acting. Conclusions: the human talent of the operating unit meets high levels of knowledge regarding basic and advanced life support.

Background: The multidisciplinary team caring for at-risk newborns should identify the risk factors for asphyxia early on. The team should be prepared for cardiopulmonary resuscitation and post-resuscitation care and have theoretical knowledge, practical skills, and ethical behaviors to minimize adverse events and provide safe care. Objective: To assess the knowledge of the multidisciplinary team about neonatal post-resuscitation care. Methodology: Cross-sectional study in a neonatal unit in Fortaleza-CE-Brazil. A questionnaire was administered to the multidisciplinary team, selected by convenience, and analyzed using the positivity index. Results: The team consisted mostly of women, aged 30 to 40 years, with a postgraduate degree, who had witnessed neonatal cardiorespiratory arrest and had attended a training course. They had an adequate knowledge positivity index (99.1%) regarding vital signs and oxygen saturation; borderline knowledge of blood gas dosage (71.8%), and poor knowledge (20.9% to 60.0%) about monitoring central venous pressure, urine output, blood glucose, and cardiac enzymes. Participants suggested using hard technologies, with only one professional recommending soft/relational technologies. Conclusion: There is a need for effective professional training, continuing education, and awareness-raising interventions to promote a holistic approach to newborn care. [ABSTRACT FROM AUTHOR]
