Abstract

Recently there has been a chorus of demands to “re‐imagine” social care. Community and faith‐based organisations, policy, and academic communities are engaged in discussions on issues such as human rights for older populations, the future of residential care, how to better support family/community care, and strengthen local place‐based community development. Moreover, the Covid‐19 pandemic has added new urgency to this mission, galvanizing developments for change and collective action and exposing public troubles of endemic system failings, prevailing discourses of ageism, tensions with health systems, and limitations of market models of care and support. Prevention is a central social welfare principle in many countries. It is associated with policy and practices that aim to meet social care needs early and is explored in this thematic issue.

Abstract

Prevention is becoming ever more central in UK care policy for older people, though precisely what this entails, and how it works most effectively in social care and support, remains ambiguous. Set against the “newness” of recent social care legislation in Wales, this article explores the perspectives of professionals on prevention and community development, particularly for older people. This draws on qualitative data collected from 11 Welsh local authorities, four NHS Wales health boards, and eight regional third-sector organisations, incorporating 64 interviews with directors, executives, and senior managers. Recent research has highlighted concerns over the slipperiness of prevention as a concept, resulting in multiple interpretations and activities operating under its banner. Consistent with this, our data suggested a kaleidoscopic picture of variously named community-based initiatives working to support the intricate web of connections that sustain older people, as well as provide practical or material help. Similarly, professionals highlighted varied agendas of community resilience, individual independence, and reducing the need for state-funded health and social care, as well as a range of viewpoints on the roles of the state, private sector, and the third sector. Analysis revealed fragments of familiar themes in community development; positive hopes for community initiatives, tensions between the mixed agendas of state-instigated activities, and the practical challenges arising from systems imbued with neo-liberal ideas. Realising the promise of prevention will require deft steering through these challenges.

14. Older people : ageism and the National Health Service

Abstract

The care of older people in the hospital setting has been the subject of increasing scrutiny from policymakers, researchers, philosophers and age campaigners over the last twenty years. However, in spite of continued attention in organisational training, dignity campaigns and government policy, reports of undignified care continue to surface through notable incidents such as the Mid Staffordshire Public Inquiry. This study examined the role that the cultural representation of older people may have on the attitudes of NHS staff towards them, hypothesising that this influence can be as meaningful as that gathered in the work or social settings. The research adopted a mixed methods approach incorporating a national survey of NHS staff cultural consumption habits, multi-modal discourse analysis of cultural texts typically consumed by that population, and semi-structured interviews with NHS staff. The findings show that older people are generally underrepresented from large swathes of culture, and when represented there is a tendency towards either negative or positive stereotypes. Market trends and operational processes within the cultural industries help to explain the variation of representation on offer, with audience segmentation being a key factor in cases where older people were absent or negatively stereotyped. These representations were found to inform the interviewees’ perceptions of the older population, although this was also mediated by experiences gathered on the job, in training or through social engagement. While efforts are ongoing to foster intergenerational understanding in NHS organisational culture and local communities, the unique position of the cultural industries makes similar interventions in this field more challenging.

Abstract

This thesis investigates means of improving, applying, and measuring the success of, the Spatial Scan Statistic (SSS) when applied to binary labelled point data (BLPD). As the SSS is an established means of detecting anomalies in spatial data (also known as clusters), this work has potential application in many fields, notably epidemiology. Firstly, the thesis considers the capacity of the SSS to correctly identify the presence of anomalies, irrespective of location. The most important contribution is the identification that p-values produced by the standard algorithm for implementing of the SSS are sometimes conservative, and thus may lead to lower-than-expected statistical power. A novel means of rectifying this is presented, along with a study of how this can be used in conjunction with an existing technique (Gumbel smoothing) for reducing the computational expense of the SSS. A novel version of the SSS for BLPD is also derived and tested, together with an alternative algorithm for selecting circular scan windows. Secondly, the thesis considers the capacity of the SSS to correctly identify the location of anomalies. This is an under-researched area, and this work is relevant to all forms of data to which the SSS is applied, not just BLPD. A synthesis of current research is presented as a five level framework, facilitating the comparison and hy- bridisation of existing spatial accuracy measures for the SSS. Two novel measures of spatial accuracy ('l!, 0) are derived, both compatible with this framework. 'l! works in conjunction with power; D is independent of power. Both use a single parameter to encapsulate complex information about spatial accuracy performance. This pre- viously required two or more parameters, or an arbitrarily weighted combination of two or more parameters. All novel techniques are benchmark tested against established software, and the statistical significance of performance improvements is measured.

23. Transition to turbulence in a turbomachinery environment

Abstract

This thesis aims to contribute to the understanding of transitional flows in the blade boundary layers of axial compressors. Two experiments are described, the first examining in detail the transitional boundary layer on a simulated controlled-diffusion blade and the second surveying the mid-height flowfield in an embedded stage of a low-speed axial compressor. The velocity distribution on the simulated blade is identical to the Velocity distribution on the suction surface of the blades in the axial compressor. At 2 Reynolds numbers and 3 levels of freestream turbulence, a single hot wire was used to conduct a boundary layer survey on a simulated controlled-diffusion blade. Integral parameters of the boundary layers are explored to dene the length and nature of transition. At low Reynolds number there is a separated or near separated region at the leading edge which does not lead to turbulence. Transition covers a length of approximately 20% of the blade chord, starting between 20% and 30% chord. The position of transition is strongly influenced by the level of freestream turbulence. Most of the transition process occurs within the decelerating flow region which exists from 20% of the chord. At high Reynolds number, a leading edge separation bubble leads to transition within 2% of the blade chord. Abu-Ghannam & Shaws correlation for the start and length of transition was found to predict the start of transition well for attached flows, but could not be relied upon for separated flows. It is apparent that the correlation was not designed for the very strong Velocity gradients in the leading edge region, and probably not for separated flow. _ Three flow conditions in the axial compressor were used: design speed, peak efficiency, low Reynolds number at peak efficiency (the machine was slowed to one-quarter speed) and design speed near the stall. Using hot wires at mid-height, axial and circumferential velocity and turbulence information was obtained. Wakes and structure within wakes are visible in the turbulence and Reynolds stress distributions. The wakes of more than one upstream blade row are visible; the region where two wakes intersect gives some information about interaction between a stator blade Wake and a rotor blade boundary layer. Some information is available about the length scale 'distribution inside and outside wakes. Secondary flow in the axial-circumferential plane shows motion within wakes and a vortex in the near-stall flowfield, shed preferentially at one point in the blade-passing cycle.

Abstract

Rheumatoid arthritis (RA) is a chronic inflammatory disorder with poorly defined aetiology characterised by synovial inflammation with variable disease severity and drug responsiveness. To investigate the peripheral blood immune cell landscape of early, drug naive RA, we performed comprehensive clinical and molecular profiling of 267 RA patients and 52 healthy vaccine recipients for up to 18 months to establish a high quality sample biobank including plasma, serum, peripheral blood cells, urine, genomic DNA, RNA from whole blood, lymphocyte and monocyte subsets. We have performed extensive multi-omic immune phenotyping, including genomic, metabolomic, proteomic, transcriptomic and autoantibody profiling. We anticipate that these detailed clinical and molecular data will serve as a fundamental resource offering insights into immune-mediated disease pathogenesis, progression and therapeutic response, ultimately contributing to the development and application of targeted therapies for RA.

Abstract

Background: For women with early-stage oestrogen receptor (ER)-positive breast cancer, adjuvant tamoxifen reduces 15-year breast cancer mortality by a third. Aromatase inhibitors are more effective than tamoxifen in postmenopausal women but are ineffective in premenopausal women when used without ovarian suppression. We aimed to investigate whether premenopausal women treated with ovarian suppression benefit from aromatase inhibitors. Methods: We did a meta-analysis of individual patient data from randomised trials comparing aromatase inhibitors (anastrozole, exemestane, or letrozole) versus tamoxifen for 3 or 5 years in premenopausal women with ER-positive breast cancer receiving ovarian suppression (goserelin or triptorelin) or ablation. We collected data on baseline characteristics, dates and sites of any breast cancer recurrence or second primary cancer, and dates and causes of death. Primary outcomes were breast cancer recurrence (distant, locoregional, or contralateral), breast cancer mortality, death without recurrence, and all-cause mortality. As distant recurrence invariably results in death from breast cancer several years after the occurrence, whereas locoregional recurrence and new contralateral breast cancer are not usually fatal, the distant recurrence analysis is shown separately. Standard intention-to-treat log-rank analyses estimated first-event rate ratios (RR) and their confidence intervals (CIs). Findings: We obtained data from all four identified trials (ABCSG XII, SOFT, TEXT, and HOBOE trials), which included 7030 women with ER-positive tumours enrolled between June 17, 1999, and Aug 4, 2015. Median follow-up was 8·0 years (IQR 6·1–9·3). The rate of breast cancer recurrence was lower for women allocated to an aromatase inhibitor than for women assigned to tamoxifen (RR 0·79, 95% CI 0·69–0·90, p=0·0005). The main benefit was seen in years 0–4 (RR 0·68, 99% CI 0·55–0·85; p<0·0001), the period when treatments differed, with a 3·2% (95% CI 1·

Despite this, procedural guidance on how clinicians should best engage patients regarding their medicine-taking is limited in the United Kingdom. Adherence for chronic conditions is notably complex, requiring clear education, communication, and behavioural shifts to initiate and sustain daily regimens successfully. This article explores current clinician guidance on assuring patient adherence to medication within the National Health Service, comparing it to that provided for healthcare workers in the field of behavioural change. Outlining the inertia of the former and the progress of the latter, we consider what steps should be taken to address this deficit, including greater focus on patient concerns, as well as knowledge translation for healthcare professionals in future adherence research. Current United Kingdom clinical guidance for assuring patient adherence is largely outdated based on inconclusive evidence for best practice. Abstract

The original LP turbine of the BR715 engine featured 'High Lift' blading, which achieved a 20-percent reduction in aerofoil numbers compared to blading with conventional levels of lift, reported in Cobley et al. (1997). This paper describes the design and test of a re-bladed LP turbine with new 'Ultra High Lift' aerofoils, achieving a further reduction of approximately 11 percent in aerofoil count and significant reductions in turbine weight. The design is based on the successful cascade experiments of Howell et al. (2000) and Brunner et al. (2000). Unsteady wake-boundary layer interaction on these low-Reynolds-number aerofoils is of particular importance in their successful application. Test results show the LP turbine performance to be in line with expectation. Measured aerofoil pressure distributions are presented and compared with the design intent. Changes in the turbine characteristics relative to the original design are interpreted by making reference to the detailed differences in the two aerofoil design styles. [DOI: 10.1115/1.1415737]
