The article presents a case study of a 30-year-old man diagnosed with schizophrenia who came to the emergency room after an attempted suicide by penile amputation with a razor blade. The amputated part was aligned with the stump in as natural position as possible with each corpus cavernosum anastomosed using interrupted 3-0 vicryl catgut sutures. The article discusses the penile reimplantation of an amputated penis.

Introduction: The bladder tumor is common in urinary system and its usual diagnostic method is cystoscopy that is an invasive method. In this study the sensitivity and specifity of the urinary FDP(Fibrin Degradation Products) in diagnosis of transitional cell carcinoma of bladder was determined and compared with urine cytology during the course of disease. Methods: In this evaluation study of diagnostic test, sixty cases with confirmed bladder carcinoma admitted to Rahnemoon hospital in Yazd were included. Urine samples were collected before any intervention for cytologic examination and also for checking the FDP according to manufacture's instruction (FDP AuraTeck kit). Patients were randomly divided into three groups based on the stages and grades of disease. Sensitivity and specifity of each test was also determined. Results: Sixty urine samples were evaluated. 34 patients were male and 26 were female. Mean age of patients was 62.3±11 years. Sensitivity of FDP and cytology in patients with grade I was 63% and 8%, in grade II 64% and 28%, and in grade III and also grade IV 89% and 50% respectively. These values in patients with stage Ta were 64% and 34%, in stage T1 and T2 65% and 22% and in stage T3 and T4 63% and 43% respectively. Overall the sensitivity of FDP was 68% and for cytology was 38% (P<0.005). The positive predictive value for FDP was 100% and the negative predictive value was 30.4%. Conclusions: Our study showed that urinary FDP is a valuable diagnostic test which has greater sensitivity and specifity than cytology in bladder carcinoma and it's accuracy is not affected by the stage and grade of the tumor. Purpose: Renal transplantation is an ideal treatment for patients with chronic renal failure. It was demonstrated that despite the adhesion to surgical and anesthetic principles, urinary output is not satisfactory after transplantation. It seems that microvascular spasm of renal vasculature is responsible for this phenomenon. We designed a study to investigate whether lidocaine injection into renal artery can relieve vasospasm and subsequently improve output and graft function better than furosemide., Materials and Methods: In a randomized clinical trial, from July 2002 to November 2003, 100 consecutive patients who were referred to our center for kidney transplantation were recruited in this study. After obtaining written informed consent, they were divided blindly into two groups. In group 1, lidocaine was injected into renal artery, before arterial anastomosis, and group 2 received furosemide as the conventional intervention. Urine volume within 1, 4, and 24 postoperative hours and serum creatinine levels in the first three weeks were recorded and compared between the two groups., Results: Urine volumes at 1, 4, and 24 hours after transplantation were higher significantly in lidocaine group (P <0.001). Serum creatinine levels were lower significantly in the first postoperative day and also 21 days after transplantation in group 1 (P <0.001)., Conclusion: Comparing to furosemide, it seems that lidocaine can cause a more effective vasodilation in renal arteries of kidney allograft, resulting in a better diuresis. This may have a role in the betterment of graft function.
