Abstract

This paper studies the problem of electing a small number of representatives (council) out of a (possible large) group of anonymous candidates. The problem arises in scenarios such as multicast where, to avoid feedback implosion, a small subset of the receivers is chosen to provide feedback on network conditions. We present several algorithms for this problem and analyze the expected number of messages and rounds required for their convergence. In particular, we present an algorithm that almost always converges in one round using a small number of messages (for typical council size) when the number of hosts is known. In the case where the number of hosts is unknown (and too large to be polled), our algorithms converge in a small number of rounds that improves previous results by Bolot et al. (1994).

Abstract

The incidence of esophageal adenocarcinoma in the United States has risen rapidly over the last 30 years, whereas the incidence of esophageal squamous cell carcinoma has fallen dramatically. In contrast, parts of Asia have extremely high rates of squamous cell carcinoma, but virtually no adenocarcinoma. Within the United States, Asian-Americans as a whole, have low rates of esophageal adenocarcinoma and higher rates of squamous cell carcinoma. It is unclear what the patterns are for those Asians born in the United States. The relative influence of ethnicity and environment on the incidence of esophageal cancer in this population is unknown. We identified all cases of esophageal adenocarcinoma and squamous cell carcinoma from the California Cancer Registry 1988-2004, including 955 cases among 6 different Asian ethnicities. Time trends were examined using Joinpoint software to calculate the annual percentage changes in regression models. Rates of esophageal squamous cell carcinoma varied substantially among different Asian ethnic groups, but squamous cell carcinoma was much more common than adenocarcinoma in both foreign-born and US-born Asian-Americans. Rates of squamous cell carcinoma were slightly higher among US-born Asian men (4.0 per 100,000) compared with foreign-born Asian men (3.2 per 100,000) and White men (2.2 per 100,000), P = 0.03. Rates of adenocarcinoma were also slighter higher among US-born Asian men (1.2 per 100,000) compared with foreign-born Asian men (0.7 per 100,000), P = 0.01. Rates of squamous cell carcinoma decreased for both US-born and foreign-born Asians during this period, whereas adenocarcinoma remained low and stable. These results provide better insight into the genetic and environmental factors affecting the changing incidence of esophageal cancer histologies in the United States and Asia.

Abstract

Deleterious microcirculatory effects of Intralipid (IL) infusion may be caused by hemorheological or vascular effects. The aim of this investigation was to study vascular and hemorheological effects of IL in preterm and fullterm neonates and children. Ten preterm newborns, 10 fullterm neonates, and 10 children received an initial infusion of IL (0.6 g/kg) over 4 h. Calf blood flow (venous occlusion plethysmography), blood pressure (Dinamap), whole blood and plasma viscosity (capillary viscometer), red blood cell deformability (rheoscope), and erythrocyte aggregation (aggregometer) were measured before and after administration of IL. Plasma triglyceride levels showed the greatest increase in preterm infants. Whole blood viscosity decreased by about 10% in all three groups because of a similar reduction in hematocrit. Red blood cell aggregation decreased by about 20% after IL infusion. Blood pressure rose by 10%, and peripheral blood flow declined by about 10% in the three groups. Vascular hindrance, a calculation of blood pressure divided by blood flow and viscosity, was raised by about 20%, suggesting marked vasoconstriction of peripheral arteries. Vasoconstriction rather than hemorheological changes during infusion of IL may play a crucial role in the pathogenesis of circulatory alterations in parenterally-fed neonates. © Springer-Verlag 2005. 21 3 197 202

Abstract

Multi-source contamination mapping is a critical aspect of radiation detection and environmental monitoring. This paper introduces an innovative algorithm for accurate and efficient multi-source contamination mapping. The algorithm comprises two main components: (1) search area identification and (2) source localization using a particle filter. The search area identification involves extrapolating directional measurements to create search areas, employing clustering algorithms to refine these areas, and generating final search areas based on weighted factors. The incorporation of directional measurements significantly enhances performance by reducing parameter search space of the particle filter and thus improving practical application potential. The particle filter-based source localization employs a four-stage process, involving generation of initial guesses, comparison of expected and acquired measurements, employing genetic algorithms for particle selection, and a convergence criterion. The algorithm’s efficacy is demonstrated through four simulated experimental scenarios, each with varying numbers and densities of point radioactive sources. Results illustrate successful search area identification and source localization with an average error of approximately 1 meter and source activity estimation of within 25% of the ground-truth values. Further improvements to the area clustering algorithm, improved mission planning for online applications, and extensive testing of the algorithm are planned in the future. With these improvements, the algorithm presented here could enhance the ability to respond to radioactive contamination incidents swiftly and effectively, ensure timely mapping of contaminated areas, and maintain human safety in the event of an incident.

Abstract

Blocking the PD-1 pathway was shown to be effective in amyloid beta mouse models, yet little is known about its therapeutic potential in models of tauopathy. The authors show here that blocking PD-L1, a PD-1 ligand, is similarly effective, and that both treatments reversed cognitive deficiencies, and modified disease pathology not only in an animal model of AD, but also in the DM-hTAU mouse tauopathy model, through a mechanism that involves monocyte-derived macrophages.

Abstract

BackgroundIn the prehospital setting, endotracheal intubation (ETI) may be required to secure the coronavirus disease 2019 (COVID-19) patient airway. It has been suggested that the use of a protective barrier can reduce possible aerosol delivery from patients to clinicians during ETI. We sought to assess the performance of ETI by paramedics wearing personal protective equipment with and without the use of a box barrier.MethodsA randomized, crossover simulation study was performed in a simulation laboratory. Study participants were 18 paramedics actively working in the clinical environment. Participants' performance of ETI via direct laryngoscopy (DL) with and without the use of a box barrier was assessed. The sequence of intubation was randomized to either BoxDL-first or DL-first. The primary outcome was the success rate of ETI on first-attempt. The secondary and tertiary outcomes were ETI success rates on three attempts and total intubation time, respectively.ResultsThere were no differences between the DL group and the BoxDL group in one-attempt success rates (14/18 vs 12/18; P = 0.754), and in overall success rates (16/18 vs 14/18; P = 0.682). The mean (standard deviation) of the total intubation times for the DL group and the BoxDL group were 27.3 (19.7) seconds and 36.8 (26.2) seconds, respectively (P < 0.015).ConclusionsThe findings of this pilot study suggest that paramedics wearing personal protective equipment can successfully perform ETI using a barrier box, but the intubation time may be prolonged. The applicability of these findings to the care of COVID-19 patients remain to be investigated.
