15. Safety by design: dialogues between designers and builders using virtual reality

Published by Taylor & Francis.Designers can contribute to enhancing the safety of construction work by considering how their decisions impact on both the physical environment in which construction workers operate and the means and methods they use. To do so, however, designers require knowledge about safety hazards on site and the opportunity to examine their designs early in projects. Through a set of studies virtual reality tools were used to examine the potential for collaborative dialogue between designers and builders to provide a forum for learning and proactive change of a design to make a project safer to build. In the tests, participants viewed proposed designs using virtual reality to examine various alternative design and construction scenarios. The study shows that consultation and dialogue with an experienced construction professional are highly beneficial for designers to appreciate the implications of designs on safety, and that designers are more willing to adapt design details than to change aesthetic aspects of their designs.

16. Ureteropelvic junction obstruction in the first year of life

Abstract

The ureteropelvic junction is the point at which the ureter joins the pelvis of the kidney; it is the most common site of urinary tract obstruction in children. Results are presented from a series of 21 patients younger than one year of age who underwent surgery for ureteropelvic obstruction. Ultrasound studies carried out before birth allowed the diagnosis of hydronephrosis (accumulation of urine in the kidney) in 14 cases. Other tests were carried out after birth. Six infants had palpable abdominal masses and five had recurrent urinary tract infections. In most cases, the obstruction was the result of narrowing of the ureter at its upper segments. All patients underwent dismembered pyeloplasty, a procedure that was successful in 18 cases (86 percent). Fetal ultrasonography enables physicians to diagnose asymptomatic cases of ureteropelvic junction obstruction early. In general, results from surgical repair are excellent in infants younger than two. No infant younger than one month was operated upon. 40. Effect of craniosacral therapy on lower urinary tract signs and symptoms in multiple sclerosis.

Abstract

To examine whether craniosacral therapy improves lower urinary tract symptoms of multiple sclerosis (MS) patients. A prospective cohort study. Out-patient clinic of multiple sclerosis center in a referral medical center. Hands on craniosacral therapy (CST). Change in lower urinary tract symptoms, post voiding residual volume and quality of life. Patients from our multiple sclerosis clinic were assessed before and after craniosacral therapy. Evaluation included neurological examination, disability status determination, ultrasonographic post voiding residual volume estimation and questionnaires regarding lower urinary tract symptoms and quality of life. Twenty eight patients met eligibility criteria and were included in this study. Comparison of post voiding residual volume, lower urinary tract symptoms and quality of life before and after craniosacral therapy revealed a significant improvement (0.001>p>0.0001). CST was found to be an effective means for treating lower urinary tract symptoms and improving quality of life in MS patients. [ABSTRACT FROM AUTHOR]

49. Prostatic Intraepithelial Neoplasia: Influence of Clinical and Pathological Data on the Detection of Prostate Cancer

Abstract

Purpose: We attempted to characterize patients diagnosed with prostatic intraepithelial neoplasia without concurrent cancer on biopsy who had prostate cancer on subsequent biopsy. Materials and Methods: The records of 93 patients with low and high grade prostatic intraepithelial neoplasia without concurrent cancer on initial biopsy were analyzed. The relationships among prostatic intraepithelial neoplasia grades, patient age, digital rectal examination, serum prostate specific antigen (PSA), transrectal ultrasound appearance and final pathological results were investigated. Results: Subsequent carcinoma was found on repeat biopsy in 13.3 percent of patients with low grade and 47.9 percent with high grade prostatic intraepithelial neoplasia (p less than 0.001). In the former group digital rectal examination, patient age, serum PSA and transrectal ultrasound were not predictive of cancer. Transrectal ultrasound appearance, digital rectal examination and serum PSA were statistically different between high grade prostatic intraepithelial neoplasia with and without subsequent cancer (p less than 0.001, p = 0.008 and p = 0.016, respectively, univariate analysis). On multivariate analysis of patients with high grade prostatic intraepithelial neoplasia only digital rectal examination and PSA were predictive of subsequent carcinoma. Conclusions: High grade prostatic intraepithelial neoplasia is a strong predictor of subsequent cancer, especially in men with abnormal digital rectal examination and elevated serum PSA. Patients with high grade prostatic intraepithelial neoplasia should undergo repeat biopsy to exclude cancer. Further investigations are needed to optimize the treatment of patients with low grade prostatic intraepithelial neoplasia.

Numerical experiments, which describe the inversion of scattering information in cases when multiple scattering is not negligible, are presented and discussed. The inversion method developed here is applied to laboratory angular scattering measurements performed for optical depths of 0.1–0.3.
