Modern drug development is often hindered by low solubility of new drug particles and the need for multiple drug molecules. A way to counteract the low solubility is to create nanoparticles of the drug. This, however, leads to issues in administration of the drug such as the need for a carrier powder in inhalation therapy. This work introduces a synthesis method of a drug particle that has multiple nanosized drugs encased within a micron-sized carrier particle. A nanosuspension of water and naproxen – a poorly soluble drug – is created using a ball mill. The resulting nanosuspension is mixed with cimetidine – a drug often administered with naproxen. Mannitol and leucine are added to form the matrix and the coating layer of the synthesized particle. The resulting solution is then run through an aerosol flow reactor. The “Nanos-in-Micros” particles and the powder made thereof are studied with multiple different methods. The particles assembled were found to exhibit most of the expected properties. As such, the “Nanos-in-Micros” concept is deemed a promising tool for pharmaceutical nanoparticle processing and further research is suggested., Usein modernia lääkekehitystä vaikeuttaa uusien lääkeaineiden alhainen liukenevuus ja useiden lääkemolekyylien tarve. Liukenevuutta voidaan parantaa tekemällä lääkeaineesta nanokokoista. Tämä johtaa kuitenkin ongelmiin hengitettävän lääkkeen annostestelussa - kuten esimerkiksi tarpeeseen käyttää kantojauhetta. Työssä esitellään synteesimenetelmä lääkehiukkaselle, jossa mikronin kokoluokassa olevan hiukkasen sisällä on nanokokoisia lääkehiukkasia. Veden ja huonosti liukenevan lääkeaineen, naproxenin, seos jauhetaan lääkemyllyllä. Muodostuneeseen nanosuspensioon lisätään cimetidineä, lääkeainetta jota usein käytetään naproxenin kanssa. Suspensioon lisätään vielä mannitolia ja leusiiniä, jotka muodostavat lopullisen hiukkasen matriisin sekä pinnoitteen suspensiota prosessoitaessa aerosolireaktorissa. Näin valmistettuja ”nanoja mikroneissa” –hiukkasia (Nanos-in-Micros) ja niistä muodostuvaa jauhetta tutkitaan useilla eri menetelmillä. Muodostetuilla lääkehiukkasilla havainnoidaan olevan niille oletettuja ominaisuuksia, ja esitelty konsepti todetaan lupaavaksi metodiksi nanokokoisten lääkkeiden kehityksessä. Lopulta tutkimusta esitetään jatkettavaksi.

Glucagon-like peptide-1 (GLP-1), an incretin hormone, is used for type 2 diabetes mellitus (T2DM) treatment because of its ability to stimulate insulin secretion and release in a glucose-dependent manner. Despite of its potent insulinotropic effect, oral GLP-1 delivery is greatly limited by its instability in the gastrointestinal tract, poor absorption efficiency and rapid degradation by dipeptidylpeptidase-4 (DPP4) enzyme leading to a short half-life (~2min). Thus, a multistage dual-drug delivery nanosystem was developed to deliver GLP-1 and DPP4 inhibitor simultaneously. The system comprised of chitosan-modified porous silicon (CSUn) nanoparticles, which were coated by an enteric polymer, hydroxypropylmethylcellulose acetate succinate MF, using aerosol flow reactor technology. A non-obese T2DM rat model induced by co-administration of nicotinamide and streptozotocin was used to evaluate the in vivo efficacy of the nanosystem. The oral administration of H-CSUn nanoparticles resulted in 32% reduction in blood glucose levels and ~6.0-fold enhancement in pancreatic insulin content, as compared to the GLP-1+DPP4 inhibitor solution. Overall, these results present a promising system for oral co-delivery of GLP-1 and DPP4 inhibitor that could be further evaluated in a chronic diabetic study.

Nanotechnology based drug delivery systems are anticipated to overcome the persistent challenges in oral protein and peptide administration, and lead to the development of long awaited non-invasive therapies. Herein, an advanced single-step aerosol flow reactor based technology was used to develop a multifunctional site specific dual protein-drug delivery nanosystem. For this purpose, mucoadhesive porous silicon (PSi) nanoparticles encapsulated into a pH-responsive polymeric nanomatrix was developed for advanced oral type 2 diabetes mellitus therapy with an antidiabetic peptide, glucagon like peptide-1 (GLP-1), and the enzyme inhibitor, dipeptidyl peptidase-4 (DPP4). Chitosan surface modification inherited the mucoadhesiveness to the nanosystem which led to enhanced cellular interactions and increased cellular compatibility. An advanced aerosol flow reactor technology was used to encapsulate the chitosan modified nanoparticles into an enteric polymeric nanomatrix. The pH-sensitive polymeric matrix simultaneously prevented the gastric degradation of the encapsulated peptide and also preserved the mucoadhesive functionality of the chitosan-modified PSi nanoparticles in the harsh stomach environment. The multidrug loaded nanosystem showed augmented intestinal permeability of GLP-1, evaluated in an in vitro cell-based intestinal epithelium model, attributed to the permeation enhancer effect of chitosan and inhibition of GLP-1 degradation by the DPP4 inhibitor. The applied technology resulted in the development of a dual-drug delivery nanosystem that synergizes the antidiabetic effect of the loaded peptide and the enzyme inhibitor, thereby indicating high clinical potential of the system and preparation technique.

Salbutamol sulphate is used in the relief of asthma. Aerosol flow reactor method produces drug particles of salbutamol sulphate, which are coated with L-leucine. Depending on the production temperature, the L-leucine layer possesses different properties. The interactions between salbutamol sulphate and L-leucine molecules are studied, to learn about the behaviour of the combination drug particles. Differential scanning calorimetry (DSC) is used to gather information about the interactions taking place during heating. DSC shows evidence of water evaporation, melting of salbutamol sulphate in the particles, and an endothermic reaction that can be assigned to hydrogen bonding. Hydrogen bonding is complex to study because of its unstable nature. However, techniques like infrared (IR) spectroscopy can be used to locate hydrogen bonding in the molecular structure. Spectra of the vibrations of the structural elements of the samples, in various temperatures, are recorded with an IR spectrometer. In the spectra, distinct bands can be assigned to specific structural elements, based on previous literature. By studying the differences in band intensities between samples and temperatures, information about the molecular structures can be gathered. The intensities of the DSC endotherms of the samples are compared with specific band intensities in the IR spectra, and the location of the hydrogen bonding between the salbutamol and L-leucine molecules, can be restricted to few locations in the salbutamol molecule, and to one specific location in the L-leucine molecule. The increasing knowledge about the hydrogen bonding in these drug particles can be used to control the release of salbutamol sulphate in the lungs, when administered through inhalation. For example information about the advantage of sustained release caused by the hydrogen bonding, can be gained from dissolution studies. Vetysidos on hyvin epästabiili luonteeltaan, ja siksi sen tutkiminen on haastavaa. Tässä tutkimuksessa infrapuna (IR) spektroskopiaa käytetään vetysidoksen tutkimiseen. IR-spektrejä näytteiden värähtelytaajuuksista kerätään useassa eri lämpötilassa. Tietyt taajuudet spektreissä voidaan yhdistää tiettyihin molekyylitason rakenteisiin aiempien tutkimusten perusteella. Muutokset värähtelyintensiteeteissä taajuuksien sekä lämpötilojen välillä antavat tietoa molekyylitason sidoksista ja niiden muutoksista. DSC-spektrien ja IR-spektrien vertailun perusteella näytteissä tapahtuva vetysitoutuminen pystytään paikallistamaan muutamaan kohtaan salbutamolmolekyylissä, sekä yhteen tiettyyn kohtaa L-leusiinimolekyylissä. Lisätiedon toivossa lääkepartikkeleilla tehtiin myös liukoisuuskokeita. Näissä kokeissa määriteltyä pH:ta ja lämpötilaa käyttämällä yritettiin luoda samanlainen ympäristö kuin ihmiskeuhkoissa, ja siten saada tietoa vetysitoutumisen vaikutuksista lääkepartikkeleiden liukoisuuteen. Paremmin näitä vaikutuksia ymmärtämällä olisi mahdollisuus esimerkiksi hallita pitkäkestoisempaa vaikutusaineiden vapautumista keuhkoissa.
