1. NMR studies of the structure, kinetics and interactions of the conserved RNA motifs in the FMDV IRES

Abstract

The structure, kinetics, and interactions of the conserved 16mer and 15mer RNA motifs of the internal ribosome entry site (IRES) of the Foot-and-Mouth Disease virus (FMDV), have been investigated by homonuclear and heteronuclear NMR techniques. The 16mer RNA is endowed with a classic GNRA tetraloop motif, which is essential for IRES activity and the 15mer RNA motif is a potential tetraloop receptor. We have determined three high resolution NMR solution structures of the 16mer apo-RNA, the 16mer Mg2+ RNA complex and the 15mer apo-RNA with RMSDs of 0.17Å, 0.16Å and 0.35Å, respectively. The high precision of these NMR structures was achieved by including a large number of NMR experimental restraints, derived from NOEs and coupling constants, and validating them using the MolProbity program. The 16mer RNA structure comprised of six base pairs with a GUAA tetraloop and the 15mer RNA structure comprised of four base pairs and a large heptaloop; this is the first heptaloop to be studied by NMR.Addition of Mg2+ to the 16mer apo-RNA caused selective chemical shift changes to the stem G177 and loop G178 imino proton resonances, suggesting Mg2+-induced conformational change to the GUAA tetraloop. This was supported by a significant chemical shift change to the selectively 19F-labelled loop U179 in the 5-FU 16mer RNA. Furthermore, variable temperature experiments revealed retarded imino proton exchange for the stem and loop imino protons, demonstrating the enhanced thermodynamic stability conferred by Mg2+. This enhancement in stability was confirmed by measuring the imino proton exchange rates for the 16mer apo-RNA and the 16mer Mg2+ RNA complex. Analysis of the 16mer apo-RNA and its Mg2+ RNA complex NMR solution structures revealed that Mg2+-induced structural changes to the GUAA tetraloop act to stabilise the loop via stronger base stacking and intramolecular interactions. Fascinatingly, we discovered that Mg2+ ions provide increased stability required for the formation of a G.A sheared base pair in the GUAA tetraloop. RNA-RNA interactions between the 16mer and 15mer RNAs and their fluorinated analogues were studied by NMR spectroscopy. Small changes to chemical shift and linewidth of proton peaks in the non-fluorinated RNA-RNA complex provided evidence for a weak interaction between the loop of the 16mer RNA and the stem of the 15mer RNA. 19F-NMR experiments revealed additional peaks for the 19F-labelled U179 of the fluorinated 16mer/15mer RNA complex providing further good evidence of RNA-RNA interaction. The NMR structures of the conserved RNA motifs and their interactions have yielded important information in understanding the properties and behaviour of RNA. This will provide the first stepping stone in understanding the IRES mechanism and its use in antiviral therapy and biotechnology.

9. The Manukau Salivary Symptoms Score for Assessing the Impact of Sialendoscopy in Recurrent Obstructive Sialadenitis.

Abstract

Objective: To examine the Manukau Salivary Symptom Score (MSSS) questionnaire as a validated tool to assess obstructive sialadenitis-specific symptoms to both indicate disease severity and assess the outcome after sialendoscopic procedures. Study Design: A prospective observational study was performed from 2010 to 2019 comprising 164 patients undergoing sialendoscopy for nonneoplastic chronic obstructive salivary gland disease (COSGD). Setting: Department of Otolaryngology–Head and Neck Surgery at the Manukau Surgical Centre, Auckland, New Zealand, between June 2010 and September 2019. Methods: A prospective observational study was performed from 2010 to 2019 comprising 164 patients undergoing sialendoscopy for nonneoplastic COSGD. Patients completed the MSSS preoperatively and at postoperative follow-up. Statistical tests were used to compare pre- and postoperative answers. Cronbach's α was used to measure internal consistency. Finally, construct validity was determined by comparing the 5-question MSSS questionnaire to the preexisting 20-question Chronic Obstructive Sialadenitis Symptoms (COSS) questionnaire. Results: Postoperatively, patients had significant improvements in pain, eating, talking, swelling, and quality of life (P <.001). The MSSS questionnaire was found to have high internal consistency (α = 0.938). Questions in the MSSS had a very strong positive correlation with 3 COSS questions, a strong positive correlation with 8, a moderate positive correlation with 4, and a weak positive correlation with 1. Four COSS questions were not considered relevant and were not included in the MSSS questionnaire. Conclusion: The MSSS questionnaire is a simple, validated questionnaire that is useful for assessing the impact of sialendoscopy in patients with COSGD. [ABSTRACT FROM AUTHOR]

10. Interventions to reduce acute paediatric hospital admissions: a systematic review.

Background: Admission rates are rising despite no change to burden of illness, and interventions to reduce unscheduled admission to hospital safely may be justified.Objective: To systematically examine admission prevention strategies and report long-term follow-up of admission prevention initiatives.Data Sources: MEDLINE, Embase, OVID SP, PsychINFO, Science Citation Index Expanded/ISI Web of Science, The Cochrane Library from inception to time of writing. Reference lists were hand searched.Study Eligibility Criteria: Randomised controlled trials and before-and-after studies.Participants: Individuals aged <18 years.Study Appraisal and Synthesis Methods: Studies were independently screened by two reviewers with final screening by a third. Data extraction and the Critical Appraisals Skills Programme checklist completion (for risk of bias assessment) were performed by one reviewer and checked by a second.Results: Twenty-eight studies were included of whom 24 were before-and-after studies and 4 were studies comparing outcomes between non-randomised groups. Interventions included referral pathways, staff reconfiguration, new healthcare facilities and telemedicine. The strongest evidence for admission prevention was seen in asthma-specific referral pathways (n=6) showing 34% (95% CI 28 to 39) reduction, but with evidence of publication bias. Other pathways showed inconsistent results or were insufficient for wider interpretation. Staffing reconfiguration showed reduced admissions in two studies, and shorter length of stay in one. Short stay admission units reduced admissions in three studies.Conclusions and Implications: There is little robust evidence to support interventions aimed at preventing paediatric admissions and further research is needed. Abstract

The article analyzes how a T-level programme in optical care services will help recruit optical assistants. It is noted that T-level focuses on vocational skills and can help students into skilled employment, higher study or apprenticeships. It is noted that within the Healthcare Science T-level, there are various occupational specialisms, including Optical Care Services to learn the role and responsibilities of an optical assistant.

Abstract

BackgroundMedical schools differ, particularly in their teaching, but it is unclear whether such differences matter, although influential claims are often made. The Medical School Differences (MedDifs) study brings together a wide range of measures of UK medical schools, including postgraduate performance, fitness to practise issues, specialty choice, preparedness, satisfaction, teaching styles, entry criteria and institutional factors.MethodAggregated data were collected for 50 measures across 29 UK medical schools. Data includeinstitutional history(e.g. rate of production of hospital and GP specialists in the past),curricular influences(e.g. PBL schools, spend per student, staff-student ratio), selection measures(e.g. entry grades),teaching and assessment(e.g. traditional vs PBL, specialty teaching, self-regulated learning), student satisfaction, Foundation selection scores,Foundation satisfaction,postgraduate examination performance andfitness to practise(postgraduate progression, GMC sanctions). Six specialties (General Practice, Psychiatry, Anaesthetics, Obstetrics and Gynaecology, Internal Medicine, Surgery) were examined in more detail.ResultsMedical school differences are stable across time (median alpha = 0.835). The 50 measures were highly correlated, 395 (32.2%) of 1225 correlations being significant withp p Problem-based learning (PBL) schools differ on many measures, including lower performance on postgraduate assessments. While these are in part explained by lower entry grades, a surprising finding is that schools such as PBL schools which reportedgreaterstudent satisfaction with feedback also showedlowerperformance at postgraduate examinations.More medical school teaching of psychiatry, surgery and anaesthetics did not result in more specialist trainees. Schools that taught more general practice did have more graduates entering GP training, but those graduates performed less well in MRCGP examinations, the negative correlation resulting from numbers of GP trainees and exam outcomes being affected both by non-traditional teaching and by greater historical production of GPs.Postgraduate exam outcomes were also higher in schools with more self-regulated learning, but lower in larger medical schools.A path model for 29 measures found a complex causal nexus, most measures causing or being caused by other measures. Postgraduate exam performance was influenced by earlier attainment, at entry to Foundation and entry to medical school (the so-called academic backbone), and by self-regulated learning.Foundation measures of satisfaction, including preparedness, had no subsequent influence on outcomes. Fitness to practise issues were more frequent in schools producing more male graduates and more GPs.ConclusionsMedical schools differ in large numbers of ways that are causally interconnected. Differences between schools in postgraduate examination performance, training problems and GMC sanctions have important implications for the quality of patient care and patient safety.

Abstract

Background: \ud What subjects UK medical schools teach, what ways they teach subjects, and how much they teach those subjects is unclear. Whether teaching differences matter is a separate, important question. This study provides a detailed picture of timetabled undergraduate teaching activity at 25 UK medical schools, particularly in relation to problem-based learning (PBL).\ud \ud Method: \ud The Analysis of Teaching of Medical Schools (AToMS) survey used detailed timetables provided by 25 schools with standard 5-year courses. Timetabled teaching events were coded in terms of course year, duration, teaching format, and teaching content. Ten schools used PBL. Teaching times from timetables were validated against two other studies that had assessed GP teaching and lecture, seminar, and tutorial times.\ud \ud Results: \ud A total of 47,258 timetabled teaching events in the academic year 2014/2015 were analysed, including SSCs (student-selected components) and elective studies. A typical UK medical student receives 3960 timetabled hours of teaching during their 5-year course. There was a clear difference between the initial 2 years which mostly contained basic medical science content and the later 3 years which mostly consisted of clinical teaching, although some clinical teaching occurs in the first 2 years. Medical schools differed in duration, format, and content of teaching. Two main factors underlay most of the variation between schools, Traditional vs PBL teaching and Structured vs Unstructured teaching. A curriculum map comparing medical schools was constructed using those factors. PBL schools differed on a number of measures, having more PBL teaching time, fewer lectures, more GP teaching, less surgery, less formal teaching of basic science, and more sessions with unspecified content.\ud \ud Discussion: \ud UK medical schools differ in both format and content of teaching. PBL and non-PBL schools clearly differ, albeit with substantial variation within groups, and overlap in the middle. The important question of whether differences in teaching matter in terms of outcomes is analysed in a companion study (MedDifs) which examines how teaching differences relate to university infrastructure, entry requirements, student perceptions, and outcomes in Foundation Programme and postgraduate training.
