Patients were divided into two groups: âwith RV dysfunctionâ (⥠1 classic parameter) and âwithout RV dysfunctionâ. We used speckle tracking to estimate myocardial deformation. For all analyses, results were considered significant if p < 0.05.Results: The mean age across all participants was 65 ± 9 years, with 53% (48/91) being male. Patients in the group with RV dysfunction were able to walk shorter distances and had higher estimated right ventricular systolic pressure (RVSP) and mean pulmonary arterial pressure (mPAP). The RV free wall longitudinal strain (RVFWLS) was the only deformation indices that showed a significant correlation with all classic measurements and indices in the diagnosis of RV dysfunction (Wald test, 10.24; p < 0.01; odds ratio, 1.61). Aims:\ud The safety and efficacy of the novel selective cardiac myosin activator, omecamtiv mecarbil, in patients with heart failure with reduced ejection fraction (HFrEF) is tested in the Global Approach to Lowering Adverse Cardiac outcomes Through Improving Contractility in Heart Failure (GALACTIC‐HF) trial. Here we describe the baseline characteristics of participants in GALACTIC‐HF and how these compare with other contemporary trials.\ud \ud Methods and Results:\ud Adults with established HFrEF, New York Heart Association functional class (NYHA) ≥ II, EF ≤35%, elevated natriuretic peptides and either current hospitalization for HF or history of hospitalization/ emergency department visit for HF within a year were randomized to either placebo or omecamtiv mecarbil (pharmacokinetic‐guided dosing: 25, 37.5 or 50 mg bid). 8256 patients [male (79%), non‐white (22%), mean age 65 years] were enrolled with a mean EF 27%, ischemic etiology in 54%, NYHA II 53% and III/IV 47%, and median NT‐proBNP 1971 pg/mL. HF therapies at baseline were among the most effectively employed in contemporary HF trials. Aims The SHIFT trial showed that ivabradine reduced heart rate (HR) and the risk of cardiovascular outcomes. Concerns remain over the efficacy and safety of ivabradine on heart failure (HF) due to Chagas disease (ChD). We therefore conducted a post hoc analysis of the SHIFT trial to investigate the effect of ivabradine in these patients. Methods and results SHIFT was a randomized, double‐blind, placebo‐controlled trial in symptomatic systolic stable HF, HR ≥ 70 b.p.m., and in sinus rhythm. The ChD HF subgroup included 38 patients, 20 on ivabradine, and 18 on placebo. The ChD HF subgroup showed high prevalence of bundle branch right block and, compared with the overall SHIFT population, lower systolic blood pressure; higher use of diuretics, cardiac glycosides, and antialdosterone agents; and lower use of angiotensin‐converting enzyme inhibitor/angiotensin II receptor blocker or target daily dose of beta‐blocker. ChD HF presented a poor prognosis (all‐cause mortality at 2 years was ~60%). The mean twice‐daily dose of ivabradine was 6.26 ± 1.15 mg and placebo 6.43 ± 1.55 mg. Ivabradine reduced HR from 77.9 ± 3.8 to 62.3 ± 10.1 b.p.m. (P = 0.005) and improved functional class (P = 0.02). A trend towards reduction in all‐cause death was observed in ivabradine arm vs. placebo (P = 0.07). Ivabradine was not associated with serious bradycardia, atrioventricular block, hypotension, or syncope. Conclusions ChD HF is an advanced form of HF with poor prognosis. Ivabradine was effective in reducing HR in these patients and improving functional class. Although our results are based on a very limited sample and should be interpreted with caution, they suggest that ivabradine may have a favourable benefit–risk profile in ChD HF patients.

Fundamento: O ecocardiograma fornece dados importantes na avaliação cardiológica de pacientes em insuficiência cardíaca. A identificação de parâmetros ecocardiográficos na cardiopatia chagásica grave auxiliaria na implementação terapêutica e na avaliação prognóstica. Objetivo: Correlacionar parâmetros ecocardiográficos com desfecho mortalidade cardiovascular em pacientes com fração de ejeção < 35%. Métodos: Estudo de análise retrospectiva de parâmetros ecocardiográficos coletados prospectivamente e pré-especificados em 60 pacientes incluídos no Estudo Multicêntrico Randomizado de Terapia Celular em Cardiopatias - braço cardiopatia chagásica. Os parâmetros foram: diâmetros e volumes diastólico e sistólico do ventrículo esquerdo, fração de ejeção, diâmetro do átrio esquerdo, volume do átrio esquerdo, volume indexado do átrio esquerdo, pressão sistólica da artéria pulmonar, integral da velocidade do fluxo aórtico, índice de performance miocárdica, taxa de aumento da pressão do ventrículo esquerdo, tempo de relaxamento isovolumétrico, velocidade das ondas E, A, Em, Am e Sm, tempo de desaceleração da onda E, relação E/A , E/Em e insuficiência mitral. Resultados: No seguimento médio de 24,18 meses, 27 pacientes faleceram. a fração de ejeção média era de 26,6 ± 5,34%. Na análise multivariada, foram incluídos os parâmetros de fração de ejeção (HR = 1,114; p = 0,3704), volume indexado do átrio esquerdo (HR = 1,033; p < 0,0001) e relação E/Em (HR = 0,95; p = 0,1261). O volume indexado do átrio esquerdo foi um fator de predição independente em relação ao desfecho e observou-se que um valor > 70,71 mL/m2 foi associado ao aumento significativo na mortalidade (log rank p < 0,0001). Conclusão: O volume indexado do átrio esquerdo mostrou-se como único fator de predição independente de mortalidade nesta população de pacientes chagásicos e com disfunção sistólica importante. Background: Echocardiography provides important information on the cardiac evaluation of patients with heart failure. The identification of echocardiographic parameters in severe Chagas heart disease would help implement treatment and assess prognosis. Objective: To correlate echocardiographic parameters with the endpoint cardiovascular mortality in patients with ejection fraction < 35%. Methods: Study with retrospective analysis of pre-specified echocardiographic parameters prospectively collected from 60 patients included in the Multicenter Randomized Trial of Cell Therapy in Patients with Heart Diseases (Estudo Multicêntrico Randomizado de Terapia Celular em Cardiopatias) - Chagas heart disease arm. The following parameters were collected: left ventricular systolic and diastolic diameters and volumes; ejection fraction; left atrial diameter; left atrial volume; indexed left atrial volume; systolic pulmonary artery pressure; integral of the aortic flow velocity; myocardial performance index; rate of increase of left ventricular pressure; isovolumic relaxation time; E, A, Em, Am and Sm wave velocities; E wave deceleration time; E/A and E/Em ratios; and mitral regurgitation. Results: In the mean 24.18-month follow-up, 27 patients died. The mean ejection fraction was 26.6 ± 5.34%. In the multivariate analysis, the parameters ejection fraction (HR = 1.114; p = 0.3704), indexed left atrial volume (HR = 1.033; p < 0.0001) and E/Em ratio (HR = 0.95; p = 0.1261) were excluded. The indexed left atrial volume was an independent predictor in relation to the endpoint, and values > 70.71 mL/m2 were associated with a significant increase in mortality (log rank p < 0.0001). Conclusion: The indexed left atrial volume was the only independent predictor of mortality in this population of Chagasic patients with severe systolic dysfunction.

Background: Dobutamine stress echocardiography (DSE) is an important tool in the diagnosis of coronary artery disease. However, there is hesitation in clinical practice for using it in patients with Chagas disease (CD) due to the arrhythmogenic potential of this heart condition. This study aimed to evaluate the incidence and variables associated with arrhythmias during DSE in a population of patients with CD. Methods: A population of 205 consecutive patients with CD and suspected coronary heart disease was assessed through a retrospective database analysis. CD was confirmed in all patients by serological testing. Results: The mean age of the patients selected was 64 years, and 65.4% of the patients were female. Significant arrhythmias occurred as follows: nonsustained ventricular tachycardia in 7.3% of patients; supraventricular tachycardia and sustained ventricular tachycardia in 1%; and atrial fibrillation in 0.5%. Nonsignificant arrhythmias occurred as follows: premature ventricular contractions in 48% of patients and bigeminy in 4.4%. Values for the wall‐motion score index at rest greater than 1.12 and 1.18 were independently correlated with the occurrence of nonsignificant arrhythmias (odds ratio [OR] = 2.90, P < 0.001) and significant arrhythmias (OR = 4.23, P = 0.044), respectively. Conclusion: DSE should be considered a safe examination in patients with CD despite the known increased risk of arrhythmias in this group of patients. The occurrence of arrhythmias was low in this study. Abnormal wall‐motion score index values at rest were associated with the occurrence of significant and nonsignificant arrhythmias during the test. Objetivo: Comparar las dos técnicas analíticas más utilizadas en la determinación de la digoxinemia y verificar cuál de ellas atiende mejor las necesidades de la rutina de seguimiento clínico de los pacientes con insuficiencia cardíaca. Métodos: En este trabajo fue investigado el perfil clínico y de laboratorio de 15 pacientes con insuficiencia cardiaca los cuales fueron atendidos en el Ambulatorio para pacientes con Insuficiencia Cardiaca del Hospital das Clínicas de la Universidad Federal de Goiás, Brasil. Las muestras de sangre de los pacientes fueron recogidas y analizadas por Inmunoensayo y Cromatografía Líquida de Alta Eficiencia acoplada a Espectrometría de Masas (LC-MS/MS). Resultados: Los resultados del test "t" de Student (p = 0,05) mostraron que hubo diferencia significativa entre los dos métodos analíticos. Las concentraciones determinadas por inmunoensayo fueron mayores que las concentraciones determinadas por LC-MS/MS, ya que ese método analiza la digoxina y también sus metabolitos y substancias endógenas, mientras que el LC-MS/MS analiza solamente la molécula de digoxina. Por otro lado, ninguno de los pacientes, presentó datos clínicos relevantes sugestivos de intoxicación digitálica, a pesar de que varios fármacos con potencial de interacción estuvieron asociados al tratamiento. Conclusiones: A partir de los resultados obtenidos puede concluirse que el método por LC-MS/MS se mostró más selectivo y específico, siendo una opción para la monitorización terapéutica de la digoxina, desde que se obtengan valores de referencia para la digoxinemia por LC-MS/MS.

Objective: To compare two analytical techniques used in the determination of plasma digoxin (LC-MS/MS and immunoassay) and to verify which one better answer the need of the clinical monitoring routine of patients with cardiac heart failure. Method: The clinical findings in 15 cardiac heart failure (CHF) outpatients of the Cardiac Heart Service of the Goias Federal University Clinical Hospital were investigated. Blood samples of the patients were collected and analysed by Immunoassay and by Liquid Chromatography coupled to Mass Spectrometry (LC-MS/MS). Results: The results of the statistic test (Student p = 0,05) showed a significant difference between the analytical methods: immunoassay concentrations were higher than the concentrations determined by LCMS/MS. The explanation may be because immunoassay method measures digoxin plus other metabolites and endogenous substances, while the LC-MS/MS method measures only the digoxin molecule. None of the patients, showed relevant clinical data suggestive of digitalis intoxication, even several drugs with potential interaction were associated with treatment. Conclusion: It was concluded, therefore, that LC-MS/MS me thod is safer, more selective and specific than immunoassay, being an option for therapeutic drug monitoring of digoxin, since the reference values would be obtain for digoxinemia by LC-MS/MS.

FUNDAMENTO: Os fatores de risco para as síndromes hipertensivas gestacionais (SHG) são vários e podem relacionar-se à região e etnia da população. No Estado de Goiás, são escassos os estudos sobre esse tema. OBJETIVO: Investigar os fatores de risco maternos para SHG. MÉTODOS: Utilizou-se um estudo caso-controle por meio da análise dos prontuários das parturientes da Maternidade do Hospital das Clínicas da Universidade Federal de Goiás (HC-UFG), em 2005. Os fatores de risco foram analisados pela análise de regressão logística e pelo teste exato de Fisher. RESULTADOS: Em 2005, houve 890 partos na Maternidade do HC-UFG, e 129 gestantes apresentaram diagnóstico de SHG (14,5%). A análise multivariada identificou a obesidade como fator de risco tanto para hipertensão gestacional (HG) (OR: 17,636; IC 95%: de 2,859 a 108,774) como para hipertensão arterial crônica superajuntada à pré-eclâmpsia (HCSPE) (OR: 27,307; IC 95%: de 4,453 a 167,440). Primiparidade constituiu fator de risco para HG (OR: 5,435; IC 95%: de 1,923 a 15,385). Idade acima de 30 anos foi fator de risco para HCSPE (OR: 5,218; IC 95%: de 1,873 a 14,536) e fator de proteção para pré-eclâmpsia (PE) (OR: 0,035; IC 95%: de 0,003 a 0,364). Raça não-branca representou risco independente para PE (OR: 13,158; IC 95%: de 1,672 a 100,000) e PE prévia para HCSPE (OR: 4,757; IC 95%: de 1,261 a 17,938). Das gestantes com hipertensão arterial crônica (HAC), 73,5% desenvolveram HCSPE (p < 0,001). CONCLUSÃO: Os fatores obesidade, raça não-branca, PE prévia, idade acima de 30 anos e HAC identificados foram semelhantes à maioria dos achados da literatura. BACKGROUND: There are several risk factors for hypertensive syndromes in pregnancy (HSP), and these can be related to regional and ethnic factors. Studies on this issue are scarce in the State of Goiás. OBJECTIVE: To investigate maternal risk factors for HSP. METHODS: Case-control study based on the analysis of medical records of parturients from the Maternity Service of Hospital das Clínicas da Universidade Federal de Goiás (HC-UFG) in 2005. Risk factors were analyzed using Logistic Regression Analysis and Fisher's Exact Test. RESULTS: In 2005, there were 890 deliveries in the Maternity Service of HC-UFG, and 129 pregnant women were diagnosed with HSP (14.5%). The multivariate analysis identified obesity as a risk factor both for gestational hypertension - GH (OR: 17.636; 95% CI: 2.859 to 108.774) and for preeclampsia superimposed on chronic hypertension - PESCH (OR: 27.307; 95% CI: 4.453 to 167.440). Primiparity was a risk factor for GH (OR: 5.435; 95% CI: 1.923 to 15.385). Age above 30 years was a risk factor for PESCH (OR: 5.218; 95% CI: 1.873 to 14.536) and a protective factor against preeclampsia - PE (OR: 0.035; 95% CI: 0.003 to 0.364). Non-white race was an independent risk for PE (OR: 13.158; 95% CI: 1.672 to 100.000) and previous PE for PESCH (OR: 4.757; 95% CI: 1.261 to 17.938). Of the pregnant women with chronic hypertension (CH), 73.5% developed PESCH (p < 0.001). CONCLUSION: The factors identified - obesity, non-white race, previous PE, age above 30 years and CH, were similar to those found in the majority of studies in the literature.
