The objective of this study was to investigate the performance of nanotopographic implants with a Sr-functionalized titanium (Ti) coating (Ti–Sr–O) with respect to osseointegration in osteoporotic bone. The trial was designed to examine the effect of sustained-release characteristics of Sr in poor-quality bone. Three Ti–Sr–O groups, which differed from each other in coating thickness, Sr contents, and Sr release, were examined. These were prepared by a magnetron sputtering process and compared to uncoated grade 4 Ti. Composition, morphology, and mechanical stability of the coatings were analyzed, and Sr release data were gained from in vitro washout experiments. In vivo investigation was carried out in an osteoporotic rat model and analyzed histologically, 6 weeks and 12 weeks after implantation. Median values of bone-to-implant contact and new bone formation after 6 weeks were found to be 84.7% and 54.9% (best performing Sr group) as compared to 65.2% and 23.8% (grade 4 Ti reference), respectively. The 12-week observation period revealed 84.3% and 56.5% (best performing Sr group) and 81.3% and 39.4% (grade 4 Ti reference), respectively, for the same measurements. The increase in new bone formation was found to correlate with the amount of Sr released in vitro. The results indicate that sputtered nanostructured Ti–Sr–O coatings showed sustained release of Sr and accelerate osseointegration even in poor-quality bone, and thus, may have impact on practical applications for medical implants. New excavations in ravines at Ounjougou in Mali have brought to light a lithic and ceramic assemblage that dates from before 9400 cal BC. The authors show that this first use of pottery coincides with a warm wet period in the Sahara. As in East Asia, where very early ceramics are also known, the pottery and small biracial arrowheads were the components of a new subsistence strategy exploiting an ecology associated with abundant wild grasses. In Africa, however, the seeds were probably boiled (then as now) rather than made into bread. Abstract

The area of Ounjougou consists of a series of gullies cut through Upper Pleistocene and Holocene formations on the Dogon Plateau in the Sahel at the south edge of the Sahara Desert. Here the authors have chronicled a stratified sequence of human occupation from the tenth to the second millennium BC, recording natural and anthropogenic strata containing artefacts and micro- and macro- palaeoecological remains, mostly in an excellent state of preservation. They present a first synthesis of the archaeological and environmental sequence for the Holocene period, define five main occupation phases for Ounjougou, and attempt to place them within the context of West African prehistory. The issue of the peopling of the Dogon Country (Mali) and surrounding regions over the past 3000 years, taking into account the influential of Sahelian paleoclimatic variations is reviewed. The pluridisciplinary research program 'Paleoenvironment and Human Population in West Africa' includes numerous archaeological sites exposed by erosion processes, indicating the presence of human populations from the Lower Paleolithic to present times.

Abstract

Due to advances in surgical methods, such as miniplate and lag screw osteosynthesis, there is current discussion about the adequate therapy of condylar fractures. Regarding the insufficient outcome of conservative treatment confirmed by refined methods of functional diagnosis, there is a trend towards surgical therapy, at least in cases of condylar displacement out of the fossa. This trend was also prompted when surgeons learned that the condyle determines local growth but does not serve as a growth center of the mandible. Along with increased interest in temporomandibular function and surgical therapy of internal disorders, surgical therapy of traumatic soft tissue lesions was emphasized. This consequently led to surgical restoration of intraarticular fractures. Conservative treatment will nonetheless be sufficient in cases of minor displacement due to a higher ability of remodeling, even more so in children.

Abstract

Radiotherapy puts bones at risk of developing osteonecrosis. Irradiation has an impact on the viability as well as the differentiation capacity of mesenchymal stem cells (MSC), which play a pivotal role in bone regeneration. To investigate the effect of irradiation on MSC, human bone-derived MSC were irradiated in vitro. With increasing doses the cells’ self-renewal capabilities were greatly reduced. Notably however, mitotically stalled cells were still capable of differentiating into osteoblasts and preadipocytes. Next the pigs mandibles were subjected to fractionized radiation of 2x9 Gy within one week. This treatment mimicks that of a standardized clinical treatment regimen of a head and neck cancer patient (30x2 Gy). Fractures, which had been deliberately generated and subsequently irradiated showed retarded osseous healing. When isolating MSC from irradiated sites at different time points post irradiation, no significant changes in comparison to cells derived from un-irradiated specimens regarding proliferation capacity and osteogenic differentiation potential became apparent. Therefore, pig mandibles were irradiated with 9 and 18 Gy in vivo, and MSC were isolated immediately afterwards. No significant differences between the untreated and 9 Gy -irradiated bone with respect to proliferation and osteogenic differentiation were unveiled. Yet, cells isolated from 18 Gy irradiated specimens exhibited a reduced osteogenic differentiation capacity, and during the first two weeks proliferation rates were greatly diminished. Thereafter, cells recovered and showed normal proliferation behaviour. These findings imply that MSC can cope with irradiation up to high doses in vivo, and could be implemented in future therapeutic concepts to protect from osteonecrosis.
