12. Extra-articular calcification of the lateral collateral ligament (LCL) presenting with recurrent acute knee periarthritis: A case-based review.

Abstract

To present a case with extra-articular calcification of the lateral collateral ligament (LCL) presenting with recurrent acute knee periarthritis. A 53-year-old Saudi male patient presented with acute onset of right-sided knee pain, localized swelling and tenderness along the lateral aspect after sport related activity. He reported a similar attack at the same knee joint a few years earlier. Plain X-ray and magnetic resonance imaging (MRI) after the initial attack showed calcification along the LCL. On examination, the patient had mild knee effusion, painful knee extension, and localized swelling along the LCL. A recent MRI and complementary computerized tomography (CT) showed a fragmented calcific shadow along the LCL. The latter was associated with soft tissue hyperintensity, which indicated extensive local inflammatory reaction. After ruling out internal derangement and infection, it was concluded that the patient had an acute episode of peri -arthritis caused by calcification along the LCL of the affected knee joint. Intramuscular (IM) betemetazone sodium dipropionate/phosphate was used to treat the peri -arthritis and as such deposits may cause a potent neutrophilic chemotactic response and intense inflammation, colchicine was added for its anti-inflammatory properties and other known mechanisms that are effective in treating crystal-induced arthritis. On the second day following treatment, a significant improvement was seen. A case-based review was presented. Despite the rare incidence of symptomatic LCL calcification, fragmentation of the calcific deposits may lead to acute intense inflammatory exacerbation and peri -arthritis. 17. Bilateral extensive steroid-associated osteonecrosis (SAON) of femur, tibia and patella: Successful early management with combined antiresorptive and anabolic bone agents.

Abstract

To present a case of bilateral extensive steroid-associated osteonecrosis (SAON) of femur, tibia and patella that was successfully managed with combined antiresorptive and anabolic bone agents. A 38-year-old female patient encountered an aggressive coronavirus disease 2019 (COVID-19) infection and was given systemic steroids for six months. The patient then began to experience bilateral lower limb pain. Tenderness over the knee joint margins was found, as well as tenderness of the lower end of the femur, upper tibia, and patella on both sides. The initial plain x-ray of the lower limb bones revealed subtle areas of sclerosis at the proximal metaphysis of tibial bones. The patient did not improve despite stopping steroids and repeated courses of simple analgesics, and the pain became progressive and more intense, to the point where the patient was unable to bear any weight and became wheel chair bound. Magnetic resonance imaging (MRI) was done revealing extensive osteonecrotic lesions involving the distal metaphysis of the femur with posterior extension into the medial and lateral condyles abutting the articular surfaces. Two anti-osteoporotic drugs were used; alendronate, used weekly to inhibit osteoclastic activity and limit the progression of the osteonecrotic lesions and teriparatide, an anabolic agent that increases osteoblasts, resulting in new trabecular and cortical bone growth. Clinical improvement, pain and ambulation, occurred after one month of initiation of treatment and follow up MRI study after 10 months showed marked radiological improvement. 18. The Economic Importance of Air Travel in High-Amenity Rural Areas

Abstract

The western United States offers a case study on the importance of access to large population centers and their markets, via road and air travel, for economic development. The vast distances between towns and cities in the American West can be a detriment to business, yet they also serve to attract technology and knowledge-based workers seeking to live in a picturesque setting. In spite of the increasing importance of amenities to migration and business location, also needed is access to markets, particularly via commercial air service. We test a new county classification system for the western United States to reflect differing degrees of access to population centers and account for the increasing importance of airports. Past classifications are based on population size and cross-county commuting. We examine the validity of this new classification and test for differences in economic performance among the three county types. Our findings show that there are three distinct Wests that can be classified using economic performance measures and socioeconomic characteristics. The results show that "metro" and "isolated" counties are clearly distinct, but "connected" counties, those that are rural in nature but have ready access to metropolitan areas via air travel, can be difficult to distinguish from "metro" and "isolated" counties. Much of the variation is explained by travel distance to airports. The findings illustrate the importance of airports in rural development, and the limitations facing those communities that are isolated from markets and population centers. The results apply to other parts of the world with similar characteristics that include large expanses of open space, natural amenities that attract migrants and stimulate new business, and different degrees of access to large population centers via road or air travel, and therefore different rates of economic growth. 23. Rheumatoid factor and anti-citrullinated protein antibodies (ACPA) in psoriatic arthritis (PsA), and skin psoriasis: Relevance and clinical implications.

Abstract

Anti-citrullinated protein antibodies (ACPAs), is a highly specific markers for rheumatoid arthritis, can also be found in psoriatic arthritis (PsA). This work aimed to look into the frequency of rheumatoid factor (RF) and ACPA in PsA and psoriasis patients without clinical evidence of arthritis (PSO), and to correlate findings with demographics, disease characteristics, laboratory findings, and radiological damage in PsA. Patients and methods: The study included 78 PsA patients, 47 PSO patients, and 69 normal controls. Full clinical assessments, RF and ACPA assays, and modified radiological Steinbroker score (Mss) for PsA were performed. Results: The age of the patients was 45.6 ± 8.7 years and the M:F 59:66 (M:F 0.9:1). Positive RF was not significantly different between PsA and PSO groups (p = 0.35), but positive ACPA was significantly more common in PsA patients (p < 0.001) than in PSO and controls. No significant difference was observed between the PSO and controls (p = 0.08). In PsA, RF titers correlated significantly with ESR (p = 0.002), swollen joint count (SJC)(p = 0.02), tender joint count (TJC)(p = 0.02), and mSS (p = 0.001), while in the PSO RF correlated significantly with ESR (p = 0.011) and CRP levels (p = 0.02). In the PsA, ACPA titer significantly correlated with CRP levels (p < 0.001), SJC (p = 0.002), TJC (p = 0.003), mSS (p < 0.001) and PASI (p < 0.001), but with none of these in the PSO. Conclusion: PsA patients have more frequent positive RF and ACPA than PSO patients. ACPA values are significantly correlated with markers of inflammation, swollen and tender joint count and radiological damage in PsA and not in PSO patients. [ABSTRACT FROM AUTHOR]

Abstract

Osteoarthritis (OA) is the most prevalent type of arthritis worldwide, resulting in painand often chronic disability and a significant burden on healthcare systems globally. Non-steroidalanti-inflammatory drugs (NSAIDs), analgesics, intra-articular corticosteroid injections are of littlevalue in the long term, and opioids may have ominous consequences. Radiotherapy of knee OAhas no added value. Physical therapy, exercises, weight loss, and lifestyle modifications may givepain relief, improve physical functioning and quality of life. However, none of them has articularcartilage regenerating potential. Due to a better understanding of osteoarthritis, innovative newtreatment options have been developed. In this narrative review, we focus on emerging OA kneetreatments, relieving symptoms, and regenerating damaged articular cartilage that includes intra-articularhuman serum albumin, conventional disease-modifying anti-rheumatic drugs (DMARDs),metformin, lipid-lowering agents (statin), nerve growth factors antagonists, bone morphogeneticprotein, fibroblast growth factors, Platelet-Rich Plasma (PRP), Mesenchymal Stem Cells (MSC),exosomes, interleukin-1 blockers, gene-based therapy, and bisphosphonate.

Abstract

Studies of the relation of fibromyalgia (FM) and widespread pain (WSP) to mortality have differed as to the presence or absence of an association and the extent of cause-specific mortality. However, no studies have investigated which definitions of FM and WSP associate with mortality, nor of FM mortality in other diseases. We investigated these issues and the meaning of mortality in patients with FM. We used Cox regression to study 35,248 rheumatic disease patients with up to 16 years of mortality follow-up in all patients and separately in those with diagnoses of rheumatoid arthritis (RA) (N = 26,458), non-inflammatory rheumatic disorders (NIRMD) (N = 5,167) and clinically diagnosed FM (N = 3,659). We applied 2016 FM criteria and other FM and WSP criteria to models adjusted for age and sex as well as to models that included a full range of covariates, including comorbid disease and functional status. We estimated the degree of explained of variance (R2) as a measure of predictive ability. We found positive associations between al'l definitions of FM and WSP and all-cause mortality, with relative risks (RR)s ranging from 1.19 (95%CI 1.15–1.24) for American College of Rheumatology (ACR) 1990 WSP to 1.38 (1.31–1.46) in age and sex adjusted revised 2016 criteria (FM 2016). However, in full covariate models the FM 2016 RR reduced further to 1.15 (1.09–1.22). The association with mortality was noted with RA (1.52 (1.43–1.61)), NIRMD (1.43 (1.24–1.66)) and clinical FM (1.41 (1.14–1.75) - where 37% of FM diagnosed patients did not satisfy FM 2016 criteria. In the all-patient analyses, the age and sex explained variation (R2) was 0.255, increasing to 0.264 (4.4%) when FM 2016 criteria were added, and to 0.378 in a full covariate model. Death causes related to FM 2016 status included accidents, 1.45 (1.11–1.91); diabetes 1.78 (1.16–2,71); suicide, 3.01 (1.55–5.84) and hypertensive related disorders, 3.01 (1.55–5.84). Cancer deaths were less common 0.77 (0.68–0.88). FM is weakly associated with mortality within all criteria definitions of FM and WSP examined (3.4% of explained variance), and across all diseases (RA, NIRMD, clinical FM) equally. Clinical and criteria-defined FM had different mortality outcomes. We found no evidence for a positive association of cancer and FM or WSP. [ABSTRACT FROM AUTHOR]
