Abstract

Macrodystrophia lipomatosa (MDL) is a rare congenital nonhereditary anomaly, in which progressive overgrowth of mesenchymal tissue leads to disproportionate enlargement of the digits of the hands and feet. We discuss the case of a 5-year-old girl who presented with progressive disproportionate enlargement of the third and fourth toes of the right foot since birth. A diagnosis of MDL was made on the basis of clinical and radiological findings.

Abstract

Trachyonychia also known as 20-nail dystrophy presents as roughness of nail plate associated with longitudinal ridging giving a sandpaper-like appearance. It has two subtypes: opaque variety and shiny variety. It commonly presents in the pediatric age group 3–12 years but can present at any age. It can be idiopathic or associated with cutaneous or systemic conditions. Alopecia areata, atopic dermatitis, vitiligo, and lichen planus are some of the commonly associated conditions. Diagnosis is clinical and in doubtful cases, biopsy has a role. Onychoscopy as a novel diagnostic modality can be used. Spontaneous resolution is seen in the majority of pediatric patients, so counseling, reassurance, and active nonintervention can be tried. Treatment is mainly for cosmetic concerns or when associated with some systemic conditions. Various treatment modalities are available with variable efficacy. Topical treatment options include tazarotene, topical steroid, and calcipotriol/betamethasone combination, whereas systemic agents such as oral mini-pulse steroid, methotrexate, cyclosporine, and low-dose acitretin have shown beneficial role. Novel treatment modalities such as the use of tofacitinib, fluocinonide 0.05% with bifonazole 1% cream, combination of fractional CO2 laser with topical therapy, and alitretinoin have shown promising results.

8. Clinicoepidemiologic profile of leprosy in geriatric population in post-elimination era: A retrospective, hospital-based, cross-sectional study from Eastern India

Abstract

Background and Aim: Geriatric populations are susceptible to leprosy infection with masked clinical signs due to lower immunity in them. Our aim was to analyze the clinicoepidemiologic profile of patients with geriatric leprosy and find out the reasons for delayed diagnosis and treatment. Materials and Methods: A retrospective, record-based study was conducted in a tertiary care center from May 2019 to May 2022. The clinicodemographic data of biopsy-confirmed leprosy cases aged ≥60 years were obtained from the leprosy clinic record. Various reasons for the delay in treatment were also recorded. Simple statistics was used for analysis. Results: Out of 605 leprosy cases, 50 (7.4%) cases belonged to the geriatric population. Males outnumbered females (M/F = 37/13). The mean age of the patients was 66.28 + 6.5 years. Maximum patients belonged to 60–69 years of age. The mean duration of illness was 35.22 months (range 1–240 months). Most of the patients were illiterate (33.6%), and 56% were farmers by occupation. The ulnar nerve was the most common nerve to be thickened in 90% (45/50) cases, followed by the common peroneal nerve in 54% (27/50) cases, radical cutaneous nerve in 52% (26/50) cases, and posterior tibial nerve in 24% (12/50) cases. Borderline tuberculoid was the most common type in 44% cases, followed by lepromatous leprosy in 22%, borderline lepromatous leprosy in 18% (9/50), and pure neuritic leprosy in 14%. Type 1 and type 2 lepra reactions were found in 18% and 14% cases, respectively. Also, 38% had grade 2 disability. Common reasons for the delay in treatment were financial constraints, lack of family support, and personal superstitious beliefs. The study was limited by its retrospective nature. Conclusion: Geriatric leprosy needs special attention as the elderly are more prone for deformities; also, because of low immunity, there is a high chance of developing multibacillary leprosy, and therefore, they are potential sources of infection to the community.

11. A challenging case of recalcitrant Hyper-IgE syndrome successfully treated with omalizumab

Abstract

Hyper-IgE syndrome (HIES) is a rare group of primary immunodeficiency diseases characterized by elevated IgE levels, eosinophilia, recurrent pyoderma, ear infection, eczematous dermatitis, and pulmonary infection. The treatment depends on the clinical presentation of the disease. The eczematous skin lesions usually respond to a topical steroid, calcineurin inhibitors, and sometimes, based on severity, systemic drugs are given. The secondary infections are usually treated with oral and topical antibiotics. We are describing a case of HIES who presented with severe recalcitrant itchy oozy eczematous skin lesions which did not respond to conventional systemic drugs and were later on controlled with injection omalizumab. In addition, we have discussed the HIES in detail in the current article.

Abstract

The study explores the preservation policy and techniques employed in preserving library materials in three libraries in South Asia: Central University of Kashmir; Islamic University of Science and Technology; and University of Kashmir. The study aims to: investigate preservation policy/plan/division of selected libraries; investigate the current preservation technique used in the libraries; explore the category of staff responsible for preservation of library materials; and identify capacity building programs attended by staff at local, national/international level on preservation. Data collection was employed: results were tabulated, interpreted, and correlated with the published literature to ferret out meaningful findings and draw logical conclusions. The study reveals that: binding (sewing) and fumigation (solid fumigation) are widely employed techniques in preservation; manuscripts are mostly prioritized for preservation; and usually professionals are involved in dealing with preservation. However, none of the libraries possess preservation policies, which are crucial for the preservation of the library collection on a systematic and scientific basis. In order to cope with the changing environment of information preservation and management, libraries regularly organize seminars and conferences on preservation, and it is hoped that stakeholders continue to identify loopholes and to build capacity for preservation. Abstract

BackgroundSARS-CoV-2 pandemic has had many significant impacts within the surgical realm, and surgeons have been obligated to reconsider almost every aspect of daily clinical practice.MethodsThis is a cross-sectional study reported in compliance with the CHERRIES guidelines and conducted through an online platform from June 14th to July 15th, 2020. The primary outcome was the burden of burnout during the pandemic indicated by the validated Shirom-Melamed Burnout Measure.ResultsNine hundred fifty-four surgeons completed the survey. The median length of practice was 10 years; 78.2% included were male with a median age of 37 years old, 39.5% were consultants, 68.9% were general surgeons, and 55.7% were affiliated with an academic institution. Overall, there was a significant increase in the mean burnout score during the pandemic; longer years of practice and older age were significantly associated with less burnout.There were significant reductions in the median number of outpatient visits, operated cases, on-call hours, emergency visits, and research work, so, 48.2% of respondents felt that the training resources were insufficient. The majority (81.3%) of respondents reported that their hospitals were included in the management of COVID-19, 66.5% felt their roles had been minimized; 41% were asked to assist in non-surgical medical practices, and 37.6% of respondents were included in COVID-19 management.ResultsNine hundred fifty-four surgeons completed the survey. 16. An inclusive analysis of the leptonic decay modes of the Z⁰ boson

Abstract

This thesis describes an analysis of the process e+e— → l+l- (where l = e, μ,, τ) at centre-of-mass energies between 88 GeV and 94 GeV, using the data collected by the DELPHI detector between the years 1991 and 1993. The leptonic decays of the Z° boson are selected without attempting to separate the three lepton types, thus making it an inclusive lepton analysis. The theory behind lepton pair production is introduced and the extraction of various electroweak parameters from the experimental observables is discussed. The LEP collider and the DELPHI detector are described, with special emphasis being given to the sub-detectors used in the analysis. The criteria used to select a high purity leptonic sample are described along with calculations of various backgrounds and efficiencies. The sample of selected leptonic events is then used to measure the cross-sections and forward-backward asymmetries. Finally, a fit to these cross-sections and asymmetries, together with the hadronic (e+e- → qq̄) cross-sections, is carried out. Various Z° parameters are obtained: the mass and total width Mz-91.1876 ± 0.0052 GeV/c2, Γz = 2.4971 ± 0.0061 GeV, the ratio of the hadronic to leptonic partial widths Rl = 20.73 ± 0.09, and the pole leptonic asymmetry (A°FB)1 = 0.0195 ± 0.0042. Using these results and the value of the strong coupling constant (αs), determined by the DELPHI collaboration, the number of light neutrino species is determined to be Nν = 3.045 ± 0.035. The leptonic partial width is found to be: Γl = 83.82 ± 0.29 MeV. Using the measured leptonic forward-backward asymmetries, the squared vector and axial-vector couplings of the Z° to charged leptons are found to be (ĝv1)2-3 and (ĝa1)2 = 0.2505 ± 0.0009. These values can be used to determine the effective rho parameter and the effective weak mixing angle: p̂ = 1.0020 ± 0.0036, and sin2 θefflept = 0.2297 ± 0.0024. A full Standard Model fit to the data gives the values of the strong coupling constant, αs, and the mass of the top quark, mtop, as being: αs = 0.123 ± 0.010, mtop = 178+22-25 (expt)+18-16(Higg s)GeV/c2, where 60 < mHiggs (GeV/c2) < 1000 with a central value of 300 GeV/c2. All the results obtained agree with the results from the lepton-identified analyses (analyses in which leptonic events are selected on the basis of their individual flavour) and with the predictions of the Standard Model.

Abstract

This thesis describes an analysis of the process e+e— → l+l- (where l = e, μ,, τ) at centre-of-mass energies between 88 GeV and 94 GeV, using the data collected by the DELPHI detector between the years 1991 and 1993. The leptonic decays of the Z° boson are selected without attempting to separate the three lepton types, thus making it an inclusive lepton analysis. The theory behind lepton pair production is introduced and the extraction of various electroweak parameters from the experimental observables is discussed. The LEP collider and the DELPHI detector are described, with special emphasis being given to the sub-detectors used in the analysis. The criteria used to select a high purity leptonic sample are described along with calculations of various backgrounds and efficiencies. The sample of selected leptonic events is then used to measure the cross-sections and forward-backward asymmetries. Finally, a fit to these cross-sections and asymmetries, together with the hadronic (e+e- → qq̄) cross-sections, is carried out. Various Z° parameters are obtained: the mass and total width Mz-91.1876 ± 0.0052 GeV/c2, Γz = 2.4971 ± 0.0061 GeV, the ratio of the hadronic to leptonic partial widths Rl = 20.73 ± 0.09, and the pole leptonic asymmetry (A°FB)1 = 0.0195 ± 0.0042. Using these results and the value of the strong coupling constant (αs), determined by the DELPHI collaboration, the number of light neutrino species is determined to be Nν = 3.045 ± 0.035. The leptonic partial width is found to be: Γl = 83.82 ± 0.29 MeV. Using the measured leptonic forward-backward asymmetries, the squared vector and axial-vector couplings of the Z° to charged leptons are found to be (ĝv1)2-3 and (ĝa1)2 = 0.2505 ± 0.0009. These values can be used to determine the effective rho parameter and the effective weak mixing angle: p̂ = 1.0020 ± 0.0036, and sin2 θefflept = 0.2297 ± 0.0024. A full Standard Model fit to the data gives the values of the strong coupling constant, αs, and the mass of the top quark, mtop, as being: αs = 0.123 ± 0.010, mtop = 178+22-25 (expt)+18-16(Higg s)GeV/c2, where 60 < mHiggs (GeV/c2) < 1000 with a central value of 300 GeV/c2. All the results obtained agree with the results from the lepton-identified analyses (analyses in which leptonic events are selected on the basis of their individual flavour) and with the predictions of the Standard Model.

27. Association between Angiographic Severity of Coronary Artery Disease and High Sensitivity C-Reactive Protein.

Abstract

Objectives: To determine correlation between blood levels of high sensitivity c-reactive protein (Hs CRP) and severity of coronary artery disease (CAD) on coronary angiography measured through Gensini scoring system. Study type, settings and duration: The descriptive case study was conducted at Rawalpindi institute of cardiology. The duration of study was six month. Patients and Methods: Adult patients of both genders between age 25 to 60 years visiting to outpatient department of the institute and registered for coronary angiogram were included. From each of the enrolled patient about 5cc venos blood sample was collected and Hs-CRP was performed. Severity was measured through Gensini score after coronary angiography. Angiographic Gensini scores were correlated with Hs-CRP using Pearson's correlation (2-tailed). Value of < 0.05 was considered statistically significant. Results: Total 87 patients fulfilling the inclusion criteria were enrolled among them 81.6% (n=71) were males and 18.4% (n=16) were females. The correlation coefficient was found to be 0.043, implying that there is negligible relationship between Hs-CRP and Gensini scores at p= 0.696. Conclusion: There is no significant correlation between Hs-CRP and severity of CAD measured through Gensini scores on coronary angiography. [ABSTRACT FROM AUTHOR]

Abstract

This paper presents a hierarchical architecture for the coordination and control of autonomous agents performing intelligent team operations. Each team consisting of multiple aerial and ground vehicles, uses a coordinated strategy through communication via a wireless network. As an exemplary case study, a pursuit-evasion scenario is developed. This paper also introduces the experimental setup for aerial and ground-based autonomous agents. The proposed scheme is currently under development for near future experiments.

35. ECRM and customers: a case of Askari Commercial Bank, Pakistan.

Abstract

Purpose – The purpose of this study is to observe and analyze current banking practices in Pakistan, regarding the electronic customer relationship management system (ECRM). The aim is to explore the variables in relation to operational problems that may occur with banks that use ECRM and the customer's perception of the usage of e-banking. Design/methodology/approach – The research design is divided into two sections using both qualitative as well as quantitative methods to represent the results. The first section explores the variables related to the operational problems with e-banking while the second section discusses the variables related to the customer's perception of the usage of e-banking systems. Descriptive statistics are used to define the demographic variables while correlation is used to describe the operational problems. Cross case analysis is used to present the customer perception of e-banking practices. In total, 40 staff members and four customers are selected as samples for this study. SPSS was used for data analysis. Results – Data analysis shows that customers are not ready to adopt new technologies, which results in a low level of satisfaction with e-banking. Internet speeds and government policies are also not supportive of e-banking in Pakistan. Owing to a lack of trust in technology and a low computer literacy rate, customers hesitate to adopt new technology. On the other hand, internet rates are too high and provided speed is in Kb/s. Practical implications – The results of this study will provide some ideas and practical suggestions, which can be implemented, particularly with E-banking in order to improve its continuance (i.e. customer satisfaction strategies as an effective means of maintaining the subscriber base, market share and the overall revenue of online banking). This paper has covered the operational issues related to e-banking as well as customer's perception on usage of e-banking a case study of Askari Bank, Pakistan. 40 staff members and four customers are selected as sample for this study. Both qualitative and quantitative methods are used to present the results. Descriptive statistics is applied to describe the demographic variables while for operational problems correlation was used. Finally cross case analysis present customers' perception about e-banking practices. Analysis shows that customer is not ready to adopt new technology that why their satisfaction level with e-banking is low. Internet speed and government policies are not supportive for e-banking in Pakistan. Abstract

Two lignan glycosides identified as 5,5'-dimethoxylariciresinol-9-O-β-D-glucopyranoside (rupaside) and lyoniresinol-9'-O-β-D-glucopyranoside along with ethyl-α-D-galactopyranoside, α-and β-D-glucose, and β-D-fructose have been isolated from Ruellia patula. Their structures were assigned on the bases of spectral evidence. Preliminary cardiovascular screening of butanol and aqueous fractions suggested possible cardiotonic activity. However, additional studies will be required to confirm this type of activity.
