Background: Although atopic dermatitis (AD) is becoming a pressing public health concern in the world, Madagascar is underrepresented in the AD literature. Objective: We aimed to study the demographic and clinical pattern of AD in adult dermatology outpatients. Methods: A cross-sectional study was conducted in the Department of Dermatology, University Hospital, Antananarivo, Madagascar. Patients >15 years old with a registered diagnosis of AD, from January 2010 to February 2019, were included. AD was diagnosed by a dermatologist according to Hanifin and Rajka criteria. The severity of AD was assessed using scoring atopic dermatitis (SCORAD). Results: Forty-two cases of AD were included. The prevalence was 0.5%. The median patient age was 37 years. The age of onset of AD was before the age of 15 years in 38% of the patients and after the age of 15 years in 61.9% of the patients. There was a female preponderance (female to male ratio, 2:1), but no correlation was found between sex and the severity of AD. People living in urban areas were the most affected. According to SCORAD, 37 cases presented moderate AD and 2 cases presented severe AD. Conclusion: The prevalence of AD in adult dermatology outpatients is still low, and moderate AD is the most frequent form, according to SCORAD.

Chromoblastomycosis and sporotrichosis are the two main implantation mycoses that are now recognized as fungal neglected tropical diseases (NTDs). Their laboratory diagnosis mainly relies on direct microscopy, histopathology, and identification of the fungus by culture. However, to be appropriately used, these techniques require mycological expertise that is not widely available and may be absent in peripheral health care facilities in endemic areas. In addition, they lack sensitivity and specificity, and the culture for isolation and identification can have a long time-to-results period. Molecular methods, including matrix-assisted laser desorption/ionization time-of-flight mass spectrometry (MALDI-TOF MS), have been developed in well-equipped reference laboratories. They greatly improve the rapidity and accuracy of diagnosis; in particular, for species identification. Recently, PCR and sequencing have paved the way for more user-friendly point-of-care tests, such as those based on LAMP or RCA technologies, which can be used in basic healthcare settings and even in field consultations.

INTRODUCTION: les cancers cutanés sont des tumeurs kératinisantes malignes, d’origines épidermique, mélanique ou annexielle. Ces cancers sont encore peu connus, pourtant existent bien à Madagascar, un pays abritant un peuple à phototype diversifié sous un fort degré d’ensoleillement. Notre étude vise à déterminer le profil épidémio-clinique des cancers cutanés à Antananarivo.Méthodes: il s’agit d’une étude rétrospective, descriptive des cancers cutanés sur une durée de sept ans au Service d’Oncologie du CHU Joseph Ravoahangy Andrianavalona, Antananarivo. Elle inclut les patients présentant cliniquement des lésions cutanées d’allure tumorale, confirmées ensuite par un examen histologique.Résultats: nous avions colligé 47 cas de cancers cutanés à prédominance masculine (sex ratio 1,13). L’âge moyen était de 49,3 ans. Les agriculteurs étaient les plus touchés. Les lésions touchaient préférentiellement la tête, le cou (44%) et les membres inférieurs (42%). Les cancers cutanés étaient découverts au stade localisé dans 61,7% des cas. Le carcinome épidermoïde constituait le premier cancer cutané des Malgaches (37%), suivi par le mélanome (21%) et le carcinome basocellulaire (11%). La prise en charge était essentiellement chirurgicale (74,19%).Conclusion: le cancer épidermoïde est le premier cancer Malgache, suivi du mélanome et du carcinome basocellulaire. Les cancers cutanés touchent surtout la population jeune nécessitant une prise en charge bien cadrée.

Neoplastic and infectious etiologies must be ruled out. IGM is a diagnostic challenge for countries with high tuberculosis endemicity like Madagascar since it may clinically and radiologically mimic breast tuberculosis. We report a case of IGM associated with erythema nodosum in a Malagasy.Case Report: A 29-year-old primiparous woman came to a dermatological consultation for typical erythema nodosum lesions that appeared one month after a breast swelling. She had no particular medical history. Examination revealed typical erythema nodosum lesions on the legs, voluminous tender mass in the right breast. Bacteriological samples and tuberculosis test were negative. Imaging showed mastitis on the right breast with no evidence of malignancy. Histology revealed a non-caseating granulomas on the lobule of the right breast. As part of an etiological work-up, COVID-19 serology was performed with a positive IgG antibody. The diagnosis of IGM associated with erythema nodosum was evocated. The evolution was favorable under systemic corticosteroid therapy.Discussion

- Abstract
Introduction. La neurofibromatose type 1 (NF1) est une maladie autosomique dominante, à expression multisystémique. Sa prévalence varie selon les pays et peu de données sur la NF1 sont connues à Madagascar. Méthodologie. Une étude transversale rétrospective a été réalisée au service de dermatologie du Centre hospitalier universitaire Joseph Raseta Befelatanana d’Antananarivo, Madagascar, sur une période de 6 ans (de janvier 2014 à décembre 2019). Ont été inclus dans cette étude les patients vus en consultation porteurs de NF1 dont le diagnostic a été confirmé et chez lesquels une enquête généalogique a pu être faite. Résultats. Vingt-huit cas de neurofibromatose ont été inclus avec un sexe-ratio H/F de 0,87. L’âge moyen était de 24 ans avec des extrêmes de 11 et 54 ans. La tranche d’âge 16-35 ans était la plus touchée. Dix-sept patients présentaient la forme sporadique. Tous les patients présentaient des taches café au lait et des neurofibromes cutanés, 15 patients portaient des nodules iridiens de Lisch. Les difficultés d’apprentissage, l’épilepsie et la scoliose étaient les complications les plus représentées. Conclusion. Les manifestations cliniques de la NF1 sont extrêmement variables. Même si les complications systémiques sont rares, le suivi des patients est indispensable. Neurofibromatosis 1 (NF1) is an inherited disease, in an autosomal dominant manner, with complex multi-system involvements. Prevalence varies from one country to another. However, little is known about neurofibromatosis in African countries, particularly in Madagascar. Methodology. A descriptive retrospective study from 2014 to 2019 was conducted at the service of dermatology at University Hospital Joseph Raseta Befelatanana in Antananarivo, including all patients with neurofibromatosis according to National Institutes of Health Consensus Conference criteria for whom genealogical investigation could be made. Results. Among 32 cases of NF1 seen during 6 years, 28 cases were included with a sex ratio M/F of 0.87. The mean age was 24 years ranging from 11 to 54 years. Seventeen patients presented sporadic forms. All patients had “café au lait” spots and cutaneous neurofibromatosis. Three cases presented plexiform neurofibromas which cause significant cosmetic and functional problems by their size and their displayed topography. Fifteen patients had Lisch nodules but no case of optic glioma was identified. Neurological symptoms such as learning difficulties, epilepsy and headache were frequent in our case series. However, access to medical imaging was very limited. Scoliosis was the most common orthopedic complication. Conclusion. The clinical manifestations of NF1 are extremely variable. A male infant presented with large hemangioma of the right parotid gland diagnosed at the age of 3 months. Starting at the age of 4 months, he was treated with oral propranolol for 10 months, which led to a marked reduction in tumor size and strawberry hemangioma. La douleur induite par les soins correspond à la douleur survenant lors des actes à visé diagnostique et/ou thérapeutique. A notre connaissance, nous n'avons pas encore des données disponibles pour les douleurs induites par les soins à l'Hôpital de Befelatanana. Nos objectifs étaient de décrire le profil épidémiologique de la douleur induite par les soins, d'identifier les principaux facteurs influençant sur l'intensité de la douleur et leurs retentissements chez les patients. Il s'agissait d'une étude rétrospective, transversale type un jour donné menée dans les douze services de Médecines au Centre Hospitalier Universitaire de Befelatanana en Novembre 2013. Cent deux patients ont été retenus dans l'étude et trois cent vingt trois actes douloureux étaient enregistrés. La fréquence de la douleur induite par les soins était de 69,86%. Le genre féminin prédominait dans 52% des cas (n=53) avec un sex-ratio à 0,92. L'âge moyen était de 46 ans. Les ponctions vasculaires étaient l'acte prédominant dans 49,54% (n=109) des cas. Les infirmiers réalisaient les soins dans 47,05% (n=48) des cas. L'information verbale était la mesure préventive utilisée dans 57,84% des cas (n=59). Le transport par marche à pied et au dos représentait 16,67% des cas (n=17). Les patients naïfs des gestes étaient plus anxieux. Ces patients gardaient de mauvais souvenir dans 64,71% des cas (n=66). La fréquence de douleur induite par les soins était trop élevée. Un effort important est nécessaire pour réduire la douleur induite par les soins.

However, significant adverse effects have been associated with continuous exposure to oral corticosteroids.Objective: We aim to assess the adverse effects of long-term corticosteroid therapy in patients seen in the Department of Dermatology at the University Hospital Joseph Raseta Befelatanana Antananarivo (UH/JRB), Madagascar, and to identify the risk factors associated with these adverse effects.Methods: A cross-sectional study was conducted during 4 months to assess the adverse effects of long-term corticosteroid therapy in patients seen in the Department of Dermatology. Patients treated with oral corticosteroids for more than 3 months were included in our study.Results: The prevalence of long-term use of oral corticosteroids in the Department of Dermatology of UH/JRB was 34.28%. A total of 51 patients were included and adverse effects occurred in 64.70% of this population. Repetitive infections and cutaneous adverse reactions were the most frequent adverse effects, in 23.52% and 11.76% of cases, respectively. There were no correlations between age, gender, type of disease treated, the molecule used or daily dose and the risk of adverse effects. Patients who received a corticosteroid dose of more than 40 mg daily (longer than 3 months) or a high cumulative dose of corticosteroids had a high risk of adverse effects.Conclusion: Repetitive infections and cutaneous adverse reactions are the most frequent adverse effects of long-term oral corticosteroid use.

BACKGROUND: Psoriasis is a chronic, inflammatory and multifactorial dermatosis that impairs quality of life (QoL). Health-related QoL has become an important element in medical decision making along with the effectiveness and the harmlessness of the treatments.OBJECTIVE: To assess the impact of psoriasis in the QoL of patients with psoriasis by using the DLQI scales.METHODS: A cross-sectional study, during ten months, was conducted in the Department of Dermatology of the University Hospital Joseph Raseta Befelatanana Antananarivo Madagascar including patients more than 15 years-old with moderate to severe psoriasis. The severity of psoriasis was assessed using the “Psoriasis Area and Severity Index (PASI)”. QoL of patients with psoriasis was evaluated by using the DLQI scales.RESULTS: 80 patients were included, their mean age was 36,52 years and the male to female was 1,5:1. The mean DLQI score was 13,85. Symptoms, feelings and psychic were the most altered dimensions. QoL was impaired in young patients, single, having medium level education. Even though patients with disease duration more than 5 years had higher DLQI score than other patients, the difference was not statistically significant (p=0,36). Furthermore, clinical presentation of psoriasis didn’t influence patient’s QoL (p=0,73). Patients with nail involvement had QoL impaired but the difference with another localization was not statistically significant (p=0,2).The quality of life was influenced by body area involved. The higher the body surface area involved, the more QoL is impaired (p=0,002). Furthermore, the higher the PASI, the more QoL is altered (p=0,002).CONCLUSION: Psoriasis has a negative impact in the quality of life in Malagasy patients with psoriasis, especially in younger and single patients. Worse quality of life is correlated to severity of psoriasis.

Our case-control study found that there is a statistically significant correlation between birth month and risk of AD in Malagasy children < 3 years. Compared with people born in December, people born in April had the highest risk of AD (OR: 2.11, 95% CI 0.93– 4.78), followed by people born in March (OR: 1.52, 95% CI 0.79– 2,88). Introduction:les cancers cutanés sont des tumeurs kératinisantes malignes, d’origines épidermique, mélanique ou annexielle. Ces cancers sont encore peu connus, pourtant existent bien à Madagascar, un pays abritant un peuple à phototype diversifié sous un fort degré d’ensoleillement. Notre étude vise à déterminer le profil épidémio-clinique des cancers cutanés à Antananarivo. Méthodes:il s’agit d’une étude rétrospective, descriptive des cancers cutanés sur une durée de sept ans au Service d’Oncologie du CHU Joseph Ravoahangy Andrianavalona, Antananarivo. Elle inclut les patients présentant cliniquement des lésions cutanées d’allure tumorale, confirmées ensuite par un examen histologique. Résultats:nous avions colligé 47 cas de cancers cutanés à prédominance masculine (sex ratio 1,13). L’âge moyen était de 49,3 ans. Les agriculteurs étaient les plus touchés. Les lésions touchaient préférentiellement la tête, le cou (44%) et les membres inférieurs (42%). Les cancers cutanés étaient découverts au stade localisé dans 61,7% des cas. Le carcinome épidermoïde constituait le premier cancer cutané des Malgaches (37%), suivi par le mélanome (21%) et le carcinome basocellulaire (11%). La prise en charge était essentiellement chirurgicale (74,19%). Conclusion:le cancer épidermoïde est le premier cancer Malgache, suivi du mélanome et du carcinome basocellulaire. Les cancers cutanés touchent surtout la population jeune nécessitant une prise en charge bien cadrée.

- Abstract
Introduction: The Dermatology Department of the University Hospital Center of Befelatanana in Antananarivo deals with both outpatient and inpatient dermatoses. Two outpatient studies were conducted in 2001 and in 2006. But, this will be the first for Madagascar regarding hospitalization. So our aim is to determine the epidemiological-clinical profile of dermatoses seen in hospitalization.Patients and methods: This descriptive retrospective study was conducted from 1 February 2006 to 28 February 2012 in Dermatology department at the University Hospital Befelatanana. The register of hospital consultants and files were used to fill out data collection form.Results : We collected 286 patients seen in the Dermatology department. The mean age was 37.70 years (5- 98 years) with a sex ratio 0.49. Our study showed the importance of autoimmune diseases (33.3%) that are dominated by systemic lupus erythematosus (n = 51). It is a serious chronic pathology evolving by push. The management of severe toxidermia (25,1%) is done in hospital as in any other country. The mean duration of hospitalization was 34 days. The mortality rate was 3.49%.Conclusion: The serious forms of dermatological pathologies require management in hospital.

- Abstract
Introduction: Therapeutic compliance is the adequacy of the patient's behavior regarding medical recommendations. In Madagascar, No survey has not yet been published in that regard. Thus, our purposes are to determine the compliancy rate of patients with chronic diseases in dermatology and to identify the factors affecting their therapeutic compliance.Methods: Monocentric descriptive and retrospective study performed in the dermatology department of Joseph Raseta Befelatanana University Hospital center, On a 6 months duration, among patients with chronic dermatoses.Results: We counted 133 patients with a compliancy rate of 35.34%. Several parameters particularly age, gender, educational level, salary, understanding of pathology and treatment, duration of follow-up, were considered and correlated with the rate of compliance to assess factors of good and bad compliance.Conclusion: In order to improve the therapeutic observance in care unit, It is necessary to intervene on the standard of living of the patients suffering from chronic dermatoses and to promote the therapeutic education accompanied by psychological support to make them understand their pathology as well as the appropriate treatment.

Introduction: Lupus erythematosus is a serious autoimmune inflammatory disease. Mucocutaneous manifestations are themain clinical manifestations, so our aim is to determine the different cutaneous manifestations of lupus erythematosus in Antananarivo.Methods: A retrospective study was conducted over a period of 5 years from January 2007 to December 2011 in the medical derpartment of Joseph Raseta Befelatanana University Hospital.. All patients who presented lupus erythematosus were included.Results: Among 57,509 patients seen in Joseph Raseta Befelatanana University Hospital, sixty seven cases of lupus erythematosus were included. The mean age was 35.08 years [minimum: 9; maximum: 69 years]. The sex ratio was 0.06. Among these 67 patients, forty six presented cutaneous manifestations (68, 65%). As specific lesions in lupus, chronic cutaneous lupus was observed in 47.82%, acute cutaneous lupus in 30.43% and subacute cutaneous lupus in 06.52% of patients. The other dermatological manifestations found were alopecia in 52.17%, photosensitivity in 43.47%, Raynaud's phenomenon in 17.39% and bullous lesions in 06.52% of patients.Conclusion: A little difference was found in epidemiological, clinical and paraclinical aspects in lupus Malagasy patients and Africans. Lupus erythematosus is still a under-diagnosed disease in Madagascar. A deeper understanding of the cutaneous manifestations of SLE is essential for diagnosis and prognosis

Rosai-Dorfman disease is a rare, benign histiocytic proliferative disorder that usually affects the lymph nodes. Although extranodal involvement has been reported in diverse sites, manifestation in the cardiovascular system is extremely rare. Specifically, cardiac involvement in Rosai-Dorfman disease is an extraordinarily infrequent event. We describe a case of a 36-year-old female who presented Rosai-Dorfman disease of multiple organs including the heart, with poor prognosis.

Sporotrichosis is a saprozoonotic fungal infection found mostly in tropical and subtropical areas. Few case reports in Madagascar have been published. To document sporotrichosis epidemiology in Madagascar, we conducted a cross-sectional study. During March 2013-June 2017, we recruited from select hospitals in Madagascar patients with chronic cutaneous lesions suggestive of dermatomycosis. Sporotrichosis was diagnosed for 63 (42.5%) of 148 patients. All but 1 patient came from the central highlands, where the prevalence was 0.21 cases/100,000 inhabitants. Frequency was high (64.7%) among patients <18 years of age. Sporotrichosis was diagnosed for 73.8% of patients with arm lesions, 32.3% with leg lesions, and 15.4% with lesions at other sites. Molecular identification identified 53 Sporothrix schenckii isolates. Among the 32 patients who were followed up, response to itraconazole was complete or major for 15 and minor for 17. Overall, endemicity of sporotrichosis in Madagascar was high, concentrated in the highlands. [ABSTRACT FROM AUTHOR]

43. A Case of Fluoroquinolone-Resistant Leprosy Discovered after 9 Years of Misdiagnosis

- Abstract
We report a case of misdiagnosed leprosy in a 21-year-old Malagasy male, who, improperly treated, developed secondary mycobacterial resistance to fluoroquinolone. The patient contracted the infection 9 years prior to the current consultation, displaying on the right thigh a single papulonodular lesion, which progressively spread to the lower leg, back, and face. Initial administration of ciprofloxacin and prednisolone led to temporary and fluctuating improvement. Subsequent long-term self-medication with ciprofloxacin and corticosteroid did not heal the foul and nonhealing ulcers on the legs and under the right sole. Histopathological findings were compatible with lepromatous leprosy. Skin biopsy was positive for acid-fast bacilli and PCR assay confirmed the presence of a fluoroquinolone-resistant strain of Mycobacterium leprae (gyrA A91V). After 6 months of standard regimen with rifampicin, clofazimine, and dapsone, clinical outcome significantly improved. Clinical characteristics and possible epidemiological implications are discussed.
