Abstract

Prostate Cancer Diagnosis and Treatment Enhancement Through the Power of Big Data in Europe (PIONEER) is a European network of excellence for big data in prostate cancer, consisting of 32 private and public stakeholders from 9 countries across Europe. Launched by the Innovative Medicines Initiative 2 and part of the Big Data for Better Outcomes Programme (BD4BO), the overarching goal of PIONEER is to provide high-quality evidence on prostate cancer management by unlocking the potential of big data. The project has identified critical evidence gaps in prostate cancer care, via a detailed prioritization exercise including all key stakeholders. By standardizing and integrating existing high-quality and multidisciplinary data sources from patients with prostate cancer across different stages of the disease, the resulting big data will be assembled into a single innovative data platform for research. Based on a unique set of methodologies, PIONEER aims to advance the field of prostate cancer care with a particular focus on improving prostate-cancer-related outcomes, health system efficiency by streamlining patient management, and the quality of health and social care delivered to all men with prostate cancer and their families worldwide.Prostate Cancer Diagnosis and Treatment Enhancement Through the Power of Big Data in Europe (PIONEER) is a European network of excellence for big data in prostate cancer, consisting of 32 private and public stakeholders from 9 countries across Europe. In this Perspectives article, the authors introduce the PIONEER project and describe its aims and plans for ultimately improving prostate cancer care through the use of big data.

Abstract

During a two-year period, intra-operative ultrasonic explorations were performed in 44 patients for the purpose of localisation of intracranial masses. This facilitated accurate intraoperative assessment of the location and consistency of the mass. We consider operative ultrasound to be an invaluable adjunct to surgery of small intracerebral masses.

Abstract

Injury to the facial nerve is of concern in surgery of cerebellopontine angle tumors. The crossed acoustic reflex provides a way to monitor the ipsilateral facial nerve with the auditory stimuli delivered to the contralateral side. Using the method of optimum digital filtering, it is possible to monitor the resulting brain stem facial evoked response (BFER) in real time. This paper presents preliminary experiences in more than 18 such operations monitored using this method. This preliminary study demonstrates a trend for a high (88.8%) correlation between BFER and postoperative facial nerve function. Identical latencies from simultaneous BFER and facial nerve recordings along with findings after facial nerve transection suggest that some portion of the complex BFER waveform derives from facial nerve depolarization. 36. Use of high dose corticosteroids in patients with inoperable brain tumours.

Abstract

Eleven patients with inoperable brain tumours were treated with high doses of corticosteroids (methylprednisolone 200-2000 mg/day) for up to 151 days (mean 55 days). Neurological improvement occurred in eight patients on high doses after deterioration on concentional doses (methylprednisolone 80-120 mg/day). In two patients steroids could be completely discontinued for several months. Serious adverse effects included sepsis in three patients and myopathy in tow. All three patients with sepsis also received chemotherapy. There were no deaths that could be attributed to steroids. The most likely effect of high dosage steroids is reduction of cerebral oedema. It is conceivable that in some instances high dose steroids may also result in tumour inhibition or oncolysis or both. [ABSTRACT FROM AUTHOR]

43. Spontaneous canine hydrocephalus: cerebrospinal fluid dynamics.

Abstract

Cerebrospinal fluid dynamics were studied in 14 dogs with spontaneous hydrocephalus. In nine of the dogs aqueductal obstruction was observed and the remainder had a `communicating type' hydrocephalus. The major histological findings consisted of severe ependymal destruction, spongy changes in the periventricular white matter, increased density of capillaries in this area, and varying degrees of thickening, fibrosis, and fusion of the choroid villi. The formation and absorption of CSF were studied by perfusion of the cerebral ventricles. The rate of formation of CSF was found to decrease with perfusion pressure by V(f) = 0·02595-0·00022 P ml./min (P = pressure in cm H(2)O). The absorption of spinal fluid was found to increase linearly with pressure by V(a) = 0·0165 + 0·00050 P. The various factors influencing the formation and absorption of the spinal fluid are discussed. The meaning and attainment of `arrest' of the hydrocephalic process in terms of the measured rates of CSF formation and absorption in these animals are considered. [ABSTRACT FROM AUTHOR]

Abstract

Within three weeks of definitive surgical intervention, 467 patients with histologically proved malignant glioma were randomized to receive one of four treatment regimens: semustine (MeCCNU), radiotherapy, carmustine (BCNU) plus radiotherapy, or semustine plus radiotherapy. We analyzed the data for the total randomized population and for the 358 patients in whom the initial protocol specifications were met (the valid study group). Observed toxicity included acceptable skin reactions secondary to radiotherapy and reversible leukopenia and thrombocytopenia due to chemotherapy. Radiotherapy used alone or in combination with a nitrosourea significantly improved survival in comparison with semustine alone. The group receiving carmustine plus radiotherapy had the best survival, but the difference in survival between the groups receiving carmustine plus radiotherapy and semustine plus radiotherapy was not statistically significant. The combination of carmustine plus radiotherapy produced a modest benefit in long-term (18-month) survival as compared with radiotherapy alone, although the difference between survival curves was not significiant at the 0.05 level. This study suggests that it is best to use radiotherapy in the post-surgical treatment of malignant glioma and to continue the search for an effective chemotherapeutic regimen to use in addition to radiotherapy.

47. Visual system toxicity following intraarterial chemotherapy

Abstract

We studied the effects of intra-arterial chemotherapy on the visual system of 29 consecutive patients with gliomas. As expected, infra-ophthalmic carotid infusion of cisplatin or carmustine (BCNU) was associated with clinically apparent anterior visual pathway lesions. Electroretinography revealed retinal dysfunction in patients without clinical abnormalities. Supra-ophthalmic carotid infusion of cisplatin or BCNU caused no retinal or optic nerve lesions. Electroretinography was abnormal in only one of these patients. Our results indicated that BCNU and cisplatin cause ischemic damage and are toxic to both retinal and neural tissue in patients with gliomas.

48. Real-Time Monitoring of Brainstem Auditory Evoked Response (BAER) during Cerebellopontine Angle (CPA) Surgery

Abstract

The signal-to-noise ratio of brainstem auditory evoked responses (BAER) can be greatly enhanced by use of optimal digital filtering before averaging. This permits accurate assessment of auditory nerve status every 5 to 10 seconds, making real-time intraoperative monitoring possible. The major advantages yielded by real-time monitoring—in our experience thus far—have been (1) identification of potentially adverse functional consequences of apparently uneventful surgical maneuvers, reducing postoperative dysfunction, (2) early indication of potential for improved clinical function, and (3) potential identification and localization of neural tissue in the face of absent surgical landmarks. Examples of these advantages will be provided from case studies, and the possibility that real-time monitoring may improve ability to preserve hearing will be discussed.
