2. Students with Disabilities and Clinical Placement: Understanding the Perspective of Healthcare Organisations

Abstract

The Australian Disability Discrimination Act (1992) has challenged higher education to meet the increasing needs of students with different abilities. This may be more so in programmes that have practice components where students must demonstrate competency, regardless of their situation. This qualitative exploratory study undertaken at the University of South Australia (UniSA) investigated host healthcare organisations' perspectives on providing clinical practice placements and supporting Bachelor of Nursing (BN) students with physical disabilities through their practice components. An advisory group consisting of the university's legal officer, disability services representative, programme directors and other academics from the BN monitored the project. Directors of nursing from 50 randomly selected South Australian metropolitan and rural healthcare organisations that host student placements nominated a representative from their organisations to participate in a series of focus groups. Focus group discussion was guided by open questions developed from relevant literature, research team deliberations, advisory group recommendations and the previous focus group discussions. Data analysis consisted of manually identifying themes from each stage of the process, and the congregate data. The findings were bridging the reality gap through joint commitment, the right of disclosure versus confidentiality and proactivity through collaboration and communication.

Abstract

Background: Clinical nursing education is a major part of any undergraduate programme aimed at preparing nurses for competent professional practice. However research most often evaluated specific clinical education models; few studies compared models and little attention has been given to its effect on student learning outcomes. The purpose of this review was to examine the effectiveness of clinical education models for undergraduate nursing programs. Method: This systematic review utilised the Joanna Briggs Institute systematic review approach. A web-based literature search was performed to identify original research articles published from2002 to 2016 using a three-step search strategy. All papers selected for retrieval were assessed by at least two independent reviewers for inclusion criteria, methodological validity prior to inclusion, and data extraction in the review. A comprehensive search using all identified keywords and index terms was undertaken in major databases. Results: This review included nine studies including two pre-posttest studies with a control group totalling 1893 participants including 1286 nursing students. Conclusion: The evidence regarding the effectiveness of clinical education models for undergraduate nursing programs is notably weak; however this review found limited evidence that the clinical facilitator model is preferable to the preceptor model and Dedicated Education Unit (DEU) model provided greater engagement and an enhanced learning environment compared with standard facilitationmodel. Abstract

We report three cases of arterial emboli in patients with lung cancer treated with cisplatin chemotherapy. All three patients were managed without surgical intervention but subsequent oncological treatment was complicated by the sequelae of arterial emboli. We discuss the issues surrounding these patients and the importance of identifying patients at risk of arterial embolic phenomena with cisplatin treatment.

To examine the effects of advanced access (same-day physician appointments) on patient and provider satisfaction and to determine its association with other variables such as physician income and patient emergency department use.Patient satisfaction survey and semistructured interviews with physicians and support staff; analysis of physician medical insurance billings and patient emergency department visits.Cape Breton, NS.Patients, physicians, and support staff of 3 comparable family physician practices that had not implemented advanced access and an established advanced access practice.Self-reported provider and patient satisfaction, physician office income, and patients' emergency department use.The key benefits of implementation of advanced access were an increase in provider and patient satisfaction levels, same or greater physician office income, and fewer less urgent (triage level 4) and nonurgent (triage level 5) emergency department visits by patients.Currently within the Central Cape Breton Region, 33% of patients wait 4 or more days for urgent appointments. Findings from this study can be used to enhance primary care physician practice redesign. This research supports many benefits of transitioning to an advanced access model of patient booking.

Abstract

PURPOSE The aim of this trial was to compare the activity and toxicity of single-agent doxorubicin with that of two multidrug regimens in the treatment of patients with adult advanced soft tissue sarcomas. PATIENTS AND METHODS This was a prospective randomized phase III trial performed by 35 cancer centers within the Soft Tissue and Bone Sarcoma Group of the European Organization for Research and Treatment of Cancer (EORTC). Six hundred sixty-three eligible patients were randomly allocated to receive either doxorubicin 75 mg/m2 (arm A), cyclophosphamide, vincristine, doxorubicin, and dacarbazine (CYVADIC) (arm B), or ifosfamide 5 g/m2 plus doxorubicin 50 mg/m2 (arm C). RESULTS The overall response rate was 24% (95% confidence interval, 20.7% to 27.3%) among eligible patients and 26% among assessable patients. No statistically significant difference was detected among the three study arms in terms of response rate (arm A, 23.3%; arm B, 28.4%; and arm C, 28.1%), remission duration (median, 46 weeks on arm A, 48 weeks on arm B, and 44 weeks on arm C), or overall survival (median, 52 weeks on arm A, 51 weeks on arm B, and 55 weeks on arm C). The degree of myelosuppression was significantly greater for the combination of ifosfamide and doxorubicin than for the other two regimens. Cardiotoxicity was also more frequent in this arm, but other toxicities were similar. CONCLUSION In advanced soft tissue sarcomas of adults, single-agent doxorubicin is still the standard chemotherapy against which more intensive or new drug treatments should be compared. Combination chemotherapy cannot be recommended outside a controlled clinical trial with the exclusion of some subsets of sarcoma patients for whom significant tumor volume reduction may be an important end point of a chemotherapy regimen.

Thirty-nine tumor-bearing patients with metastatic melanoma were treated with 3 subcutaneous injections of the MAGE-3.A1 peptide at monthly intervals. No significant toxicity was observed. Of the 25 patients who received the complete treatment, 7 displayed significant tumor regressions. All but one of these regressions involved cutaneous metastases. Three regressions were complete and 2 of these led to a disease-free state, which persisted for more than 2 years after the beginning of treatment. No evidence for a cytolytic T lymphocyte (CTL) response was found in the blood of the 4 patients who were analyzed, including 2 who displayed complete tumor regression. Our results suggest that injection of the MAGE-3.A1 peptide induced tumor regression in a significant number of the patients, even though no massive CTL response was produced.

46. Glycoprotein hormone α-subunit secretion in patients with pituitary adenomas: influence of TRH, LRH and bromocriptine

Abstract

Twelve patients with pituitary adenomas and increased serum concentration of the glycoprotein hormone α-subunit were studied. Eight patients were acromegalic and one had a FSH1producing tumour. The adenomas in 9 patients had undergone subtotal operative removal and/or external irradiation but no patient was studied within 3 months of these treatments.Many of the acromegalic patients, with moderately elevated α-levels, showed marked increases in α-concentration after TRH and/or LRH, compared with controls. The non-acromegalic patients, with the highest α-levels, showed poor responses to releasing hormones. These results suggest that excessive α-subunit secretion in acromegalic patients is often under hypothalamic control whereas in non-acromegalic patients it is often autonomous.Seven patients, 4 with acromegaly, were then given oral bromocriptine, 5 mg over 3 h. There was a significant fall in log mean α-level at 4 and 5 h (P< 0.02). Six patients took bromocriptine for 1–2 months. Log mean α-concentration was significantly reduced at the end of treatment (P< 0.02) and then recovered to basal levels after stopping treatment for one week. α-Subunit hypersecretion in some patients with pituitary adenomas is therefore modulated by dopaminergic control mechanisms.
