Background Sleep can be defined as a state of reduced attention from where the person can be woken up by any kind of stimuli. Sleep difficulties are a major group of disorders affecting one third of the adult population. The present study was taken up to assess the sleep quality and prevalence of sleep disorders among the adult population in the urban slum area of H Siddaiah Road Urban Primary Health Center (UPHC), which is in the Urban Field Practice Area, BMCRI. Methods Stratified random sampling was used to select 821 adults in the population of 18-60 years of age. Ethical clearance was obtained from the Institutional Ethical Committee. A pretested semi-structured questionnaire was used to interview the adults after obtaining their consent. The data was entered in Microsoft Office Excel and analysed using SPSS ver20.0. Results The study population was 52.81% females and 77.5% in the age group of 18-30 years. Most of the study population were Hindus (78.90%), and only 3.8% of the study population were illiterate. Most of them were employed (86.12%). Substance use was present in 82.9% of the study participants and overcrowding was present in 51.3% of the subjects. Female gender, being unemployed, living with relatives, overcrowding, and substance use such as alcohol and smokeless tobacco were the factors associated with poor sleep quality as measured using the Pittsburgh Sleep Quality Index. Among the study participants having poor sleep quality, most of participants needed further clinical assessment for insomnia (86%) followed by assessment for sleep apnoea (50.5%). Conclusions 200 (24.36%) study participants were determined to have poor sleep quality. Gender, marital status and overcrowding were the factors associated with poor sleep quality. A significant number of study participants need further assessment on insomnia, sleep apnoea and psychiatric disorders.

Background: Adolescent girls are concerned about their weight, physical appearance, and how society perceives them. The identities they seek and create in this phase influence society and their peers. In this process, a lot of Emotional, Biological, and Social problems are faced by them. One such problem is Menstrual disorders. Keeping this rationale in mind, the above study was undertaken to Assess Body weight perception, Weight control practices, and their association with Menstruation among Adolescent girls. Objective: 1. To assess Body weight perception and Weight Control Practices, among Adolescent girls 2. To compare Body weight perception and Weight control practices among Adolescent girls with normal and abnormal Menstrual cycles Methodology: A Cross-sectional study was conducted among Adolescent girls residing in the rural field practice area of Bangalore Medical College and Research Institute, Bengaluru. A Sample size of 192 was determined based on a previous study done by Bhurtun DD et al, and Simple random sampling was used for participant recruitment. Data collection utilized a pre-tested structured questionnaire. Data analyzed using SPSS 21.0 Results: Among the 192 participants, based on Body mass index (BMI), 13(6.7%) were Overweight and the rest of the participants 135(70.3%) were of Normal weight. Out of 118(61.5%) respondents reported trying to lose weight, reducing fat intake 100(84.7%) was the most common method. The majority of the Adolescent girls had a positive perception of weight control but had problems accepting their body image as healthy. Body weight perception and Weight control practices affected normal menstrual cycles as well as girls with Menstrual disorders. Conclusion: The findings revealed Body weight perception affected the girls with Normal Menstruation as well as with Menstrual disorders. Therefore, Adolescent girls need to be educated on the need for a Healthy body image as well as a Healthy lifestyle.

Background: Diabetes is a chronic metabolic disease marked by high blood glucose levels. Type-2 diabetes has been far more common during the last three decades worldwide. Approximately 422 million individuals worldwide and 77 million in India suffer from diabetes; most of them reside in low- and middle-income nations, Quality of Life (QoL) is how good or bad a person feels their life to be. It measures physical, psychological, and social well-being; thus, the purpose of this study is to assess the QoL among type-2 diabetics residing in the urban field practice area of a Government medical college, Bengaluru. Objectives: To assess the quality of life and the factors affecting it in type 2 diabetics in the urban field practice area of a Government medical college, Bengaluru. Methodology: A community-based cross-sectional study was conducted among type 2 diabetics residing in the urban field practice area of a Government medical college, Bengaluru, from August 2023 to November 2023. With a total sample size of 173, Study participants were interviewed using the WHOQOL– BREF scale to elicit quality of life, and a semi-structured questionnaire was used to elicit the socio-demographic and behavioral profile. The data collected was entered in MS Excel and analyzed using SPSS 21.0. Results: For WHOQOL-BREF, the mean scores for all the domains were physical (51.5), psychological (45.5), social (36.0), and environmental (40.5) In addition, the age, duration of diabetes, associated co-morbidities, and regular medication intake in patients showed a highly significant correlation with QoL. Conclusion: The Quality of life of type 2 diabetics was poor to average. Hence, to improve the quality of life and to raise awareness among them, public health initiatives and patient education are crucial.

Background: WHO defines health literacy as cognitive and social skills that determine the motivation and ability of people to understand, gain access and use the information to obtain good health. Inadequate health literacy has been linked to increased hospital admissions, poorer physical and mental health, lower use of preventive health services like vaccinations and screening tests. These ultimately lead to higher healthcare costs. This study was conducted to assess health literacy among adults attending outpatient clinics at a government tertiary care hospital in urban Bangalore. Objectives: To estimate proportion of inadequate health literacy and its associated factors among adults attending outpatient clinics at a government tertiary care hospital in urban Bangalore. Methodology: A cross-sectional study was conducted among adult attendees (i.e., patients and companions) of outpatient clinics of a government tertiary care hospital in urban Bangalore. Based on a study done by Almaleh. R et al. (P = 34.3%) with 7% absolute precision, the sample size was calculated as 200. A total of 200 adults attending outpatient clinics at the government tertiary care hospital were selected using simple random sampling. Data was collected using a pre-tested, semi-structured questionnaire for sociodemographic factors and HLS-EU-Q16 (European Health Literacy Survey Questionnaire short version) for assessing health literacy. The data collected was entered in MS Excel and analysed using SPSS 21.0. Results: The proportion of inadequate health literacy among study participants was observed to be 38%. Factors like educational status, age, and gender were found to have an association with it. Health literacy was found to be least in lower-educated individuals. Conclusion: From the present study, it was found that a large part of adult population had limited health literacy. It should be addressed through proper interventions, like ensuring basic education and health awareness programmes.

Context/Background: Rapid urbanization has resulted in a growing number of people living in under-served areas with inadequate and overburdened infrastructure and services, worsening of environmental factors. Sustainable Development Goal 11: “sustainable cities and communities”, highlights, there is an ur-gent need to re-orient the community development strategies by adopting economically & technologically sustainable models. Aims/Objectives: An application-based household survey to assess the vulnerability and Health condi-tions among the urban poor of slums in Bengaluru. Methodology: The tool for Vulnerability Assessment by Government of India was modified to develop a user-friendly tool for Community Health Workers in an Android–based smartphone application (Namma Samudaya). Data was collected through house-to-house survey. Results: A total of 3165 households (HHs) were surveyed. Among these, 54.2% of the HHs belong to vul-nerable category and 45.8% to highly vulnerable group. Conclusions: Domain-wise analysis showed, Residential -25.2% of HHs were located besides polluted wa-ter Social - “No social support” and “disability” was reported by 3% of households each; Health-Related - Catastrophic Health expenditure in the past 1 year was reported by 40.4% HHs. The baseline information of slums & their geographic mapping obtained during the survey can be used as foundation for effective plan-ning of health care services.

Introduction: Non-communicable diseases (NCDs) are among the rising trend and are often caused by modifiable risk factors. Ado- lescents are frequent victims due to junk food consumption and un- healthy diet leading to overweight and obesity. A nutritionally ad- equate and balanced diet is essential for optimal growth and devel- opment. Objectives: A cross-sectional study to assess the knowledge, atti- tude and dietary practices among the adolescents of urban slums. Also, to find the association of Body Mass Index (BMI) with fre- quency of junk foods consumption. Materials and methods: 230 adolescents aged 13-15 years in Benga- luru Urban slums were assessed using multi-stage random sam- pling technique and data was collected. Bowl-method was em- ployed to quantify the measurements of foods consumed. Standard precautions were taken to measure height and weight. Chi-square test was used to find association between BMI and frequency of junk food consumption. Results: Mean age of the students was 14.02 (SD 0.96) and majority were in 8th standard (42.2%). 116 (50.4%) of them were boys. Mean BMI of the adolescents was 20.7kg/m2 (SD 3.6) where, 68 (29.6%) of them were underweight, 62 (27%) were overweight and 4 (1.7%) were obese. Only 39% had good knowledge, 71% had favourable attitude and good practice in 31% of adolescents. There was signif- icant association with frequency of junk food consumption and BMI variation. Conclusion: Adolescents had poor knowledge and practice. Over- weight was more prevalent among boys and Underweight in girls. There was significant association with frequency of junk food con- sumption and BMI variation.

Context: A high suicide rate is an index of social disorganization. In India, it is the second leading cause of death among 15-29 years age group. Young age, female sex, poor education, unemployment and socio economic deprivation are some of the potential risk factors. The study conducted to assess the risk factors and socio- demographic profile in patients with attempted suicide in a tertiary care centre. Methods and Material: A Cross Sectional Study was conducted among 476 suicide attempted patients by convenient sampling from January 2016 to May 2017. Data was collected by using a pre- tested, semi-structured questionnaire. Results: Mean age of study participants was 30.65+0.75 years. 57.78% of them were males. Hindus (82.98%) were predominant among the cases. Most of the suicide victims (82.97%) were from nuclear families.24.57% of study participants had family history of suicide. There was no significant association between socio- demographic factors like education, occupation and socio- economic class and the number of previous attempts. Conclusion: Suicides and attempted suicides are slowly but stead- ily assuming the levels of a public health problem caused by multi- ple factors. Hence there is a need to address the risk factors for sui- cide attempts and preventing them by taking proper measures at individual, family and societal level.

Background: Despite their public health benefit, vaccination pro- grams face obstacles. The objectives of the study were to determine the vaccination coverage among the children in urban slums in Bangalore and to determine the factors associated with partial immunization. Methodology: This cross sectional study was carried out among 210 children aged between 12-23 months in urban slums of Banga- lore from June 2015 to May 2016 using 30-cluster survey method. Information regarding immunization status, socio demographic and personal details was collected using pretested semi structured questionnaire after obtaining the consent. Data was entered in MS EXCEL and was analyzed, using percentages and chi square test. Results: Among 210 children, Male children constituted 53.8%. Full immunization coverage was 83.3% and Partial immunization coverage was 16.7%. The relation between socio demographic var- iables like religion, type of family, total number of children in fam- ily, birth order, place of delivery, father's and mother's educational status, socioeconomic status and immunization status was found to be statistically significant. Conclusion: Illiterate father, illiterate mother, low socio economic status, no exclusive breast feeding and non availability of immun- ization card were determinants of partial immunization.

Background: Infant nutrition is a key factor for child survival, optim- al growth and development and breastfeeding improves immunity to prevent childhood illnesses. We aimed to estimate exclusive breast- feeding rates and assess complementary feeding practices among 9 months old infants. Methods: A cross-sectional study was carried out among mothers of infants coming for measles vaccination at three Primary Health Cen- tres from January to December, 2013. Trained medical interns inter- viewed the mothers about breastfeeding and feeding practices using a structured pretested questionnaire. Results: Of the 4232 women, 954 children came for measles vaccina- tion. Majorities (94%) were fed with colostrum, and about 83% were initiated breastfeeding within an hour after delivery. Only about 34% were exclusively breastfed up to six months. A total of 602 infants (66%) were given herbal drops (janam ghutti), animal milk, formula feeds were introduced at various ages prior to 6 months and the main reason cited by the mothers was insufficient breast milk production. Conclusions: Though feeding practices at birth were good, preva- lence of exclusive breastfeeding was low. Measures should be under- taken to improve exclusive breastfeeding. Mothers attending vaccina- tion clinics should be counselled about child feeding practices.

ABSTRACT: Context: The annual incidence cases report depicts India as having the highest tuberculosis (TB) burden globally. Following a programmatic change, the daily fixed-dose combination (FDC) anti-TB treatment regimens were introduced by the Indian government's National Tuberculosis Elimination Program (NTEP). Aims: The aim of the study was to assess the treatment outcomes among drug-sensitive pulmonary TB patients receiving daily FDC drugs and the associated factors influencing the treatment outcomes. Settings and Design: A prospective study was conducted among 300 drug-sensitive pulmonary TB cases in the Bruhat Bengaluru Mahanagara Palike (BBMP) area. Materials and Methods: The TB units and designated microscopic centers (DMCs) were selected by multistage random sampling. Data were collected through a pre-tested and semi-structured questionnaire. Patients were followed up until treatment completion. Statistical Analysis Used: Data were compiled and analyzed using IBM Statistical Package for Social Sciences (SPSS) statistics version 20.0. Descriptive statistics and the Chi-square test were used for interpretation. A P -value less than 0.05 was considered statistically significant. Results: Around 86.33% of patients were cured, 4% had completed treatment, and 1% had treatment failure. Older age, human immunodeficiency virus (HIV) reactive status, alcohol intake, tobacco use, and migrants were associated with poor outcomes. Conclusions: The daily FDC regimen had better outcomes than intermittent regimens. Smokers, alcoholics, migrants, and patients with co-morbidity need to be given priority in management as they are prone to poorer outcomes. [ABSTRACT FROM AUTHOR]

Background: Children are a supreme asset to a nation. Beginning six years of life is pivotal for the social, emotional, cognitive, physical, motor, and psychological upbringing of the child. National Family Health Survey (NFHS) 5 data shows 41.9% of children die before the age of five years, with greater mortality in the rural areas (45.7%). The present study was conducted to assess the anthropometric profile and immunization status of children in Anganwadi centers., Materials and Methods: A community-based cross-sectional study was conducted in 21 Anganwadi centers in a rural area in South India. The anthropometry (height, weight, mid-upper arm circumference), morbidity profile, and immunization status of the enrolled and available children were assessed with the help of WHO growth charts, Salter scale, clinical examination, immunization cards, and data from Anganwadi registers., Statistical Analysis: Data were compiled and analyzed using SPSS software version 20. Descriptive data were interpreted as mean and percentages and associations were tested with the Chi-square test, and Fisher's exact test. A P value of <0.05 was considered to be statistically significant., Results: Around 400 children, aged 0-6 years, participated in the study. The mean age of the study population was 42.08 ± 12.55 months. The overall prevalence of underweight, stunting, and wasting was 52%, 54%, and 47.5% respectively. The most common morbidity was upper respiratory tract infection (11.5%). A majority (97.5%) of children were found to be immunized appropriately as per age., Conclusion: The prevalence of malnutrition was alarmingly high which warrants urgent referral and follow-up. Context: In the absence of any specific treatment available for COVID-19, people started practicing traditional nonpharmacological preventive home remedies such as salt water gargling and steam inhalation. The available research evidence on some of these measures opines that steam inhalation, saline gargling, and povidone-iodine gargling does have virucidal properties and do provide symptomatic relief., Aims: The aim is to test this hypothesis, and the present trial was undertaken with an objective to assess the effect of steam inhalation, saline gargling, and povidone-iodine gargling among the COVID-19-positive patients with respect to early test negativity and clinical recovery., Methodology: Open-labeled, parallel, randomized controlled trial was conducted among asymptomatic or mild COVID-19-positive patients in Bangalore from September 2020 to February 2021. In each group of steam inhalation, saline gargling, povidone-iodine gargling, and control, twenty participants were allocated. Daily follow-up was done for 21 days to assess early test negativity and clinical recovery. Trial Registry Number: Clinical Trial Registry India/2020/09/027687., Results: Among 80 participants recruited, 65 (81.3%) were symptomatic. Early test negativity was seen in povidone-iodine gargling group of 6 days (KaplanMeier survival curve, BreslowGeneralized Wilcoxon test P = 0.7 as per the intention-to-treat and as per-protocol P = 0.8). Setting: Five select districts of Karnataka, India, providing anti-tubercular and antiretroviral therapy (ATT and ART) to people with Human Immunodeficiency Virus (HIV) - associated Tuberculosis (TB) through a single window care approach at the ART centres (seven ART centres and 16 link ART centres)., Objectives: To determine the factors associated with non-adherence to concurrent therapy., Design: We conducted a case-control study involving primary and secondary data collection. Starting January 2019, we consecutively enrolled people on at least three months of ATT until we enrolled 125 cases (non-adherent to concurrent therapy) and 375 controls (adherent to concurrent therapy). Adherence was defined as taking >95% ART doses and >90% ATT doses, every month over the last three months. We performed multivariable logistic regression to identify factors associated with non-adherence., Results: The mean age of the cases and control was similar: 39.8 (standard deviation: 8.8) years. Objective: To estimate the burden of active infection and anti-SARS-CoV-2 IgG antibodies in Karnataka, India, and to assess variation across geographical regions and risk groups., Methods: A cross-sectional survey of 16,416 people covering three risk groups was conducted between 3-16 September 2020 using the state of Karnataka's infrastructure of 290 healthcare facilities across all 30 districts. Participants were further classified into risk subgroups and sampled using stratified sampling. All participants were subjected to simultaneous detection of SARS-CoV-2 IgG using a commercial ELISA kit, SARS-CoV-2 antigen using a rapid antigen detection test (RAT) and reverse transcription-polymerase chain reaction (RT-PCR) for RNA detection. Maximum-likelihood estimation was used for joint estimation of the adjusted IgG, active and total prevalence (either IgG or active or both), while multinomial regression identified predictors., Results: The overall adjusted total prevalence of COVID-19 in Karnataka was 27.7% (95% CI 26.1-29.3), IgG 16.8% (15.5-18.1) and active infection fraction 12.6% (11.5-13.8). The case-to-infection ratio was 1:40 and the infection fatality rate was 0.05%. Influenza-like symptoms or contact with a COVID-19-positive patient were good predictors of active infection. RAT kits had higher sensitivity (68%) in symptomatic people compared with 47% in asymptomatic people., Conclusion: This sentinel-based population survey was the first comprehensive survey in India to provide accurate estimates of the COVID-19 burden. The findings provide a reasonable approximation of the population immunity threshold levels. We used social network analysis (SNA) to study the novel coronavirus (COVID-19) outbreak in Karnataka, India, and to assess the potential of SNA as a tool for outbreak monitoring and control. We analysed contact tracing data of 1147 COVID-19 positive cases (mean age 34.91 years, 61.99% aged 11-40, 742 males), anonymised and made public by the Karnataka government. Software tools, Cytoscape and Gephi, were used to create SNA graphics and determine network attributes of nodes (cases) and edges (directed links from source to target patients). Outdegree was 1-47 for 199 (17.35%) nodes, and betweenness, 0.5-87 for 89 (7.76%) nodes. Men had higher mean outdegree and women, higher mean betweenness. Delhi was the exogenous source of 17.44% cases. Bangalore city had the highest caseload in the state (229, 20%), but comparatively low cluster formation. Thirty-four (2.96%) 'super-spreaders' (outdegree ⩾ 5) caused 60% of the transmissions. Real-time social network visualisation can allow healthcare administrators to flag evolving hotspots and pinpoint key actors in transmission. Prioritising these areas and individuals for rigorous containment could help minimise resource outlay and potentially achieve a significant reduction in COVID-19 transmission.

Setting: Tuberculosis (TB) and diabetes mellitus (DM) clinics at Bowring and Lady Curzon Hospital, a tertiary care centre in Bangalore, India., Obejctive: To assess the feasibility and results of TB-DM bidirectional screening., Methods: A descriptive study conducted from 1 March to 30 September 2012, in which all TB patients were assessed for DM and vice versa. Fasting blood glucose values of ≥126 mg/dl and 110-125 mg/dl were considered as DM and pre-diabetes, respectively., Results: Of 510 TB patients, 32 (6.3%) had been previously diagnosed with DM. Screening among the remaining 478 patients yielded 15 (2.9%) with pre-diabetes and 15 (2.9%) newly diagnosed cases of DM. A higher prevalence of DM was found among patients aged ≥40 years, patients with pulmonary TB and smokers. Of the 47 TB-DM patients, 45 were enrolled in DM care. Of 1670 DM patients followed up in DM clinics, 45 already had TB. Among the remaining 1625 patients screened, 152 (9%) had symptoms suggestive of TB; two of these were found to have the disease., Conclusion: Bidirectional screening for DM and TB is feasible and produces a high yield for DM among TB patients. The yield of TB among DM patients was low and needs future research using new, improved TB diagnostic tools.
