Abstract

Pediatric cardiothoracic surgical patients experience varying degrees of pain in the immediate postoperative period. Multiple analgesic modalities can be used to effectively manage pain in this patient population. Routinely, intravenous (IV) opioid is prescribed postoperatively to manage acute pain needs in this select group of patients. Rectal acetaminophen as an adjunct to IV opioid to improve comfort levels and decrease overall opioid use in the immediate postoperative period in this specific pediatric population has not been examined. This retrospective study was designed to compare comfort levels and overall opioid use in full-term infants with congenital heart disease within 24 hours or less post-open heart repair. A total of 74 infants out of 203 participants who met inclusion criteria were compared. Fiftythree infants received rectal acetaminophen in conjunction with IV opioid, and 21 infants received IV opioid only. Infants prescribed rectal acetaminophen in conjunction with IV opioid had no significant difference in comfort levels as compared to infants who received IV opioid only (p < 0.08), but had a statistically significant (p < 0.01) decrease in overall opioid exposure (0.11 mg/kg/day) as compared to infants prescribed IV opioid alone (0.28 mg/kg/day)., Optimizing pain management modalities for pediatric patients remains a priority for practitioners and researchers alike. Abstract

Introduction: The optimal treatment for small, asymptomatic, nonfunctioning pancreatic neuroendocrine neoplasms (NF-PanNEN) is still controversial. European Neuroendocrine Tumor Society (ENETS) guidelines recommend a watchful strategy for asymptomatic NF-PanNEN <2 cm of diameter. Several retrospective series demonstrated that a non-operative management is safe and feasible, but no prospective studies are available. Aim of the ASPEN study is to evaluate the optimal management of asymptomatic NF-PanNEN ≤2 cm comparing active surveillance and surgery. Methods: ASPEN is a prospective international observational multicentric cohort study supported by ENETS. The study is registered in ClinicalTrials.gov with the identification code NCT03084770. Based on the incidence of NF-PanNEN the number of expected patients to be enrolled in the ASPEN study is 1,000 during the study period (2017–2022). Primary endpoint is disease/progression-free survival, defined as the time from study enrolment to the first evidence of progression (active surveillance group) or recurrence of disease (surgery group) or death from disease. Inclusion criteria are: age >18 years, the presence of asymptomatic sporadic NF-PanNEN ≤2 cm proven by a positive fine-needle aspiration (FNA) or by the presence of a measurable nodule on high-quality imaging techniques that is positive at 68Gallium DOTATOC-PET scan. Conclusion: The ASPEN study is designed to investigate if an active surveillance of asymptomatic NF-PanNEN ≤2 cm is safe as compared to surgical approach.

Abstract

Event-related potentials (ERPs) are an ideal tool for measuring neural responses in a wide range of participants, including children diagnosed with neurodevelopmental disorders (NDDs). However, due to perceived barriers regarding participant compliance, much of this work has excluded children with low IQ and/or reduced adaptive functioning, significant anxiety symptoms, and/or sensory processing difficulties, including heterogeneous samples of children with autism spectrum disorder (ASD) and children with fragile X syndrome (FXS). We have developed a behavioral support protocol designed to obtain high-quality ERP data from children in a single session. Using this approach, ERP data were successfully collected from participants with ASD, FXS, and typical development (TD). Higher success rates were observed for children with ASD and TD than children with FXS. Unique clinical–behavioral characteristics were associated with successful data collection across these groups. Higher chronological age, nonverbal mental age, and receptive language skills were associated with a greater number of valid trials completed in children with ASD. In contrast, higher language ability, lower autism severity, increased anxiety, and increased sensory hyperresponsivity were associated with a greater number of valid trials completed in children with FXS. This work indicates that a “one-size-fits-all” approach cannot be taken to ERP research on children with NDDs, but that a single-session paradigm is feasible and is intended to promote increased representation of children with NDDs in neuroscience research through development of ERP methods that support inclusion of diverse and representative samples.

Abstract

Objective: To integrate speaking practice with rhetorical theory. Type of speech: Persuasive. Point value: 100 points (i.e., 30 points based on peer evaluations, 30 points based on individual performance, 40 points based on the group presentation), which is 25% of course grade. Requirements: (a) References: 7-10; (b) Length: 20-30 minutes; (c) Visual aid: Yes; (d) Outline: Yes; (e) Prerequisite reading: The appropriate chapter (e.g., generic, feminist, metaphoric, fantasy theme, pentadic criticism, neo-Aristotelian) in Foss (1996); (f) Additional requirements: Each group member should be given an equal amount of time to speak and an equal amount of work in preparing the final presentation. This assignment gives students a chance to engage in the study of popular culture as public address, giving them insight into their own worlds and the ways popular culture may help shape/instruct that world. This assignment also allows students to engage in critical analysis, which not only means that students can learn about rhetorical theory, but they also get to engage in the types of analysis used by their instructor during the course. Abstract

This essay employs a qualitative and quantitative audience study to explore the reception of racially controversial books, such as The Bell Curve, and their dominantly negative reviews in the popular press. Findings do not support the hypothesis that reviews increase racism by recirculating racist views under cover of disapprobrium. Instead, responses suggest that audience members can incorporate components of both genetic and environmental accounts to their prior views. Thus, relative acceptance of genetic vs. environmental accounts of human difference did not correlate with racist attitudes; rather those whites who had strong negative affect toward persons of other races appropriated both genetic and environmental accounts to bolster their racism, while both blacks and whites with more egalitarian attitudes were able to incorporate genetic accounts into their schemas. We conclude that the agenda setting effects of the book and its reviews may have encouraged a less than ideal stasis point in the debate, focusing the debate on nature vs. nurture as causative agents for individuals rather than focusing on issues of individual vs. social responsibility. We suggest further exploration of the functioning of popular book reviews and of audience interpretations of popular reception of mass mediated arguments about race.

Abstract

The rhetoric of the First Wave of American feminism was consistently responsive to its cultural contexts and evolved into many different forms as contexts changed and the movement matured. This analysis examines the rhetorical invention strategies of suffrage rhetoric as it responded to the cultural context of World War I. Specifically, I focus on the political cartoons that were published in the mainstream Suffrage Movement's chief organ at this time, The Woman Citizen. Through a construction of woman as strong, competent, and essential to the war effort, suffragists exploited the persuasive resources of this specific cultural context to construct a new definition of 'loyal citizen' and offer up new identification possibilities for women.

Abstract

This piece makes a claim for the transformative power of hermeneutics by seeing in the tradition places of undischarged Utopian potential, which are often found in a close hermeneutic engagement with poetry. All such readings, it is argued, ought to meet the four watchwords of any contemporary interpretation viz., that such readings be non-orthodox, non-nostalgic, non-rejectionist, and non-apocalyptic. Such a reading is attempted of Langston Hughes’s poem “Let America Be America Again” against the backdrop of an interpretation of insights from Gianni Vattimo. All this is meant to be evidence as well of the dire necessity of the Humanities.
