Abstract: A search for proton decay into $e^+/\mu^+$ and a $\eta$ meson has been performed using data from a 0.373 Mton$\cdot$year exposure (6050.3 live days) of Super-Kamiokande. Compared to previous searches this work introduces an improved model of the intranuclear $\eta$ interaction cross section, resulting in a factor of two reduction in uncertainties from this source and $\sim$10\% increase in signal efficiency. No significant data excess was found above the expected number of atmospheric neutrino background events resulting in no indication of proton decay into either mode. Lower limits on the proton partial lifetime of $1.4\times\mathrm{10^{34}~years}$ for $p\rightarrow e^+\eta$ and $7.3\times\mathrm{10^{33}~years}$ for $p\rightarrow \mu^+\eta$ at the 90$\%$ C.L. were set. These limits are around 1.5 times longer than our previous study and are the most stringent to date.

Abstract: Quasi-periodic Eruptions (QPEs) are luminous bursts of soft X-rays from the nuclei of galaxies, repeating on timescales of hours to weeks. The mechanism behind these rare systems is uncertain, but most theories involve accretion disks around supermassive black holes (SMBHs), undergoing instabilities or interacting with a stellar object in a close orbit. It has been suggested that this disk could be created when the SMBH disrupts a passing star, implying that many QPEs should be preceded by observable tidal disruption events (TDEs). Two known QPE sources show long-term decays in quiescent luminosity consistent with TDEs, and two observed TDEs have exhibited X-ray flares consistent with individual eruptions. TDEs and QPEs also occur preferentially in similar galaxies. However, no confirmed repeating QPEs have been associated with a spectroscopically confirmed TDE or an optical TDE observed at peak brightness. Here we report the detection of nine X-ray QPEs with a mean recurrence time of approximately 48 hours from AT2019qiz, a nearby and extensively studied optically-selected TDE. We detect and model the X-ray, ultraviolet and optical emission from the accretion disk, and show that an orbiting body colliding with this disk provides a plausible explanation for the QPEs.

Abstract: We present multi-wavelength data of SN2020acct, a double-peaked stripped-envelope supernova (SN) in NGC2981 at ~150 Mpc. The two peaks are temporally distinct, with maxima separated by 58 rest-frame days, and a factor of 20 reduction in flux between. The first is luminous (M$_{r}$ = -18.00 $\pm$ 0.02 mag), blue (g - r = 0.27 $\pm$ 0.03 mag), and displays spectroscopic signatures of interaction with hydrogen-free circumstellar material. The second peak is fainter (M$_{r}$ = -17.29 $\pm$ 0.03 mag), and spectroscopically similar to an evolved stripped-envelope SNe, with strong blended forbidden [Ca II] and [O II] features. No other known double-peak SN exhibits a light curve similar to that of SN 2020acct. We find the likelihood of two individual SNe occurring in the same star-forming region within that time to be highly improbable, while an implausibly fine-tuned configuration would be required to produce two SNe from a single binary system. We find that the peculiar properties of SN2020acct match models of pulsational pair instability (PPI), in which the initial peak is produced by collisions of shells of ejected material, shortly followed by a terminal explosion. Pulsations from a star with a 72 M$_{\odot}$ helium core provide an excellent match to the double-peaked light curve. The local galactic environment has a metallicity of 0.4 Z$_{\odot}$, a level where massive single stars are not expected retain enough mass to encounter the PPI. However, late binary mergers or a low-metallicity pocket may allow the required core mass. We measure the rate of SN 2020acct-like events to be $<3.3\times10^{-8}$ Mpc$^{-3}$ yr$^{-1}$ at z = 0.07, or <0.1% of the total core-collapse SN rate., Comment: 31 pages, 14 figures. Abstract: The Super-Kamiokande and T2K collaborations present a joint measurement of neutrino oscillation parameters from their atmospheric and beam neutrino data. It uses a common interaction model for events overlapping in neutrino energy and correlated detector systematic uncertainties between the two datasets, which are found to be compatible. Using 3244.4 days of atmospheric data and a beam exposure of $19.7(16.3) \times 10^{20}$ protons on target in (anti)neutrino mode, the analysis finds a 1.9$\sigma$ exclusion of CP-conservation (defined as $J_{CP}=0$) and a preference for the normal mass ordering., Comment: 12 pages, 4 figures

Abstract: Preceding a core-collapse supernova, various processes produce an increasing amount of neutrinos of all flavors characterized by mounting energies from the interior of massive stars. Among them, the electron antineutrinos are potentially detectable by terrestrial neutrino experiments such as KamLAND and Super-Kamiokande via inverse beta decay interactions. Once these pre-supernova neutrinos are observed, an early warning of the upcoming core-collapse supernova can be provided. In light of this, KamLAND and Super-Kamiokande, both located in the Kamioka mine in Japan, have been monitoring pre-supernova neutrinos since 2015 and 2021, respectively. Recently, we performed a joint study between KamLAND and Super-Kamiokande on pre-supernova neutrino detection. A pre-supernova alert system combining the KamLAND detector and the Super-Kamiokande detector was developed and put into operation, which can provide a supernova alert to the astrophysics community. Fully leveraging the complementary properties of these two detectors, the combined alert is expected to resolve a pre-supernova neutrino signal from a 15 M$_{\odot}$ star within 510 pc of the Earth, at a significance level corresponding to a false alarm rate of no more than 1 per century. Abstract: Neutrinos from very nearby supernovae, such as Betelgeuse, are expected to generate more than ten million events over 10\,s in Super-Kamokande (SK). At such large event rates, the buffers of the SK analog-to-digital conversion board (QBEE) will overflow, causing random loss of data that is critical for understanding the dynamics of the supernova explosion mechanism. In order to solve this problem, two new DAQ modules were developed to aid in the observation of very nearby supernovae. The first of these, the SN module, is designed to save only the number of hit PMTs during a supernova burst and the second, the Veto module, prescales the high rate neutrino events to prevent the QBEE from overflowing based on information from the SN module. In the event of a very nearby supernova, these modules allow SK to reconstruct the time evolution of the neutrino event rate from beginning to end using both QBEE and SN module data. This paper presents the development and testing of these modules together with an analysis of supernova-like data generated with a flashing laser diode. We demonstrate that the Veto module successfully prevents DAQ overflows for Betelgeuse-like supernovae as well as the long-term stability of the new modules. During normal running the Veto module is found to issue DAQ vetos a few times per month resulting in a total dead time less than 1\,ms, and does not influence ordinary operations. Additionally, using simulation data we find that supernovae closer than 800~pc will trigger Veto module resulting in a prescaling of the observed neutrino data., Comment: 28 pages, 18 figures. Abstract: We present the results of the charge ratio ($R$) and polarization ($P^{\mu}_{0}$) measurements using the decay electron events collected from 2008 September to 2022 June by the Super-Kamiokande detector. Because of its underground location and long operation, we performed high precision measurements by accumulating cosmic-ray muons. We measured the muon charge ratio to be $R=1.32 \pm 0.02$ $(\mathrm{stat.}{+}\mathrm{syst.})$ at $E_{\mu}\cos \theta_{\mathrm{Zenith}}=0.7^{+0.3}_{-0.2}$ $\mathrm{TeV}$, where $E_{\mu}$ is the muon energy and $\theta_{\mathrm{Zenith}}$ is the zenith angle of incoming cosmic-ray muons. This result is consistent with the Honda flux model while this suggests a tension with the $\pi K$ model of $1.9\sigma$. We also measured the muon polarization at the production location to be $P^{\mu}_{0}=0.52 \pm 0.02$ $(\mathrm{stat.}{+}\mathrm{syst.})$ at the muon momentum of $0.9^{+0.6}_{-0.1}$ $\mathrm{TeV}/c$ at the surface of the mountain; this also suggests a tension with the Honda flux model of $1.5\sigma$. This is the most precise measurement ever to experimentally determine the cosmic-ray muon polarization near $1~\mathrm{TeV}/c$. Abstract: The first loading of gadolinium (Gd) into Super-Kamiokande in 2020 was successful, and the neutron capture efficiency on Gd reached 50\%. To further increase the Gd neutron capture efficiency to 75\%, 26.1 tons of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ was additionally loaded into Super-Kamiokande (SK) from May 31 to July 4, 2022. As the amount of loaded $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$ was doubled compared to the first loading, the capacity of the powder dissolving system was doubled. We also developed new batches of gadolinium sulfate with even further reduced radioactive impurities. In addition, a more efficient screening method was devised and implemented to evaluate these new batches of $\rm Gd_2(\rm SO_4)_3\cdot \rm 8H_2O$. Following the second loading, the Gd concentration in SK was measured to be $333.5\pm2.5$ ppm via an Atomic Absorption Spectrometer (AAS). From the mean neutron capture time constant of neutrons from an Am/Be calibration source, the Gd concentration was independently measured to be 332.7 $\pm$ 6.8(sys.) $\pm$ 1.1(stat.) ppm, consistent with the AAS result. Abstract: Among multi-messenger observations of the next galactic core-collapse supernova, Super-Kamiokande (SK) plays a critical role in detecting the emitted supernova neutrinos, determining the direction to the supernova (SN), and notifying the astronomical community of these observations in advance of the optical signal. On 2022, SK has increased the gadolinium dissolved in its water target (SK-Gd) and has achieved a Gd concentration of 0.033%, resulting in enhanced neutron detection capability, which in turn enables more accurate determination of the supernova direction. Accordingly, SK-Gd's real-time supernova monitoring system (Abe te al. 2016b) has been upgraded. SK_SN Notice, a warning system that works together with this monitoring system, was released on December 13, 2021, and is available through GCN Notices (Barthelmy et al. 2000). When the monitoring system detects an SN-like burst of events, SK_SN Notice will automatically distribute an alarm with the reconstructed direction to the supernova candidate within a few minutes. In this paper, we present a systematic study of SK-Gd's response to a simulated galactic SN. Assuming a supernova situated at 10 kpc, neutrino fluxes from six supernova models are used to characterize SK-Gd's pointing accuracy using the same tools as the online monitoring system. The pointing accuracy is found to vary from 3-7$^\circ$ depending on the models. However, if the supernova is closer than 10 kpc, SK_SN Notice can issue an alarm with three-degree accuracy, which will benefit follow-up observations by optical telescopes with large fields of view., Comment: 38 pages, 29 figures, 6 tables

Abstract: We present an extensive analysis of the optical and UV properties of AT2023clx, the closest TDE to date, that occurred in the nucleus of the interacting LINER galaxy, NGC3799 (z=0.01107). After correcting for the host reddening (E(B-V) = 0.179 mag), we find its peak absolute g-band magnitude to be -18.03{+/-}0.07 mag, and its peak bolometric luminosity to be L=(1.57{+/-}0.19)x10^43 erg/s. AT2023clx displays several distinctive features: first, it rose to peak within 10.4{+/-}2.5 days, making it the fastest rising TDE to date. Our SMBH mass estimate of M_BH ~ 10^6 Msol rules out the possibility of an intermediate-mass BH as the reason for the fast rise. Dense spectral follow-up reveals a blue continuum that cools slowly and broad Balmer and HeII lines as well as weak HeI 5876,6678 emission features that are typically seen in TDEs. The early, broad (width ~ 15000 km/s) profile of Ha matches theoretical expectations from an optically thick outflow. A flat Balmer decrement (~ 1.58) suggests that the lines are collisionally excited rather than being produced via photoionisation, in contrast to typical active galactic nuclei. A second distinctive feature, seen for the first time in TDE spectra, is a sharp, narrow emission peak at a rest wavelength of ~ 6353 A. This feature is clearly visible up to 10d post-peak; we attribute it to clumpy material preceding the bulk outflow, which manifests as a high-velocity component of Ha (-9584 km/s). Its third distinctive feature is the rapid cooling during the first ~ 20 days after peak, reflected as a break in the temperature evolution. Combining these findings, we propose a scenario for AT2023clx involving the disruption of a very low-mass star (<=0.1 Msol) with an outflow launched in our line of sight and with disruption properties that led to efficient circularisation and prompt accretion disc formation, observed through a low-density photosphere., Comment: Accepted for publication in Astronomy and Astrophysics (in production; official acceptance date: 28/06/2024)

Abstract: An analysis of solar neutrino data from the fourth phase of Super-Kamiokande~(SK-IV) from October 2008 to May 2018 is performed and the results are presented. The observation time of the data set of SK-IV corresponds to $2970$~days and the total live time for all four phases is $5805$~days. For more precise solar neutrino measurements, several improvements are applied in this analysis: lowering the data acquisition threshold in May 2015, further reduction of the spallation background using neutron clustering events, precise energy reconstruction considering the time variation of the PMT gain. The observed number of solar neutrino events in $3.49$--$19.49$ MeV electron kinetic energy region during SK-IV is $65,443^{+390}_{-388}\,(\mathrm{stat.})\pm 925\,(\mathrm{syst.})$ events. Corresponding $\mathrm{^{8}B}$ solar neutrino flux is $(2.314 \pm 0.014\, \rm{(stat.)} \pm 0.040 \, \rm{(syst.)}) \times 10^{6}~\mathrm{cm^{-2}\,s^{-1}}$, assuming a pure electron-neutrino flavor component without neutrino oscillations. The flux combined with all SK phases up to SK-IV is $(2.336 \pm 0.011\, \rm{(stat.)} \pm 0.043 \, \rm{(syst.)}) \times 10^{6}~\mathrm{cm^{-2}\,s^{-1}}$. Based on the neutrino oscillation analysis from all solar experiments, including the SK $5805$~days data set, the best-fit neutrino oscillation parameters are $\rm{sin^{2} \theta_{12,\,solar}} = 0.306 \pm 0.013 $ and $\Delta m^{2}_{21,\,\mathrm{solar}} = (6.10^{+ 0.95}_{-0.81}) \times 10^{-5}~\rm{eV}^{2}$, with a deviation of about 1.5$\sigma$ from the $\Delta m^{2}_{21}$ parameter obtained by KamLAND. The best-fit neutrino oscillation parameters obtained from all solar experiments and KamLAND are $\sin^{2} \theta_{12,\,\mathrm{global}} = 0.307 \pm 0.012 $ and $\Delta m^{2}_{21,\,\mathrm{global}} = (7.50^{+ 0.19}_{-0.18}) \times 10^{-5}~\rm{eV}^{2}$., Comment: 47 pages, 61 figures

Abstract: We present a measurement of neutrino oscillation parameters with the Super-Kamiokande detector using atmospheric neutrinos from the complete pure-water SK I-V (April 1996-July 2020) data set, including events from an expanded fiducial volume. The data set corresponds to 6511.3 live days and an exposure of 484.2 kiloton-years. Measurements of the neutrino oscillation parameters $\Delta m^2_{32}$, $\sin^2\theta_{23}$, $\sin^2 \theta_{13}$, $\delta_{CP}$, and the preference for the neutrino mass ordering are presented with atmospheric neutrino data alone, and with constraints on $\sin^2 \theta_{13}$ from reactor neutrino experiments. Abstract: We report the first measurement of the atmospheric neutrino-oxygen neutral-current quasielastic (NCQE) cross section in the gadolinium-loaded Super-Kamiokande (SK) water Cherenkov detector. In June 2020, SK began a new experimental phase, named SK-Gd, by loading 0.011% by mass of gadolinium into the ultrapure water of the SK detector. The introduction of gadolinium to ultrapure water has the effect of improving the neutron-tagging efficiency. Using a 552.2 day data set from August 2020 to June 2022, we measure the NCQE cross section to be 0.74 $\pm$ 0.22(stat.) $^{+0.85}_{-0.15}$ (syst.) $\times$ 10$^{-38}$ cm$^{2}$/oxygen in the energy range from 160 MeV to 10 GeV, which is consistent with the atmospheric neutrino-flux-averaged theoretical NCQE cross section and the measurement in the SK pure-water phase within the uncertainties. Furthermore, we compare the models of the nucleon-nucleus interactions in water and find that the Binary Cascade model and the Liege Intranuclear Cascade model provide a somewhat better fit to the observed data than the Bertini Cascade model. Since the atmospheric neutrino-oxygen NCQE reactions are one of the main backgrounds in the search for diffuse supernova neutrino background (DSNB), these new results will contribute to future studies - and the potential discovery - of the DSNB in SK., Comment: 8 pages, 3 figures

Abstract: We report a search for time variations of the solar $^8$B neutrino flux using 5804 live days of Super-Kamiokande data collected between May 31, 1996, and May 30, 2018. Super-Kamiokande measured the precise time of each solar neutrino interaction over 22 calendar years to search for solar neutrino flux modulations with unprecedented precision. Periodic modulations are searched for in a dataset comprising five-day interval solar neutrino flux measurements with a maximum likelihood method. We also applied the Lomb-Scargle method to this dataset to compare it with previous reports. The only significant modulation found is due to the elliptic orbit of the Earth around the Sun. Abstract: We present multi-wavelength photometry and spectroscopy of SN 2022jli, an unprecedented Type Ic supernova discovered in the galaxy NGC 157 at a distance of $\approx$ 23 Mpc. The multi-band light curves reveal many remarkable characteristics. Peaking at a magnitude of $g=15.11\pm0.02$, the high-cadence photometry reveals 12.5$\pm0.2\ $day periodic undulations superimposed on the 200 day supernova decline. This periodicity is observed in the light curves from nine separate filter and instrument configurations with peak-to-peak amplitudes of $\simeq$ 0.1 mag. This is the first time that repeated periodic oscillations, over many cycles, have been detected in a supernova light curve. SN 2022jli also displays an extreme early excess which fades over $\approx$ 25 days followed by a rise to a peak luminosity of $L_{\rm opt} = 10^{42.1}$ erg s$^{-1}$. Although the exact explosion epoch is not constrained by data, the time from explosion to maximum light is $\gtrsim$ 59 days. The luminosity can be explained by a large ejecta mass ($M_{\rm ej}\approx12\pm6$M$_{\odot}$) powered by $^{56}$Ni but we find difficulty in quantitatively modelling the early excess with circumstellar interaction and cooling. Collision between the supernova ejecta and a binary companion is a possible source of this emission. We discuss the origin of the periodic variability in the light curve, including interaction of the SN ejecta with nested shells of circumstellar matter and neutron stars colliding with binary companions., Comment: Accepted in ApJL

Abstract: Abstract Background The health issues experienced by older people can often be severe and complex, and an increasing number are using residential aged care services to meet their care needs. High-quality nursing care is fundamental to the health and safety of aged care residents and is contingent on nurses’ accurate assessment, informed decision-making, and delivery of timely interventions. However, the role of the aged care nurse is often challenging, impeded by factors such as understaffing, high workloads, and a lack of access to clinical infrastructure and resources. When these challenges mount, residents are put at greater risk of adverse outcomes, such as avoidable clinical deterioration and hospital transfers. This study describes the adaptation and implementation of the emergency nursing framework, HIRAID® (History including Infection risk, Red Flags, Assessment, Interventions, Diagnostics, reassessment, communication and plan)—a tool to assist nurses’ assessment, decision-making and care in residential aged care. Methods The HIRAID® framework will be adapted for residential aged care using a real-time Delphi and panel of aged care and nursing experts. The co-designed HIRAID® Aged Care framework will be trialled in 23 residential aged care homes in Sydney, Australia, in a modified stepped-wedge cluster randomised controlled trial design. All homes will be randomised into one of four clusters. Outcomes of interest include the rate of clinical deterioration events resulting from nurses’ actions, the rate of hospital transfers determined to be inappropriate, performance against the national mandatory aged care quality indicators, resident satisfaction with care, nurse and medical staff satisfaction with communication, and the quality of nursing documentation. These outcomes will be evaluated using a combination of qualitative and quantitative analysis of routinely collected resident data, expert assessments of facility documentation events against validated criteria, and pre- and post-intervention surveys of residents, family carers, nurses, and medical staff. Discussion This protocol describes a pragmatic trial that aims to translate an evidence-based framework from the emergency care context into residential aged care. The adapted HIRAID® Aged Care framework will be the first of its kind to standardise and guide holistic nursing assessment, decision-making, and documentation in residential aged care in Australia. Ethics and dissemination This research has been approved by the Western Sydney Local Health District Human Research Ethics Committee: 2023/ETH00523. A waiver of consent has been approved to access resident health data and nursing documentation at each participating site. 19. Loss of mitochondrial Ca2+ response and CaMKII/ERK activation by LRRK2R1441G mutation correlate with impaired depolarization-induced mitophagy

Abstract: Abstract Background Stress-induced activation of ERK/Drp1 serves as a checkpoint in the segregation of damaged mitochondria for autophagic clearance (mitophagy). Elevated cytosolic calcium (Ca2+) activates ERK, which is pivotal to mitophagy initiation. This process is altered in Parkinson’s disease (PD) with mutations in leucine-rich repeat kinase 2 (LRRK2), potentially contributing to mitochondrial dysfunction. Pathogenic LRRK2 mutation is linked to dysregulated cellular Ca2+ signaling but the mechanism involved remains unclear. Methods Mitochondrial damages lead to membrane depolarization. To investigate how LRRK2 mutation impairs cellular response to mitochondrial damages, mitochondrial depolarization was induced by artificial uncoupler (FCCP) in wild-type (WT) and LRRK2R1441G mutant knockin (KI) mouse embryonic fibroblasts (MEFs). The resultant cytosolic Ca2+ flux was assessed using live-cell Ca2+ imaging. The role of mitochondria in FCCP-induced cytosolic Ca2+ surge was confirmed by co-treatment with the mitochondrial sodium-calcium exchanger (NCLX) inhibitor. Cellular mitochondrial quality and function were evaluated by Seahorse™ real-time cell metabolic analysis, flow cytometry, and confocal imaging. Mitochondrial morphology was visualized using transmission electron microscopy (TEM). Activation (phosphorylation) of stress response pathways were assessed by immunoblotting. Results Acute mitochondrial depolarization induced by FCCP resulted in an immediate cytosolic Ca2+ surge in WT MEFs, mediated predominantly via mitochondrial NCLX. However, such cytosolic Ca2+ response was abolished in LRRK2 KI MEFs. This loss of response in KI was associated with impaired activation of Ca2+/calmodulin-dependent kinase II (CaMKII) and MEK, the two upstream kinases of ERK. Treatment of LRRK2 inhibitor did not rescue this phenotype indicating that it was not caused by mutant LRRK2 kinase hyperactivity. KI MEFs exhibited swollen mitochondria with distorted cristae, depolarized mitochondrial membrane potential, and reduced mitochondrial Ca2+ store and mitochondrial calcium uniporter (MCU) expression. These mutant cells also exhibited lower cellular ATP: ADP ratio albeit higher basal respiration than WT, indicating compensation for mitochondrial dysfunction. These defects may hinder cellular stress response and signals to Drp1-mediated mitophagy, as evident by impaired mitochondrial clearance in the mutant. Conclusions Pathogenic LRRK2R1441G mutation abolished mitochondrial depolarization-induced Ca2+ response and impaired the basal mitochondrial clearance. Inherent defects from LRRK2 mutation have weakened the cellular ability to scavenge damaged mitochondria, which may further aggravate mitochondrial dysfunction and neurodegeneration in PD.

Abstract: Abstract Background Both pregnant women and those with multiple long-term conditions are under-served groups in clinical research. Informing and improving research through patient and public involvement, including pregnant women with two or more long-term health conditions, is critical to increasing their inclusion in maternity research. Giant PANDA is a randomised controlled trial, evaluating the effect of a treatment initiation strategy with nifedipine versus labetalol on severe maternal hypertension and a composite outcome of fetal/neonatal death, or neonatal unit admission. We aimed to undertake a mixed methods study-within-a-project within the Giant PANDA trial to understand barriers and facilitators to participation, understand and optimise current representativeness of clinical trial delivery of those with multiple long-term conditions and co-create a checklist to support their inclusion in pregnancy research. Methods We undertook online workshops with women with lived experience and hybrid workshops with healthcare professionals who look after women with multiple long-term conditions. A site audit of Giant PANDA sites provided insights into research delivery capacity and health system set-up, and how this influences inclusion. An extension to the Giant PANDA screening log captured data on multiple long-term conditions enabling analysis of the impact of these health conditions on women’s inclusion in the trial. We co-created a checklist of recommendations for those designing and recruiting to similar clinical trials. Results Five key recommendations were identified including a need to (1) involve women with multiple long-term conditions as partners in maternity research and (2) minimise barriers that stop them from taking part through (3) designing and delivering research that is flexible in time and place (4) consider research as part of care for everyone, including those with multiple long-term conditions and (5) measure and report inclusion of those with two or more health conditions in maternity research. Multiple long-term conditions were not a barrier to recruitment or randomisation in the Giant PANDA trial. Conclusion Women with multiple long-term conditions would like opportunities to find out about and participate in research which accounts for their needs. Our checklist aims to support those designing and delivering maternity research to optimise inclusion of individuals with multiple-long term conditions. Abstract: We present the discovery and extensive follow-up of a remarkable fast-evolving optical transient, AT2022aedm, detected by the Asteroid Terrestrial impact Last Alert Survey (ATLAS). AT2022aedm exhibited a rise time of $9\pm1$ days in the ATLAS $o$-band, reaching a luminous peak with $M_g\approx-22$ mag. It faded by 2 magnitudes in $g$-band during the next 15 days. These timescales are consistent with other rapidly evolving transients, though the luminosity is extreme. Most surprisingly, the host galaxy is a massive elliptical with negligible current star formation. X-ray and radio observations rule out a relativistic AT2018cow-like explosion. A spectrum in the first few days after explosion showed short-lived He II emission resembling young core-collapse supernovae, but obvious broad supernova features never developed; later spectra showed only a fast-cooling continuum and narrow, blue-shifted absorption lines, possibly arising in a wind with $v\approx2700$ km s$^{-1}$. We identify two further transients in the literature (Dougie in particular, as well as AT2020bot) that share similarities in their luminosities, timescales, colour evolution and largely featureless spectra, and propose that these may constitute a new class of transients: luminous fast-coolers (LFCs). All three events occurred in passive galaxies at offsets of $\sim4-10$ kpc from the nucleus, posing a challenge for progenitor models involving massive stars or massive black holes. The light curves and spectra appear to be consistent with shock breakout emission, though usually this mechanism is associated with core-collapse supernovae. Abstract: Abstract Background Australian Rural Resident Medical Officer Cadetships are awarded to medical students interested in a rural medical career. The Rural Residential Medical Officer Cadetship Program (Cadetship Program) is administered by the Rural Doctors Network on behalf of the NSW Ministry of Health. This study aimed to assess the overall experience of medical students and key factors that contributed to their satisfaction with the Cadetship Program. Methods A quantitative cross-sectional study was conducted among 107 former cadets who had completed the Cadetship Program. Data on medical students’ experience with the Cadetship Program (outcome variable) and potential explanatory variables were collected using a structured self-administered questionnaire. Explanatory variables included gender, geographical location, rural health club membership, rural clinical school attendance, financial support, mentorship benefits, networking opportunities, influence on career decisions, opportunity for preferential placements, and relocation. Both bivariate (Pearson’s chi-squared test) and multiple logistic regression analysis were employed to identify the factors associated with medical students’ overall experience with the Cadetship Program. The non-linear analysis was weighted to represent the rural/remote health workforce, in Stata/SE 14.1. Results Our results indicate that 91% of medical students were satisfied with the Cadetship Program. The logistic regression model identified two significant predictors of a positive experience with the Cadetship Program. Medical students who perceived financial support as beneficial were significantly more likely to report a satisfactory program experience (aOR = 6.22, 95% CI: 1.36–28.44, p = 0.019) than those who perceived financial support as not beneficial. Similarly, those who valued networking opportunities were more likely to have a positive view of their cadetship experience (aOR = 10.06, 95% CI: 1.11–91.06, p = 0.040) than their counterparts. Conclusion Our study found that students who valued financial support and networking opportunities had the most positive views of the Cadetship Program. These findings demonstrate that the Cadetship Program may be most helpful for those who need financial support and for students who seek networking opportunities. These findings increase our knowledge about the characteristics of medical students who have the most positive experiences with the Cadetship Program. They help us to understand the mechanisms of influence of such programs on individuals’ decisions to be part of the future rural health workforce.

Abstract: Abstract Trastuzumab deruxtecan (T‐DXd; DS‐8201; ENHERTU®) is a human epithelial growth factor receptor 2 (HER2)‐directed antibody drug conjugate (ADC) with demonstrated antitumor activity against a range of tumor types. Aiming to understand the relationship between antigen expression and downstream efficacy outcomes, T‐DXd was administered in tumor‐bearing mice carrying NCI‐N87, Capan‐1, JIMT‐1, and MDA‐MB‐468 xenografts, characterized by varying HER2 levels. Plasma pharmacokinetics (PK) of total antibody, T‐DXd, and released DXd and tumor concentrations of released DXd were evaluated, in addition to monitoring γΗ2AX and pRAD50 pharmacodynamic (PD) response. A positive relationship was observed between released DXd concentrations in tumor and HER2 expression, with NCI‐N87 xenografts characterized by the highest exposures compared to the remaining cell lines. γΗ2AX and pRAD50 demonstrated a sustained increase over several days occurring with a time delay relative to tumoral‐released DXd concentrations. In vitro investigations of cell‐based DXd disposition facilitated the characterization of DXd kinetics across tumor cells. These outputs were incorporated into a mechanistic mathematical model, utilized to describe PK/PD trends. The model captured plasma PK across dosing arms as well as tumor PK in NCI‐N87, Capan‐1, and MDA‐MB‐468 models; tumor concentrations in JIMT‐1 xenografts required additional parameter adjustments reflective of complex receptor dynamics. γΗ2AX longitudinal trends were well characterized via a unified PD model implemented across xenografts demonstrating the robustness of measured PD trends. This work supports the application of a mechanistic model as a quantitative tool, reliably projecting tumor payload concentrations upon T‐DXd administration, as the first step towards preclinical‐to‐clinical translation.

Abstract: Purpose: During the course of demyelinating inflammatory diseases, myelin-derived proteins, including myelin basic protein(MBP), are secreted into extracellular space. MBP shows extensive post-translational modifications, including deimination/citrullination. Deiminated MBP is structurally less ordered, susceptible to proteolytic attack, and more immunogenic than unmodified MBP. This study investigated the effect of the deiminated/citrullinated isomer of MBP(C8) and the unmodified isomer of MBP(C1) on cultured primary astrocytes. Methods: MBP charge isomers were isolated/purified from bovine brain. Primary astrocyte cultures were prepared from the 2-day-old Wistar rats. For evaluation of glutamate release/uptake a Fluorimetric glutamate assay was used. Expression of peroxisome proliferator-activated receptor-gamma(PPAR-γ), excitatory amino acid transporter 2(EAAT2), the inhibitor of the nuclear factor kappa-B(ikB) and high mobility group-B1(HMGB1) protein were assayed by Western blot analysis. IL-17A expression was determined in cell medium by ELISA. Results: We found that MBP(C8) and MBP(C1) acted differently on the uptake/release of glutamate in astrocytes: C1 increased glutamate uptake and did not change its release, whereas C8 decreased glutamate release but did not change its uptake. Both isomers increased the expression of PPAR-γ and EAAT2 to the same degree. Western blots of cell lysates revealed decreased expression of ikB and increased expression of HMGB1 proteins after treatment of astrocytes by C8. Moreover, C8-treated cells released more nitric oxide and proinflammatory IL-17A than C1-treated cells. Conclusions: These data suggest that the most immunogenic deiminated isomer C8, in parallel to the decreases in glutamate release, elicits an inflammatory response and enhances the secretion of proinflammatory molecules via activation of nuclear factor kappa B(NF-kB). Summary statement: The most modified-citrullinated myelin basic protein charge isomer decreases glutamate release, elicits an inflammatory response and enhances the secretion of proinflammatory molecules via activation of nuclear factor kappa B in astrocytes.

Explicit expressions are derived for the resolvents and two-sided exit problem when killing depends on the present level of the process. Similar results in the reflected case are also considered. Abstract: In this paper we develop the Gerber-Shiu theory for the classic and dual discrete risk processes in a Markovian (regime switching) environment. In particular, by expressing the Gerber-Shiu function in terms of potential measures of an upward (downward) skip-free discrete-time and discrete-space Markov Additive Process (MAP), we derive closed form expressions for the Gerber-Shiu function in terms of the so-called (discrete) $\boldsymbol{W}_v$ and $\boldsymbol{Z}_v$ scale matrices, which were introduced in arXiv:2008.06697. We show that the discrete scale matrices allow for a unified approach for identifying the Gerber-Shiu function as well as the value function of the associated constant dividend barrier problems.

Abstract: A previous report showed that 12-week lowering of dietary omega-6 linoleic acid (LA) coupled with increased omega-3 polyunsaturated fatty acid (PUFA) intake (H3-L6 diet) reduced headache frequency and improved quality of life in patients with chronic daily headaches (CDHs) compared to dietary LA reduction alone (L6 diet). The trial also showed that targeted dietary manipulation alters PUFA-derived lipid mediators and endocannabinoids. However, several additional classes of lipid mediators associated with pain in preclinical models were not measured. The current secondary analysis investigated whether the clinical benefits of the H3-L6 diet were related to changes in plasma unesterified PUFA-derived lipid mediators known to be involved in nociception, including prostanoids. Lipid mediators were measured by ultra-high-pressure liquid chromatography coupled with tandem mass-spectrometry. Compared to baseline, dietary LA lowering with or without added omega-3 fatty acids did not alter unesterified n-6 PUFA-derived lipid mediators, although several species derived from LA, di-homo-gamma-linolenic acid, and arachidonic acid were positively associated with headache frequency and intensity, as well as mental health burden. Alpha-linolenic acid (ALA)-derived metabolites were also associated with increased headache frequency and intensity, although they did not change from the baseline in either dietary group. Compared to baseline, docosahexaenoic acid (DHA)-derived epoxides were more elevated in the H3-L6 group compared to the L6 group. Diet-induced elevations in plasma DHA-epoxides were associated with reduced headache frequency, better physical and mental health, and improved quality of life (p < 0.05). Prostanoids were not detected, except for PGF2-alpha, which was not associated with any outcomes. This study demonstrates that diet-induced changes in DHA-epoxides were associated with pain reduction in patients with chronic headaches, whereas n-6 PUFA and ALA metabolites were associated with nociception. Lipid mediator associations with mental health and quality of life paralleled pain management outcomes in this population. The findings point to a network of multiple diet-modifiable lipid mediator targets for pain management in individuals with CDHs.

Abstract: Objective Health programs for Indigenous people are most effective, acceptable, and sustainable when Indigenous perspectives are prioritized. Codesign builds on Indigenous people's creativity and propensity to experiment with new technologies and ensures research is designed and implemented in a culturally safe and respectful manner. Limited research has focused on older Indigenous people as partners in digital health. No research has focused on the acceptability and feasibility of older Indigenous people using wearables for heart health monitoring. This study provides insights into the acceptability and feasibility for ≥55-year-old Indigenous people living in remote locations to use wearables (watches and patches) to detect atrial fibrillation (AF) and high blood pressure. Methods This mixed methods study was codesigned and coimplemented with the local Aboriginal Controlled Health Service in a remote area of New South Wales, Australia. It included active involvement and codesign with the participants. The devices used in this study included a Withings Scan watch and a Biobeat patch. Results Despite challenging conditions (>36°C) and variable internet connectivity, 11 Indigenous older adults participated in a five-day wearables program in a remote location. Participants indicated that using digital health devices was acceptable and feasible for older Indigenous users. They described high levels of comfort, safety and convenience when using wearables (patches and watches) to detect AF. They were active participants in codesigning the program. Conclusion Older Indigenous Australians are motivated to use wearable health devices. They are keen to participate in codesign innovative health tech programs to ensure new health technologies are acceptable to Indigenous people and feasible for remote locations.

We will conduct a mixed methods Canadian interprovincial project in Quebec, New-Brunswick and Nova Scotia. It will include a scale-up phase and an evaluation phase. At inception, a scale-up committee will be formed in each province to oversee the scale-up phase. We will assess scale-up using a realist evaluation guided by the RAMESES checklist to develop an initial programme theory on CM scale-up. Then we will test and refine the programme theory using a mixed-methods multiple case study with 10 cases, each case being the scalable unit of the intervention in a region. Each primary care clinic within the case will recruit 30 adult patients with complex needs who frequently use healthcare services. Qualitative data will be used to identify contexts, mechanisms and certain outcomes for developing context–mechanism–outcome configurations. Quantitative data will be used to describe patient characteristics and measure scale-up outcomes.Ethics and dissemination Ethics approval was obtained. Engaging researchers, decision-makers, clinicians and patient partners on the study Steering Committee will foster knowledge mobilisation and impact. The dissemination plan will be developed with the Steering Committee with messages and dissemination methods targeted for each audience.

Abstract: Abstract Case management (CM) is an intervention for improving integrated care for patients with complex care needs. The implementation of this complex intervention often raises opportunities for change and collective leadership has the potential to optimize the implementation. However, the application of collective leadership in real-world is not often described in the literature. This commentary highlights challenges faced during the implantation of a CM intervention in primary care for people with complex care needs, including stakeholders’ buy-in and providers’ willingness to change their practice, selection of the best person for the case manager position and staff turnover. Based on lessons learned from PriCARE research program, this paper encourages researchers to adopt collective leadership strategies for the implementation of complex interventions, including promoting a collaborative approach, fostering stakeholders’ engagement in a trusting and fair environment, providing a high level of communication, and enhancing collective leadership attitudes and skills. The learnings from the PriCARE program may help guide researchers for implementing complex healthcare interventions.

42. Frequency and duration measurements of children’s outdoor free play: A Scoping review

Abstract: Outdoor free play encompasses unstructured, self-directed play in the outdoors and has been shown to support children’s health and development. Accurate and reliable measures are required to conduct research on children’s outdoor free play and examine cross-sectional and longitudinal variation. This study systematically reviews and evaluates measurement approaches for children’s outdoor free play used in existing literature. A scoping review was conducted to identify English-language peer-reviewed and grey literature that included measurements of the occurrence, frequency or duration of outdoor free play with children aged 2 to 17 years old. Studies were excluded if the outdoor free play measure included structured settings or activities, or focused on a specific location or play activity. Quantitative and qualitative content analysis was used to consider outdoor free play terminology, definition, and operationalization; positioning in relation to other variables and the topic of interest; and data collection context. A total of 4,860 unique studies were identified. After screening and full-text review, 184 papers were taken forward for analysis. Parent-recall questionnaires were used in 70.1% of included studies to measure outdoor free play, often using a single question to capture the variable. A lack of differentiation between outdoor play emanating from structured and unstructured settings was common, as was limited consideration of contextual factors such as season, school or non-school days, and time of day. The implications of existing approaches to measuring children’s outdoor free play and the need for valid and reliable measures to further research examining children’s outdoor free play are discussed.

Abstract: Background: Sleep efficiency and sleep onset latency are two measures that can be used to assess sleep quality. Factors that are related to sleep quality include age, sex, sociodemographic factors, and physical and mental health status. This study examines factors related to sleep efficiency and sleep onset latency in one First Nation in Saskatchewan, Canada. Methods: A baseline survey of the First Nations Sleep Health project was completed between 2018 and 2019 in collaboration with two Cree First Nations. One-night actigraphy evaluations were completed within one of the two First Nations. Objective actigraphy evaluations included sleep efficiency and sleep onset latency. A total of 167 individuals participated, and of these, 156 observations were available for analysis. Statistical analysis was conducted using logistic and linear regression models. Results: More females (61%) than males participated in the actigraphy study, with the mean age being higher for females (39.6 years) than males (35.0 years). The mean sleep efficiency was 83.38%, and the mean sleep onset latency was 20.74 (SD = 27.25) minutes. Age, chronic pain, ever having high blood pressure, and smoking inside the house were associated with an increased risk of poor sleep efficiency in the multiple logistic regression model. Age, chronic pain, ever having anxiety, heart-related illness, and smoking inside the house were associated with longer sleep onset latency in the multiple linear regression model. Conclusions: Sleep efficiency and sleep onset latency were associated with physical and environmental factors in this First Nation.

44. The bulge masses of TDE host galaxies and their scaling with black hole mass

Abstract: Tidal disruption events (TDEs) provide a means to probe the low end of the supermassive black hole (SMBH) mass distribution, as they are only observable below the Hills mass ($\lesssim 10^8$ M$_\odot$). Here we attempt to calibrate the scaling of SMBH mass with host galaxy bulge mass, enabling SMBH masses to be estimated for large TDE samples without the need for follow-up observations or extrapolations of relations based on high-mass samples. We derive host galaxy masses using Prospector fits to the UV-MIR spectral energy distributions for the hosts of 29 well-observed TDEs with BH mass estimates from MOSFiT. We then conduct detailed bulge/disk decomposition using SDSS and PanSTARRS imaging, and provide a catalog of bulge masses. We measure a positive correlation between SMBH and bulge mass for the TDE sample, with a power-law slope of 0.28 and significance $p=0.06$ (Spearmans) and $p=0.05$ (Pearsons), and an intrinsic scatter of 0.2 dex. Applying MC resampling and bootstrapping, we find a more conservative estimate of the slope is $0.18\pm0.11$, dominated by the systematic errors from Prospector and MOSFiT. This is shallower than the slope at high SMBH mass, which may be due to a bias in the TDE sample towards lower mass BHs that can more easily disrupt low-mass stars outside of the event horizon. When combining the TDE sample with that of the high mass regime, we find that TDEs are successful in extending the SMBH - stellar mass relationship further down the mass spectrum and provide a relationship across the full range of SMBH masses., Comment: Updated to match accepted MNRAS version

Abstract: With the sample of observed tidal disruption events (TDEs) now reaching several tens, distinct spectroscopic classes have emerged: TDEs with only hydrogen lines (TDE-H), only helium lines (TDE-He), or hydrogen in combination with He II and often N III/O III (TDE-H+He). Here we model the light curves of 32 optically-bright TDEs using the Modular Open Source Fitter for Transients (MOSFiT) to estimate physical and orbital properties, and look for statistical differences between the spectroscopic classes. For all types, we find a shallow distribution of star masses, compared to a typical initial mass function, between $\sim 0.1-1$ M$_\odot$, and no TDEs with very deep ($\beta \gg 1$) encounters. Our main result is that TDE-H events appear to come from less complete disruptions (and possibly lower SMBH masses) than TDE-H+He, with TDE-He events fully disrupted. We also find that TDE-H events have more extended photospheres, in agreement with recent literature, and argue that this could be a consequence of differences in the self-intersection radii of the debris streams. Finally, we identify an approximately linear correlation between black hole mass and radiative efficiency. We suggest that TDE-H may be powered by collision-induced outflows at relatively large radii, while TDE-H+He could result from prompt accretion disks, formed more efficiently in closer encounters around more massive SMBHs., Comment: Accepted for publication in MNRAS

Abstract: Abstract In sporadic Alzheimer’s disease (sAD) specific regions, layers and neurons accumulate hyperphosphorylated Tau (pTau) and degenerate early while others remain unaffected even in advanced disease. ApoER2-Dab1 signaling suppresses Tau phosphorylation as part of a four-arm pathway that regulates lipoprotein internalization and the integrity of actin, microtubules, and synapses; however, the role of this pathway in sAD pathogenesis is not fully understood. We previously showed that multiple ApoER2-Dab1 pathway components including ApoE, Reelin, ApoER2, Dab1, pP85αTyr607, pLIMK1Thr508, pTauSer202/Thr205 and pPSD95Thr19 accumulate together within entorhinal-hippocampal terminal zones in sAD, and proposed a unifying hypothesis wherein disruption of this pathway underlies multiple aspects of sAD pathogenesis. However, it is not yet known whether ApoER2-Dab1 disruption can help explain the origin(s) and early progression of pTau pathology in sAD. In the present study, we applied in situ hybridization and immunohistochemistry (IHC) to characterize ApoER2 expression and accumulation of ApoER2-Dab1 pathway components in five regions known to develop early pTau pathology in 64 rapidly autopsied cases spanning the clinicopathological spectrum of sAD. We found that (1) these selectively vulnerable neuron populations strongly express ApoER2; and (2) multiple ApoER2-Dab1 components representing all four arms of this pathway accumulate in abnormal neurons and neuritic plaques in mild cognitive impairment (MCI) and sAD cases and correlate with histological progression and cognitive deficits. Multiplex-IHC revealed that Dab1, pP85αTyr607, pLIMK1Thr508, pTauSer202/Thr205 and pPSD95Thr19 accumulate together within many of the same ApoER2-expressing neurons and in the immediate vicinity of ApoE/ApoJ-enriched extracellular plaques. Collective findings reveal that pTau is only one of many ApoER2-Dab1 pathway components that accumulate in multiple neuroanatomical sites in the earliest stages of sAD and provide support for the concept that ApoER2-Dab1 disruption drives pTau-associated neurodegeneration in human sAD.
