Η παρούσα διατριβή πραγματεύεται την εφαρμογή και ανάπτυξη μεθόδων πολλαπλών επιπέδων, για την επίλυση γραμμικών συστημάτων εξισώσεων και υπολογισμού ιχνών συναρτήσεων πινάκων, στη ρύθμιση δικτυωτού Κβαντική Χρωμοδυναμική (ΚΧΔ). ́Οταν τα εξαιρετικά μεγάλα και κακώς διαμορφωμένος γραμμικά συστήματα επιλύονται μέσω μεθόδων πολλαπλών δικτύων, η επεκτασιμότητα της υλοποίησης συνήθως διακυβεύεται καθώς μετακινούμαστε σε μεγάλο αριθμό κόμβων υπολογιστών. Η πρώτη συμβολή αυτής της διατριβής είναι στη μείωση αυτών των ζητημάτων επεκτασιμότητας στο ΔΔ-αΑΜΓ (μια βιβλιοθήκη για την επίλυση γραμμικών συστημάτων σε δικτυωτά ΚΧΔ). Αυτό το κάνουμε ενσωματώνοντας τέσσερις μεθόδους στο πιο χονδροειδές επίπεδο: ανακύκλωση, πολυωνυμική προετοιμασία, προετοιμασία με διαγώνιο μπλοκ και διοχέτευση. Με αυτόν τον τρόπο, έχουμε αξιοσημείωτα αλγοριθμικά και υπολογιστικά κέρδη στην περίπτωση Ωιλσον και είμαστε επίσης σε θέση να απαλλαγούμε από μια κάπως τεχνητή παράμετρο στο πιο χονδροειδές επίπεδο στην περίπτωση συνεστραμμένης μάζας. Στη συνέχεια, στρεφόμαστε σε περισσότερες υπολογιστικές πτυχές και, ως δεύτερη συνεισφορά από την εργασία μας, επεκτείνουμε τη βιβλιοθήκη ΔΔ-αΑΜΓ για να γίνει μια υβριδική δι- αλύτης ΓΠΥ+ΞΠΥ, μεταφέροντας ορισμένα μέρη του κώδικα μέσω ΞΥΔΑ Ξ . ́Ετσι, αντιλαμβανόμαστε τη σημασία της ομαλότερης αποσύνθεσης με βάση τον τομέα κατά την εκτέλεση σε ΓΠΥ, καθώς και της χρήσης μιας (πιο) επιθετικής χονδροειδοποίησης στην ιεραρχία πολλαπλών δικτύων, κατά την εκτέλεση με τον υβριδικό μας επιλύτη. Τέλος, συζητάμε την ανάπτυξη και τη δοκιμή μιας νέας μεθόδου για τον υπολογισμό των ιχνών των συναρτήσεων των πινάκων, tr(f (A)). Αυτή η νέα μέθοδος βασίζεται στο πολυεπίπεδο Μοντε Ξαρλο, σε συνδυασμό με μια ιεραρχία πολλαπλών δικτύων. Δοκιμάσαμε τη μέθοδο στην αντίστροφη συνάρτηση (δηλαδή f (A) = A −1 ), για τρεις πίνακες: Σξηωινγερ, Ωιλ- σον και στριμμένη μάζα. Βλέπουμε αξιοσημείωτα αποτελέσματα στο Σξηωινγερ, και πολύ καλά και πολλά υποσχόμενα αποτελέσματα για τον Ωιλσον και τη στριφτή μάζα. Αυτά τα αποτελέσματα ανοίγουν νέους δρόμους έρευνας, όπου η πολυεπίπεδη μέθοδος Μοντε Ξαρλο πολλαπλών δικτύων μπορεί να χρησιμοποιηθεί σε συνδυασμό με άλλες μεθόδους όπως ο αποπληθωρισμός και η ιεραρχική ανίχνευση. This thesis deals with the implementation and development of multilevel methods, for solving linear systems of equations and computing traces of functions of matrices, in the lattice Quantum Chromodynamics (QCD) setting. When extremely large and ill- conditioned linear systems are being solved via multigrid methods, the scalability of the implementation is typically compromised as we move to a large number of compute nodes. The first contribution of this thesis is on diminishing these scalability issues in DD-αAMG (a library for solving linear systems in lattice QCD). We do this by in- tegrating four methods into the coarsest level: recycling, polynomial preconditioning, block-diagonal preconditioning and pipelining. In doing so, we get remarkable algo- rithmic and computational gains in the Wilson case, and we are also able to get rid of a somewhat artificial parameter at the coarsest level in the twisted mass case. We then turn to more computational aspects and, as a second contribution from our work, we extend the DD-αAMG library to become a hybrid GPU+CPU solver, by porting some parts of the code via CUDA C. In doing so, we realize the importance of having a smoother based on domain decomposition when running on GPUs, as well as of us- ing an (more) aggressive coarsening in the multigrid hierarchy, when running with our hybrid solver. Finally, we discuss the development and testing of a new method for the computation of traces of functions of matrices, tr(f (A)). This new method is based on multilevel Monte Carlo, in combination with a multigrid hierarchy. We have tested the method on the inverse (i.e. f (A) = A −1 ), for three matrices: Schwinger, Wilson and twisted mass. We see remarkable results in Schwinger, and very good and promising results for Wilson and twisted mass. These results open new paths of research, where the multigrid multilevel Monte Carlo method can be used in combination with other methods such as deflation and hierarchical probing. Purpose Patients with pulmonary tuberculosis (PTB) disease and positive sputum cultures are the main source of infection. Culture conversion time is inconsistent and defining the length of respiratory isolation is challenging. The objective of this study is to develop a score to predict the length of isolation period. Methods A retrospective study was carried out to evaluated risk factors associated with persistent positive sputum cultures after 4 weeks of treatment in 229 patients with PTB. A multivariable logistic regression model was used to determinate predictors for positive culture and a scoring system was created based on the coefficients of the final model. Results Sputum culture was persistently positive in 40.6%. Fever at consultation (1.87, 95% CI:1.02–3.41), smoking (2.44, 95% CI:1.36–4.37), > 2 affected lung lobes (1.95, 95% CI:1.08–3.54), and neutrophil-to-lymphocyte ratio > 3.5 (2.22, 95% CI:1.24–3.99), were significantly associated with delayed culture conversion. Therefore, we assembled a severity score that achieved an area under the curve of 0.71 (95% CI:0.64–0.78). Conclusions In patients with smear positive PTB, a score with clinical, radiological and analytical parameters can be used as a supplemental tool to assist clinical decisions in isolation period.

A high degree of vigilance and appropriate diagnostic methods are required to detect Clostridioides difficile infection (CDI). We studied the effectiveness of a multimodal training program for improving CDI surveillance and prevention. Between 2011 and 2016, this program was made available to healthcare staff of acute care hospitals in Catalonia. The program included an online course, two face-to-face workshops and dissemination of recommendations on prevention and diagnosis. Adherence to the recommendations was evaluated through surveys administered to the infection control teams at the 38 participating hospitals. The incidence of CDI increased from 2.20 cases/10 000 patient-days in 2011 to 3.41 in 2016 (P < 0.001). The number of hospitals that applied an optimal diagnostic algorithm rose from 32.0% to 71.1% (P = 0.002). Hospitals that applied an optimal diagnostic algorithm reported a higher overall incidence of CDI (3.62 vs. 1.92, P < 0.001), and hospitals that were more active in searching for cases reported higher rates of hospital-acquired CDI (1.76 vs. 0.84, P < 0.001). The results suggest that the application of a multimodal training strategy was associated with a significant rise in the reporting of CDI, as well as with an increase in the application of the optimal diagnostic algorithm.

The aim of our study was to characterize the etiology of prosthetic joint infections (PJIs)&mdash, including multidrug-resistant organisms (MDRO)&mdash, by category of infection. A multicenter study of 2544 patients with PJIs was performed. We analyzed the causative microorganisms according to the Tsukayama&rsquo, s scheme (early postoperative, late chronic, and acute hematogenous infections (EPI, LCI, AHI) and &ldquo, positive intraoperative cultures&rdquo, (PIC)). Non-hematogenous PJIs were also evaluated according to time since surgery: &lt, 1 month, 2&ndash, 3 months, 4&ndash, 12 months, &gt, 12 months. AHIs were mostly caused by Staphylococcus aureus (39.2%) and streptococci (30.2%). EPIs were characterized by a preponderance of virulent microorganisms (S. aureus, Gram-negative bacilli (GNB), enterococci), MDROs (24%) and polymicrobial infections (27.4%). Conversely, coagulase-negative staphylococci (CoNS) and Cutibacterium species were predominant in LCIs (54.5% and 6.1%, respectively) and PICs (57.1% and 15.1%). The percentage of MDROs isolated in EPIs was more than three times the percentage isolated in LCIs (7.8%) and more than twice the proportion found in AHI (10.9%). There was a significant decreasing linear trend over the four time intervals post-surgery for virulent microorganisms, MDROs, and polymicrobial infections, and a rising trend for CoNS, streptococci and Cutibacterium spp. The observed differences have important implications for the empirical antimicrobial treatment of PJIs.

La presente Tesis tiene como objetivo explorar conceptos relacionados con el modelamiento de estándares y buenas prácticas a través de herramientas como el modelo de referencia SCOR y conceptos claves como VMI y análisis financiero, con el fin de realizar una propuesta de mejora en la cadena de suministro para una empresa concesionaria de gas natural. La propuesta se enfoca en la reducción de los días de inventario y la optimización del costo en sus operaciones sin afectar el nivel de servicio. La tesis tiene como referente a una empresa concesionaria de gas natural que opera en Lima Metropolitana y Callao, la cual tiene un contrato de concesión del sistema de distribución de gas natural por un periodo de 33 años, susceptibles de prórroga, a través de un contrato de concesión, que incluye todas las etapas necesarias para poner en marcha el servicio.

La planeación financiera es una parte importante de las funciones de la empresa, al proporcionar una guía para dirigir, coordinar, controlar las acciones y alcanzar sus objetivos. El punto de partida para una planeación financiera a corto plazo es el entendimiento de las fuentes y los usos de fondos. Si el saldo de efectivo previsto es insuficiente para cubrir el día a día de las operaciones y proporcionar un amortiguador frente a las contingencias, se necesitara encontrar financiamiento adicional. La planeación financiera no intenta minimizar riesgos. Es un proceso de decidir que riesgos tomar y cuales son innecesarios o no merece la pena asumir. Es ordenar los recursos para que el futuro sea favorable, por ello se deben controlar los acontecimientos que sean manejables y adaptar los que no lo sean. Existen tres elementos claves en el proceso de planeación financiera: La planeación del efectivo, Planeación de utilidades, los presupuestos de caja y los estados proformas. En nuestro caso práctico trabajamos la Empresa Auto Forros S, A. Es una empresa que se dedica a fabricar forros para asientos de vehículos y les presentamos los Presupuesto de Ventas, Presupuesto de Compras, Presupuesto de costo de producción, Presupuestos de gastos Operativos, Presupuesto de efectivo, Análisis Horizontal y Vertical de la empresa Auto Forros S, A. En síntesis nuestro trabajo muestra una planeación financiera adecuada que busca optimizar los recursos y mejorar su desarrollo dentro del mercado en que se desenvuelve la empresa. Así también nos permitirá mejorar la calidad de los bienes que ofrecemos a nuestros clientes, optimizando así el nivel de satisfacción de los consumidores lo que hará que la empresa sea más competitiva a nivel local y a nivel nacional.

To determine the clinical importance of repeating the Hybrid Capture 2 (HC2) high-risk human papillomavirus (HPV) test when the result obtained falls between 2.7 and 0.8 pg/mL, the so-called "grey zone."Repeat testing was performed on 139 thin-layer liquid-based cytology cervicovaginal samples that were found to fall in the grey zone. The samples represented a variety of cytological diagnoses. The results of both HPV HC2 tests were analyzed related to the cytological diagnosis of both the previous and the follow-up cytological samples.The change of the HPV HC2 result from positive (2.7-1 pg/mL) to negative (1 pg/mL) when repeating the test is not related with the diagnosis obtained in the cytological follow-up of the patient.The HC2 test should not be repeated when the result falls between 2.7 and 0.8 pg/mL. In these cases the result of the cytology is more relevant and should be used in the clinical assessment of the patient.
