8. Augmented histopathology: Enhancing colon cancer detection through deep learning and ensemble techniques.

- Abstract
Colon cancer poses a significant threat to human life with a high global mortality rate. Early and accurate detection is crucial for improving treatment quality and the survival rate. This paper presents a comprehensive approach to enhance colon cancer detection and classification. The histopathological images are gathered from the CRC‐VAL‐HE‐7K dataset. The images undergo preprocessing to improve quality, followed by augmentation to increase dataset size and enhance model generalization. A deep learning based transformer model is designed for efficient feature extraction and enhancing classification by incorporating a convolutional neural network (CNN). A cross‐transformation model captures long‐range dependencies between regions, and an attention mechanism assigns weights to highlight crucial features. To boost classification accuracy, a Siamese network distinguishes colon cancer tissue classes based on probabilities. Optimization algorithms fine‐tune model parameters, categorizing colon cancer tissues into different classes. The multi‐class classification performance is evaluated in the experimental evaluation, which demonstrates that the proposed model provided highest accuracy rate of 98.84%. In this research article, the proposed method achieved better performance in all analyses by comparing with other existing methods. It is extremely difficult to ensure the security of the Vehicular Internet of Things (VIoT) because of how diverse and infrastructure-free it is deployed, which exposes it to a variety of security concerns. There is an urgent need for a mutual authentication method among the linked devices to solve this problem and safeguard sensitive data inside the deployed region. However, when used in VIoT systems, previous methods have significant computational costs and insecure connectivity. Therefore, it is essential to provide an authentication system that protects privacy and can successfully protect VANET devices from attacks. In order to provide private and secure authentication between VANET devices, this study offers a special, compact, and secure protocol based on network-centric Henon Chaotic Maps (NC-HCM). The proposed method enables real devices to vary their keys for each transmission iteration while ensuring secure transmission of sensitive data from the source to the destination. The security level of the proposed model is assessed and analysed formally using the Burrows-Abadi-Needham Logic (BAN). The given model is also thoroughly verified using AVISPA and the Profverif tool. The results of the investigation show that the suggested strategy acts as a strong deterrent against both active and passive attacks. It successfully protects the VANET devices and guarantees the veracity, integrity, and secrecy of the data sent.

In present times, unmanned aerial vehicles (UAVs) are widely employed in several real time applications due to their autonomous, inexpensive, and compact nature. Aerial image classification in UAVs has gained significant interest in surveillance systems that assist object detection and tracking processes. The advent of deep learning (DL) models paves a way to design effective aerial image classification techniques in UAV networks. In this view, this paper presents a novel optimal Squeezenet with a deep neural network (OSQN-DNN) model for aerial image classification in UAV networks. The proposed OSQN-DNN model initially enables the UAVs to capture images using the inbuilt imaging sensors. Besides, the OSQN model is applied as a feature extractor to derive a useful set of feature vectors where the coyote optimization algorithm (COA) is employed to optimally choose the hyperparameters involved in the classical SqueezeNet model. Moreover, the DNN model is utilized as a classifier that aims to allocate proper class labels to the applied input aerial images. Furthermore, the usage of COA for hyperparameter tuning of the SqueezeNet model helps to considerably boost the overall classification performance. For examining the enhanced aerial image classification performance of the OSQN-DNN model, a series of experiments were performed on the benchmark UCM dataset. The experimental results pointed out that the OSQN-DNN model has resulted in a maximum accuracy of 98.97% and a minimum running time of 1.26mts.

The presented mathematical model is used to form machine cells, optimize costs of exceptional elements and design the shop floor layout for various demands of components. The complete similarity measure algorithm forms machine cells and part families in a refined form. Later, exceptional elements are eliminated in linear programming optimization model by using machine duplication and part subcontract. Then the shop floor layout is designed to have optimized material movements between cells and within a cell. The performance evaluation of cell formation algorithm is done on case studies of various batch sizes to give the process capability compared with other similar methods. The result from a linear programming optimization model is cost savings, machines duplicated, parts subcontracted, inter intra cellular movements. Finally, the output of inbound facility design is the floor layout which has machine cell clusters with optimized floor area.

BACKGROUND AND OBJECTIVES: All the four serotypes of dengue virus are found in our country. Case fatality rates in endemic countries like India are 2.5%. During epidemics of dengue, attack rates among susceptible are 40-90%. Early recognition and prompt treatment are vital if disease related morbidity and mortality are to be limited. Clinical features that can be used in the initial assessment of febrile patients are essential tools for clinicians, especially in limited resource settings. Awareness of bradycardia as a clinical finding, could help in the early recognition of dengue and potentially reduce complications and death. METHOD AND RESULTS: The study was conducted in the Department of Medicine, Mysore Medical College & Research Institute, Mysore from the period of July 2010 to December 2010 who met the inclusion and exclusion criteria. A total of 100 patients of Dengue fever were analyzed. The most common age group affected in our study was 20-39 years (53%). The clinical spectrum of cases included 22% cases of Dengue fever (DF), 72% cases of Dengue Hemorrhagic fever (DHF), 6% cases of Dengue Shock Syndrome (DSS). Clinical pulse rate distribution amongst cases showed 27% with bradycardia, 61% with relative bradycardia, and 12% with tachycardia. Electrocardiographic changes in our study showed 37% with sinus bradycardia, 48% with normal sinus rhythm, 1% with first degree heart block, and 14% with sinus tachycardia. CONCLUSION: Bradycardia was a predominant occurrence amongst total of 100 cases of Dengue fever analyzed. 88% of the cases had bradycardia. (61% had relative bradycardia and 27% bradycardia. Majority of the patients on ECG showed sinus bradycardia (37%) and normal sinus rhythm (48%). Hence, awareness of bradycardia as a clinical finding, can help in the early recognition of dengue and potentially reduce complications and death associated with dengue virus infection.

BACKGROUND: Few data are available regarding the use of antithrombotic strategies in coronary artery disease patients with atrial fibrillation (AF) in everyday practice. We sought to describe the prevalence of AF and its antithrombotic management in a contemporary population of patients with stable coronary artery disease.METHODS AND FINDINGS: CLARIFY is an international, prospective, longitudinal registry of outpatients with stable coronary artery disease, defined as prior (≥12 months) myocardial infarction, revascularization procedure, coronary stenosis >50%, or chest pain associated with evidence of myocardial ischemia. Overall, 33,428 patients were screened, of whom 32,954 had data available for analysis at baseline; of these 2,229 (6.7%) had a history of AF. Median (interquartile range) CHA2DS2-VASc score was 4 (3, 5). Oral anticoagulation alone was used in 25.7%, antiplatelet therapy alone in 52.8% (single 41.8%, dual 11.0%), and both in 21.5%. OAC use was independently associated with permanent AF (pCONCLUSIONS: In this contemporary cohort of patients with stable coronary artery disease and AF, most of whom are theoretical candidates for anticoagulation, oral anticoagulants were used in only 47.2%. Half of the patients received antiplatelet therapy alone and one-fifth received both antiplatelets and oral anticoagulants. Background. Left ventricular (LV) function might be altered in type 2 diabetes (DM) and microalbuminuria (MA) may accentuate the abnormalities. We sought to investigate whether additional LV dysfunction could be unmasked using tissue Doppler (TVE)-enhanced dobutamine stress echocardiography (TVE-DSE) in patients with DM+MA. Methods. Twenty seven DM subjects with MA, (DM+MA), 31 DM subjects without MA (DM-MA), and 13 Controls were evaluated using TVE-DSE. LV basal peak systolic (PSV), early (E') and late (A') diastolic velocities (cm/sec) at rest and peak stress were post-processed. LV filling pressure was assessed using E/E'ratio. Results. PSV and E'velocity at peak stress in the respective three groups were 13.7±1.0, 10.1±1.1, 10.0±1.2 for PSV; and 10.0±1.6, 5.0±1.4, 4.8±1.4 for E' (p < 0.001 for controls vs. both groups). E/E' at rest was 7.9±0.7 in the controls, 10.8±2.4 in DM-MA, and 11.0±2.2 in DM+MA (p < 0.01 Controls vs. both the DM groups). Conclusions. Patients with DM+MA do not have additional LV regional systolic and diastolic dysfunctions compared with DM-MA, as revealed by TVE-DSE, when controlled for glycemia levels, lipids, and treatment strategies., QC 20100709

Background: Although the impact of type 2 diabetes mellitus (DM) and hypertension (HTN) on myocardial function has recently been studied using tissue Doppler echocardiography (TDE), the independent role of both conditions, and the influence of other risk factors on myocardial function has not been completely defined, particularly in absence of coronary artery disease (CAD). The aim of this study was to assess the myocardial functional reserve in patients with DM or HTN with apparently normal left ventricular (LV) systolic function. Methods: Standard and dobutamine stress echocardiography using TDE was performed in 128 subjects: 59 had DM, 20 had HTN, 27 had both DM and HTN (HTN + DM), and 22 subjects were controls (C). Subjects with known CAD and depressed LV function were excluded. In addition, standard two-dimensional and Doppler measurements, LV regional peak systolic (PSV), early (E') and late (A') diastolic velocities, strain (S%) and strain rate (SR), were assessed at rest and peak stress. Results: The LV mass did not differ, although relative wall thickness was significantly higher in subjects with HTN + DM and HTN. The PSV did not differ at rest but was lowest in subjects with HTN + DM at peak stress. The E' wave velocity was significantly lower in subjects with HTN + DM both at rest and during peak stress, as were S% and SR. Conclusions: The addition of DM to HTN has a negative effect on LV systolic and diastolic functions. A depressed myocardial functional reserve might be postulated as one of the pathophysiologic mechanisms for the excessive occurrence of congestive heart failure in patients with DM or HTN., QC 20100709

Background: Though hemodialysis (HD) acutely improves cardiac function, the impact of background diseases like coronary artery disease (CAD) and Type 2 diabetes (DM) in the setting of end-stage renal disease (ESRD) is not known. Tissue velocity echocardiography (TVE) offers a fast choice to follow changes in myocardial function after HD in ESRD with concomitant DM and/or CAD. Methods: 46 subjects (17 with ESRD, Group 1; 15 with DM, Group 2; 14 with DM+CAD, Group 3) underwent standard and TVE prior to and shortly after HD. Besides standard Doppler variables, regional myocardial systolic and diastolic velocities, as well as systolic strain rate were post processed. Results: Compared with pre-HD, post-HD body weight (kg) significantly decreased in all the three groups (51 ± 9 vs. 48 ± 8, 62 ± 10 vs.59 ± 10, and 61 ± 9 vs. 58 ± 9 respectively; all p < 0.01). Left ventricular end diastolic dimensions (mm) also decreased post- HD (46 ± 5 vs. 42 ± 7, 53 ± 7 vs. 50 ± 7, 51 ± 7 vs. 47 ± 8 respectively; all p < 0.01). Regional longitudinal peak systolic velocity in septum (cm/s) significantly increased post-HD in Group 1(5.7 ± 1.6 vs. 7.2 ± 2.3; p < 0.001) while remained unchanged in the other two groups. Similar trends were noted in other left ventricular walls. When the myocardial velocities (cm/s) were computed globally, the improvement was seen only in Group 1 (6.3 ± 1.5 vs. 7.9 ± 2.0; p < 0.001). Global early regional diastolic velocity (cm/s) improved in Group 1, remained unchanged in Group 2, while significantly decreased in Group 3(-5.9 ± 1.3 vs. -4.1 ± 1.8; p < 0.01). Global systolic strain rate (1/sec) increased in the first 2 Groups but remained unchanged (-0.87 ± 0.4 vs. -0.94 ± 0.3; p = ns) in Group 3. Conclusion: A single HD session improves LV function only in ESRD without coexistent DM and/or CAD. The present data suggest that not only dialysis-dependent changes in loading conditions but also co-existent background diseases determine the myocard, QC 20100709

Background: Thyroid abnormalities are common in chronic heart failure. Severity of heart failure rises by several fold in patients with thyroid dysfunction. Objectives: The purpose of this prospective study is to determine the correlation between low T3 syndrome and chronic heart failure with 2D echocardiography features & predicting the severity of chronic heart failure. Methods: In this descriptive, prospective cross sectional study, all patients who presented to the department of medicine with chronic heart failure during this study period of 12 months from January 2010-December 2011 in K.R.Hospital, Mysore were included. Patients were grouped into Low T3 chronic heart failure, hypothyroid chronic heart failure and chronic heart failure. Results: Mean age of low T3 chronic heart failure patients was higher than other two groups [60.50±6.15(SD) years, Systolic dysfunction on 2D Echo was more in low T3 dilated cardiomyopathy (20%), Diastolic dysfunction on 2D Echo was more in low T3 dilated cardiomyopathy group (30%), Pericardial effusion was seen in more number of patients with low T3 dilated cardiomyopathy (10%).Global hypokinesia was seen in more number of patients with low T3 dilated cardiomyopathy (30%).Segmental hypokinesia was seen in more number of patients with low T3 dilated cardiomyopathy (3%). Mean ejection fraction was seen in more number of patients with low T3 dilated cardiomyopathy [36.78±5.08 (SD)%]. Mean ejection fraction was lower in low T3 dilated cardiomyopathy [34.8±3.293 (SD) %].The high pulmonary artery systolic pressure was seen in more number of patients in low T3 dilated cardiomyopathy (70%).Conclusion: There is significant percentage of chronic heart failure patients having low T3 alone as biochemical parameter. It is important to recognize patients with chronic heart failure as it is associated with increased severity of heart failure. [ABSTRACT FROM AUTHOR]

- Abstract
Background: Although the impact of type 2 diabetes mellitus (DM) and hypertension (HTN) on myocardial function has recently been studied using tissue Doppler echocardiography (TDE), the independent role of both conditions, and the influence of other risk factors on myocardial function has not been completely defined, particularly in absence of coronary artery disease (CAD). The aim of this study was to assess the myocardial functional reserve in patients with DM or HTN with apparently normal left ventricular (LV) systolic function., Methods: Standard and dobutamine stress echocardiography using TDE was performed in 128 subjects: 59 had DM, 20 had HTN, 27 had both DM and HTN (HTN + DM), and 22 subjects were controls (C). Subjects with known CAD and depressed LV function were excluded. In addition, standard two-dimensional and Doppler measurements, LV regional peak systolic (PSV), early (E') and late (A') diastolic velocities, strain (S%) and strain rate (SR), were assessed at rest and peak stress., Results: The LV mass did not differ, although relative wall thickness was significantly higher in subjects with HTN + DM and HTN. The PSV did not differ at rest but was lowest in subjects with HTN + DM at peak stress. The E' wave velocity was significantly lower in subjects with HTN + DM both at rest and during peak stress, as were S% and SR., Conclusions: The addition of DM to HTN has a negative effect on LV systolic and diastolic functions. A depressed myocardial functional reserve might be postulated as one of the pathophysiologic mechanisms for the excessive occurrence of congestive heart failure in patients with DM or HTN.

- Abstract
Background: Streptokinase is the most widely used thrombolytic agent and can now be made using recombinant DNA technology. The present trial was initiated to assess an indigenous recombinant streptokinase (Shankinase, r-SK)., Aim: To compare the efficacy and safety of indigenous recombinant streptokinase (Shankinase, r-SK) and natural streptokinase (Streptase, n-SK)., Settings and Design: Double blind, randomized, non-inferiority, multicentric, parallel study., Materials and Methods: Patients of AMI < 6 hours of chest pain and 2 mm ST elevation in 2 contiguous chest leads V(1)-V(6) or 1 mm in limb leads were randomized to receive 1.5 miu of either r-SK or n-SK. CK Peaking and decrease of > or = 50% ST segment were used to assess reperfusion., Statistical Analysis: Difference in the groups was assessed by chi-square or paired t test as required. Probability value < 0.05 was considered significant with 95% confidence interval., Results: Overall 150 patients were recruited (96 r-SK group and 54 in n-SK group) and demographic and clinical profile of the groups was comparable. Reperfusion was seen in 68.2% (58) and 69.4% (34) patients in r-SK and n-SK groups respectively. Commonly seen adverse events were fever in 7 (8.5%), hypotension in 3 (3.6%), nausea in 2 (2.4%) patients. Minor bleeding were seen in 4 (4.8%) of patients., Conclusion: Indigenous recombinant Streptokinase (r-SK) is as efficacious as natural streptokinase (n-SK) in establishing reperfusion as assessed by non-invasive parameters with comparable side effect profile.
