1. Association between COVID-19 Vaccination (ChAdOx1-S) and Thromboembolic, Thrombocytopenic, Hemorrhagic Events: A Systematic Review and Meta-analysis of Analytical Epidemiological Studies

Abstract

We conducted a systematic review of analytical epidemiological studies to assess the association between ChAdOx1-S vaccination and thromboembolic, thrombocytopenic, and hemorrhagic events. We searched Medline, Embase, Google Scholar, WHO-COVID-19 database, and medRxiv for studies evaluating the association between ChAdOx1-S and vascular events. Primary outcomes of interest were cerebral venous sinus thrombosis, peripheral venous thrombosis (PVT), and thrombocytopenia. Two independent reviewers screened for eligible studies, extracted data, and assessed the risk of bias. The DerSimonian-Laird random effects model was used to pool the incidence rate ratios (IRRs) separately for the first and second doses. Heterogeneity was assessed using I2 statistics. Twenty studies were included, of which 11 were self-controlled case series, and nine were cohort studies (254 million participants). Pooling of 17 studies showed a higher risk of cerebrovascular thrombosis (IRR = 3.5, 95% CI = 2.2–5.4, I2 = 79%), PVT (IRR = 2.0, 95% CI = 1.1–3.5, I2 = 95%) and thrombocytopenia (IRR = 1.6, 95% CI = 1.4–1.9, I2 = 93%) among those who received ChAdOx1-S vaccination as compared to controls. No increased risk was seen after the second dose or for secondary outcomes. There is moderate-to-high certainty of the evidence for the increased risk of cerebral venous sinus thrombosis, PVT, and thrombocytopenia following the first dose of the ChAdOx1-S vaccine. 2. Numerical Method for Modeling Nucleation and Growth of Particles that Prevents Numerical Diffusion

Abstract

State-of-the-art models for aerosol particle nucleation and growth from a cooling vapor primarily use a nodal method to numerically solve particle growth kinetics. In this method, particles that are smaller than the critical size are omitted from consideration, because they are thermodynamically unfavorable. This omission is based on the assumption that most of the newly formed particles are above the critical size and that the subcritical-size particles are not important to take into account. Due to the nature of the nodal method, it suffers from the numerical diffusion, which can cause an artificial broadening of the cluster size distribution leading to significant overestimation of the number of large-size particles. To address these issues, we propose a more accurate numerical method that explicitly models particles of all sizes, and uses a special numerical scheme that eliminates the numerical diffusion. We extensively compare this novel method to the commonly used nodal solver of the General Dynamics Equation (GDE) for particle growth and demonstrate that it offers GDE solutions with higher accuracy without generating numerical diffusion. Incorporating small subcritical clusters into the solution is crucial for: 1) more precise determination of the entire shape of the particle size distribution function and 2) wider applicability of the model to experimental studies with non-monotonic temperature variations leading to particle evaporation. The computational code implementing this numerical method in Python is available upon request.

3. Closed Loop Testing of Microphonics Algorithms Using a Cavity Emulator

Abstract

An analog crystal filter based cavity emulator is modified with reverse biased varactor diodes to provide a tuning range of around 160 Hz. The piezo drive voltage of the resonance controller is used to detune the cavity through the bias voltage. A signal conditioning and summing circuit allows the introduction of microphonics disturbance from a signal source or using real microphonics data from cavity testing. 4. LLRF System for the Fermilab PIP-II Superconducting LINAC

Abstract

PIP-II is an 800 MEV superconducting linac that is in the initial acceleration chain for the Fermilab accelerator complex. The RF system consists of a warm front-end with an ion source, RFQ and buncher cavities along with 25 superconducting cryo-modules comprised of five different acceleration \(\beta\). The LLRF system for the LINAC has to provide field and resonance control for a total of 125 RF cavities.The LLRF system design is in the final design review phase and will enter the production phase next year. The PIP-II project is an international collaboration with various partner labs contributing subsystems. Abstract

Purpose/objective(s)Radiographic MRI response following prostate radiotherapy, particularly stereotactic body radiotherapy (SBRT), remains poorly understood. Our objective was to describe radiographic changes to the prostate gland and prostate tumor following SBRT of men treated on a prospective, randomized trial.Materials/methodsMIRAGE (NCT04384770) is a single center, randomized phase III trial of patients receiving either CT or MRI guided SBRT for localized prostate cancer. Patients underwent pre-treatment and annual post-treatment MRIs, in addition to routine PSA surveillance. Outcomes reported include percent gland shrinkage, percent PSA response at one year, and presence of residual tumor based on radiographic interpretation. Patient characteristics were compared via two-sample t-test or Fischer's exact test. Both univariate and multivariable logistical analysis were employed to identify potential clinical predictors of residual tumor on 1-year follow up MRI.ResultsThis study cohort included 94 eligible patients with baseline characteristics in Table 1. Residual lesions were seen in 13 patients (14%), 5/27 (18.5%) treated without ADT and 8/67 (12%) with ADT. PSA ablation was deep, with a 79% median decrease without ADT and 98% median decrease with ADT. Patients receiving ADT showed more gland shrinkage (17% vs. 34% shrinkage, p = 0.0001), while radiographic non-responders and responders experienced similar gland shrinkage (median 21% vs 29% shrinkage, p > 0.05). No significant clinical predictors of residual tumor were identified on univariate and multivariate analysis. No patient had any clinical or biochemical evidence of failure.ConclusionA total of 14% of patients were found to have residual tumor detected on MRI one year after SBRT. These data highlight the protracted nature of radiographic tumor response to radiation therapy, even with ablative radiation techniques. The analysis is limited by the lack of biopsy data to quantify whether visualized residual tumor harbor active cancer.

Abstract

Nano-scale fluid flow is unlike transport on the macro-scale. Pressure gradients typically dominate effects on a large scale while thermal gradients contribute negligibly to the motion of fluid. The situation entirely reverses on the nano-scale. At a microscopic level, flows induced by thermal gradients are caused by forces that act on atoms or molecules near an interface. These thermo-osmotic forces cannot, at present, be derived analytically or measured experimentally. Clearly, it would be useful to calculate these forces via molecular simulations, but direct approaches fail because in the steady-state, the average force per particle vanishes, as the thermo-osmotic force is balanced by a gradient in shear stress. In our journey to indirectly calculate the osmotic force, we met another unknown in the field of molecular theory at interfaces: the microscopic pressure tensor. The latter is an open problem since the microscopic pressure near an interface is not uniquely defined. Using local thermodynamics theories, we relate the thermo-osmotic force to the gradient of the microscopic pressure tensor. Yet, because the pressure is not uniquely defined, we arrive at multiple answers for the thermo-osmotic force, where at most one can be correct. To resolve the latter puzzle, we develop a direct, non-equilibrium simulation protocol to measure the thermo-osmotic force, whereby a thermal gradient is imposed and the osmotic force is measured by eliminating the shear force. Surprisingly, we find that the osmotic force cannot be derived from the gradient of well-known microscopic pressure expressions. We, therefore, derive a thermodynamic expression that gets close. In this work, we report the first, direct calculation of the thermo-osmotic force while simultaneously showing that standard microscopic pressure expressions fail to predict pressure gradients.

Abstract

State-of-the-art numerical models describing the kinetics of aerosol particle nucleation and growth from a cooling vapor primarily use a nodal method, in which particles that are smaller than the critical size are omitted from consideration because they are thermodynamically unfavorable. This omission is based on the assumption that most newly formed particles are above the critical size, so that subcritical-size particles are not important to take into account. Due to the nature of the nodal method, it suffers from numerical diffusion, which can cause an artificial broadening of the cluster size distribution leading to a significant overestimation of the number of large-size particles. To address these issues, we propose a more accurate numerical method that explicitly models particles of all sizes, and uses a special numerical scheme that substantially reduces the numerical diffusion and provides high solution accuracy and numerical stability. We extensively compare this novel method to the commonly used nodal solver of the general dynamic equation (GDE) for particle growth and demonstrate that it offers GDE solutions with higher accuracy with low numerical diffusion. Incorporating small subcritical clusters into the solution is crucial for: 1) more precise determination of the entire particle size distribution function and 2) wider applicability of the model to experimental studies with non-monotonic temperature variations leading to particle evaporation. Abstract

We conducted a systematic review of analytical epidemiological studies to assess the association between ChAdOx1-S vaccination and thromboembolic, thrombocytopenic, and hemorrhagic events. We searched Medline, Embase, Google Scholar, WHO-COVID-19 database, and medRxiv for studies evaluating the association between ChAdOx1-S and vascular events. Primary outcomes of interest were cerebral venous sinus thrombosis, peripheral venous thrombosis (PVT), and thrombocytopenia. Two independent reviewers screened for eligible studies, extracted data, and assessed the risk of bias. The DerSimonian-Laird random effects model was used to pool the incidence rate ratios (IRRs) separately for the first and second doses. Heterogeneity was assessed using I2 statistics. Twenty studies were included, of which 11 were self-controlled case series, and nine were cohort studies (254 million participants). Pooling of 17 studies showed a higher risk of cerebrovascular thrombosis (IRR = 3.5, 95% CI = 2.2-5.4, I² = 79%), PVT (IRR = 2.0, 95% CI = 1.1-3.5, I² = 95%) and thrombocytopenia (IRR = 1.6, 95% CI = 1.4-1.9, I² = 93%) among those who received ChAdOx1-S vaccination as compared to controls. No increased risk was seen after the second dose or for secondary outcomes. There is moderate-to-high certainty of the evidence for the increased risk of cerebral venous sinus thrombosis, PVT, and thrombocytopenia following the first dose of the ChAdOx1-S vaccine. BackgroundHepatitis B virus (HBV) infection is common with major clinical consequences. In Asian Americans, the HBsAg carrier rate ranges from 2% to 16% which approximates the rates from their countries of origin. Similarly, HBV is the most important cause of cirrhosis, hepatocellular carcinoma (HCC) and liver related deaths in HBsAg positive Asians worldwide.AimTo generate recommendations for the management of Asian Americans infected with HBV.MethodsThese guidelines are based on relevant data derived from medical reports on HBV from Asian countries as well as from studies in the HBsAg positive Asian Americans. 38. Investigation of oxygen targets and hypoxic adaptation in paediatric critical illness

Abstract

Background: The effect of varying oxygen tension on the outcome of critically ill patients is contentious. Many interventions employed in the intensive care unit aimed at increasing oxygen delivery can cause harm. During the natural history of critical illness, adaption to tissue hypoxia may occur to a greater or lesser extent. I used the principle of Near Infrared Spectroscopy (NIRS) with a vascular occlusion test to investigate forearm muscle oxygen consumption. Methods: The relationship between arterial oxygenation and mortality in critically ill children was explored with a retrospective observational study, a national survey and a systematic review in chapter 3. Chapter 4 describes a study of Near infrared spectroscopy with a vascular occlusion test (NIRS VOT) in children with suspected mitochondrial disease. The aim was ascertain if NIRS VOT as a technique is able to reflect tissue oxygen consumption. Chapter 5 covers the Young Everest Study 2. Healthy volunteers (8-16 years) who travelled to Nepal and trekked to 3525 meters altitude were tested at two altitudes (sea level and 3525 meters). The aim was to investigate the physiological response to a hypobaric hypoxic environment including the NIRS VOT. Chapter 6 covers studies conducted on critically ill children admitted to the PICU in GOSH. These include serial NIRS VOT study, an in-vivo measure of oxygen consumption of peripheral blood mononuclear cells and the expression of oxidative phosphorylation (OXPHOS) genes changes in the first 48 hours after admission to the PICU in children with meningococcal septic shock. Results: Chapter 3: The epidemiological study of the children admitted to the intensive care unit showed that 40 children had a P aO2 of less than 20 mmHg (2.6 kPa) out of a total 7751 admissions in a 9 year period. Of these, 33 children survived to hospital discharge. There was a U shaped relationship between arterial oxygen tension and mortality. The results from the survey showed a varied practice. Of the total, 21 respondents (42%, 95%CI : 29.3 55.7%) stated that they do not follow specific P aO2 targets. Of the rest, as equal number (21, 42%) aimed for targets between 8.1 (61 mmHg) and 10 kPa (76 mmHg). Only 8(16%) aimed for what would be considered a normal range (10.1 13kP a). A majority of the units (96%) reported having an alarm target on their oxygen saturation monitor. However, 73% of the respondents worked in units that did not have an oxygen weaning protocol for mechanically ventilated patients. In acute respiratory distress syndrome, cardiac arrest and sepsis, there was a tendency to aim for lower P aO2 (< 10 kPa) as the F iO2 increased. Following traumatic brain injury and in pulmonary hypertension, there was a propensity to aim for normal P aO2 (10.1-13 kPa) even as the F iO2 rose (28-33% when F iO2 >0.4). The systematic review identified 11 studies (n=5280) related to hypoxia with combined odds ratio (OR) for death of 3.13 (95%CI: 1.79-5.48, p < 0.001) compared to normoxia. Six studies (n=2012) investigated the effect of hyperoxia and suggest no e↵ect on mortality compared to normoxia: OR 1.15 (95%CI: 0.42-3.17, p = 0.77). Chapter 4: A DropTOI value of 14 was noted to have the least crossover between the mitochondrial disease group and controls. This was empirically set as the cutoff for the 2x2 table. The Fisher exact test statistic was 0.22 and showed that NIRS VOT is unable to differentiate between mitochondrial disease and controls. The NIRS VOT was able to exclude mitochondrial disease in healthy children with a high degree of certainty (positive LR - 5.66, 95%CI:0.84-38, negative LR - 0.61, 95%CI:0.47-0.80). Chapter 5: The SpO2 at rest decreased from 99±0.79 to 89±2.54 % from London to Namche (p=0.0009). The SpO2 after exercise decreased from 93.5 ± 7.16 to 83.3 ± 5.06 % from London to Namche (p=0.008). The heart rate Z-scores at rest changed from 0.44±0.70 to 0.10±0.76 bpm (p=0.30) whilst the heart rate Z-scores after exercise increased from 1.20 ± 1.30 to 2.07 ± 1.46. Cardiac index (at rest) did not show a statistically significant change with an ascent to moderate altitude. Respiratory rate after exercise increased consistently from sea level (22.2 ± 4.4) to Namche Bazaar (27.6 ± 10.9). With a multilevel functional regression analysis, the baseline TOI was significantly lower in Namche Bazaar than London (-4.05, 95%CI: -3.23, -4.86, p < 0.001). The lowest TOI was also significantly lower in Namche Bazaar (-1.95, 95%CI: -1.13, -2.76, p < 0.001). The DropTOI at Namche bazaar compared to sea level was 2.1 points lower (p < 0.0001). Chapter 6: Tissue Oxygen Index varied during the first 5 days of illness. The DropTOI decreased until day 4 (3.02 lower) and showed relative increase on day 5 (0.93 lower) compared to day 1. The net routine control ratio (measured with oroboros analyser) dropped from day 1 to day 5. On the group level analysis (all five patients assessed together), with a false discovery rate (FDR) at < 25% and ranked according to their Normalised Enrichment Score (NES), 1039 out of 3655 gene sets showed a decreasing profile with time. The respiratory electron transport chain gene set was ranked 75th of the 1039 gene sets. Conclusions: The oxygen administration practices in the paediatric intensive care units in the UK vary widely. Of note, admission PaO2 has a ‘U-shaped’ relationship to mortality in children admitted to the paediatric intensive care unit. Healthy children, when exposed to hypobaric hypoxic conditions, show a tendency to decrease oxygen consumption from forearm muscle. There was some evidence that critically ill children demonstrate similar signs of early adaptive response by reducing oxygen consumption.

Abstract

Semaglutide, a GLP-1 receptor agonist, reduces the risk of major adverse cardiovascular events (MACE) in people with overweight or obesity, but the effects of this drug on outcomes in patients with atherosclerotic cardiovascular disease and heart failure are unknown. We report a prespecified analysis of the effect of once-weekly subcutaneous semaglutide 2·4 mg on ischaemic and heart failure cardiovascular outcomes. We aimed to investigate if semaglutide was beneficial in patients with atherosclerotic cardiovascular disease with a history of heart failure compared with placebo; if there was a difference in outcome in patients designated as having heart failure with preserved ejection fraction compared with heart failure with reduced ejection fraction; and if the efficacy and safety of semaglutide in patients with heart failure was related to baseline characteristics or subtype of heart failure.

Abstract

Introduction: This study aims to: (1) describe perioperative complications amongst patients who underwent primary total hip arthroplasty (THA) using a short cementless, titanium, flat, tapered stem; (2) estimate this stem's early- to mid-term survival; (3) identify factors associated with revision arthroplasty; and (4) describe femoral remodelling at minimum 6 years postoperatively. Methods: A retrospective review of consecutive patients who underwent THA using a Taperloc Microplasty stem (Zimmer-Biomet, Warsaw, Indiana, USA) with minimum 2-year follow-up was performed. Surgeries were performed by 1 of 6, non-designer, arthroplasty surgeons between 2014 and 2018. Outcomes included perioperative complications including revision arthroplasty, and survival. Cox analysis was used to analyse the effect of different factors on risk of revision arthroplasty. Radiographs with 6-year follow-up served to describe femoral remodelling. Results: In 1205 patients, followed for 5.1 ± 1.4 years, the incidence of perioperative complication was 5.2% for which 29 patients (2.4%) required revision arthroplasty. The 5- and 7-year survival rates were 97.8% (95% CI, 96.9–98.5) and 97.0% (95% CI, 95.6–98.0), respectively. The only factor associated with revision arthroplasty was proximal femur morphology, as per Dorr classification (HR 1.24 [95%CI, 1.09–1.41]; p = 0.005). During radiographic assessment, 12% of patients showed ⩾25% of relative change in cortical thickness in Gruen zones 3 or 5. We observed calcar remodelling in 50% of radiographs while 10% showed presence of a pedestal sign. Conclusions: The 7-year survivorship of the Taperloc Microplasty stem is within National Institute for Health and Care Excellence (NICE) guidelines. Patients ⩽65 years with osteoarthritis and Dorr A/B femoral morphology may be ideal candidates for THA with this stem. Femoral remodelling is common and not associated with adverse outcome.
