Background: Despite Bell's palsy is not a life-threatening condition, it has a lot of psychological impact, up to now the exact underlying pathophysiology of bell's palsy is not fully understood, so we are trying to recognize the factors that affect its recovery. Methods: This prospective cohort study was executed on 60 patients with Bell's palsy who underwent complete blood count including calculation of neutrophil lymphocyte ratio, evaluation of severity using B-H scale and neurophysiological testing of the facial nerve with evaluation of motor amplitude and degeneration index of the nerve, then follow up was done after three months. Results: 18.3% of our patients had severe bell's palsy, the mean NLR in mild cases was 3.1 ± 0.17 versus 3.71 ± 0.36 for severe cases, high NLR and comorbid diabetes were found to be associated with increased severity of bell's palsy, old age and high NLR were associated with poor recovery after three months. Conclusion: Neutrophil lymphocyte ratio, old age, initial B-H scale, degeneration index and diabetes were found to possess a potent statistical value in predicting short term outcome of bell's palsy. Background: Multiple sclerosis (MS) is considered an immune-mediated disorder with different disease course., Recently, oxidative stress was reported to play a role in the pathogenesis of MS. Aim: to investigate the role of the serum glutathione peroxidase enzyme in the pathogenesis of Multiple sclerosis, especially the relapsing-remitting type (RRMS), and to assess the relationship between it and disease characteristics. Methods: 45 patients with RRMS (23 in relapse and 22 in remission) (18 males and 27 females) and 45 age, sex, and body mass index (BMI)-matched healthy subjects were included in this study. Clinical assessment, general, and neurological examination were done. MS Severity was calculated using the expanded disability status scale (EDSS). Magnetic resonance imaging (MRI) of the brain and spine with gadolinium enhancement and visual evoked potential was done for all MS patients. Serum glutathione peroxidase (GPX) enzyme and routine laboratory tests were assessed. Results: reduced levels of GP in patients compared to controls. No significant difference was detected between the remitting and relapsing MS groups. In addition, there was a statistically significant negative correlation between serum GPx, disease duration, and physical MSIS. Also, an adverse correlation between serum GPx and gadolinium enhancement was recorded. Conclusion: Our study demonstrates a significantly lower level of GPX among patients than among controls; this indicates its role in MS pathogenesis, which might be important for new therapeutic strategies based on an antioxidant approach in MS patients. Background: Multiple sclerosis (MS) is an autoimmune neurodegenerative disease of central nervous system characterized by inflammation, demyelination, as well as neuro-degeneration. The aim of the study is to detect factors affecting the degree of disability in multiple sclerosis patients for early identification and prognosis estimation. Methods: Ninety relapsing remitting multiple sclerosis (RRMS) patients (36 males and 54 females) were included in this retrospective cohort study in the period from February 2020 to October 2022 in Zagazig University hospital and outpatient MS clinic. Full general and neurological assessments were done to all patients. Severity was evaluated by expanded disability status scale (EDSS). Physical and psychological MS impact scales were also done. Magnetic resonance imaging (MRI) for brain and spine were done with gadolinium enhancement; visual evoked potential was done for the patients. Results: Among 90 patients with RRMS, 52 (57.8 %) patients had EDSS ≤3 (group A), while 38 patients (42.2%) had EDSS >3 (group B). Multilogistic regression analysis showed that black holes, total cholesterol (TC) and disease modifying therapy (DMT) use were the independent factors affecting disability in RRMS patients. There was significant positive correlation between EDSS and Visual evoked potential (VEP) latency of RT eye and there was a negative correlation between EDSS and VEP amplitude of the RT eye. Conclusions: Black holes, TC and DMT use were independent factors affecting prognosis of RRMS that help early estimation and intervention. [ABSTRACT FROM AUTHOR]

Background: Epidemiology of MS in Sharkia is not properly recognized. Clinical evaluation including demographic, laboratory and radiological features of MS patients will help to achieve better understanding of the distribution of disease for a good management that offers better quality of life. This clinical study aims at detection and evaluation of multiple sclerosis patients in Sharkia Governorate in Egypt. Methods: This was a retrospective, observational study undertaken in the period between January 2021 and July 2021 in the multiple sclerosis unit (MSU), outpatient clinics of the neurology department, Zagazig University Hospitals. Medical records of all patients were reviewed and data were identified and extracted. Any patient not from or living outside Sharkia Governorate had excluded. Results: The total number of patients recorded in MSU was 258 patients. Female to male ratio was 2.03:1. The mean age (± SD) of onset of the disease was 27 (± 4.3). Family history of MS was reported in 1.94% of patients. RRMS was the most common MS type in our study (77.1%). The overall are suffering from motor symptoms (22.48%), followed by the visual (20.5%) then sensory symptoms (17%). The mean (± SD) EDSS score of our patients was 2.17 (±1.87). Conclusion: This is the first MS register from Sharkia Governorate. The frequency of MS in Sharkia Governorate is 3.376 / 100,000 population. The most common type is RRMS, and the motor symptoms are the most frequent presenting symptoms. Our results are comparable to the results of other literature. [ABSTRACT FROM AUTHOR]
