Veterinary hematology serves as an important screening procedure to assess general health conditions, diagnosis, and treatment of disease. This study aims to interpret and establish a set of hematology reference ranges for Asian Elephants managed by private and government facilities in Nepal. Blood samples from 50 elephants around Chitwan National Park, Sauraha were collected and hematological parameters such as total erythrocyte count and total leukocyte count were determined. The results show that the majority of hematological values were in line with the values previously published by different authors. The mean erythrocyte and leukocyte counts were reported as 3.32±0.93 × 10^6 cell/µL and 10448±335.49 cells/µL respectively. No sex-associated significant difference was observed in the case of total erythrocyte count, whereas total leukocyte counts varied significantly within sexes. Our findings revealed no significant difference in hematological parameters between government and privately owned elephants. The hematological values from our study can be used as reference values for assessing the health condition of elephants in Nepal. Further research work should be conducted to evaluate the factors affecting hematological parameters.

Introduction: Surgical removal of the mandibular third molar has its own set of complications. The mandibular impacted teeth are in proximity to the Inferior Alveolar Nerve (IAN), Buccal Nerve, and Lingual Nerve (LN). Therefore, each of these nerves is always at risk of injury during extraction. Objectives: This study was to evaluate the anatomical risk factors of nerve injury after the surgical extraction of mandibular third molars in patients visiting the department of oral and maxillofacial surgery of People’s Dental College and Hospital. Methods: This prospective study was conducted with 315 participants who presented with a mandibular third molar impaction and underwent Intraoral Periapical Radiograph (IOPAr), panoramic radiograph, as well as Cone Beam, Computed Tomography (CBCT). CBCT was done in those patients in which the mandibular third molar was in close contact with the mandibular canal. Results: Collected data from 315 patients showed that the incidence of Inferior alveolar nerve (IAN) and lingual nerve(LN)injury was 0.31%. Of which one had mesioangular class B, level II type of impaction in 17year male and the other had horizontal class C, level II type of impaction in 47year female respectively. In both cases, the tooth was lingually placed in relation to IAN. Conclusions: Various factors are responsible for nerve injury after the removal of the mandibular third molar. In our study, the incidence of nerve injury to IAN and LN was comparatively low and the most common risk factor was angulation and anatomical position of the impacted mandibular third molar.

Background: Dengue fever is one of the most common tropical diseases caused by an arbo­virus. This virus is a member of genus Flavivirus and flaviviradae family, is transmitted by bite of Aedes aegypti mosquito. Its outbreak is being reported from different parts of the world including Nepal. This disease is prevalent throughout Nepal mostly in terai regions. The aim was to study the epidemiological and clinical spectrum of dengue fever at tertiary care hos­pital in Chitwan. Methods: This study was a hospital based prospective study where a total of 241 patients di­agnosed with dengue fever were assessed. This study was carried out from September 2019 to February 2020. Data was entered on predesigned proforma and analyzed by using SPSS version 16.0. Mean and standard deviations was calculated for continuous variables and frequencies for categorical variables. Results: More than half (56%) of the dengue patients belonged to the age group of 25-44 years. Out of 241 patients, 61.4% were males. Apart from fever the other common symptoms were ret­roorbital pain/headache comprising around 80% of patients. The mean duration of hospital stay in our study was 3.86 days. Background: In the last few decades, the incidence of stroke has doubled in low- and middle-income countries, whereas, during the same period stroke incidence has declined by 42% in high-income countries. The major type of stroke is ischemic, which occurs in about 87% of all stroke cases. According to the Global Burden of Disease (GBD) study in 2010, out of 11 million ischemic strokes occurred, among them, 63% were from a low- and middle-income country. The present study aimed to investigate the clinical presentation and risk factors of stroke patients. Materials and methods: This is a descriptive cross-sectional study. Proforma was designed for socio-demographic factors and common risk factors for stroke. A Chi-square test was conducted for a bivariate relationship between clinical presentation and stroke type. Results: Altogether 50 participants were included in the study. Among them, 58% were age 65 and over,36% belonged to the age group 45 to 65 years and 6% belonged to the age group less than 45 years. Among the risk factors, the most common risk factor was hypertension (50%). Among the clinical presentation aphasia (46%) was the most common presentation, whereas, gait abnormality (6%) and eye gaze impairment (6%) were the least common presentation. Headache and speech abnormality was significantly associated with stroke type. Conclusion: Headache and speech abnormality are significantly associated with different types of stroke.

Background: Globally prevalence of metabolic syndrome ranges from 10 to 84%, depending upon sex, age, race, and ethnicity, region and the definition used. There is limited literature in the prevalence of metabolic syndrome in Nepal. One nationwide survey study reported prevalence of metabolic syndrome is 15% and 16% according to ATP III and IDF criteria respectively. Aims and Objectives: The aims and objective of our study is to study the prevalence of metabolic syndrome and its components in tertiary care center. Material and Methods: This was a descriptive cross-sectional study. We used convenient method of sampling and data collection was done in between May 2019 to July 2019. Bivariate and multivariate logistic regression analysis was done for association of metabolic syndrome with different variables. Results: The total of 362 participants was included in the study. Among them 54.4% were 53 years and above and 45.6% were below 53 years of age. Among all participants 25% were diabetic and 55% were hypertensive. Among all participants metabolic syndrome was found in 45.3%. All components of metabolic syndrome were found to be significantly associated with metabolic syndrome both in bivariate and multiple regression analysis. Conclusion: The metabolic syndrome is very prevalent in our population. These findings prompt the concerned authorities to formulate strategies to prevent the risk factors.

This paper describes the application of Deep Mixing Method (DMM) for the mitigation of potential hazards induced by natural disasters such as hurricane-induced ground flooding and seismic ground shaking for two projects in the USA. The two projects completed in 2014 and 2016 are used as case examples to present the design, construction, and quality control of the DMM work for hazard mitigation. DMM was installed in the first project called P-17A primarily to reinforce soft soils within and below a levee to provide resistance against lateral loads that will be induced from high flood generated by hurricane. The second project called Perris Dam included the use of DMM to improve potentially liquefiable soils within the foundation of an earthen dam primarily to limit the deformation of the dam during a seismic event.

Objectives: Acute kidney injury is one of the most common cause of hospitalization in developing countries. Causes of AKI are multifactorial. Most of AKI are community acquired. The objective of the study was to identify the clinical profile and outcome of acute kidney disease. Subject and Methodology: 30 patients from Chitwan Medical College outpatient clinic were included in the study from November 2014 to April 2015. A brief history and clinical examinations were taken from all patients along with laboratory tests for Renal function tests, urine output, metabolic parameters and hematological profile. Results: 19 males (63.3% and 11 females (36.7%) were studied. The main causes for AKI were sepsis (46.6%) followed by hepatic causes (16.6%), gastroenteritis (10%) and others (10.2%). Out of 30 patients, 19 recovered (63.3%) and were discharged and 11(36.7%) died. Most of the deaths were in injury (37.5%) and failure (42.8%) stages of RIFLE criteria. Out of 19 recovered 16(84.21%) patients did not need any renal replacement therapy whereas 3(15.8%) patients had to undergo hemodialysis. Conclusion: Early identification of kidney injury may lead to lesser renal replacement therapy and better prognosis. However late presentations of AKI have higher hospital mortality rate.

Parotid gland is a rare site of metastasis from gastrointestinal (GI) tract primary cancer. Here, we present an interesting case report of parotid metastasis from unknown primary with probable lower GI tract as a primary site of cancer, with a normal colonoscopy and upper GI endoscopy. This case report highlights the importance of immunohistochemistry in taking treatment decisions during dilemmatic situations.

Myiasis is occurs in humans and vertebrate animals mostly due to dipterous flies of the genera Chrysomyia and Cochliomyia. There have been reported cases of myiasis of skin, breast, intestine, eyes and ear but very few cases of vulvar myiasis have been reported in the medical literature. We report a case of a 20 years old female with vulvar myiasis. A 20-year old unmarried girl presented to our hospital with complains of vaginal discharge and vulvar irritation for 3 days with evidence of some worms around her vulvar region. Her menstrual history revealed the use of folded cloths instead of sanitary pads during her menstruation. Moreover, she washed those clothes with water and hanged them in the clothesline outside to let them dry before reuse. Examination of the external genitalia multiple ulcers measuring 1X1 cm on the inner surface of the labia minora, heavily infested with large number of maggots. The patient was admitted for 4 days during which she was given antibiotics, analgesics and anti-inflammatory medication. Daily local asepsis of the wound with antiseptic solution (betadine) was done. We removed more than 30 larvae by the fourth day after which no more larvae were seen and she gradually recovered in the following days. It is our opinion that the cause of vulvar myiasis in our patient was her poor sanitary hygiene. Simple measures such as washing clothes thoroughly, drying and ironing of clothes are also necessary to reduce the risk of this human myiasis. Keywords: external genitalia; female; maggots; myiasis.

Factors that affect the engineering properties of cement stabilized soils such as strength are discussed in this paper using data on these factors. The selected factors studied in this paper are initial soil water content, grain size distribution, organic matter content, binder dosage, age and curing temperature, which has been collated from a number of international deep mixing projects. Some resulting correlations from this data are discussed and presented. The concept of Artificial Neural Networks and its applicability in developing predictive models for deep mixed soils is presented and discussed using a subset of the collated data. The results from the neural network model were found to emulate the known trends and reasonable estimates of strength as a function of the selected variables were obtained. © 2012 American Society of Civil Engineers.

Relatively new in the UK, soil mix technology applied to the in-situ remediation of contaminated land involves the use of mixing tools and additives to construct permeable reactive in-ground barriers and low-permeability containment walls and for hot-spot soil treatment by stabilisation/ solidification. It is a cost effective and versatile approach with numerous environmental advantages. Further commercial advantages can be realised by combining this with ground improvement through the development of a single integrated soil mix technology system which is the core objective of Project SMiRT (Soil Mix Remediation Technology). This is a large UK-based R&D project involving academia-industry collaboration with a number of tasks including equipment development, laboratory treatability studies, field trials, stakeholder consultation and dissemination activities. This paper presents aspects of project SMiRT relating to the laboratory treatability study work leading to the design of the field trials. © 2012 American Society of Civil Engineers.

This paper introduces current work in collating data from different projects using soil mix technology and establishing trends using artificial neural networks (ANNs). Variation in unconfined compressive strength as a function of selected soil mix variables (e.g., initial soil water content and binder dosage) is observed through the data compiled from completed and on-going soil mixing projects around the world. The potential and feasibility of ANNs in developing predictive models, which take into account a large number of variables, is discussed. The main objective of the work is the management and effective utilization of salient variables and the development of predictive models useful for soil mix technology design. Based on the observed success in the predictions made, this paper suggests that neural network analysis for the prediction of properties of soil mix systems is feasible. © ASCE 2011.
