4. A review of the impact of sensory processing sensitivity on mental health in university students

Abstract

Introduction The concept of sensory processing sensitivity (SPS) was first introduced by Aron and Aron (1996) as an innate trait characterized by heightened processing of sensory, emotional, and physical stimuli. Since the concept’s introduction in 1996, high SPS has been shown to be associated with poor physical and mental health. It is possible that this is especially true in university students, who are frequently faced with numerous stressors, such as intense workloads and test anxiety. Methods This article is a systematic literature review conducted through EBSCOHost using the following databases: Academic Search Complete, APA PsycArticles, APA PsycInfo, Education Research Complete, ERIC, MEDLINE Complete, and SocINDEX. Search terms included terms regarding high sensory processing sensitivity, university or professional students, and mental health. Results A total of 6 studies were included. University students with high SPS experienced heightened reactions to sensory, emotional, and physical stimuli. The studies demonstrate correlations of high SPS with outcomes such as depressive tendencies and difficulty adjusting to college. Discussion These findings highlight that SPS is associated with poor mental health outcomes. The results underscore the importance of developing support methods for students with high SPS. Future studies should further explore SPS in university students to develop targeted support methods and programs.

5. Nonmedical Use of Prescription Stimulants and Nicotine among Community College Students

Abstract

Objective: Despite community colleges accounting for 34% of all undergraduate enrollment, research on substance-use patterns among community college students is limited. Community college students may engage in substance use differently than their 4-year university counterparts due to differences in psychosocial factors and decreased availability of mental health services. The current study aimed to elucidate risk factors underlying non-medical use of prescription simulants (NMUS) and nicotine use by community college students. Methods: A web-based survey was administered to 10 of 13 community colleges within a southeastern state's Board of Regents school system. The survey included questions related to NMUS, nicotine use, alcohol use, mental health diagnosis, and demographics. Results: Overall, 9% of the participants reported NMUS, and 24.6% used nicotine. Multivariate analysis of variance and ?[subscript 2] tests revealed group differences among individuals using only nicotine, only NMUS, both nicotine and NMUS, and neither nicotine nor NMUS. Post-hoc 2 × 2 ?[subscript 2] tests indicated that individuals using both nicotine and NMUS had higher incidence of mental health diagnoses, were more likely to live in urban areas, reported higher weekly alcohol consumption, and were more likely to be male as compared to individuals using neither substance. Attention-deficit hyperactivity disorder (ADHD) symptoms were higher in individuals using only NMUS and both NMUS and nicotine as compared to those using only nicotine or neither substance. Conclusions: These findings provide insight into demographic and psychological variables associated with NMUS and nicotine use among community college students that could be benefitted by greater access to affordable mental health services.

Abstract

Background: Individual sleep and activity patterns show large variations and are interfered considerably by social schedules. Social jetlag (SJL) is the difference between intrinsic circadian rhythm and extrinsically enforced sleep-wake cycle. However, little is known about the variables affecting the severity of SJL. Methodology: We evaluated whether sleep- or smartphone-related variables affected the severity of SJL among college students in India. A total of 1175 students from medicine, dental, engineering, paramedical, and other colleges in Gujarat, India, completed a web-based survey. The survey included demographic questions and questions from the Smartphone Addiction Scale-Short Version (SAS-SV), reduced Horne and Ostberg Morningness-Eveningness Questionnaire (rMEQ), and Munich Chronotype Questionnaire (MCTQ). The responses to the MCTQ determined SJL scores. Results: Outcomes from multiple linear regression analysis indicated that the sleep length on free-day (B = 0.42), chronotypes (B = 0.44, B2 = 0.40) maximum smartphone usage time after waking up (B = 0.92), smartphone addiction severity (B = ‒0.01) and free-day sleep onset range (B = ‒0.02) significantly predicted SJL scores (P < 0.03). The SJL severity was 0.42 and 0.40 units greater in individuals with morning-type and evening-type, respectively, compared to the neutral-type rMEQ category. The SJL severity was 0.92 units greater in individuals whose smartphone usage was maximum right after waking up compared to those whose usage was maximum during other times of the day. Every unit increase in SAS score decreased SJL by 0.01 units. Conclusion: These results indicate that SJL severity is affected by several factors, which can be targeted for developing interventions for reducing SJL among college students in India.

Abstract

The scope of the research is to compare conventional concrete and the 10% Novel E-waste replaced concrete material of M30 grade concrete. The composition of various items present in concrete mix are segregated with Novel E-waste 20mm, superplasticizer, cement (53 grade PPC), fine aggregate (sieve size 4.75mm), coarse aggregate (sieve size 20mm), and water. In this research the main set of events are grouped in two categories. Group-1 (control group/conventional concrete) and group-2 (experimental group/Novel E-waste concrete). In this project first group has 18 samples and the Second group has 18 samples, Total samples of N=36 The analysis was performed with a G-power of 0.8, alpha and beta values set at 0.05 and 0.2 respectively, and a 95% confidence interval. The independent sample T-Test yielded a value of (p>0.05), specifically (p=0.256). The One-way ANOVA conducted using SPSS confirmed differences in the mechanical properties between conventional concrete and Novel E-waste Concrete. The average compressive strength for conventional concrete was found to be (31.12 N/mm2), while for Novel E-waste concrete it was (34.15 N/mm2). A significant difference was noted with a significance value of (p<0.05), precisely (p=0.000), and standard deviations of 0.65 and 0.79, respectively. It was determined that Novel E-waste concrete, when mixed with a superplasticizer, exhibited a lower compressive strength percentage compared to conventional concrete. [ABSTRACT FROM AUTHOR]

Abstract

BackgroundMethamphetamine (MA) use disorders are major public health problems nationally and worldwide and treatment remains an unmet need.Objectives(1) To review preclinical and clinical studies identifying the dopamine D3 receptor as a therapeutic target for substance use disorders (SUDs), including MA dependence, (2) to consider buspirone (Buspar®) as a potential medication based on its dopamine D3 receptor antagonist properties, and (3) to evaluate the safety and initial efficacy of buspirone in a pilot study of MA-dependent individuals.MethodsLiterature on the dopamine D3 receptor as a therapeutic target and on the potential of buspirone as a novel therapy for MA dependence was reviewed. The cardiovascular and subjective effects of intravenous MA challenge were assessed in five non-treatment seeking individuals. Participants met DSM-IV criteria for MA dependence and were treated subacutely (9 days) with buspirone (60 mg daily).ResultsThe literature identified the dopamine D3 receptor as a therapeutic target for MA dependence, a safe and approved medication, and a valuable opportunity to re-purpose buspirone for treating MA dependence and perhaps other SUDs. Pilot data (n = 5) indicated that buspirone is safe in MA-using individuals and comparison against historical placebo data from this laboratory suggested that at least some aspects of the subjective properties of MA may be diminished during buspirone treatment.ConclusionFuture studies should include a small-scale, placebo-controlled Phase IIa trial of buspirone in MA dependence.

One relatively unexplored approach is modulation of cholinergic system function. Animal research suggests that enhancement of central cholinergic activity, possibly at nicotinic acetylcholine receptors (nAChRs), can reduce methamphetamine-related behaviors. Further, preliminary findings indicate that rivastigmine, a cholinesterase inhibitor, may reduce craving for methamphetamine after administration of the drug in human subjects. We therefore performed a double-blind, placebo-controlled, crossover pilot study of the safety and tolerability of varenicline in eight methamphetamine-dependent research subjects. Varenicline is used clinically to aid smoking cessation, and acts as a partial agonist at α4β2 nAChRs with full agonist properties at α7 nAChRs. Oral varenicline dose was titrated over 1 week to reach 1 mg bid, and then was co-administered with 30 mg methamphetamine, delivered in ten intravenous infusions of 3 mg each. Varenicline was found to be safe in combination with IV methamphetamine, producing no cardiac rhythm disturbances or alterations in vital sign parameters. No adverse neuropsychiatric sequelae were detected either during varenicline titration or following administration of methamphetamine. The results suggest that varenicline warrants further investigation as a potential treatment for methamphetamine dependence.

Abstract

Postgraduate pharmacy residency and fellowship positions have remained competitive. We evaluated factors predicting students' pursuit and attainment of postgraduate pharmacy training positions.A web-based survey was administered to students from a three-year accelerated pharmacy program. The survey asked questions regarding the participants' demographics, cumulative academic pharmacy grade point average (GPA), school leadership positions in organizations or committees, and research activities. Students' resilience was measured by the Academic Pharmacy Resilience Scale questionnaire. Multiple logistic regression was used to determine variables that predicted the pursuit and attainment of residency positions.The survey response rate was 46.7%. Students with greater cumulative GPA (odds ratio [OR] 6.3; 95% CI, 2.25-17.68), research experience (OR 3.3; 95% CI, 1.29-8.45), resilience scores (OR 1.07; 95% CI, 1.03-1.12), and leadership in an organization (OR 3.27; 95% CI, 1.46-7.33) or school committee (OR 2.29; 95% CI, 1.04-5.07) were more likely to apply to a residency program. Students with greater cumulative GPA (OR 9.93; 95% CI, 1.33-74.23), self-rated performance score in the residency interview (OR 5.32; 95% CI, 2.47-11.44), and leadership experience on a school committee (OR 15.37; 95% CI, 3.94-59.93) were more likely to match with a residency program. The average scores on interview performance and networking were significantly higher in students who obtained a fellowship position compared to those who did not obtain that.This study identified several predictors for the pursuit and attainment of residency or fellowship positions, which could inform pharmacy educators, students, and program directors.

36. Single-Incision Multi-Port Appendectomy for a Patient with Situs Inversus Totalis : First case report

Abstract

Situs inversus totalis (SIT) is a rare autosomal recessive condition involving the complete lateral transposition of the organs. When individuals with this condition suffer from appendicitis, associated pain and symptoms are usually present on the left side, resulting in diagnostic difficulties. Moreover, the laparoscopic removal of the left-sided appendix may pose practical problems during surgery. Removal of an inflamed appendix is generally performed using a multiple-port laparoscopy. We report a 22-year-old male who presented to the Lifeline Institute of Minimal Access Surgery in Chennai, India, in April 2015 with pain in the left iliac fossa. Chest X-rays and ultrasonography confirmed SIT with an acutely inflamed appendix on the left side. The patient underwent a single-incision multi-port laparoscopic appendectomy with a successful outcome. To the best of the authors’ knowledge, this is the first report in the literature of a single-incision multi-port appendectomy in a patient with SIT.

Abstract

Background Postoperative pain management solely with opioids elevates the risk of opioid-related adverse events during hospitalization and after discharge from the hospital. Clinical trials have demonstrated gabapentinoids as viable adjunctive treatments for spinal surgeries. However, only a few practice-based studies have examined the efficacy of gabapentin as an opioid-sparing agent for patients undergoing lumbar laminectomy in rural hospital settings. Objective To determine the effects of gabapentin on opioid consumption and pain perception in patients who underwent lumbar laminectomy at a rural community hospital. Methods Data were collected by retrospective chart reviews of 99 patients who underwent lumbar laminectomy at Yavapai Regional Medical Center from January 1, 2017, to July 1, 2019. The patients were stratified into 2 groups: those who were taking gabapentin as outpatients before surgery and were continued on the same dose postoperatively (n = 50, gabapentin group) and those who were not taking gabapentin preoperatively or postoperatively (n = 49, usual-treatment group). The primary end points were opioid consumption in morphine milligram equivalents (MME) and pain for 24 hours postsurgery. Results Outcomes from the mixed-model analysis of variance showed significant main effects of group and time for opioid consumption in MME (F1,97 = 4.3, P Conclusion Patients on gabapentin showed reductions in pain perception and postoperative opioid consumption. The results extend the findings from randomized trials to a real-world clinical setting. These data support using gabapentin in conjunction with opioids for pain management of patients undergoing lumbar laminectomy.
