The process of collecting blood from donors and making it available for transfusion requires a complex series of operations involving multiple actors and resources at each step. Ensuring hospitals receive adequate and safe blood for transfusion is a common challenge across low- and middle-income countries, but is rarely addressed from a system level. This paper presents the first use of discrete event simulation to study the blood system in Kenya and to explore the effect of variations and perturbations at different steps of the system on meeting patient blood demand. A process map of the Kenyan blood system was developed to capture critical steps from blood donation to transfusion using interviews with blood bank, hospital, and laboratory personnel at four public hospitals across three counties in Kenya. The blood system was simulated starting with blood collection, a blood bank where blood is tested and stored before it is issued, a major hospital attached to the blood bank, and several smaller hospitals served by the same blood bank. Values for supply-side parameters were based mainly on expert opinion; demand-side parameters were based on data from blood requisitions made in hospital wards, and dispatch of blood from the hospital laboratory. Illustrative examples demonstrate how the model can be used to explore the impacts of changes in blood collection (e.g., prioritising different donor types), blood demand (e.g., differing clinical case mix), and blood distribution (e.g., restocking strategies) on meeting demand at patient level. The model can reveal potential process impediments in the blood system and aid in choosing strategies for improving blood collection, distribution or use. Detecting changes in high-dimensional vectors presents significant challenges, especially when the post-change distribution is unknown and time-varying. This paper introduces a novel robust algorithm for correlation change detection in high-dimensional data. The approach utilizes the summary statistic of the maximum magnitude correlation coefficient to detect the change. This summary statistic captures the level of correlation present in the data but also has an asymptotic density. The robust test is designed using the asymptotic density. The proposed approach is robust because it can help detect a change in correlation level from some known level to unknown, time-varying levels. The proposed test is also computationally efficient and valid for a broad class of data distributions. The effectiveness of the proposed algorithm is demonstrated on simulated data.

The Traveling Salesman Problem is one of the most intensively studied combinatorial optimization problems due both to its range of real-world applications and its computational complexity. When combined with the Set Covering Problem, it raises even more issues related to tractability and scalability. We study a combined Set Covering and Traveling Salesman problem and provide a mixed integer programming formulation to solve the problem. Motivated by applications where the optimal policy needs to be updated on a regular basis and repetitively solving this via MIP can be computationally expensive, we propose a machine learning approach to effectively deal with this problem by providing an opportunity to learn from historical optimal solutions that are derived from the MIP formulation. Vaccination has been proven to be the most effective method to prevent infectious diseases. However, in many low and middle-income countries with geographically dispersed and nomadic populations, last-mile vaccine delivery can be extremely complex. Because newborns in remote population centers often do not have direct access to clinics and hospitals, they face significant risk from diseases and infections. An approach known as outreach is typically utilized to raise immunization rates in these situations. A set of these remote locations is chosen, and over an appropriate planning period, teams of clinicians and support personnel are sent from a depot to set up mobile clinics at these locations to vaccinate people there and in the immediate surrounding area. In this paper, we model the problem of optimally designing outreach efforts as a mixed integer program that is a combination of a set covering problem and a vehicle routing problem. In addition, because elements relevant to outreach (such as populations and road conditions) are often unstable and unpredictable, we address uncertainty and determine the worst-case solutions. This is done using a multi-period stochastic modeling approach that considers updated model parameter estimates and revised plans for subsequent planning periods. We also conduct numerical experiments to provide insights on how demographic characteristics affect outreach planning and where outreach planners should focus their attention when gathering data.

The World Health Organization's Expanded Programme on Immunization (WHO-EPI) was developed to ensure that all children have access to common childhood vaccinations. Unfortunately, because of inefficient distribution networks and cost constraints, millions of children in many low and middle-income countries still go without being vaccinated. In this paper, we formulate a mathematical programming model for the design of a typical WHO-EPI network with the goal of minimizing costs while providing the opportunity for universal coverage. Vaccination has been proven to be the most effective method to prevent infectious diseases. However, there are still millions of children in low and middle-income countries who are not covered by routine vaccines and remain at risk. The World Health Organization - Expanded Programme on Immunization (WHO-EPI) was designed to provide universal childhood vaccine access for children across the world and in this work, we address the design of the distribution network for WHO-EPI vaccines. In particular, we formulate the network design problem as a mixed integer program (MIP) and present a new algorithm for typical problems that are too large to be solved using commercial MIP software. We test the algorithm using data derived from four different countries in sub-Saharan Africa and show that the algorithm is able to obtain high-quality solutions for even the largest problems within a few minutes.

In recent years there has been a strong interest on the part of universities, particularly in Latin America and Southeast Asia, to increase the percentage of their engineering faculty with doctoral degrees. Since there are relatively few doctoral programs in engineering in these countries, many universities would like to upgrade their current faculty by encouraging them to obtain doctoral degrees from nationally ranked schools in the U.S. or Europe. Unfortunately, given their teaching workloads and faculty shortages it is extremely difficult for these universities to grant their faculty a protracted leave of absence of four or more years to complete their education abroad. In this paper, authors present and discuss alternative models for doctoral programs designed for such faculty, that are flexible enough to accommodate their unique circumstances while at the same time maintain academic rigor. The key factors enabling such an approach are the growth in distance-education technology, the Internet and sophisticated means of rapid and interactive communications. Authors detail a number of academic issues, along with several alternative strategies for the actual delivery of these programs. A typical doctoral program is dissected into its components and models for flexible programs within each are detailed. Finally, a model program developed at the University of Pittsburgh is described.
