Background: Hepatitis C virus (HCV) is a global prevalent pathogen causes both acute and chronic hepatitis and leading to serious liver damage. Correct and rapid diagnosis is pivotal for the management of HCV disease. Rapid card tests are superior alternatives for the large-scale screening of HCV infection. Methods: The present observational study evaluates analytical performance of four different anti-HCV rapid tests. A total of 200 ELISA confirmed, HCV positive (n=100) and HCV negative (n=100) clinical specimens were selected and re-tested for anti-HCV antibodies by using commercially available four different immunochromatography cards (Meriscreen, Accurate, Oscar and Biolab). Results: Among all, Biolab rapid card test shown highest (98%) sensitivity. On the other hand, all rapid card test kits showed identical 100% specificity. Conclusions: Overall BioLab anti-HCV rapid card tests found to be superior in the present study and strongly suggest in house validation of rapid card tests before their diagnostics use on clinical specimens.

We study the observational implications of a class of inflationary models wherein the inflaton is coupled to the Einstein tensor through a generalized non-minimal derivative coupling (GNMDC). In particular, we explore whether these models can generate suitable features in the primordial spectrum of curvature perturbations as a possible explanation for the large-scale anomalies associated with the angular power spectrum of CMB temperature anisotropies. We derive model-independent constraints on the GNMDC function for such a scenario, considering both the scalar and tensor perturbations. We modify CosmoMC to accommodate our GNMDC framework and investigate different classes of inflationary models using a fully consistent numerical approach. We find that the hilltop-quartic model with a specific choice of the GNMDC function provides a considerable improvement over the best-fit reference $\Lambda$CDM model with a nearly scale-invariant power spectrum. While the large-scale structure observations should be able to provide independent constraints, future CMB experiments, such as CMB-S4 and CMB-Bharat, are expected to constrain further the parameter space of such beyond canonical single-field inflationary models., Comment: v1: 34 pages, 7 figures, 4 tables; v2: 26 pages, 8 figures, 4 tables, published in Phys. The present study was conducted to determine an optimum cut-off IgM/IgG ratio for differentiating between primary and secondary Dengue. The calculated cut-off was used to estimate the relative prevalence of primary and secondary Dengue in central India during the 2021 outbreak.To differentiate between primary and secondary Dengue, optimum cut-off IgM/IgG ratio was determined using ROC curve analysis and Area under the curve (AUC) was calculated. For this 84 IgM positive samples were selected during the study period and Dengue IgM and IgG testing was performed.Based on sero-positivity for IgG antibodies, we observed a prevalence of 77.4% for secondary Dengue during this outbreak. IgM/IgG ratio of 1.59 delivered a sensitivity of 100% and specificity of 90.8%. Comparison of clinical characteristics revealed significant differences in rigors (p ​= ​0.04) and haemorrhagic manifestations (p ​= ​0.01) between the two types of infection.This study revealed an overwhelming predominance of secondary Dengue during the reported outbreak, which hints at increasing endemicity in this region and raises the risk of severe clinical manifestations. An optimal IgM/IgG cut-off ratio, tailored according to the endemicity of this geographical region, is therefore suggested in this study.

Coronary atherosclerosis results from the delicate interplay of genetic and exogenous risk factors, principally taking place in metabolic organs and the arterial wall. Here we show that 224 gene-regulatory coexpression networks (GRNs) identified by integrating genetic and clinical data from patients with (n = 600) and without (n = 250) coronary artery disease (CAD) with RNA-seq data from seven disease-relevant tissues in the Stockholm–Tartu Atherosclerosis Reverse Network Engineering Task (STARNET) study largely capture this delicate interplay, explaining >54% of CAD heritability. Within 89 cross-tissue GRNs associated with clinical severity of CAD, 374 endocrine factors facilitated inter-organ interactions, primarily along an axis from adipose tissue to the liver (n = 152). This axis was independently replicated in genetically diverse mouse strains and by injection of recombinant forms of adipose endocrine factors (EPDR1, FCN2, FSTL3 and LBP) that markedly altered blood lipid and glucose levels in mice. Altogether, the STARNET database and the associated GRN browser (http://starnet.mssm.edu) provide a multiorgan framework for exploration of the molecular interplay between cardiometabolic disorders and CAD.

[Abridged] As is well known, in order to generate magnetic fields of observed amplitudes during inflation, the conformal invariance of the electromagnetic field has to be broken by coupling it either to the inflaton or to the scalar curvature. Couplings to scalar curvature pose certain challenges even in slow roll inflation and it seems desirable to consider couplings to the inflaton. It can be shown that, in slow roll inflation, to generate nearly scale invariant magnetic fields of adequate strengths, the non-conformal coupling to the inflaton has to be chosen specifically depending on the inflationary model at hand. In a recent work, we had found that, when there arise sharp departures from slow roll inflation leading to strong features in the scalar power spectra, there inevitably arise sharp features in the spectra of the electromagnetic fields, unless the non-conformal coupling functions are extremely fine tuned. In particular, we had found that, if there occurs an epoch of ultra slow roll inflation, then the strength of the magnetic field over large scales can be severely suppressed. In this work, we examine whether these challenges can be circumvented in models of inflation involving two fields. We show that the presence of the additional scalar field allows us to construct coupling functions that lead to magnetic fields of required strengths even when there arise intermediate epochs of ultra slow roll inflation. However, we find that the features in the spectra of the magnetic fields that are induced due to the departures from slow roll inflation cannot be completely ironed out. Primordial magnetic fields are generated during inflation by considering actions that break the conformal invariance of the electromagnetic field. To break the conformal invariance, the electromagnetic fields are coupled either to the inflaton or to the scalar curvature. Also, a parity violating term is often added to the action in order to enhance the amplitudes of the primordial electromagnetic fields. In this work, we examine the effects of deviations from slow roll inflation on the spectra of non-helical as well as helical electromagnetic fields. We find that, in the case of the coupling to the scalar curvature, there arise certain challenges in generating electromagnetic fields of the desired shapes and strengths even in slow roll inflation. When the field is coupled to the inflaton, it is possible to construct model-dependent coupling functions which lead to nearly scale invariant magnetic fields in slow roll inflation. However, we show that sharp features in the scalar power spectrum generated due to departures from slow roll inflation inevitably lead to strong features in the power spectra of the electromagnetic fields. Moreover, we find that such effects can also considerably suppress the strengths of the generated electromagnetic fields over the scales of cosmological interest. We illustrate these aspects with the aid of specific inflationary models that have been considered to produce specific features in the scalar power spectrum. Further, we find that, in such situations, if the strong features in the electromagnetic power spectra are to be undone, the choice of the coupling function requires considerable fine tuning. Primordial electromagnetic fields can strongly affect the cosmic evolution of axions, and vice versa. We show that if helical electromagnetic fields are coherently produced in the early universe, their remnants source a field velocity to the coupled axions and enhance the relic abundance of axion dark matter. We discuss the implications for the QCD axion and axion-like particles that are coupled to the SM or hidden gauge groups. For a QCD axion coupled to hidden photons, we find that the conventional window for the axion decay constant $10^{8}\, \mathrm{GeV} \lesssim f \lesssim 10^{12}\, \mathrm{GeV}$ can be completely closed due to overproduction of axion dark matter by helical electromagnetic fields as little as $\alpha \, \Delta N_{\mathrm{eff}} \gtrsim 10^{-12}$, where $\alpha$ is the gauge coupling and $\Delta N_{\mathrm{eff}}$ is the effective extra relativistic degrees of freedom of the hidden photons., Comment: 24 pages, 3 figures, v2: published in JCAP

In all inflationary scenarios of primordial black holes (PBH) formation, amplified scalar perturbations inevitably accompany an induced stochastic gravitational waves background (ISGWB) at smaller scales. In this paper, we study the ISGWB originating from the inflationary model, introduced in our previous paper [1] wherein PBHs can be produced with a nearly monochromatic mass fraction in the asteroid mass window accounting for the total dark matter in the universe. We numerically calculate the ISGWB in our scenario for frequencies ranging from nanoHz to KHz that covers the observational scales corresponding to future space based GW observatories such as IPTA, LISA, DECIGO and ET. Interestingly, we find that ultralight PBHs ($M_{\rm PBH} \sim 10^{-20} M_\odot$) which shall completely evaporate by today with exceedingly small contribution to dark matter, would still generate an ISGWB that may be detected by a future design of the ground based Advanced LIGO detector. Using a model independent approach, we obtain a stringent lower mass limit for ultralight PBHs which would be valid for a large class of ultra slow roll inflationary models. Further, we extend our formalism to study the imprints of a reheating epoch on both the ISGWB and the derived lower mass bound. We find that any non-instantaneous reheating leads to an even stronger lower bound on PBHs mass and an epoch of a prolonged matter dominated reheating shifts the ISGWB spectrum to smaller frequencies. In particular, we show that an epoch of an early matter dominated phase leads to a secondary amplification of ISGWB at much smaller scales corresponding to the smallest comoving scale leaving the horizon during inflation or the end of inflation scale. We study the generation of primordial black holes (PBH) in a single field inflection point model of inflation wherein the effective potential is expanded up to the sextic order and the inversion symmetry is imposed such that only even powers are retained in the potential. Such a potential allows the existence of an inflection point which leads to a dynamical phase of ultra slow roll evolution, thereby causing an enhancement of the primordial perturbation spectrum at smaller scales. Working with a quasi-inflection point in the potential, we find that PBHs can be produced in our scenario in the asteroid-mass window with a nearly monochromatic mass fraction which can account for the total dark matter in the universe. For different choices of quasi-inflection points and other parameters of our model, we can also generate PBHs in higher mass windows but the primordial spectrum of curvature perturbations becomes strongly tilted at the CMB scales. Moreover, we study the effects of a reheating epoch after the end of inflation on the PBHs mass fraction and find that an epoch of a matter dominated reheating can shift the mass fraction to a larger mass window as well as increase their fractional contribution to the total dark matter even for the case of a monochromatic mass fraction., v1: 36 pages, 8 figures; v2: 33 pages, 7 figures, version accepted in JCAP

Introduction: The COVID-19 pandemic has resulted in an increased need for molecular diagnostics. Ribonucleic acid (RNA) extraction is a crucial step in the detection of severe acute respiratory syndrome coronavirus-2 (SARS-COV-2). The resulting outcome depends on the performance of the RNA extraction kit used. Methods: This study evaluated the efficiency of four different commercial RNA extraction kits for the detection of SARS-COV 2. The reproducibility and performance efficiency of each kit were assessed and compared. The performance, processing time and storage conditions were also considered in the evaluation. Results: A slightly lesser cycle threshold (Ct) values were observed in RNA extracted using HiPurA Viral RNA Purification Kit. Invitrogen PureLink RNA Mini Kit was found to be time-saving and user-friendly. Conclusions: All the RNA isolation kit yielded the best results; however, if time is the important considerations, Invitrogen PureLink RNA Mini Kit has the shortest processing time.

Adenocarcinoma of oropharynx is a rarely encountered entity. It may be primary or secondary as metastasis from a distant site. Secondary adenocarcinoma of base of tongue is even more uncommon, few cases have been reported in the literature till date including one from stomach as a primary site. We are presenting a case of secondary adenocarcinoma of base of tongue metastasised from stomach after one and half years of successful treatment consisting of surgery and adjuvant chemotherapy.

Models of spontaneous baryogenesis have an interaction term $\partial_\mu\theta j^\mu_B$ in the Lagrangian, where $j^\mu_B$ is the baryonic current and $\theta$ can be a pseudo-Nambu-Goldstone boson. Since the time component of this term, $\dot{\theta} j^0_B$, equals $\dot{\theta} n_B$ for a spatially homogeneous current, it is usually argued that this term implies a splitting in the energy of baryons and antibaryons thereby providing an effective chemical potential for baryon number. In thermal equilibrium, one {then obtains} $n_B \sim \dot{\theta} T^2$. We however argue that a term of this form in the Lagrangian does not contribute to the single particle energies of baryons and antibaryons. We show this for both fermionic and scalar baryons. But, similar to some recent work, we find that despite the above result the baryon number density obtained from a Boltzmann equation analysis can be proportional to $\dot{\theta} T^2$. Type 2 diabetes mellitus (T2DM) is typically progressive, with sequential addition of therapies often needed to address increasing hyperglycemia over the disease course. Using treatments in combination may be preferred to sequential addition, as a means of providing a more rapid clinical response and potentially avoiding clinical inertia. In such cases, a single-pill combination can help to reduce pill burden. Although various single-pill combinations of oral glucose-lowering agents are available, empagliflozin/linagliptin was the first approved combination of a sodium glucose co-transporter 2 (SGLT2) inhibitor with a dipeptidyl peptidase 4 (DPP-4) inhibitor in the United States. Areas covered: Two publications of the clinical trial investigating the efficacy and safety of single-pill combinations of empagliflozin/linagliptin in treatment-naive or metformin-treated patients with T2DM (NCT01422876) are reviewed, and their potential impact on clinical practice is discussed. Expert opinion: The study discussed provides evidence for the efficacy and safety of empagliflozin/linagliptin single pills. Addition of an empagliflozin/linagliptin single pill may be considered in patients with inadequate glycemic control on metformin, or as an alternative to first-line treatment with empagliflozin or linagliptin when metformin is not suitable, particularly in patients with very poor glycemic control, or those who need to achieve target more quickly.

Leading order gravitational corrections to the Einstein-Hilbert action can lead to a consistent picture of the universe by unifying the epochs of inflation and dark energy in a single framework. While the leading local correction induces an inflationary phase in the early universe, the leading non-local term leads to an accelerated expansion of the universe at the present epoch. We argue that both the leading UV and IR terms can be obtained within the framework of a covariant effective field theory of gravity. The perturbative gravitational corrections therefore provide a fundamental basis for understanding a possible connection between the two epochs., 5 pages, 2 figures. Recently, we had numerically shown that, for a non-minimal coupling that is a simple power of the scale factor, scale invariant magnetic fields arise in a class of bouncing universes. In this work, we {\it analytically} evaluate the spectrum of magnetic and electric fields generated in a sub-class of such models. We illustrate that, for cosmological scales which have wavenumbers much smaller than the wavenumber associated with the bounce, the shape of the spectrum is preserved across the bounce. Using the analytic solutions obtained, we also illustrate that the problem of backreaction is severe at the bounce. This is a summary of the activities of the working group on cosmology at WHEPP-X. The three main problems that were discussed at some length by the group during the course of the workshop were (i) canceling a 'large' cosmological constant, (ii) non-Gaussianities in inflationary models and (iii) stability of interacting models of dark energy and dark matter. We have briefly outlined these problems and have indicated the progress made.

We study interacting scalar field theory non-minimally coupled to gravity in the FRW background. We show that for a specific choice of interaction terms, the energy-momentum tensor of the scalar field vanishes, and as a result the scalar field does not gravitate. The naive space dependent solution to equations of motion gives rise to singular field profile. We carefully analyze the energy-momentum tensor for such a solution and show that the singularity of the solution gives a subtle contribution to the energy-momentum tensor. The space dependent solution therefore is not non-gravitating. Our conclusion is applicable to other space-time dependent non-gravitating solutions as well. Following our previous work wherein the leading order effective action was computed in the covariant effective field theory of gravity, here we specialize the effective action to the FRW spacetime and obtain the effective Friedmann equations. In particular, we focus our attention on studying the cosmological implications of the non-local terms when each of them is combined with the Einstein-Hilbert action. We obtain both analytical and iterative solutions to the effective background equations in all the cases and also briefly comment on the consistency between the iterative and numerical solutions whenever possible. We find that among all the non-local terms, the imprints induced by $R\frac{1}{\square^2}R$ are very significant. Interpreting these corrections as an effective dark energy component characterized by an equation of state parameter, we find that the $R\frac{1}{\square^2}R$ correction can indeed lead to an accelerated expansion of the universe at the present epoch even in the absence of a cosmological constant. We construct the covariant effective field theory of gravity as an expansion in inverse powers of the Planck mass, identifying the leading and next-to-leading quantum corrections. We determine the form of the effective action for the cases of pure gravity with cosmological constant as well as gravity coupled to matter. By means of heat kernel methods we renormalize and compute the leading quantum corrections to quadratic order in a curvature expansion. The final effective action in our covariant formalism is generally non-local and can be readily used to understand the phenomenology on different spacetimes. We investigate radiative corrections to the inflaton potential from heavy fields undergoing a fast-roll phase transition. We find that a logarithmic one-loop correction to the inflaton potential involving this field can induce a temporary running of the spectral index. The induced running can be a short burst of strong running, which may be related to the observed anomalies on large scales in the cosmic microwave spectrum, or extend over many e-folds, sustaining an effectively constant running to be searched for in the future. We implement this in a general class of models, where effects are mediated through a heavy messenger field sitting in its minimum. Interestingly, within the present framework it is a generic outcome that a large running implies a small field model with a vanishing tensor-to-scalar ratio, circumventing the normal expectation that small field models typically lead to an unobservable small running of the spectral index. An observable level of tensor modes can also be accommodated, but, surprisingly, this requires running to be induced by a curvaton. If upcoming observations are consistent with a small tensor-to-scalar ratio as predicted by small field models of inflation, then the present study serves as an explicit example contrary to the general expectation that the running will be unobservable., Comment: 35 pages, 4 figures, matches published version

Scalar-tensor theories of gravity can lead to modifications of the gravitational force inside astrophysical objects. We exhibit that compact stars such as white dwarfs provide a unique set-up to test beyond Horndeski theories of ${\rm G}^3$ type. We obtain stringent and independent constraints on the parameter $\Upsilon$ characterizing the deviations from Newtonian gravity using the mass-radius relation, the Chandrasekhar mass limit and the maximal rotational frequency of white dwarfs. We consider the generation of primordial magnetic fields in a class of bouncing models when the electromagnetic action is coupled non-minimally to a scalar field that, say, drives the background evolution. For scale factors that have the power law form at very early times and non-minimal couplings which are simple powers of the scale factor, one can easily show that scale invariant spectra for the magnetic field can arise before the bounce for certain values of the indices involved. It will be interesting to examine if these power spectra retain their shape after the bounce. However, analytical solutions for the Fourier modes of the electromagnetic vector potential across the bounce are difficult to obtain. In this work, with the help of a new time variable that we introduce, which we refer to as the ${\rm e}$-${\cal N}$-fold, we investigate these scenarios numerically. Imposing the initial conditions on the modes in the contracting phase, we numerically evolve the modes across the bounce and evaluate the spectra of the electric and magnetic fields at a suitable time after the bounce. As one could have intuitively expected, though the complete spectra depend on the details of the bounce, we find that, under the original conditions, scale invariant spectra of the magnetic fields do arise for wavenumbers much smaller than the scale associated with the bounce. We also show that magnetic fields which correspond to observed strengths today can be generated for specific values of the parameters. But, we find that, at the bounce, the backreaction due to the electromagnetic modes that have been generated can be significantly large calling into question the viability of the model. The simplest gauge invariant models of inflationary magnetogenesis are known to suffer from the problems of either large backreaction or strong coupling, which make it difficult to self-consistently achieve cosmic magnetic fields from inflation with a field strength larger than $10^{-32} G$ today on the $\Mpc$ scale. Such a strength is insufficient to act as seed for the galactic dynamo effect, which requires a magnetic field larger than $10^{-20} G$. In this paper we analyze simple extensions of the minimal model, which avoid both the strong coupling and back reaction problems, in order to generate sufficiently large magnetic fields on the Mpc scale today. First we study the possibility that the coupling function which breaks the conformal invariance of electromagnetism is non-monotonic with sharp features. Subsequently, we consider the effect of lowering the energy scale of inflation jointly with a scenario of prolonged reheating where the universe is dominated by a stiff fluid for a short period after inflation. In the latter case, a systematic study shows upper bounds for the magnetic field strength today on the Mpc scale of $10^{-13} G$ for low scale inflation and $10^{-25} G$ for high scale inflation, thus improving on the previous result by 7-19 orders of magnitude. If cosmic magnetic fields are indeed produced during inflation, they are likely to be correlated with the scalar metric perturbations that are responsible for the Cosmic Microwave Background anisotropies and Large Scale Structure. Within an archetypical model of inflationary magnetogenesis, we show that there exists a new simple consistency relation for the non-Gaussian cross correlation function of the scalar metric perturbation with two powers of the magnetic field in the squeezed limit where the momentum of the metric perturbation vanishes. We emphasize that such a consistency relation turns out to be extremely useful to test some recent calculations in the literature. Apart from primordial non-Gaussianity induced by the curvature perturbations, such a cross correlation might provide a new observational probe of inflation and can in principle reveal the primordial nature of cosmic magnetic fields., 5 pages, version published in PRD

We compute the three-point cross-correlation function of the primordial curvature perturbation generated during inflation with two powers of a vector field in a model where conformal invariance is broken by a direct coupling of the vector field with the inflaton. If the vector field is identified with the electromagnetic field, this correlation would be a non-Gaussian signature of primordial magnetic fields generated during inflation. We find that the signal is maximized for the flattened configuration where the wave number of the curvature perturbation is twice that of the vector field and in this limit, the magnetic non-linear parameter becomes as large as |b_{NL}| ~ 10^3. In the squeezed limit where the wave number of the curvature perturbation vanishes, our results agree with the magnetic consistency relation derived in arXiv:1207.4187., 18 pages. The origin of large-scale magnetic fields in cosmic structures and the intergalactic medium is still poorly understood. We explore the effects of non-minimal couplings of electromagnetism on the cosmological evolution of currents and magnetic fields. In this context, we revisit the mildly non-linear plasma dynamics around recombination that are known to generate weak magnetic fields. We use the covariant approach to obtain a fully general and non-linear evolution equation for the plasma currents and derive a generalised Ohm law valid on large scales as well as in the presence of non-minimal couplings to cosmological (pseudo-)scalar fields. Due to the sizeable conductivity of the plasma and the stringent observational bounds on such couplings, we conclude that modifications of the standard (adiabatic) evolution of magnetic fields are severely limited in these scenarios. We propose a novel scenario to generate primordial magnetic fields during inflation induced by an oscillating coupling of the electromagnetic field to the inflaton. This resonant mechanism has two key advantages over previous proposals. First of all, it generates a narrow band of magnetic fields at any required wavelength, thereby allaying the usual problem of a strongly blue spectrum and its associated backreaction. Secondly, it avoids the need for a strong coupling as the coupling is oscillating rather than growing or decaying exponentially. Despite these major advantages, we find that the backreaction is still far too large during inflation if the generated magnetic fields are required to have a strength of order 10^{-15} Gauss today on observationally interesting scales. Certain oscillatory features in the primordial scalar power spectrum are known to provide a better fit to the outliers in the cosmic microwave background data near the multipole moments of $\ell=22$ and 40. These features are usually generated by introducing a step in the popular, quadratic potential describing the canonical scalar field. Such a model will be ruled out, if the tensors remain undetected at a level corresponding to a tensor-to-scalar ratio of, say, $r\simeq 0.1$. In this work, in addition to the popular quadratic potential, we investigate the effects of the step in a small field model and a tachyon model. With possible applications to future datasets (such as PLANCK) in mind, we evaluate the tensor power spectrum exactly, and include its contribution in our analysis. We compare the models with the WMAP (five as well as seven-year), the QUaD and the ACBAR data. As expected, a step at a particular location and of a suitable magnitude and width is found to improve the fit to the outliers (near $\ell=22$ and 40) in all these cases. Recently, we have shown that scalar spectra with lower power on large scales and certain other features naturally occur in {\it punctuated inflation}, i.e. the scenario wherein a brief period of rapid roll is sandwiched between two stages of slow roll inflation. Such spectra gain importance due to the fact that they can lead to a better fit of the observed Cosmic Microwave Background (CMB) anisotropies, when compared to the conventional, featureless, power law spectrum. In this paper, with examples from the canonical scalar field as well as the tachyonic models, we illustrate that, in punctuated inflation, a drop in the scalar power on large scales is {\it always} accompanied by a rise in the tensor power and, hence, an even more pronounced increase in the tensor-to-scalar ratio $r$ on these scales. Interestingly, we find that $r$ actually {\it exceeds well beyond unity} over a small range of scales. {\it To our knowledge, this work presents for the first time, examples of single scalar field inflationary models wherein $r\gg 1$.} This feature opens up interesting possibilities. For instance, we show that the rise in $r$ on large scales translates to a rapid increase in the angular power spectrum, $C_{\ell}^{\rm BB}$, of the B-mode polarization of the CMB at the low multipoles. We investigate inflationary scenarios driven by a class of potentials which are similar in form to those that arise in certain minimal supersymmetric extensions of the standard model. We find that these potentials allow a brief period of departure from inflation sandwiched between two stages of slow roll inflation. We show that such a background behavior leads to a step like feature in the scalar power spectrum. We set the scales such that the drop in the power spectrum occurs at a length scale that corresponds to the Hubble radius today--a feature that seems necessary to explain the lower power observed in the quadrupole moment of the Cosmic Microwave Background (CMB) anisotropies. We perform a Markov Chain Monte Carlo analysis to determine the values of the model parameters that provide the best fit to the recent WMAP 5-year data for the CMB angular power spectrum. Recent observational claims of magnetic fields stronger than $10^{-16}$ G in the extragalactic medium motivate a new look for their origin in the inflationary magnetogenesis models. In this work we shall review the constraints on the simplest gauge invariant model $f^2(\phi)F_{\mu \nu}F^{\mu \nu}$ of inflationary magnetogenesis, and show that in the optimal region of parameter space the anisotropic constraints coming from the induced bispectrum, due to the generated electromagnetic fields, yield the strongest constraints. In this model, only a very fine tuned scenario at an energy scale of inflation as low as $10^{-2}$ GeV can explain the observations of void magnetic fields. These findings are consistent with the recently derived model independent constraints. However, if the detection of primordial tensor modes by BICEP2 is confirmed, the possibility of low scale inflation is excluded. Assuming the validity of the BICEP2 claim of a tensor-to-scalar ratio $r=0.2^{+0.07}_{-0.05}$, we provide the updated constraints on inflationary magnetogenesis. The generation of helical magnetic fields during single field inflation due to an axial coupling of the electromagnetic field to the inflaton is discussed. We find that such a coupling always leads to a blue spectrum of magnetic fields during slow roll inflation. We propose a fully nonlinear framework to construct consistency relations for testing generic cosmological scenarios using the evolution of large scale structure. It is based on the covariant approach in combination with a frame that is purely given by the metric, the normal frame. As an example, we apply this framework to the LambdaCDM model, by extending the usual first order conditions on the metric potentials to second order, where the two potentials start to differ from each other. We argue that working in the normal frame is not only a practical choice but also helps with the physical interpretation of nonlinear dynamics. In this frame, effective pressures and anisotropic stresses appear at second order in perturbation theory, even for "pressureless" dust. We quantify their effect and compare them, for illustration, to the pressure of a generic clustering dark energy uid and the anisotropic stress in the DGP model. We study the generation of helical magnetic fields during single field inflation induced by an axial coupling of the electromagnetic field to the inflaton. During slow roll inflation, we find that such a coupling always leads to a blue spectrum with $B^2(k) \propto k$, as long as the theory is treated perturbatively. The magnetic energy density at the end of inflation is found to be typically too small to backreact on the background dynamics of the inflaton. We also show that a short deviation from slow roll does not result in strong modifications to the shape of the spectrum. We calculate the evolution of the correlation length and the field amplitude during the inverse cascade and viscous damping of the helical magnetic field in the radiation era after inflation. In the slow-roll inflationary scenario, the amplitude of the curvature perturbations approaches a constant value soon after the modes leave the Hubble radius. However, relatively recently, it was shown that the amplitude of the curvature perturbations induced by the canonical scalar field can grow at super-Hubble scales if there is either a transition to fast roll inflation or if inflation is interrupted for some period of time. In this work, we extend the earlier analysis to the case of a non-canonical scalar field described by the Dirac-Born-Infeld action. With the help of a specific example, we show that the amplitude of the tachyonic perturbations can be enhanced or suppressed at super-Hubble scales if there is a transition from slow roll to fast roll inflation. We also illustrate as to how the growth of the entropy perturbations during the fast roll regime proves to be responsible for the change in the amplitude of the curvature perturbations at super-Hubble scales. Furthermore, following the earlier analysis for the canonical scalar field, we show that the power spectrum evaluated in the long wavelength approximation matches the exact power spectrum obtained numerically very well. Surgery and/or Radiotherapy offers poor cure rate in the cases of advanced laryngeal cancer. Due to this fact a number of combined modality programmes have been piloted in which sequential chemotherapy and radiotherapy play an important role in the primary treatment plan. In present paper we have compared the results of sequential chemotherapy followed by external radiotherapy and radiotherapy alone. This study was carried out in 55 cases of advanced laryngeal carcinoma (Stage III and Stage IV). The chemotherapeutic agent used in this study was parenteral methotrexate. There was marked improvement in cure rate of advanced squamous cell carcinoma of larynx who received sequential methotrexate followed by external beam radiotherapy.
