Purpose: Intracameral mydriatic agents (ICMAs) are replacing the conventional method of topical mydriasis for its fast action and no need for repeated instillation before cataract surgery. Its application for the management of intraoperative miosis needs to be studied with different doses of mydriatic agent. The objective of the study is to study cardiovascular effects of diluted intracameral combination of 0.02% tropicamide, 0.31% phenylephrine, and 1% lidocaine during phacoemulsification to manage intraoperative miosis., Materials and Methods: The study was conducted at Mojiram Lions Eye Hospital Akbarpur Majra village, Palla Bakhtawarpur Road, Delhi, during the year 2021-2022 after taking approval from the Ethical Committee of the hospital. Patients undergoing cataract surgery who were not dilated adequately during the preoperative period or developed intraoperative miosis were managed by injecting diluted intracameral combination of 0.02% tropicamide, 0.31% phenylephrine, and 1% lidocaine (phenocaine plus). 0.5 ml of phenocaine plus was diluted with 1.5 ml of ringer lactate solution and 0.50 ml of this solution was injected intracameral and its effect on pulse rate, blood pressure (BP), and oxygen saturation were monitored continuously on pulse oximeter., Results: There was no statistically significant effect of diluted ICMA (Phenocaine Plus) on diastolic BP and oxygen saturation. However, systolic BP showed a little change from mean of 133.78 (standard deviation [SD] =16.04) mmHg to 133.92 (SD = 15.33) mmHg which was statistically significant but clinically not significant. Pulse rate increased slightly from mean 76.46 per minutes (SD = 15.14) to 79.40 (SD = 14.95) at 10 s and 76.49 (SD = 15.15) at 60 s. The difference was again statistically significant but clinically insignificant., Conclusion: Low concentration of intracameral combination of 0.02% tropicamide, 0.31% phenylephrine, and 1% lidocaine is a very safe and effective method for the management of intraoperative miosis., Competing Interests: The author declares that there are no conflicts of interest in this paper., (Copyright: © 2023 Taiwan J Ophthalmol.)

Strong-motion records of earthquakes are used not only to evaluate the source rupture process, seismic wave propagation and strong ground motion characteristics, but also to provide valuable data for earthquake disaster mitigation. The Kathmandu Valley, Nepal, which is characterised by having soft sediments that have been deposited in an earthquake-prone zone, has experienced numerous earthquakes. We have operated four strong-motion stations in the Kathmandu Valley since 2011. These stations recorded the 2015 magnitude 7.8 Gorkha Nepal earthquake that occurred in the Himalayan continental collision zone. For several months after the mainshock, we deployed four additional temporary stations. Here, we describe the seismic data for 18 earthquakes over magnitude 5.0 collected by this array, including the 2015 magnitude 7.3 Dolakha earthquake of maximum aftershock and three large aftershocks of magnitude 6-class. These data are essential for validating the sedimentary structure of the basin and for evaluating the hazard and risk of future earthquakes in the Kathmandu Valley., (© 2022. The Author(s).)

The Syabru-Bensi hydrothermal zone, Langtang region (Nepal), is characterized by high radon-222 and CO(2) discharge. Seasonal variations of gas fluxes were studied on a reference transect in a newly discovered gas discharge zone. Radon-222 and CO(2) fluxes were measured with the accumulation chamber technique, coupled with the scintillation flask method for radon. In the reference transect, fluxes reach exceptional mean values, as high as 8700+/-1500 gm(-2)d(-1) for CO(2) and 3400+/-100 x 10(-3) Bq m(-2)s(-1) for radon. Gases fluxes were measured in September 2007 during the monsoon and during the dry winter season, in December 2007 to January 2008 and in December 2008 to January 2009. Contrary to expectations, radon and its carrier gas fluxes were similar during both seasons. The integrated flux along this transect was approximately the same for radon, with a small increase of 11+/-4% during the wet season, whereas it was reduced by 38+/-5% during the monsoon for CO(2). In order to account for the persistence of the high gas emissions during monsoon, watering experiments have been performed at selected radon measurement points. After watering, radon flux decreased within 5 min by a factor of 2-7 depending on the point. Subsequently, it returned to its original value, firstly, by an initial partial recovery within 3-4h, followed by a slow relaxation, lasting around 10h and possibly superimposed by diurnal variations. Monsoon, in this part of the Himalayas, proceeds generally by brutal rainfall events separated by two- or three-day lapses. Thus, the recovery ability shown in the watering experiments accounts for the observed long-term persistence of gas discharge. This persistence is an important asset for long-term monitoring, for example to study possible temporal variations associated with stress accumulation and release.
