Abstract

Background: Peripheral T-Cell Lymphomas (PTCL) are aggressive malignant lymphoproliferative disorders that represent 10-15% of all non-Hodgkin’s lymphomas (NHL). PTCL- not otherwise specified (PTCL-NOS), comprises approximately 6% of all NHLs, which do not meet the criteria for other mature peripheral T-Cell lymphoma as defined in WHO classification. With limited long-term data evaluating the outcome of PTCL-NOS in the Indian population, we aimed to determine the clinico-epidemiological profile and treatment outcome of patients with PTCL-NOS in a cancer care center from North-East India. Material and Methods: A retrospective observational analysis was performed on the medical records of patients who were diagnosed with PTCL-NOS between January 2015 and December 2018 at Dr. B. Borooah Cancer Institute. Results: Our study evaluated fifty-three patients with PTCL-NOS with the mean age at presentation being 45.3 years (SD ± 16.3 years). The majority of the patients (77.3%) presented with advanced stage (III/IV) with bone marrow involvement in 35.8% of the patients. B-symptoms were present in 58.4% of the patients. Extra-nodal sites of involvement were seen in 64.1% of the patients. The anthracycline-based chemotherapy regimen was the most commonly used in the frontline setting in 71.6% of the patients. Radiation therapy (IFRT) was received by 26.4% of the patients. The overall response rate (ORR) to first-line chemotherapy was 94.3% with complete response (CR) achieved by 62.3% of patients. Twenty-nine (58%) patients had relapsed/ progressive disease. With a median follow-up duration of 38 months, the 3-year progression-free survival (PFS) rate and 3-year overall survival (OS) rate were 27.6% and 33.9% respectively. The median PFS was 22 ± 3.3 months and the median OS was 28 ± 7.1 months. Conclusion: PTCL-NOS is one of the aggressive subtypes of NHL with limited therapeutic options and poor outcomes. Proper treatment adherence and the availability of newer therapeutic options can further enhance the overall outcome.

It is classically considered to be a disease of young and middle adulthood, with females more commonly affected than males. FMD is a systemic disease. Although historically considered to be rare, cerebrovascular FMD (C-FMD) has now been recognized to be as common as the renovascular counterpart. Extracranial carotid and vertebral arteries are the most commonly involved vascular territories in C-FMD with the clinical presentation determined by vessels affected. Common symptoms include headaches and pulsatile tinnitus, with transient ischemic attacks, ischemic stroke and subarachnoid or intracerebral hemorrhage constituting the more severe clinical manifestations. Cervical artery dissection involving carotids more often than vertebral arteries and intracranial aneurysms account for the cerebrovascular pathologies detected in C-FMD. Our understanding regarding C-FMD has been augmented in the recent past on account of dedicated C-FMD data from North American, European and other international FMD cohorts. 31. Assessment of the Learning Curve for Virtual Surgical Planning in Orbital Fractures

Abstract

Virtual surgical planning (VSP) is becoming more widely used in maxillofacial reconstruction and can be surgeon-based or industry-based. Surgeon-based models require software training but allow surgeon autonomy. We evaluate the learning curve for VSP through a prospective cohort study in which planning times and accuracy of 7 otolaryngology residents with no prior VSP experience were compared to that of a proficient user after a single training protocol and 6 planning sessions for orbital fractures. The average planning time for the first session was 21 minutes 41 seconds ± 6 minutes 11 seconds with an average maximum deviation of 2.5 ± 0.8 mm in the lateral orbit and 2.3 ± 0.6 mm in the superior orbit. The average planning time for the last session was 13 minutes 5 seconds ± 10 minutes and 7 seconds with an average maximum deviation of 1.4 ± 0.5 mm in the lateral orbit and 1.3 ± 0.4 mm in the superior orbit. Novice users reduced planning time by 40% and decreased maximum deviation of plans by 44% and 43% in the lateral and superior orbits, respectively, approaching that of the proficient user. Virtual surgical planning has a quick learning curve and may be incorporated into surgical training.

Nasopharyngeal oncocytic lesions are a spectrum of benign lesions that represent a reactive or hyperplastic response to chronic inflammation such as smoking and age-related degeneration. The lesions differ based on location, origin, and presence of cystic and inflammatory components. While these oncocytic lesions are frequently asymptomatic, larger lesions in the nasopharynx can result in significant symptoms such as eustachian tube dysfunction.

Though oncocytic lesions are typically asymptomatic, unilateral, and benign, this article discusses a rare case of large, bilateral oncocytic cysts and downstream otologic sequelae with a focus on identifying and discussing similar disease processes.MethodsCase report and literature review.Case Presentation: A 67-year-old patient with 57 pack year smoking history presented for one year of left sided hearing loss and aural fullness. Clinic endoscopic exam demonstrated severe inflammatory and cystic changes lining the bilateral tori. Imaging and tissue sampling confirmed the cause was minor salivary gland cysts with papillary projections lined by oncocytic cells within bilateral tori tubarius. He was successfully treated with myringotomy with pressure equalizing tube, counseling on tobacco cessation, and surveillance with serial nasopharyngoscopy.ConclusionsChronic eustachian tube dysfunction is a possible rare presentation and sequelae of large oncocytic cysts of the nasopharynx. Oncocytic cysts should be considered on the differential diagnosis for nasopharyngeal masses causing such dysfunction.
