Abstract

AIMS: This meta-analysis aims to quantify the association of reduced coronary flow with all-cause mortality and major adverse cardiovascular events (MACE) across a broad range of patient groups and pathologies. METHODS AND RESULTS: We systematically identified all studies between 1 January 2000 and 1 August 2020, where coronary flow was measured and clinical outcomes were reported. The endpoints were all-cause mortality and MACE. Estimates of effect were calculated from published hazard ratios (HRs) using a random-effects model. Seventy-nine studies with a total of 59 740 subjects were included. Abnormal coronary flow reserve (CFR) was associated with a higher incidence of all-cause mortality [HR: 3.78, 95% confidence interval (CI): 2.39-5.97] and a higher incidence of MACE (HR 3.42, 95% CI: 2.92-3.99). Each 0.1 unit reduction in CFR was associated with a proportional increase in mortality (per 0.1 CFR unit HR: 1.16, 95% CI: 1.04-1.29) and MACE (per 0.1 CFR unit HR: 1.08, 95% CI: 1.04-1.11). In patients with isolated coronary microvascular dysfunction, an abnormal CFR was associated with a higher incidence of mortality (HR: 5.44, 95% CI: 3.78-7.83) and MACE (HR: 3.56, 95% CI: 2.14-5.90). Abnormal CFR was also associated with a higher incidence of MACE in patients with acute coronary syndromes (HR: 3.76, 95% CI: 2.35-6.00), heart failure (HR: 6.38, 95% CI: 1.95-20.90), heart transplant (HR: 3.32, 95% CI: 2.34-4.71), and diabetes mellitus (HR: 7.47, 95% CI: 3.37-16.55). CONCLUSION: Reduced coronary flow is strongly associated with increased risk of all-cause mortality and MACE across a wide range of pathological processes. This finding supports recent recommendations that coronary flow should be measured more routinely in clinical practice, to target aggressive vascular risk modification for individuals at higher risk.

10. Structuring Communities for Sharing Human Digital Memories in a Social P2P Network

Abstract

A community is sub-network inside P2P networks that partition the network into groups of similar peers to improve performance by reducing network traffic and high search query success rate. Large communities are common in online social networks than traditional file-sharing P2P networks because many people capture huge amounts of data through their lives. This increases the number of hosts bearing similar data in the network and hence increases the size of communities. This article presents a Memory Thread-based Communities for our entity-based social P2P network that partition the network into groups of peers sharing data belonging to an entity - person, place, object or interest, having its own digital memory or be a part another memory. These connected peers having further similarities by organizing the network using linear orderings. A Memory-Thread is the collection of digital memories having a common reference key and organized according to some form of correlation. The simulation results show an increase in network performance for the proposed scheme along with a decrease in network overhead and higher query success rate compared to other similar schemes. 20. Integration, Implementation and Reporting Outlooks of Sustainability in Higher Education Institutions (HEIs): Index and Case Base Validation

Abstract

Purpose: The integration of sustainability is more matured in the corporate sector. However, the topic received less attention in higher education institutions (HEIs). Therefore, this study aims to conceptualize an integration framework and proposes sustainability integration and implementation index in the HEIs. Design/methodology/approach: The index was developed from previous literature. For the implementation of the index, three universities were selected. The required data collected from websites, internal documents and interviews with the high officials of these universities. Findings: Sample universities have somehow inclination toward the broad agenda of sustainable development, but still, they lack integration, implementation and reporting of their sustainable practices. Moreover, the departments have positive and practical intent toward sustainability especially regarding the environment, designing curriculum, teaching, research activities and volunteerism. However, in assessment, the social and economic contributions of these universities revealed to be somewhat mild. The findings also evidenced heterogeneity among the sample institutions to follow the said agenda which reflects a lack of awareness and policy or a centralized or regulatory push from the top. Research limitations/implications: The study is limited to three universities in Khyber Pakhtunkhwa, Pakistan. Hence, in the future, the framework and index may be tested in other universities of the country or region. Nevertheless, the study carries theoretical significance in the literature of sustainability. Practical implications: The study has practical implications for academia, regulatory bodies and universities to integrate, implement and report sustainability in HEIs. Originality/value: The study proposes and validates the sustainability integration and implementation index in HEIs. The index is unique and has originality or value particularly for HEIs of the developing countries.

22. Do corporate sustainability practices mitigate earnings management? The moderating role of firm size.

Abstract

The majority of the prior empirical literature that inquired about the direct role of corporate sustainability practices (CSP) as a composite construct in mitigating earnings management (EM) has produced equivocal outcomes. Therefore, this study examines the role of CSP not only as a combined construct but also as its three separate dimensions—social, economic, and environmental sustainability in restricting EM directly and with the distinct moderation of firm size that has rarely been probed in the past. Using the data of 255 Pakistani companies from 2018 to 2022, the estimations of ordinary least squares with panel‐corrected standard errors revealed that CSP and its separate dimensions significantly control accruals‐based earnings management, and all these relationships are further amplified by the moderation of firm size. However, neither CSP nor any of its dimensions, either directly or with the moderation of firm size, have a significant role in reducing real‐based earnings management. The additional analysis also validated these findings but only for large companies rather than small companies after splitting the sample based on the firm's size. The findings endorse the stakeholder theory and ethical perspective but oppose managerial opportunism in Pakistan. Abstract

The nexus between manufacturing firms and the green environment is unavoidable. It is merely due to the huge waste generation and greenhouse gas emissions resulting from large‐scale production and complex supply chain management processes involved in manufacturing firms. Continuation of the traditional business practices during Industry 4.0 made it impossible for the manufacturing firm to commit to green environmental practices and comply with environmental SDGs. At odd, it requires novel business strategies to ensure compliance of manufacturing firms with environmental sustainable development goals (SDGs). Blockchain is an ideal technology for delivering green environmental‐related information because it offers real‐time, shareable, and entirely transparent data kept on an immutable ledger that is accessible to members of a network. These green environmental‐related information received in real‐time through blockchain technology help in the transformation of the traditional supply chain management practices into a more green supply chain management practice. This green transformation ensures business compliance with environmental SDGs and that compliance is shared with multiple stakeholders through blockchain technology in real‐time, thus approving a positive mediating role of green supply chain management practices between blockchain technology and environmental SDGs performance. The proposed strategic framework through various propositions development will mainly offer policy insights toward (a) identifying novel antecedents of blockchain technology, (b) offering insights toward upgrading the traditional supply chain management framework into a green supply chain management framework, and (c) illuminating novel mediating role of green supply chain management practices between blockchain technology and environmental SDGs performance. Abstract

This study aims to inquire whether it is the mere female directors or their certain attributes that improve the quality of corporate sustainability disclosures (QCSD). Annual and sustainability reports are used to obtain data for 300 non‐financial Pakistani listed companies selected through stratified random sampling for the period 2012–2021. The study employed ordinary least squares with panel‐corrected standard errors to test research hypotheses. The findings that neither support "tokenism" nor "critical mass" assumptions revealed that firms with female directors on the board, regardless of how many, have better QCSD than others. Similarly, the proportion of female directors on the board and audit committee also showed a positive association with QCSD. The positive role of independent female directors and their experience was slightly more pronounced than the executive female directors and their experience in improving QCSD. Likewise, the positive effect of female directors' business‐related education was marginally higher than their non‐business education in improving QCSD. Furthermore, female directors' master's or above level of education had a significant positive, while their bachelor's or below level of education had no significant association with QCSD. Abstract

BACKGROUND: Myocardial bridges (MBs) are prevalent and can be associated with acute and chronic ischemic syndromes. We sought to determine the substrates for ischemia in patients with angina with nonobstructive coronary arteries and a MB in the left anterior descending artery. METHODS: Patients with angina with nonobstructive coronary arteries underwent the acquisition of intracoronary pressure and flow during rest, supine bicycle exercise, and adenosine infusion. Coronary wave intensity analysis was performed, with perfusion efficiency defined as accelerating wave energy/total wave energy (%). Epicardial endothelial dysfunction was defined as a reduction in epicardial vessel diameter ≥20% in response to intracoronary acetylcholine infusion. Patients with angina with nonobstructive coronary arteries and a MB were compared with 2 angina with nonobstructive coronary arteries groups with no MB: 1 with coronary microvascular disease (CMD: coronary flow reserve, <2.5) and 1 with normal coronary flow reserve (reference: coronary flow reserve, ≥2.5). RESULTS: Ninety-two patients were enrolled in the study (30 MB, 33 CMD, and 29 reference). Fractional flow reserve in these 3 groups was 0.86±0.05, 0.92±0.04, and 0.94±0.05; coronary flow reserve was 2.5±0.5, 2.0±0.3, and 3.2±0.6. Perfusion efficiency increased numerically during exercise in the reference group (65±9%–69±13%; P=0.063) but decreased in the CMD (68±10%–50±10%; P<0.001) and MB (66±9%–55±9%; P<0.001) groups. The reduction in perfusion efficiency had distinct causes: in CMD, this was driven by microcirculation-derived energy in early diastole, whereas in MB, this was driven by diminished accelerating wave energy, due to the upstream bridge, in early systole. Epicardial endothelial dysfunction was more common in the MB group (54% versus 29% reference and 38% CMD). Overall, 93% of patients with a MB had an identifiable ischemic substrate. CONCLUSIONS: MBs led to impaired coronary perfusion efficiency during exercise, which was due to diminished accelerating wave energy in early systole compared with the reference group. Additionally, there was a high prevalence of endothelial and microvascular dysfunction. These ischemic mechanisms may represent distinct treatment targets. [ABSTRACT FROM AUTHOR]

Abstract

This study endeavors to explore the impact of Corporate Social Responsibility (CSR) on the financial performance of firms within the Pakistani cement industry. The research utilized quarterly data extracted from the annual and other financial reports of eight selected cement companies, representing half of the total 16 cement companies in Pakistan's cement sector, spanning the period from 2009 to 2016. The predictor variable, CSR, was gauged by factors such as donations, insurance, and worker welfare fund. In parallel, the performance of the firms, considered as the outcome variable, was assessed through metrics including Earnings Per Share (EPS) and Net Profit Margin (NPM). Employing Ordinary Least Squares with Panel Corrected Standard Errors (OLS-PCSEs), the study uncovered a positive correlation between CSR initiatives and the financial performance of these cement companies. In addition to substantiating the Stakeholder Theory, these findings shed light on the social inclinations of Pakistani cement firms. They suggest that firms integrating CSR activities into their operations possess a competitive advantage over those abstaining from such endeavors. The study elucidates that engaging in CSR activities not only aligns with social responsibility but also imparts strategic advantages to businesses. Companies actively participating in CSR activities may find themselves at a competitive advantage compared to counterparts that do not partake in such practices.
